Sex does not affect changes in body composition and insulin-like growth factor-I during US army basic combat training. J Strength Cond Res XX(X): 000-000, 2023-Insulin-like growth factor 1 (IGF-I) has been implicated as a biomarker of health and body composition. However, whether changes in body composition are associated with changes in IGF-I is unclear. Therefore, we examined the relationship between body composition changes (i.e., fat mass and lean mass) and total serum IGF-I levels in a large cohort of young men (n = 809) and women (n = 397) attending US Army basic combat training (BCT). We measured body composition using dual energy x-ray absorptiometry and total serum IGF-I levels during week 1 and week 9 of BCT. We found that pre-BCT lean mass (r = 0.0504, p = 0.082) and fat mass (r = 0.0458, p = 0.082) were not associated with pre-BCT IGF-I. Body mass, body mass index, body fat percentage, and fat mass decreased, and lean mass increased during BCT (all p < 0.001). Mean (±SD) IGF-I increased from pre-BCT (176 ± 50 ng·ml-1) to post-BCT (200 ± 50 ng·ml-1, p < 0.001). Inspection of the partial correlations indicated that even when considering the unique contributions of other variables, increases in IGF-I during BCT were associated with both increased lean mass (r = 0.0769, p = 0.023) and increased fat mass (r = 0.1055, p < 0.001) with no sex differences. Taken together, our data suggest that although changes in IGF-I weakly correlated with changes in body composition, IGF-I, in isolation, is not an adequate biomarker for predicting changes in body composition during BCT in US Army trainees.

Peak tibial accelerations (TAs) during running are strongly related to early stance vertical ground reaction forces (GRFs), which are associated with musculoskeletal injury. However, few studies have examined these correlations during walking, and none have evaluated them during walking with loads, a relevant activity for military personnel. Our purpose was to determine the relationships between GRFs and TAs in US Army trainees (n = 649) walking with loads. An inertial measurement unit was attached over their distal antero-medial tibia. Participants walked on an instrumented treadmill at 1.21-1.34 m/s, with a pack loaded with 18.1 kg, for a 3-min warm-up followed by a minimum of 14 strides of data collection. Simple linear regression models were calculated for peak vertical and resultant TAs with vertical and posterior GRF loading rates and peak forces. The strongest relationships were between vertical loading rates and peak vertical TA (R = 0.43-0.50), however the relationships were weaker than has been reported for unloaded walking and running (R > 0.7). All other relationships were trivial to small (R = 0.06-0.27). The weaker relationships for vertical GRFs and TAs may be due to methodological differences between studies, or differences in gait mechanics, such as a longer double-limb support phase in loaded vs. unloaded walking.

In 1981, the US military adopted body fat standards to promote physical readiness and prevent obesity. Separate circumference-based equations were developed for women and men. Both predictive equations were known to underestimate %BF. However, it was not known how well these abdominal circumference-based methods tracked changes in %BF. This study examined the validity of the circumference-based %BF equations for assessing changes in %BF in young adult recruits during Army Basic Combat Training (BCT). Dual-energy X-ray absorptiometry (DXA) and circumference-based measures of %BF were obtained in women (n = 481) and men (n = 926) at the start (pre-BCT) and end (post-BCT) of 8 weeks of BCT. Repeated-measure ANOVAs were used to assess differences between DXA and circumference pre-BCT and for the change during BCT. Pre-BCT, circumferences underestimated %BF relative to DXA, with mean errors of -6.0% ± 4.4% for women and -6.0% ± 3.5% for men (both p < 0.01), and no difference between sexes was observed (p = 0.77). DXA detected a -4.0% ± 2.4% and -3.3% ± 2.8% change in %BF for women and men in response to BCT, respectively (both p < 0.01), whereas circumference estimates of %BF indicated a 0.0% ± 3.3% (p = 0.86) change in women and a -2.2% ± 3.3% (p < 0.01) change in men (sex difference by technique p < 0.01). In conclusion, circumference-based measures underestimated %BF at the start of BCT in both sexes as compared to DXA. Circumference measures underestimated changes in %BF during BCT in men and did not detect changes in women. These findings suggest that circumference-based %BF metrics may not be an appropriate tool to track changes in body composition during short duration training.

