RESUMEN

OBJECTIVE: To evaluate the student's perspective about educational and training efficacy of workplace based learning (WBL) during surgical clerkship in Orthopaedics amongst 4th and 5th year medical students. STUDY DESIGN: Cross-sectional study. Place and Duration of the Study: Department of General Surgery, Shifa Tameer-e-Millat University, Islamabad, from September 2020 to December 2021. METHODOLOGY: The inclusion criteria was fourth and final year MBBS students who had completed their orthopaedic surgery mini-clerkship. The exclusion criteria was medical students who did not attend the orthopaedics clerkship in the study period. The research instrument was a questionnaire based on the principles of experiential based learning. The participants were sent an online questionnaire as well as a consent form through e-mail. The responses were recorded and analysed for descriptive statistics. RESULTS: From the target group, 140 responses were received, 94 students (67%) expressed that workplace environment was conducive to learning and 98 (70%) agreed that their diagnostic and management skills further developed following exposure to the orthopaedic workplace. A short duration of 2 weeks served as a hindrance with 53 (38%) of students expressing that they were not able to effectively inculcate all concepts within that limited time-frame. Moreover, 52 (37.5%) students described limited patient's clerking time. CONCLUSION: Though majority of students were satisfied with teaching and learning strategies, design and implementation of the curriculum at the orthopaedics department; yet, there were significant limitations requiring further evaluation and cooperation by both students and faculty in order to establish ecosystem focusing on experiential learning. OBJECTIVE: To explore if a positive association existed between breast cancer and increased breast density. METHODS: The retrospective cross-sectional study was conducted at Shifa International Hospital, Islamabad and comprised data from July 10, 2018, to July 10, 2020, of all patients who underwent mammography for screening or diagnostic purposes. Data was collected by reviewing patients' charts, and was divided into diagnostic group A and screening group B according to mammography target. Breast Imaging Reporting and Data System category was also noted. Data was analysed using SPSS 21. RESULTS: Of the 1,035 women with mean age 46.8±2.5 years (range: 35-82 years), 928(89.7%) were in group A and 107(10.3%) were in group B Prevalent breast densities overall were category A 67(6.3%), B 349(33.7%), C 530(51.2%) and D 89(8.5%). In group A, a lump was detected in 542(58.4%) patients. Of them, 367(67.7%) lesions were malignant and 175(32.3%) were benign. Breast density and malignant tumours had significant association (p<0.05). CONCLUSIONS: Mammographic breast density wsa found to have a significant association with breast cancer.

OBJECTIVE: To determine the causes of interventions in patients with acute limb ischemia (ALI), the time lapse of presentation, and the outcome. STUDY DESIGN: An observational study. METHODOLOGY: All delayed acute limb ischemia cases (presenting later than 6 hours after the onset of symptoms) were included in the study. Cases of ALI secondary to accidental trauma were excluded except those of iatrogenic trauma like patients with intravenous drug abuse and intra-arterial accidental drug infiltration. Patients' demographic data, clinical history, aetiology, examination findings, and treatment data; including type of surgery, level of amputation, adjunctive treatment were recorded. RESULTS: Total number of delayed ALI cases was 147. Mean age was 59.52 ±17.77 years. Seventy-five (51%) were females while 72(49%) were males. The right lower limb was involved in 56(38%) cases. A hundred (68%) thromboembolectomies were successful and limbs were saved, while 19(12.9%) had failure after the procedure. Three (2%) patients expired within 24 hours of thromboembolectomy. Twenty-five (17%) had frank gangrene at presentation and ended up in amputations while 122 (82.9%) had questionable viability and underwent limb salvage procedures. CONCLUSION: Delayed presentation of ALI is very common; timely management with effective thromboembolectomies can save limbs in most of the patients. Basilic vein transposition (BVT) is the preferred permanent haemodialysis access due to better patency and lower infection rates compared to synthetic grafts. The outcomes of BVT cases, performed at Shifa International Hospital, Islamabad, from March 2006 to June 2018, were ambispectively investigated. The primary patency of the fistula was assessed immediately after surgery, at 24 hours, at 7-14 days, at 6-8 weeks and then at 3-6 months. A total of 160 patients were included in the study, out of which 83 (51.87%) were males while 77 (48.12%) were females. Of the total 160 patients, 119 (74.4%) underwent one stage BVT, while 41 (25.6%) underwent two stage BVT. One hundred and thirty-five (84.4%) procedures were successful and survived while in 25 (15.6%) cases it failed. Mean basilic vein diameter was 2.712±0.772 mm. Overall, 10(6.3%) patients had bleeding, 15(9.4%) fistulae thrombosed, 6(3.8%) had steal syndrome and only 1 (0.6%) patient developed pseudo aneurysm. We conclude that BVT is a feasible technique with very good patency rate especially for those patients who have multiple forearm AVF surgeries.

