RESUMEN

This study investigated the structure of acid Alhagi camelorum Fischa polysaccharide (aAP) and its impact on intestinal activity in mice. The results showed that aAP comprised of the fucose, arabinose, rhamnose, galactose, glucose, xylose, mannose, galacturonic acid, glucuronic acid with the molar ratio of 0.81:14.97:10.84:11.14:3.26:0.80:0.80:54.92:2.47 with the molecular weight (Mw) of 22.734 kDa. Additionally, the composition of aAP was assessed via FT-IR, methylation, and NMR analyses, indicating that the backbone of the aAP was consisted of →4)-α-D-GalpA-6-OMe-(1 â†’ 4)-α-GalpA-(1 â†’ and →4)-α-D-GalpA-6-OMe-(1 â†’ 2)-α-L-Rhap-(1→, as well as →4)-ß-D-Galp- and →5)-α-L-Araf- for the branched chain. Furthermore, ICR mice underwent intragastric administration of different concentrations of aAP for 7 consecutive days. The results showed that aAP enhanced the murine spleen and thymus indices, promoted the secretion of serum lgG antibody, intestinal lgA antibody and intestinal cytokines, improved the morphology of intestinal villi and crypts, enhanced quantity of intestinal IELs and IgA+ cells, and activated T lymphocytes and DC cells in MLNs. In summary, these findings suggest that the utilization of aAP could enhance the immune response of the murine intestinal mucosa.

Bovine mastitis, caused by Streptococcus agalactiae (Group B Streptococcus; GBS), poses significant economic challenges to the global dairy industry. Mouse models serves as valuable tools for assessing GBS-induced infections as an alternative to large animals. This study aimed to investigate the LD50 dose, organ bacterial load, and quantification of peritoneal leukocyte populations for GBS serotypes Ia and II isolates from China and Pakistan. Additionally, we measured indicators such as lactoferrin, albumin, and myeloperoxidase (MPO) activity. Pro-inflammatory cytokines (TNF-α, IL-1ß, IL-6, and IL-2) and anti-inflammatory cytokines (IL-10 and TGF-ß) in serum and tissue samples were evaluated using ELISA and qPCR, respectively. BALB/c mice (4 mice per group) received individual intraperitoneal injections of 100 µl containing specific bacterial inoculum concentrations (ranging from 105 to 109 CFU per mouse) of Chinese and Pakistani GBS isolates (serotypes Ia and II). Control groups received 100 µL of sterile PBS. Results revealed that the LD50 bacterial dose causing 50 % mortality in mice was 107 CFU. The highest bacterial load in all experimental groups was quantified in the peritoneum, followed by blood, mammary gland, liver, spleen, lungs, and brain. The most significant bacterial dissemination was observed in mice inoculated with Pakistani serotype Ia at 24 h, with a subsequent notable decline in bacterial counts at day 3. Notably, infection with Pakistani serotype Ia showed a trend of increased total leukocyte counts, significantly higher than Pakistani serotype II, Chinese Serotype Ia, and Chinese serotype II. A substantial influx of neutrophils and lymphocytes was observed in response to all tested serotypes, with Pakistani serotype Ia inducing a significantly higher influx compared to other groups (Pakistani serotype II, Chinese serotype Ia, and Chinese serotype II). Furthermore, TNF-α, IL-1ß, IL-2, and IL-6 expressions were significantly increased in mice one day after infection with the Pakistani serotype Ia. Compared to mice infected with the Pakistani serotype II, Chinese Serotype Ia, and Chinese serotype II, those infected with the Pakistani serotype Ia isolate exhibited the highest production of IL-10 and TGF-ß, along with significantly increased concentrations of lactoferrin, albumin, and MPO. These findings suggest that the persistence and severity of infection caused by the Pakistani serotype Ia may be linked to its ability to spread to deeper tissues. This study enhances our understanding of the clinical characteristics of bovine mastitis caused by S. agalactiae in China and Pakistan.