Basic combat training (BCT) is a physically rigorous period at the beginning of a soldier's career that induces bone formation in the tibia. Race and sex are determinants of bone properties in young adults but their influences on changes in bone microarchitecture during BCT are unknown. The purpose of this work was to determine the influence of sex and race on changes in bone microarchitecture during BCT. Bone microarchitecture was assessed at the distal tibia via high-resolution peripheral quantitative computed tomography at the beginning and end of 8 weeks of BCT in a multiracial cohort of trainees (552 female, 1053 male; mean ± standard deviation [SD] age = 20.7 ± 3.7 years) of which 25.4% self-identified as black, 19.5% as race other than black or white (other races combined), and 55.1% as white. We used linear regression models to determine whether changes in bone microarchitecture due to BCT differed by race or sex, after adjusting for age, height, weight, physical activity, and tobacco use. We found that trabecular bone density (Tb.BMD), thickness (Tb.Th), and volume (Tb.BV/TV), as well as cortical BMD (Ct.BMD) and thickness (Ct.Th) increased following BCT in both sexes and across racial groups (+0.32% to +1.87%, all p < 0.01). Compared to males, females had greater increases in Tb.BMD (+1.87% versus +1.40%; p = 0.01) and Tb.Th (+0.87% versus +0.58%; p = 0.02), but smaller increases in Ct.BMD (+0.35% versus +0.61%; p < 0.01). Compared to black trainees, white trainees had greater increases in Tb.Th (+0.82% versus +0.61%; p = 0.03). Other races combined and white trainees had greater increases in Ct.BMD than black trainees (+0.56% and + 0.55% versus +0.32%; both p ≤ 0.01). Changes in distal tibial microarchitecture, consistent with adaptive bone formation, occur in trainees of all races and sexes, with modest differences by sex and race. Published 2023. PURPOSE: To determine whether changes in menstruation develop in female trainees during BCT and whether changes in body mass, body composition and/or physical activity are associated with menstrual interruption during BCT. METHODS: Female trainees grouped according to self-reported menstrual status in the 12 months before BCT as having regular cycles (RC; n = 352) or MD ( n = 97) completed height, body mass, and body composition assessments and questionnaires before and after BCT. Fisher's exact test and Mann-Whitney U test were used to compare between-group differences in categorical and continuous variables, respectively. Among RC trainees, odds ratios were calculated to examine the influence of changes in body mass, lean mass, and fat mass on a trainee's likelihood to miss a period during BCT. RESULTS: There were no differences in race, height, body mass, body mass index, or physical activity history at pre-BCT between RC and MD ( P > 0.05). Overall, 86% of trainees experienced changes to menstruation during BCT. RC were more likely than MD to have at least one period during BCT (81% vs 69%, respectively, P = 0.01). Among RC, gaining more body mass and lean mass and losing less fat mass were associated with increased odds of missing a period during BCT. CONCLUSIONS: These findings demonstrate that most female trainees experience menstrual changes during BCT. Menstrual cycle interruptions do not appear to align with loss of body or fat mass.

ABSTRACT: Spiering, BA, Clark, BC, Schoenfeld, BJ, Foulis, SA, and Pasiakos, SM. Maximizing strength: the stimuli and mediators of strength gains and their application to training and rehabilitation. J Strength Cond Res 37(4): 919-929, 2023-Traditional heavy resistance exercise (RE) training increases maximal strength, a valuable adaptation in many situations. That stated, some populations seek new opportunities for pushing the upper limits of strength gains (e.g., athletes and military personnel). Alternatively, other populations strive to increase or maintain strength but cannot perform heavy RE (e.g., during at-home exercise, during deployment, or after injury or illness). Therefore, the purpose of this narrative review is to (a) identify the known stimuli that trigger gains in strength; (b) identify the known factors that mediate the long-term effectiveness of these stimuli; (c) discuss (and in some cases, speculate on) potential opportunities for maximizing strength gains beyond current limits; and (d) discuss practical applications for increasing or maintaining strength when traditional heavy RE cannot be performed. First, by conceptually deconstructing traditional heavy RE, we identify that strength gains are stimulated through a sequence of events, namely: giving maximal mental effort, leading to maximal neural activation of muscle to produce forceful contractions, involving lifting and lowering movements, training through a full range of motion, and (potentially) inducing muscular metabolic stress. Second, we identify factors that mediate the long-term effectiveness of these RE stimuli, namely: optimizing the dose of RE within a session, beginning each set of RE in a minimally fatigued state, optimizing recovery between training sessions, and (potentially) periodizing the training stimulus over time. Equipped with these insights, we identify potential opportunities for further maximizing strength gains. Finally, we identify opportunities for increasing or maintaining strength when traditional heavy RE cannot be performed.