Kimura's disease is an uncommon chronic condition that often masquerades as a malignancy. It is reported predominantly in males of South-East Asia in their second and third decade of life. Most of the patients present with increased eosinophilia, follicular hyperplasia and high levels of IgE circulating in their blood. In this case, we report a patient of 29 years age who presented with complaints of left parotid and left post-auricular swellings. It was painless and growing gradually in size over the period of 8 years. It was asymptomatic during the whole period of the disease. Patient was diagnosed on histology and subsequent immunohistochemistry reports. Treatment in this patient included parotidectomy and lymph node dissection. It was followed by local radiation to prevent recurrence. It can be concluded that Kimura's disease may vary in its presentation but a few features remain characteristic. No reports of malignant change have been reported as of yet making its prognosis quite favorable for the patient. Surgery remains the choice of treatment but for more effective approach, it should be followed by radiation.

Laparoscopic cholecystectomy is one of the most common procedures done worldwide. Post-surgical site infections are common, yet there are occurrences of uncommon complications, including port site tuberculosis (TB). We report a case of a 62-year-old man who was the victim of post-surgical site infection of port sites caused probably by improper sterilization. The patient lacked any common symptoms of tuberculosis and his initial lab investigations were not affirmative. A biopsy depicting the growth of multiple epithelioid granulomas finally led to the diagnosis of port site tuberculosis. The patient was treated by incision and drainage followed by anti-tubercular therapy. This treatment regime showed complete resolution of disease on follow-ups. Such cases require multidisciplinary team (Surgery, Pathology and Infectious disease department in our case) inputs for prompt diagnosis and treatment.

Extra-cranial arteriovenous malformations of head and neck are rare and may present with acute haemorrhage. Their management varies and requires a multidisciplinary approach. Intra-arterial embolisation alone or followed by surgery is recommended. We present here a case of a huge pulsatile lesion extending from the neck to the temporo-parietal region with the main feeding vessel being the right external carotid artery. It was treated with surgical resection only as the feeding artery was very tortuous making selective embolization of artery impossible. The excision of the portion of the artery in the neck was carried out a week later. The patient recovered smoothly except for transient facial nerve palsy.

Mucinous cystadenoma is a rare tumour of the appendix. It accounts for only 0.4% of the gastrointestinal tract malignancies and is reported rarely in the literature. Therefore, surgical management is not yet established. Here we report the case of a 65-year-old female who presented with a dragging sensation and a feeling of a mass in the right iliac fossa. Her computed tomography (CT) suggested formation of an abscess in the parietal peritoneum. She was scheduled for laparotomy and upon exploration, a mass was found arising from the tip of the retroperitoneal appendix. The whole of the appendix was studded with mucoid material. Right hemicolectomy was performed and histopathology of the appendix showed mucinous cystadenoma with no evidence of malignant changes. The recovery was uneventful and the patient was discharged on the fourth post-operative day. The unusual presentation of retroperitoneal pseudomyxoma without any intraperitoneal pathology prompted us to report this case.