The Rho guanosine triphosphatase hydrolase enzyme (GTPase) is required for the control of the actin cytoskeleton, but its activation in vivo condition is unknown. The study's goal was to find a new synthetic nanobody VHH (P-36 tagged with mNeonGreen) that interacts strongly with the Rho GTPase. We present the first novel synthetic nanobody, VHH (P-36 tagged with mNeonGreen), tested in fission yeast cells and found to have a particular interaction with Rho1GTPase. Plasmids were constructed by using of certain enzymes to digest the pDUAL-pef1a vector plasmid to produce a protein that was encoded by cloned genes. A varied VHH library was created synthetically, then transformed into yeast cells, and positive clones were chosen using chemical agents. To investigate protein interactions and cellular reactions, several studies were carried out, such as live cell imaging, growth curve analysis, coimmunoprecipitation, structural analysis, and cell therapies. Prism and RStudio were used for the statistical analysis. The presence of VHH (P-36) has no effect on the growth pattern making it an appropriate model for studying cytokinesis in vivo. According to a computational biological study, its affinity to interact with Rho1GTPase with all the complementarity-determining region (CDR) regions found on VHH (P-36) is extremely strong. We were able to track its subcellular target by localization using a fluorescent confocal microscope, ensuring the maintenance of cell polarity and morphology. Spheroplast analysis revealed a circular-shaped cell with an even distribution of Rho1 tagged VHH (P-36), indicating that the interaction occurs near the plasma membrane. The introduction of latrunculin-A (Lat-A) disrupted Rho GTPase localization, demonstrating the control over actin production, and the cell did not show evidence of mitotic phase commencement while Lat-A was present. Finally, this important biological tool can aid in our understanding of the mechanics and dynamics of cytokinesis in relation to Rho1GTPase.

INTRODUCTION AND IMPORTANCE: Degeneration process occurs in humans after reaching their maximum potential. The degeneration process in the spine includes osteoporosis and degenerative disc disease, however, the conventional treatment causes many post-operative complications. Minimally invasive procedures have recently been carried out considering the same expected outcome and minimally injuring other tissues. Biological approaches using mesenchymal stem cells (MSCs) and secretomes are more promising for bone-related issues. We report a degenerative spine case managed with minimally invasive procedures combined with a biological approach. CASE PRESENTATION: An 83-year-old woman with a chief complaint of back pain after a fall, the physical examination found a painful area in the lower back accompanied by motor weakness in both legs, causing daily use of a wheelchair. Radiology examinations showed compression fracture, bulging disc, and osteoporosis. The patient underwent multiple minimally invasive procedures, namely vertebroplasty, MSCs implantation, PLDD, and secretome implantation. CLINICAL DISCUSSION: From 6 months of follow-up, it was found that the patient's posture getting better, the pain was reduced, and the results of the BMD examination were improved. The patient was able to carry out normal activities. This is due to vertebroplasty which strengthens the structure, PLDD which decompresses the disc, and implantation of MSCs and secretomes which improves the quality of the bone and surrounding tissue. CONCLUSION: The multi-minimally invasive procedure is potential for complex degenerative spine cases, particularly when combined with biological approaches using stem cells and secretomes in elderly, considering that complications from conventional treatment are quite common in elderly.

Camel milk (CM) is the key component of human diet specially for the population belongs to the arid and semi-arid regions of the world. CM possess unique composition as compare to the cow milk with abundant amount of medium chain fatty acids in fat low lactose and higher concentration of whey protein and vitamin C. Besides the nutritional significance of CM, it also contains higher concentration of bioactive compounds including bioactive peptides, lactic acid bacteria (LAB), lactoferrin (LF), lactoperoxidase, lysozyme casein and immunoglobulin. Recently, CM and their bioactive compounds gaining more attention toward scientific community owing to their multiple health benefits, especially in the current era of emerging drug resistance and untold side effects of synthetic medicines. Consumption of fresh or fermented CM and its products presumed exceptional nutraceutical and medicinal properties, including antimicrobial, anti-inflammatory, antioxidant, anti-diabetic, hepatoprotective, nephroprotective, anticancer and immunomodulatory activities. Moreover, CM isolated LAB exhibit antioxidant and probiotic effects leading to enhance the innate and adaptive immune response against both gram-negative and gram-positive pathogenic bacteria. The main objective of this review is to highlight the nutritional significance, pharmaceutical potential, medicinal value and salient beneficial health aspect of CM for human and animals.