J Strength Cond Res 37(4): 894-901, 2023-This study measured the impact of occupational experience (i.e., time spent deployed, in military service, and in job and task performance frequency in training, deployment, and study practice) on the physiological (heart rate [HR] and oxygen consumption [VO 2 ]) and perceptual (rate of perceived exertion [RPE]) responses to performance of critical physically demanding tasks (CPDTs). Five CPDTs (road march, build a fighting position, move under fire, evacuate a casualty, and drag a casualty to safety), common to all soldiers, were performed by 237 active duty soldiers. Linear regression models examined the association between measures of experience and physiological and perceptual performance responses to task demands. The level of significance was adjusted for multiple comparisons and set at ρ ≤ 0.0125 for this study. Significant and notable effect sizes included the impact of time spent deployed on the physiological measures of the road march (PostHR F = 24.84, p < 0.0001, ß=-9.65), sandbag fill (PostHR F = 8.26, p = 0.005, ß = -2.83), and sandbag carry (MeanHR F = 7.51, p = 0.007, ß = -1.12; PostHR F = 7.35, p = 0.007, ß = -0.87). For the road march task, there was a nearly 10 bpm decrease in postperformance HR for every year spent deployed. Road march, sandbag fill, and sandbag carry tasks PostHRs were also notably negatively associated with the experience measures of time in their MOS (job and time in military service but not for other physiological and perceptual responses, including VO 2 and RPE. Frequency of task performance in training, deployment, and study practice was not meaningfully associated with experience. The results suggest that increasing task familiarization through on-the-job occupational operational experience may result in greater proficiency and reduced physiological effort.

Exercise can be both anabolic and catabolic for bone tissue. The temporal response of both bone formation and resorption following an acute bout of exercise is not well described. We assayed biochemical markers of bone and calcium metabolism for up to 3 days after military-relevant exercise. In randomized order, male (n = 18) and female (n = 2) Soldiers (means ± SD; 21.2 ± 4.1 years) performed a 60-min bout of load carriage (30% body mass; 22.4 ± 3.7 kg) treadmill exercise (EXER) or a resting control trial (REST). Blood samples were collected following provision of a standardized breakfast before (PRE), after (POST) exercise/rest, 1 h, 2 h, and 4 h into recovery. Fasted samples were also collected at 0630 on EXER and REST and for the next three mornings after EXER. Parathyroid hormone and phosphorus were elevated (208% and 128% of PRE, respectively, P < 0.05), and ionized calcium reduced (88% of PRE, P < 0.05) after EXER. N-terminal propeptide of type 1 collagen was elevated at POST (111% of PRE, P < 0.05), and the resorption marker, C-terminal propeptide of type 1 collagen was elevated at 1 h (153% of PRE, P < 0.05). Osteocalcin was higher than PRE at 1 through 4 h post EXER (119%-120% of PRE, P < 0.05). Sclerostin and Dickkopf-related protein-1 were elevated only at POST (132% and 121% of PRE, respectively, P < 0.05) during EXER. Trivial changes in biomarkers during successive recovery days were observed. These results suggest that 60 min of load carriage exercise elicits transient increases in bone formation and resorption that return to pre-exercise concentrations within 24 h post-exercise.NEW & NOTEWORTHY In this study, we demonstrated evidence for increases in both bone formation and resorption in the first 4 h after a bout of load carriage exercise. However, these changes largely disappear by 24 h after exercise. Acute formation and resorption of bone following exercise may reflect distinct physiological mechanoadaptive responses. Future work is needed to identify ways to promote acute post-exercise bone formation and minimize post-exercise resorption to optimize bone adaptation to exercise.

