RESUMEN

With the purpose of increase of efficiency of postoperative pain relief in maxillofacial surgery it is offered and for 20 years was used a method of prolonged conductor blockade PCB) of the second and the third branches of the trigeminal nerve by means of a catheter. The comparative analysis with traditional at 86 patients anesthesia of narcotic analgesics was performed. Questioning, measurement of a blood pressure, oxigemometry, spirography were used, pneumotachometry, parameters of the acid-basic condition of blood, the maintenance of glucose in blood, temperature of a skin in the field of a chin, small es, Cyrillicomplications on the part of easy and other parameters were used. We also determined need for the medicines appointed for anesthesia in the postoperative period. Results of the study and experience of successful application of the method on 576 patients have shown, that efficiency of the was 91.3%, PCB of branches of a trigeminal nerve was worthy alternative to opioid analgesics.

RESUMEN

The study of 6 groups of patients with an Angle Class II, Division I malocclusions different in age and a method of treatment was made. Mechanisms of correction of an Angle Class II, Division I malocclusion which are typical for each group were determined. The knowledge of mechanisms of correction allows to plan stages of treatment from early primary up to a permanent dentition.

RESUMEN

Epidemiological research of partial secondary adentia was done by means of telephone interviewing the population of city of Samara (1104 persons are interrogated). Prevalence and intensity of the disease in various age groups, and also average indices for all adult population are established. The share of the persons requiring for dental prosthetics among patients with defects of dental lines is determined. The same method investigates awareness of the population about dental implantation, and also the estimation is given to public opinion on treatment with the use of implants.

RESUMEN

Based on the results of statistical analyses, the authors validate the need in development of a new optimal design of implants for replacement of removed mandibular molars. Two-cog (P-shaped) dental implants are suggested, which have been patented. Study of their biomechanical characteristics by digital experiments using three-dimensional final element simulation demonstrated the efficiency of this design of implants, particularly for direct and early implantation and confirmed the adequacy and validity of the two-cogs implants.

RESUMEN

Immediate and early dental implantation helps timely restore the functional and cosmetic intactness of the maxillodental system. Seventy-four patients were operated on by these methods and 116 implants were inserted. The results were evaluated by common clinical, special computer-aided x-ray methods, and biopsy. Comparative studies proved that allogenic lyophilized dura mater membrane promoted recovery of the alveolar bone tissue structure and compactness, improved the implant osteointegration, and reduced the incidence of unfavorable outcomes.

RESUMEN

Investigation of the reparative histogenesis in patients with zygomatic bone fractures suffered in different periods in the past included histologic study of the tissue specimens taken from fracture sites during surgical intervention, studies of the osteogenesis, topographic-anatomic position of bone fragments, their mobility and configuration, and clinical and pathomorphologic changes in the soft tissues adjacent to the fracture site. Specific features of the reparative osteogenesis of the facial skull flat bones and changes in the soft tissues adjacent to the fracture site were detected.

RESUMEN

A new functional classification of muscles, determining movements of the mandible is presented. This classification reflects anatomo-topographic and histogenetic peculiarities of the muscles in question. Their functional differentiation in phylogenesis is motivated, coming from specific tasks they perform. Two groups of muscles of direct and mediated action on the mandible are distinguished. The groups are divided into functional subgroups. Mathematical correlations between various subgroups of the muscles at normal functioning of the mandible are presented. This makes possible to perform calculations of the acting muscle forces at planning reconstructive operative interventions in the maxillofacial area.

RESUMEN

Studies of bioelectrical activity of masticatory muscles in patients with phlegmons of the infratemporal space revealed that limited opening of the mouth in such patients is not related to increased tone of the masticatory muscles and that the pain syndrome is accompanied by tonic tension of the muscles. A new functionally based method for opening the phlegmons of the infratemporal space is suggested.