Background: Polysaccharide metal chelate exhibit both immunoregulatory activity and metal element supplementation effects. Methods: In this study, Ruoqiang jujube polysaccharide copper chelate (RJP-Cu) was prepared and the preparation conditions were optimized using the response surface method. Subsequently, RJP-Cu was administered to lambs to evaluate its impact on growth performance, copper ion (Cu2+) supplementation, immune enhancement, and intestinal flora was evaluated. Results: The results indicated that optimal RJP-Cu chelation conditions included a sodium citrate content of 0.5 g, a reaction temperature of 50°C, and a solution pH of 8.0, resulting in a Cu2+ concentration of 583°mg/kg in RJP-Cu. Scanning electron microscopy (SEM) revealed significant structural changes in RJP before and after chelation. RJP-Cu displaying characteristic peaks of both polysaccharides and Cu2+ chelates. Blood routine indexes showed no significant differences among the RJP-Cu-High dose group (RJP-Cu-H), RJP-Cu-Medium dose group (RJP-Cu-M), RJP-Cu-low dose group (RJP-Cu-L) and the control group (p > 0.05). However, compared with the control group, the RJP-Cu-H, M, and L dose groups significantly enhanced lamb production performance (p < 0.05). Furthermore, RJP-Cu-H, M, and L dose groups significantly increased serum Cu2+ concentration, total antioxidant capacity (T-AOC), catalase (CAT), and total superoxide dismutase (T-SOD) contents compared with control group (p < 0.05). The RJP-Cu-H group exhibited significant increases in serum IgA and IgG antibodies, as well as the secretion of cytokines IL-2, IL-4, and TNF-α compared to the control group (p < 0.05). Furthermore, RJP-Cu-H group increased the species abundance of lamb intestinal microbiota, abundance and quantity of beneficial bacteria, and decrease the abundance and quantity of harmful bacteria. The RJP-Cu-H led to the promotion of the synthesis of various Short Chain Fatty Acids (SCFAs), improvements in atrazine degradation and clavulanic acid biosynthesis in lambs, while reducing cell apoptosis and lipopolysaccharide biosynthesis. Conclusion: Thus, these findings demonstrate that RJP-Cu, as a metal chelate, could effectively promote lamb growth performance, increase Cu2+ content, and potentially induce positive immunomodulatory effects by regulating antioxidant enzymes, antibodies, cytokines, intestinal flora, and related metabolic pathways.

INTRODUCTION AND IMPORTANCE: Congenital scoliosis is abnormal vertebral column growth and development during embryogenesis. The most common type of congenital scoliosis is failure of growth which is called as hemivertebra. However, the recent surgical treatment of hemivertebra has several complications especially in young patient. The mesenchymal stem cells (MSCs) have been used to treat several bone problems including bone defect and may be have potential to treat the defect in hemiverterbra. We reported a hemivertebra treated by umbilical cord-derived MSCs (UC-MSCs). CASE PRESENTATION: A two-year-old boy presented with scoliosis deformity. The mother noticed the patient's deformity when he was 10th month of age as he learned to stand and progressed since then. There were no growth and development problems. On physical examination, the patient appeared to have scoliosis at lumbar level with bending to the right and asymmetry of waist fold with left shoulder depression. Based on X-ray and CT-Scan investigations, the patient was diagnosed with single fully segmented hemivertebra at 3rd lumbar level. 20 × 106 UC-MSCs were implanted into the bone defect of hemivertebra. CLINICAL DISCUSSION: At three-year follow-up, the X-ray and MRI investigations showed a decrease of Cobb angle and increase of hemivertebra ratio. These findings may be due to improvement of the bone defect, which is consistent with several studies that MSCs have abilities to promote bone formation by maintaining the osteoblast cells and improving vascularization. CONCLUSION: We found that MSCs therapy for hemivertebra represent a potential therapy to correct scoliosis curvature and prevent further curvature. Further clinical studies are required to investigate the efficacy of this therapy in hemivertebra.