BACKGROUND: Wilson's disease is a genetically transmitted disease and has a variety of clinical manifestations. We evaluated the various clinical and biochemical presentations of Wilson's disease (WD) at different ages. METHODS: This cross-sectional study was conducted in Shifa international hospital and Shifa Foundation Falahi Clinic (SFFC), Islamabad. Data from Jan 2010 to Dec 2018 was retrieved from hospital medical record on a structured proforma. All patients who had twenty-four hours urinary copper level of ≥100 mcg/day were included in the study. Their presenting symptoms, clinical signs and lab investigations were noted. RESULTS: Mean age was 13±4.588 years. Male to female ratio was 1.5:1. Hepatic disease was seen in 35 (68.6%) patients mainly in <10 yrs age group. Pure neurological Wilson's was seen in 14 (27.45%) cases, which were >10 years of age while 18(35.3%) had hepato-neurological manifestations. Keyser Fleischer rings were present in 26 (51%) of total patients and 14 (100%) of neurological cases. Hepatic transaminases were elevated in 36 (70 %) patients. Low serum cerruloplasmin was seen in 37 (72.5%) cases. Mean value of haemoglobin was 10.38±2.772. Mean 24 hours urinary copper was 597.6±605.446. Consanguinity was seen among 33 (64.7%) families. Family history of WD was positive in 21 (41.2%) patients. CONCLUSIONS: Hepatic form of WD is more common, yet neurological presentation is seen in patients >10 years of age.

OBJECTIVE: End-stage renal disease patients with vein diameter of ≤2.2 mm can undergo autogenous arteriovenous fistula (AVF) formation with the acceptable results. METHODS: This observational retrospective study of prospectively collected data analyzed end-stage renal disease patients with a vein diameter of ≤ 2.2 mm, who underwent AVF formation at Shifa International Hospital Islamabad from January 2009 to December 2017. The fistulae were observed for immediate success and maturity at 3 months. The chi-square test was used to determine the effect of vein diameter on final maturity. All data were analyzed using SPSS. RESULTS: The total number of patients with vein diameter of ≤2.2 mm was 38, with a mean age of 46.76 ± 12.790 years. Vein diameters ranged from 1.6 to 2.2 mm. Immediate success was observed in 35 (92.1%) cases. Veins of 31 (81.6%) patients showing maturity at 3 months and were used for hemodialysis. The overall success rate for the small caliber veins was 82%. CONCLUSION: Although end-stage renal disease patients present late with very small diameter veins, these veins should still be accommodated for permanent vascular access, because their maturity rates are still acceptable, even though these are lower than those of patients with adequate sized veins.

BACKGROUND: Gall stone disease (GSD) is one of the commonest gastrointestinal disorders. Causative factors can be non-modifiable as genetics and modifiable like dietary habits. High-fat diet and high BMI are well known contributing factors world-over. Local and regional data is deficient about these factors. Moreover, Indo-Pakistani dietary patterns are very different from the western population. This study was conducted to see the association of high BMI and diet with GSD in our population. MATERIALS AND METHODS: This case-control study was conducted in Shifa International Hospital, Islamabad, from 2012 to 2017. We included all female patients above 25 years of age with symptomatic gall stones on ultrasonography as cases and all females above 25 years of age with no gall stones on ultrasonography (done for any reason) as controls. The patients with gall stones having some other clear predisposing cause for gallstone formation, e.g., stoma, sudden weight loss, etc. were excluded. BMI, demographic details, dietary habits, and clinical profile of cases and controls were recorded. Data were analyzed by SPSS v 21. p-values were calculated. RESULTS: A total of 396 patients were included in this study, with 103 cases and 293 controls. The mean BMI of GSD cases was 27.576±5.753, while controls had a mean BMI of 25.638±7.008 (p=0.08). About 26.4% of cases had an average fat consumption greater than 100g/day as compared to 11.9% controls (p=0.035). The average red meat intake per week was 222g among cases while 210g among controls (p=0.001). The average intake of fried food per week was 270g among controls and 250g among cases (p=0.012). The average intake of bakery items per week was 240g among cases and 210g among controls (p=0.038). CONCLUSION: Gall stone disease is significantly related to high BMI and high dietary fat and meat intake in our population.

OBJECTIVE: To assess frequency of complications of total thyroidectomy for benign thyroid diseases. METHODS: The retrospective descriptive study was conducted at Shifa College of Medicine, Islamabad, Pakistan, and comprised patient data from March 2016 to March 2018 of all patients who had undergone total thyroidectomy at the centre. Operative findings and post-op complications were documented in a self-generated proforma. Followup visits documented in the charts were also reviewed. Patients who did not keep the follow-up appointment were interviewed over the phone. Data was analysed using SPSS 20. RESULTS: Of the 96 patients, 78(81.2%) were female and 18(18.7%) were male. The overall mean age was 32.9 } 9.84 years. Hypocalcaemia was the most common complication in 4(4.1%) cases, transient in 3(3.1%) and permanent in only 1(1.04%) case. Haemorrhage occurred in 1(1.04) patient, and recurrent laryngeal nerve injury in 2(2.08%). . CONCLUSIONS: Simple multi-nodular goitre was effectively treated with total thyroidectomy with minor complications.

