RESUMEN

Se tiene presente el principio martiano de que "Educar es depositar en cada hombre toda la obra humana, es ponerlo a nivel de su tiempo, es preparar al hombre para la vida", por eso, esta semblanza es un homenaje a la ilustre académica Alicia Evarista Fernández Hernández, quien con 12 años edad comenzó su labor como educadora al incorporarse a la Campaña de Alfabetización y a otras tareas asignadas por la Revolución; martiana ferviente, educadora por excelencia, ha transmitido durante más de cuatro décadas sus experiencias y sus doctrinas de amor y sabiduría a distintas generaciones de estudiantes de Enfermería, Medicina, profesores, profesionales de la salud de Cuba y otros pueblos del mundo. Es objetivo de esta semblanza referir los aspectos más relevantes de su magistral obra educativa, académica y revolucionaria, apoyados en testimonios, revisión de documentos, entrevistas a familiares y compañeros, que, sin dudas, describen de modo fehaciente su trayectoria como estudiante, enfermera, profesora, académica e investigadora. Alicia Fernández constituye un referente teórico-práctico, ejemplo de dedicación, altruismo y un modelo de conducta a seguir por las nuevas generaciones de galenos, enfermeros, profesores y otros profesionales de la salud que se forman integralmente en las universidades de las Ciencias Médicas cubanas(AU)


We keep in mind Marti's principle that "to educate is to deposit all human work in each man, to put him at the level of his time, and to prepare him for life"; therefore, this semblance is a tribute to the illustrious academician Alicia Evarista Fernández Hernández who began her work as an educator at the age of twelve by joining the literacy campaign and other tasks assigned by the Revolution. She has been a passionate defender of Marti's ideas and an educator par excellence who has passed her experiences and doctrines of love and wisdom to different generations of nursing and medical students, professors and other health professionals from Cuba and other peoples of the world for more than four decades. The objective of this semblance is to present the most relevant aspects of her masterful educational, academic and revolutionary work supported by evidence, document reviews, and interviews with family members and colleagues which undoubtedly describe her career as a student, nurse, professor, academician, and researcher. Alicia Fernández is a theoretical and practical referent, an example of dedication and altruism, and a behavior model to the new generations of doctors, nurses, professors and other health professionals who are fully trained in the Cuban universities of Medical Sciences(AU)

RESUMEN

Cow's milk allergy (CMA) is an immune-based disease that has become an increasing problem. The diagnosis and management of CMA varies from one clinical setting to another and represents a challenge in pediatric practice. In addition, because nonallergic food reactions can be confused with CMA symptoms, there is an overdiagnosis of the disease. In response to these situations, pediatric specialties from recognized institutions throughout Latin America decided to develop a clinical guideline for diagnosis and management of cow's milk allergy. These guidelines include definitions, epidemiology, pathophysiology overview, clinical and evidencebased recommendations for the diagnosis and treatment of CMA. They also include prevention and prognosis sections and identify gaps in the current knowledge to be addressed through future research.

OBJECTIVE: Regulators of adipose tissue hormones remain incompletely understood, but may include sex hormones. As adipose tissue hormones have been shown to contribute to numerous metabolic and cardiovascular disorders, understanding their regulation in midlife women is of clinical importance. Therefore, we assessed the associations between testosterone (T) and sex hormone binding globulin (SHBG) with leptin, high molecular weight (HMW) adiponectin, and the soluble form of the leptin receptor (sOB-R) in healthy midlife women. DESIGN AND METHODS: Cross-sectional analyses were performed using data from 1,881 midlife women (average age 52.6 (±2.7) years) attending the sixth Annual follow-up visit of the multiethnic Study of Women's Health Across the Nation. RESULTS: T was weakly negatively associated with both HMW adiponectin and sOB-R (r = -0.12 and r = -0.10, respectively; P < 0.001 for both), and positively associated with leptin (r = 0.17; P < 0.001). SHBG was more strongly and positively associated with both HMW adiponectin and sOB-R (r = 0.29 and r = 0.24, respectively; P < 0.001 for both), and more strongly and negatively associated with leptin (r = -0.27; P < 0.001). Adjustment for fat mass, insulin resistance, or waist circumference only partially diminished associations with HMW adiponectin and sOB-R, but attenuated associations with leptin. In conclusion, in these midlife women, lower SHBG values, and to a lesser extent, higher T levels, were associated with lower, or less favorable, levels of adiponectin and sOB-R, independent of fat mass. CONCLUSIONS: These data suggest that variation in these adipose hormones resulting from lower SHBG levels, and possibly, though less likely, greater androgenicity, may contribute to susceptibility for metabolic and cardiovascular outcomes during midlife in women.