Resilience is a significant attribute used to measure the sustainable development of the environment, and research on optimal measurement models is very important. This study took 15 farms in the Jiansanjiang Administration of Heilongjiang Province in China as the research object and constructed a resilience evaluation indicator system containing 31 indicators for the regional agricultural soil-water resource composite system (ASWRS). The combined weight (BFCM-CRITIC) and entropy weight (EW) combined with the variable fuzzy assessment (VFA) model and the improved technique for order preference by similarity to an ideal solution (TOPSIS) model were used to calculate the resilience exponent and to analyse the characteristics of space-time variation. The stability and reliability of the two models under different weights were verified by the Spearman correlation coefficient and discrimination theory to determine the optimal resilience exponent diagnosis method. The results show that according to the BFCM-CRITIC-VFA model, the levels of resilience were the highest at the Nongjiang, Hongwei and Erdaohe farms, and the resilience levels were strong and scattered. The resilience of the Jiansanjiang Administration has been increasing over time, and the spatial distribution has generally decreased from north to south and from the Heilong River and Wusuli River basins inland. A comparison of the reliability and stability of the two models for different weights indicates that the VFA model optimized based on combined weights was superior to the other methods in terms of stability and reliability, which verifies that the BFCM-CRITIC-VFA model is the most suitable for measuring the resilience exponent.

Given the central role of dendritic cells (DCs) in directing cell-mediated immunity, this study investigated the capability of Eimeria tenella 14-kDa phosphohistidine phosphatase (EtPHP14) to mature chicken DCs and initiate DC-induced T cell immunity. With the aim of identifying novel protective Eimeria antigen, EtPHP14 gene was successfully cloned and EtPHP14 recombinant protein (rEtPHP14) was expressed in Escherichia coli expression system. rEtPHP14 binding was identified on the surface of chicken DCs by Immunofluorescence assay. DC phenotypes were evaluated by flow cytometry and results indicated that MHCII, CD80, CD86, CD1.1 and CD11c were up-modulated in DCs following rEtPHP14 treatment. RT-qPCR showed increased transcript levels of DC maturation markers CCL5, CCR7 and CD83 in rEtPHP14-treated DCs. Moreover, transcript profile of genes associated with intracellular signaling pathways that characterize the immunogenic (TLR signaling) or tolerogenic (Wnt signaling) state of DCs revealed that TLR signaling was stimulated and Wnt signaling was inhibited in rEtPHP14-treated DCs. Furthermore, proliferation of T cells and differentiation of CD4+ cells were promoted when rEtPHP14-treated DCs were co-cultured with autologous T cells. DCs incubated with rEtPHP14 alone expressed increased IL-12 and IFN-γ levels while IL-10 and TGF-ß levels remained unaffected. Likewise, similar trend of IFN-γ expression was noted in rEtPHP14 treated DC-T cell coculture, whereas IL-4 expression remained unchanged. These findings indicate that EtPHP14 is an important molecule that can upregulate host immune response, particularly Th1, during host-parasite interaction, suggesting its importance as a novel candidate for coccidiosis vaccine.