INTRODUCTION/BACKGROUND: As a proxy for adiposity, body mass index (BMI) provides a practical public health metric to counter obesity-related disease trends. On an individual basis, BMI cannot distinguish fat and lean components of body composition. Further, the relationship between BMI and body composition may be altered in response to physical training. We investigated this dynamic relationship by examining the effect of US Army basic combat training (BCT) on the association between BMI and per cent body fat (%BF). METHODS: BMI and %BF were measured at the beginning (week 1) and end (week 9) of BCT in female (n=504) and male (n=965) trainees. Height and weight were obtained for BMI, and body composition was obtained by dual X-ray absorptiometry. Sensitivity and specificity of BMI-based classification were determined at two BMI thresholds (25 kg/m2 and 27.5 kg/m2). RESULTS: A progressive age-related increase in fat-free mass index (FFMI) was observed, with an inflection point at age 21 years. In soldiers aged 21+, BMI of 25.0 kg/m2 predicted 33% and 29% BF in women and 23% and 20% BF in men and BMI of 27.5 kg/m2 predicted 35% and 31% BF in women and 26% and 22% BF in men, at the start and end of BCT, respectively. Sensitivity and specificity of BMI-based classification of %BF were poor. Soldiers below BMI of 20 kg/m2 had normal instead of markedly reduced %BF, reflecting especially low FFMI. CONCLUSIONS: BCT alters the BMI-%BF relationship, with lower %BF at a given BMI by the end of BCT compared with the beginning, highlighting the unreliability of BMI to try to estimate body composition. The specific BMI threshold of 25.0 kg/m2, defined as 'overweight', is an out-of-date metric for health and performance outcomes. To the extent that %BF reflects physical readiness, these data provide evidence of a fit and capable military force at BMI greater than 25.0 kg/m2.

INTRODUCTION: The U.S. Army Occupational Physical Assessment Test (OPAT) is a pre-enlistment physical employment screening assessment developed to place recruits and soldiers into Military Occupational Specialties (MOSs) based on their physical capabilities in order to optimize performance and limit injury. The OPAT consists of the seated power throw (SPT), strength deadlift (SDL), standing long jump, and interval aerobic run. During the scientific validation of the OPAT, two variants of the SPT and two variants of the SDL were used. Although the OPAT was validated using both variants for each test, U.S. Army scientists and policymakers have received queries regarding how these variants compare to each other. Therefore, the purpose of this study was to compare different variants of the SPT and SDL. MATERIALS AND METHODS: Thirty-two participants (14 male and 18 female) between the ages of 18 and 42 years visited the laboratory on one occasion and performed two variants of the SPT (seated on the ground [the current OPAT standard] versus seated in a chair with a 35 cm seat height) and two variants of the SDL (using a hex-bar [the current OPAT standard] versus using paired dumbbells). Testing order for the different variants was randomized. The protocol was approved by the U.S. Army Medical Research and Development Command Institutional Review Board. RESULTS: Performing the SPT from a chair significantly (P < .05) increased performance when compared to performing the SPT from the ground (5.4 ± 1.3 m versus 5.0 ± 1.4 m, respectively). Values for the two SPT variants were correlated (tau = 0.90). Performing the SDL using the hex-bar significantly increased the maximal weight lifted when compared to performing the SDL using paired dumbbells (86.9 ± 18.4 kg versus 83.1 ± 18.0 kg, respectively). Values for the two SDL variants were correlated (tau = 0.83). CONCLUSIONS: Performing different variants of the SPT and SDL influenced the resulting score. Although these findings do not alter the administration or scoring of the OPAT, they do provide a valuable reference in the event of future inquiries regarding the development of the OPAT.