D-003 is a mixture of very long chain aliphatic acids purified from sugar cane wax, wherein octacosanoic acid represents the major component. Previous experimental studies have shown that D-003 inhibits platelet aggregation in rodents. Also, its lowers total (TC) and low-density lipoprotein cholesterol (LDL-C) in normocholesterolemic rabbits in a dose-dependent manner and inhibits cholesterol biosynthesis in fibroblast cultures. The present study was performed to investigate the in vitro cytotoxic and genotoxic potential effects of D-003 assessed through two tests: the neutral red (NR) assay and the Ames test. Positive and negative controls were included in each experimental series. Compared with controls, no cytotoxicity was evident after 24 and 72 h of treatment with doses up to 1,000 microg/ml in the NR assay. On the other hand, D-003 (5-5,000 microg/plate) did not increase the frequency of reverse mutations in the Ames test in both alternatives with or without S9 mix metabolic activation and a pre-incubation step. The positive control chemicals included in each experiment, namely, treatment with sodium dodecyl sulphate (SDS) in the NR assay and sodium azide (NaAz), 2-aminofluorene (AF), and dimethylnitrosamine (DMNA) in the Ames test, induced the expected changes, such as a decrease in optical density (OD) values in the NR assay and an increase in the frequency of reverse mutations in the Ames test. The present results indicate that D-003 did not show evidence of cytotoxic or genotoxic potential in tests able to detect the ability of chemicals to disrupt cells (NR assay) or to induce gene mutations (Ames test).

D-003 is a mixture of very long chain aliphatic acids purified from sugar cane wax with cholesterol-lowering effects. The present study was undertaken to investigate the in vivo cytotoxic and genotoxic potential of D-003 using three established assays: bone marrow micronucleus, sperm morphology, and single cell gel electrophoresis (Comet) assay. In a first experimental series, CEN/NMRI mice (6-8 animals per sex per group) were administered D-003 by gastric gavage at 5, 50, or 500 mg/kg for 90 days, then sacrificed 24 hours after the last administration. The effects on bone marrow micronucleus were evaluated only in female mice. D-003 (5-500 mg/kg) did not increase the frequency of micronucleated polychromatic erythrocytes, nor the ratio of polychromatic to normochromatic erythrocytes, compared with the controls. The assessment of the effects on sperm morphology showed that D-003 did not change the sperm count or the frequency of all types of abnormal head shapes, compared with the controls. In a second series, the micronucleus assay was performed in mice of both sexes given 2,000 mg/kg for 6 days. Likewise, in this series, neither cytotoxic nor genotoxic effects were found. Finally, five male Sprague-Dawley rats were treated with D-003 (1,250 mg/kg) by oral gavage for 90 days, and Comet assay on liver cells was performed. No single-strand breaks or alkali-labile site induction on DNA was observed. These results indicate that D-003 does not show evidence of cytotoxic or genotoxic activity on either somatic or germ cells in rodents.

Se describen los casos de maltrato infantil (MI) o violencia intrafamiliar (VIF) identificados durante un año en un consultorio de atención primaria, en las atenciones habituales a niños y embarazadas. Las sospechas de MI y VIF se referían al equipo social del consultorio para entrevista y visita domiciliaria, éstas en conjunto con las psicólogas, descartaban o confirmaban los casos. Según su patología fueron referidos a instituciones especializadas. Se organizaron actividades educativas, en la sala de espera, destinadas a los usuarios del consultorio y se desarrollaron talleres y psicoterapia para los grupos familiares involucrados. Se confirmaron 67 casos. El 41,8 por ciento de las familias eran víctimas de VIF, la mayoría asociada a abuso de alcohol y drogas; hubo maltrato físico en el 62,4 por ciento de los casos, psicológico en 17,9 por ciento; negligencia o abandono en 20,9 por ciento; abuso sexual en 22,4 por ciento. En el 55,2 por ciento se encontró más de un factor de maltrato. Sólo un tercio de los casos fueron detectados por el equipo de salud del consultorio, el resto fue denunciado por algún miembro de la familia de la víctima. Se calcula que 1,67 por ciento de las familias con niños menores de 15 años, inscritas en el consultorio y que acudieron por alguna razón en un año, presentan MI