Haemonchus contortus dynein light intermediate chain (HcLIC), an essential excretory/secretory protein of Haemonchus contortus, has been shown to have antigenic features. Neverthless, understanding of its immunomodulatory roles on host immune cells remains limited. Herein, HcLIC gene was amplified by polymerase chain reaction (PCR) and cloned in prokaryotic expression vector pET32a. The protein was expressed by IPTG and purified by affinity chromatography using HisTrap™ FF column. The localization of HcLIC in adult H. contortus woms was detected by immunohistochemical analysis. Immunofluorescence assay (IFA) was carried out to test the binding ability of rHcLIC to goat peripheral blood mononuclear cells (PBMCs). Furthermore, the effects of HcLIC on cell migration and cell apoptosis were evaluated when goat PBMCs were co-incubated with rHcLIC protein. The results revealed that rHcLIC was expressed in the cuticle tissues of adult H. contortus. IFA confirmed the binding of HcLIC on the surface of goat PBMCs. Moreover, functional analysis revealed that the interaction between rHcLIC and host immune cells significantly suppressed cell migration, suggesting that parasite might lessen the production of cytokines and chemokines that signal the migration of host immune cells towards infection site. Moreover, rHcLIC treatment improved cell apoptosis efficiency which might lower the immune cells quantity and thereby downregulate host immunity, enabling parasite survival within host. These results suggested that decrease trend of migration along with induction of apoptosis might be an immunosuppressive strategy of H. contortus. Overall, these findings add to our understanding of HcLIC, and the mechanisms involved in H. contortus immune escape during host-parasite interaction.

RESUMEN

The future development of personalized medicine depends on a vast exchange of data from different sources, as well as harmonized integrative analysis of large-scale clinical health and sample data. Computational-modelling approaches play a key role in the analysis of the underlying molecular processes and pathways that characterize human biology, but they also lead to a more profound understanding of the mechanisms and factors that drive diseases; hence, they allow personalized treatment strategies that are guided by central clinical questions. However, despite the growing popularity of computational-modelling approaches in different stakeholder communities, there are still many hurdles to overcome for their clinical routine implementation in the future. Especially the integration of heterogeneous data from multiple sources and types are challenging tasks that require clear guidelines that also have to comply with high ethical and legal standards. Here, we discuss the most relevant computational models for personalized medicine in detail that can be considered as best-practice guidelines for application in clinical care. We define specific challenges and provide applicable guidelines and recommendations for study design, data acquisition, and operation as well as for model validation and clinical translation and other research areas.

Evidence revealed that climate change has a significant impact on grain production in China. Northeast China has abundant agricultural resources which can make the maximum contribution to national food security. This study examines the effects of climate variability and price anomalies on grain yield and land use in Northeast China. The analysis showed that different climate variability phase combinations based on Pacific Decadal Oscillation and North Atlantic Oscillation present variations in signals and different magnitude of effects over the study area. The results revealed that land use by total grain crop negatively responds to the increase in price anomalies in Heilongjiang and Jilin Provinces. To assess the impact of climate change on crop yield model, the yield models under dynamically downscaled regional climate models revealed that climate variables significantly contribute to total grain yields. In the near future, minimum temperature (- 0.26 °C under CanESM2-4.5, - 4.42 °C under HadGEM2-ES), maximum temperature (- 2.82 °C under CanESM2-4.5, - 0.84 under HadGEM2-ES), and precipitation (ranged from 3.59 to 11.10%) positively contribute to total grain yields under both models. Overall, analysis showed that climate change has a significant contribution to grain production. In conclusion, the implications for future research and policymakers have been addressed. Particularly, the importance of considering regional differences in adaptation planning in agricultural regions was also considered.

Early diagnosis of mammary gland tumors is a challenging task in animals, especially in unspayed dogs. Hence, this study investigated the role of microsatellite instability (MSI), MMR gene mRNA transcript levels and SNPs of MMR genes in canine mammary gland tumors (CMT). A total of 77 microsatellite (MS) markers in 23 primary CMT were selected from four breeds of dogs. The results revealed that 11 out of 77 MS markers were unstable and showed MSI in all the tumors (at least at one locus), while the other markers were stable. Compared to the other markers, the ABC9TETRA, MEPIA, 9A5, SCNA11 and FJL25 markers showed higher frequencies of instability. All CMT demonstrated MSI, with eight tumors presenting MSI-H. The RT-qPCR results revealed significant upregulation of the mRNA levels of cMSH3, cMLH1, and cPMSI, but downregulation of cMSH2 compared to the levels in the control group. Moreover, single nucleotide polymorphisms (SNPs) were observed in the cMSH2 gene in four exons, i.e., 2, 6, 15, and 16. In conclusion, MSI, overexpression of MMR genes and SNPs in the MMR gene are associated with CMT and could be served as diagnostic biomarkers for CMT in the future.