INTRODUCTION: Musculoskeletal injuries and insufficient sleep are common among U.S. Army Rangers. There has been limited research into whether indices of sleep differ between injured and uninjured Rangers. The purpose of this study was to investigate the association between self-reported sleep and musculoskeletal injury in Rangers. MATERIALS AND METHODS: A total of 82 Army Rangers (male, 25.4 ± 4.0 years) were asked if they currently have any musculoskeletal injuries; completed the Pittsburgh Sleep Quality Index (PSQI), the Insomnia Severity Index (ISI), and the Stanford Sleepiness Scale; and were asked about their average sleep quality/sleep duration over the preceding week. Rangers were then dichotomized into groups, one that reported a current musculoskeletal injury and another that did not. RESULTS: The reported musculoskeletal injury prevalence was 15.9% (n = 13). The Rangers that reported an injury, compared to those that did not, had a significantly higher Global PSQI score (6.7 ± 3.7 versus 4.5 ± 2.7, P = .012) and ISI score (10.9 ± 3.7 versus 7.2 ± 4.1, P = .003), both indicative of poorer sleep. The group reporting an injury rated their average sleep quality over the preceding week significantly lower compared to those that did not report an injury (50.8 ± 17.5 versus 68.9 ± 18.3, P = .001). There was no significant group difference in the average nightly sleep duration (6.1 ± 1.0 hours versus 6.5 ± 0.9 hours, P = .099). CONCLUSION: In this cohort of male Army Rangers, In this cohort of male Army Rangers, those with a musculoskeletal injury reported poorer sleep quality than uninjured Rangers. Sleep duration was not associated with reported injuries; however, both the injured group and uninjured group averaged less than the recommended amounts of sleep. Further investigation into the relationship between musculoskeletal injury and sleep in military personnel is warranted.

BACKGROUND: In 2013 the U.S. Army began developing physical tests to predict a recruit's ability to perform the critical physically demanding tasks (CPDTs) of combat arms jobs not previously open to women. OBJECTIVE: To revalidate 15 CPDTs chosen by subject matter experts (SMEs) and researchers through questionnaires pertaining to task performance frequency, perceived importance, and performance expectations. METHOD: Web-administered job analysis questionnaires were completed by 2,090 soldiers. Seventy-three percent ranged between 25-38 years of age, 66%were staff sergeants or above, and 73%were in service for 7 + years. RESULTS: Overall, the nine SME-endorsed CPDTs were conducted more frequently and rated as more important than the six tasks identified by researchers. Foot march, dragging a casualty to safety, and connecting a tow bar (vehicle transport) were identified as the combined most important, most frequently performed and highly expected CPDTs to be performed. The canonical correlation between task performance frequencies and ratings of task importance across all 15 CPDTs was 0.82 (p < 0.001). Expectations of task completion were strongly associated with more frequent task performance (Cramer's Vs ranged 0.22 to 0.71; all p's < 0.001), but not task importance (only four CPDTs at p < 0.05). CONCLUSION: This study revalidates the value of CPDTs chosen by SMEs and researchers. Soldier readiness should reflect tasks identified by incumbents as important to success (e.g., evacuating a casualty) be trained more often, whereas others classified as frequently performed, but less important (e.g., filling sandbags), be deemphasized while ensuring that standards are met.

ABSTRACT: Maintaining physical performance: the minimal dose of exercise needed to preserve endurance and strength over time, Spiering, BA, Mujika, I, Sharp, MA, and Foulis, SA. J Strength Cond Res 35(5): 1449-1458, 2021-Nearly every physically active person encounters periods in which the time available for exercise is limited (e.g., personal, family, or business conflicts). During such periods, the goal of physical training may be to simply maintain (rather than improve) physical performance. Similarly, certain special populations may desire to maintain performance for prolonged periods, namely athletes (during the competitive season and off-season) and military personnel (during deployment). The primary purpose of this brief, narrative review is to identify the minimal dose of exercise (i.e., frequency, volume, and intensity) needed to maintain physical performance over time. In general populations, endurance performance can be maintained for up to 15 weeks when training frequency is reduced to as little as 2 sessions per week or when exercise volume is reduced by 33-66% (as low as 13-26 minutes per session), as long as exercise intensity (exercising heart rate) is maintained. Strength and muscle size (at least in younger populations) can be maintained for up to 32 weeks with as little as 1 session of strength training per week and 1 set per exercise, as long as exercise intensity (relative load) is maintained; whereas, in older populations, maintaining muscle size may require up to 2 sessions per week and 2-3 sets per exercise, while maintaining exercise intensity. Insufficient data exists to make specific recommendations for athletes or military personnel. Our primary conclusion is that exercise intensity seems to be the key variable for maintaining physical performance over time, despite relatively large reductions in exercise frequency and volume.