INTRODUCTION: Total thyroidectomy has been considered to be a treatment of choice for thyroid malignancies. It has recently gained popularity as gold standard for benign thyroid disorders requiring surgical treatment. Because of higher number of complications associated with total thyroidectomy, it is still considered an adventurous endeavor. Most important improvements in thyroid surgery include laparoscopic thyroidectomy, energy based devices (EBD) like Harmonic Focus® and LigaSure® for dissection and hemostasis, intraoperative neuromonitoring, and parathyroid hormone (PTH) assay technology. AIM OF STUDY: Recent studies have suggested that despite lesser complication rates with LigaSure dissection method in total thyroidectomy, it is associated with prolonged operation time. Aim of our study was to compare conventional suture tie technique and LigaSure thyroidectomy, in terms of perioperative complications including bleeding, recurrent laryngeal nerve (RLN) injury, postoperative hematoma, hypocalcemia, operation time, and cost-effectiveness.  Results: This is a retrospective descriptive study done at Shifa Internationall Hospital/Shifa College of Medicine/Shifa Tameer e' Millat University, Islamabad, Pakistan from 1st of June 2016 to 1st of June 2018. One hundred and two (102) patients who underwent surgical treatment for benign thyroid diseases were included in the study. Study was done after approval from ethical committee of Shifa International Hospital/Shifa Tameer e' Millat University. Group A included patients in whom LigaSure was used for hemostasis and dissection during total thyroidectomy. Group B included patients who underwent total thyroidectomy by traditional clamp, tie, and electrocautery method for hemostasis and dissection. Frequency of female patients in group A was 45 (88.2%) and in group B was 41 (80.3%). In group A frequency of male patients was 9 (17.6%) and in group B it was 7 (13.7%). Mean operative time in group A was significantly lower (92 ± 9.61 min) than group B (123 ± 7.96 min). Mean intraoperative blood loss in group A was estimated to be 51.73 ± 5.65 mL and 139.42 ± 7.31 mL in group B. Transient hypocalcemia was the most common complication in both the groups. In group A 6.8% (n=7) patients developed transient postoperative hypocalcemia. Three patients in group B (2.9%) had transient hypocalcemia. CONCLUSION: LigaSure was significantly advantageous over conventional technique in reducing risk of complications and operation time as well as perioperative and postoperative blood loss. The reduction of operative times resulted in decreased operating room occupancy costs but the overall cost of surgery was significantly higher in LigaSure group.

Introduction An important aspect of a modern academic curriculum is assessment, which can be clinical and written. Written assessment includes both multiple-choice questions (MCQs) and short answer questions (SAQs). Debate continues as to which is more reliable. It is important to assess the correlation between the two different formats of written assessments, especially in the clinical subjects as they are different from the basic science subjects. Moreover, data are lacking in the correlation of the two formats of the written assessment in the clinical subjects. Therefore, we conducted this study to see the correlation between MCQs and SAQs in the end-of-clerkship examinations for final-year medical students. Materials and methods The end-of-clerkship written assessment results of the four disciplines of medicine, surgery, gynecology, and pediatrics were included. This was a retrospective correlational analytical study conducted at Shifa Tameer-e-Millat University, Islamabad, from 2013 to 2017. Data were analyzed using IBM SPSS Statistics for Windows, version 23.0 (IBM Corp., Armonk, NY); mean, standard deviation, Pearson coefficient, and p values were calculated both for MCQs and SAQs. Results A total of 481 students were involved in our study. The mean percentage scores of MCQs and SAQs in medicine were the most similar, and scores in obstetrics and gynecology had the most disparity. As compared to MCQs, the wider standard deviations were found in SAQs. Pearson correlations were 0.49, 0.47, 0.23, and 0.38 for medicine, surgery, gynecology, and pediatrics, respectively. Conclusion While we found mild to moderate significant correlation between MCQs and SAQs for final-year medical students, further investigations are required to explore the correlation and enhance the validity of our written assessments.