BACKGROUND: The objectives of the study were to: 1) determine the frequency of incidental malignancy in unsuspected/grossly normal looking gall bladders; 2) determine the frequency of malignancy in suspected/grossly abnormal looking gall bladders. MATERIALS AND METHODS: This prospective, cross sectional study was carried out at a tertiary care hospital in Pakistan, during a four year period (Jan 2009-dec2012). All the cholecystectomy cases performed for gallstone diseases were examined initially by a surgeon and later on by a pathologist for macroscopic abnormalities and accordingly assigned to one of the three categories i.e. grossly normal, suspicious, abnormal/malignant. Frequency of incidental carcinoma in these categories was observed after receiving the final histopathology report. RESULTS: A total of 426 patients underwent cholecystectomy for cholelithiasis, with a 1:4 male: female ratio. Mean age of the patients was 45 years with a range of 17-80 years. The frequency of incidental gallbladder carcinoma was found to be 0.70 %(n=3). All the cases of gallbladder carcinoma were associated with some macroscopic abnormality. Not a single case of incidental carcinoma gallbladder was diagnosed in 383 'macroscopically normal looking' gallbladders. CONCLUSIONS: Incidental finding of gall bladder cancer was not observed in any of macroscopically normal looking gall bladders and all the cases reported as carcinoma gallbladder had some gross abnormality that made them suspicious. We suggest histopathologic examination of only those gall bladders with some gross abnormality.

RESUMEN

In several advanced fields like control engineering, computer science, fuzzy automata, finite state machine, and error correcting codes, the use of fuzzified algebraic structures especially ordered semigroups plays a central role. In this paper, we introduced a new and advanced generalization of fuzzy generalized bi-ideals of ordered semigroups. These new concepts are supported by suitable examples. These new notions are the generalizations of ordinary fuzzy generalized bi-ideals of ordered semigroups. Several fundamental theorems of ordered semigroups are investigated by the properties of these newly defined fuzzy generalized bi-ideals. Further, using level sets, ordinary fuzzy generalized bi-ideals are linked with these newly defined ideals which is the most significant part of this paper.

RESUMEN

PURPOSE: To investigate the use of the whole-breast sound speed measurement as a marker of breast density (BD), a known risk factor for breast cancer. METHODS: As part of an ongoing study of breast cancer detection, 249 patients were scanned with a clinical prototype that operates on the principles of ultrasound tomography. Typically, 40-100 sound speed tomograms were reconstructed from the scan data, corresponding to the entire volume of the breast of each patient. The data were used to estimate the volume averaged sound speed (VASS) of the breast for each patient. The corresponding mammograms were used to calculate mammographic percent density (MPD) using CUMULUS software. Film mammograms were available for 164 patients while 85 digital mammograms were available for the remaining patients. Standard statistical techniques were used to determine associations of breast sound speed with a variety of mammographic measures such as percent density, area of dense tissue, and area of nondense tissue. Furthermore, associations of breast sound speed with continuous variables such as age and weight and dichotomous variables such as parity and menopausal status were also assessed. RESULTS: VASS was found to be significantly associated with MPD. The Spearman correlation coefficient (r(s)) between VASS and MPD was found to be 0.77 and 0.71 for film and digital mammography, respectively. VASS was positively correlated with dense areas by mammography, both digital (r(s) = 0.46) and film (r(s) = 0.56). VASS was negatively associated with nondense area by mammography, both digital (r(s) = -0.58) and film (r(s) = -0.63). BD by all methods was less in postmenopausal than in premenopausal women. The MPD was lower in the postmenopausal group (by 6.6%, p < 0.08, for the digital group and 7.73%, p < 0.007, for the film group). The VASS was also lower in the postmenopausal group (by 15 m∕s, p < 0.001 for the digital group and 8 m∕s, p < 0.08, for the film group). The association of MPD with age was characterized with r(s) = -0.06 (p < 0.6) for digital mammography and r(s) = -0.53 (p < 0.002) for film mammography. For weight, the MPD associations were characterized by r(s) = -0.53 (p < 0.0001) for digital mammography and -0.38 (p < 0.0001) for film mammography. The association of VASS with age was r(s) = -0.33 (p < 0.002) for the digital group and -0.17 (p < 0.03) for the film group. For weight, the relationship was characterized with r(s) = -0.45 (p < 0.001) for the digital group and -0.37 (p < 0.0001) for the film group. CONCLUSIONS: The association between VASS and MPD is strong for both film and digital mammography, suggesting that VASS is a viable measure of breast density. This result sets the stage for future work that will focus on directly testing the association of VASS with breast cancer risk.