INTRODUCTION: U.S. Army Basic Combat Training (BCT) prepares new recruits to meet soldier physical demands. It also serves as a model of physical changes in healthy young nonobese women and men during an intensive 10-week training program without diet restriction. In this prospective observational study, we quantified the changes in lean mass and body fat induced by BCT in a large sample of men and women undergoing the same physical training program. METHODS: Young women (n = 573) and men (n = 1071) meeting Army health and fitness recruitment standards volunteered to provide DXA-derived body composition data at the beginning and end of BCT. RESULTS: During BCT, there was no change in body mass in women and a 1.7-kg loss in men. Relative body fat (%BF) declined by an average of 4.0 ± 2.4 and 3.4 ± 2.8 percentage points (±SD) for women and men, respectively. The greatest predictor of change in %BF during BCT for both sexes was %BF at the beginning of training. Women and men gained an average 2.7 ± 1.6 kg and 1.7 ± 2.0 kg of lean mass during BCT. CONCLUSIONS: Army BCT produced significant effects on body composition despite minimal changes in total body mass. These findings demonstrate the ability of a 10-week sex-integrated physical training program to positively alter body composition profiles of young adults.

OBJECTIVE: To examine the sleep health of incoming Army trainees and how it is impacted during basic combat training (BCT). DESIGN: Prospective. SETTING: BCT site (Fort Jackson, SC). PARTICIPANTS: A total of 1349 trainees (936 = male, 413 = female, 20.73 ± 3.67 years). MEASUREMENTS: Participants completed the Pittsburgh Sleep Quality Index (PSQI), the Morningness/Eveningness Questionnaire, and the Epworth Sleepiness Scale at the start of BCT and a modified PSQI at the end of BCT. RESULTS: At baseline, trainees reported an average sleep duration of 7.65 ± 1.68 hours per night, with 81.8% rating their sleep quality as "Very Good or Fairly Good." The mean reported Morningness/Eveningness Questionnaire score was 50.63 ± 8.11 and the mean Epworth Sleepiness Scale was 8.60 ± 4.02. Reported sleep duration was significantly less during BCT (6.73 ± 0.90 hours) compared to baseline (P< .001). There was no significant difference in the mean PSQI Global score at the end of BCT compared to the start (5.33 ± 3.00 vs. 5.42 ± 2.85, P = .440), however, 6 of the 7 component scores were significantly different (with Sleep Quality, Sleep Duration, and Daytime Dysfunction scores being higher/worse and Sleep Latency, Sleep Efficiency, and Sleep Medication Use scores being lower/improved [all P < .01]). CONCLUSIONS: Army BCT trainees in this large sample reported good sleep health characteristics at entry to training, including achieving recommended sleep amounts (>7 hours per night) and reporting good sleep quality. During BCT, negative changes were observed in reported sleep duration and quality in trainees. Further investigation into the factors contributing to changes in trainees' sleep health during BCT and the implications on subsequent readiness, injury risk, and performance is warranted.