OBJECTIVE: To describe the acute and late complications of organophosphate (OP) poisoning. STUDY DESIGN: Case series. PLACE AND DURATION OF STUDY: Medicine Department at Peoples Medical College Hospital, Nawabshah, from June 2008 to December 2009. METHODOLOGY: A total of 300 patients with organophosphate poisoning admitted to the Medical ICU were included. Baseline investigations included blood complete picture, urea, creatinine, arterial blood gas values, and serum cholinesterase levels. Data was retrieved from the files on a structured proforma. Studied variables included gender, mode of exposure, acute (occuring within 4 weeks) and delayed (occuring after 4 weeks onwards) complications RESULTS: There were 50 (16.66%) males and 250 (83.33%) females with ratio of 1:5. Two hundred and forty eight (82.6%) had ingested while 18 (6%) had inhaled the poison. Acute complications included fits in 50 (16.66) bradycardia in 30 (10%) and hyperglycemia in 15 (5%) patients. Delayed complications (after 4 weeks and later) included monoplegia and mild sensory loss of lower limbs in 4 (2.66) and paraplegia and weakness of upper limbs in 2 (0.66%) patients each. A total of 50 patients died due to different complications in acute period making a mortality rate of 16.66%. CONCLUSION: Frequency of acute organophosphate (OP) poisoning complications is much higher and related with high mortality and morbidity and where as late complications are less frequent and less life threatening.

OBJECTIVE: To assess the reasons for paradoxical increase in cases of meningitis in the summer of the year 2001. DESIGN: An observational study. PLACE AND DURATION OF STUDY: Medical Unit III, Peoples Medical College Hospital, Nawabshah, Pakistan, from 1st January, 2001 to 31st December, 2001. SUBJECTS AND METHODS: A total number of 229 cases of meningitis were admitted throughout the year with typical clinical features or CSF findings. Maximum number (179) was noted from April to July, which was alarming. The reason for this change was assessed in these cases. RESULTS: One hundred and seventy-nine cases of meningitis presented from 1st April to 31st July 2001. They were predominantly males n= 107 (59.7%) with age range of 15 to 64 years. Presenting symptoms were fever and headache in all and altered sensorium in 160 (89%) cases with positive signs of meningeal irritation in 175 (97.8%). Cerebrospinal fluid examination revealed altered biochemistry and neutrophil leucocytosis in all cases. Organism was identified in 139 (77.65%) cases on gram staining. Culture was done only in 61 (34.07%). Predisposing factors were smoking in 93 (52%) and diabetes in 17 (9.5%) cases. Five female patients were pregnant. CONCLUSION: Common organisms detected were gram positive diplococci (Streptococcus pneumoniae) in 83 (59.7%), gram negative diplococci (Nisseria meningitides) in 51 (36.7%) and Haemophilus influenzae in 5 (3.6%). As the frequency of isolates were approximately same as in other seasons, the increased in meningitis cases during summer was probably due to hot and dry weather causing low absolute humidity.