INTRODUCTION: This study compared the relationship between height (HT), body mass (BM), and body mass index (BMI) of female trainees and active duty female soldiers and their performance on simulated common soldiering tasks (CSTs) with high physical demands. METHODS: Female trainees (n = 133) and soldiers (n = 229) completed the following CSTs: sandbag carry, move under fire, casualty drag, casualty evacuation, and road march. Quartiles were created among HT, BM, and BMI by which task performance was compared using ANOVAs with Tukey post hoc comparisons. RESULTS: For both trainees and soldiers, HT, BM, and BMI were positively associated with improved road march, casualty drag, casualty evacuation, and sandbag carry performance. On the move under fire task, only soldier HT was positively associated with improved performance. CONCLUSION: Female trainees and soldiers who are taller and heavier with a higher BMI may demonstrate better performance on CSTs required of all soldiers. In addition to task-specific training, performance of CSTs may be enhanced in tasks requiring strength and power by recruiting and retaining taller and heavier females with a higher BMIs. Allowances should be considered for soldiers and trainees who can successfully perform soldiering tasks with high physical demands despite less desirable anthropometric measurements.

INTRODUCTION: There are many ways to quantify the training loads required to perform soldiering tasks. Although indirect calorimetry may provide the most accurate measures, the equipment can be burdensome and expensive. Simpler measures may provide sufficient data, while being more practical for measuring soldiers in the field. The purpose of this study was to examine the relationship between total relative oxygen uptake (TotalRelVO2) measured by indirect calorimetry during three soldiering tasks, with two field-expedient measures of training load: summated heart rate zone (sumHR) and session rate of perceived exertion (sRPE). MATERIALS AND METHODS: 33 male and 28 female soldiers performed three soldiering tasks while wearing a 32.3-kg fighting load: sandbag fill, sandbag carry, and ammunition can carry. Metabolic measurements were monitored and completion times were recorded (min). TotalRelVO2 (average relative VO2*time) and age-predicted maximal heart rate (220-age) were calculated. SumHR was calculated by multiplying time spent in each of the five heart rate zones by a multiplier factor for each zone (50-59% = 1, 60-69% = 2, 70-79% = 3, 80-89% = 4, and ≥90% = 5). RPE (Borg 6-20 scale) was collected at the end of each task, then sRPE was calculated (RPE*time). Pearson and Spearman correlations were performed to examine the relationship between TotalRelVO2, sumHR and sRPE. Wilcoxon signed rank tests were conducted to determine if there was a difference in median rankings between the three variables for each task. Linear regressions were performed to determine predictability of TotalRelVO2 from sumHR and sRPE. The study was approved by the U.S. Army Research Institute of Environmental Medicine Institutional Review Board. RESULTS: Significant, positive correlations were revealed for all three tasks between TotalRelVO2, sumHR and sRPE (r ≥ 0.67, p ≤ 0.01; rho≥0.74, p ≤ 0.01). Wilcoxon signed rank tests revealed no significant differences in rankings between TotalRelVO2, sumHR and sRPE for all three tasks (p ≥ 0.43). Both sumHR and sRPE are significant predictors of TotalRelVO2 (p ≤ 0.01). CONCLUSIONS: SumHR and sRPE are acceptable alternatives to TotalRelVO2 when attempting to quantify and/or monitor training load during soldiering tasks.

BACKGROUND: Beginning in 2017, successfully passing the four-part pre-enlistment Occupational Physical Assessment Test (OPAT) became a requirement for all U.S. Army recruits. To ensure the test accurately identifies individuals who are qualified for their job, it was necessary to examine classification errors. OBJECTIVE: The objectives were to 1) determine the accuracy of OPAT cut-scores for combat arms Soldiers, and 2) determine which events contribute to the individuals that are misclassified as passing or failing the OPATMETHODS:A total of 741 trainees were tested on the OPAT within two weeks of entering their initial entry training. At the end of their training, trainees were tested on simulations of the most physically demanding tasks of their job. RESULTS: There was a high classification concordance (76.5%) between success on the OPAT and job task simulations. False positives (6.7%) were misclassified because they did not perform as well on the strength-dominant task simulations. While the interval aerobic run was the greatest contributor to false negatives (16.8%), previous studies indicated high performance on this event as a potential key indicator of injury and attrition risk. CONCLUSIONS: The findings provide insight on how the accuracy of the OPAT, and similar pre-employment tests, could be improved.
