RESUMEN

Cardiopulmonary exercise testing (CPET) is a non-invasive approach to measure the maximum oxygen uptake ([Formula: see text]), which is an index to assess cardiovascular fitness (CF). However, CPET is not available to all populations and cannot be obtained continuously. Thus, wearable sensors are associated with machine learning (ML) algorithms to investigate CF. Therefore, this study aimed to predict CF by using ML algorithms using data obtained by wearable technologies. For this purpose, 43 volunteers with different levels of aerobic power, who wore a wearable device to collect unobtrusive data for 7 days, were evaluated by CPET. Eleven inputs (sex, age, weight, height, and body mass index, breathing rate, minute ventilation, total hip acceleration, walking cadence, heart rate, and tidal volume) were used to predict the [Formula: see text] by support vector regression (SVR). Afterward, the SHapley Additive exPlanations (SHAP) method was used to explain their results. SVR was able to predict the CF, and the SHAP method showed that the inputs related to hemodynamic and anthropometric domains were the most important ones to predict the CF. Therefore, we conclude that the cardiovascular fitness can be predicted by wearable technologies associated with machine learning during unsupervised activities of daily living.

Abstract Background The incidence of diabetes mellitus in younger adults is rising over the years. The diabetic population has an increased risk of developing heart failure, and diabetic individuals with heart failure have four times greater mortality rate. Studies results about exercise effect on left ventricular function in type 2 diabetes mellitus are heterogenous. Objective This review aimed to analyze the effects of physical exercise on left ventricular dysfunction in type 2 diabetes mellitus (T2DM). Methods Only randomized clinical trials with humans published in English were included. Inclusion criteria were studies with type 2 diabetes patients, physical exercise, control group and left ventricular function. Exclusion criteria were studies with animals, children, teenagers, elderly individuals and athletes, presence of diet intervention, and patients with type 1 diabetes, cancer, cardiac, pulmonary, or neurological diseases. Electronic databases PubMed, Web of Science, Cochrane, and Scopus were last searched in September 2021. Risk of bias was assessed by the Physiotherapy Evidence Database (PEDro) scale. Results Five studies were included, representing 314 diabetic individuals submitted to resistance and aerobic exercise training. Of the variables analyzed, physical exercise improved peak torsion (PTo), global longitudinal strain, global strain rate (GSR), time to peak untwist rate (PUTR), early diastolic filling rate (EDFR) and peak early diastolic strain rate (PEDSR). Conclusion To our knowledge, this is the first systematic review on the effects of exercise on left ventricular function in T2DM including only randomized clinical trials with humans. Physical exercise seems to improve systolic and diastolic strain, twist, and torsion. High intensity exercise was reported to be superior to moderate intensity exercise in one study. This review was limited by the small number of studies and their heterogeneity regarding exercise protocols, follow-up period, exercise supervision and left ventricular function variables analyzed. This review was registered in PROSPERO (CRD42021234964).

Manual hyperinflation is used in neonatal and pediatric intensive care units to promote expiratory flow bias, but there is no consensus on the benefits of the technique. Thus, a review that presents supporting evidence is necessary. This study aims to review the literature on the manual hyperinflation maneuver in neonatal and pediatric intensive care units to analyze the evidence for this technique in terms of the forms of application (associated with other techniques or not), its safety, the performance of manual resuscitators and the influence of the physical therapist's experience, in addition to evaluating the methodological quality of the identified articles. A search was performed in the following databases: Web of Science, ScienceDirect, PubMedⓇ, Scopus, CINAHL and SciELO. Two researchers independently selected the articles. Duplicate studies were assessed, evaluated by title and abstract and then read in full. The quality of the articles was analyzed using the PEDro scale. Six articles were included, two of which had high methodological quality. The main results provided information on the contribution of the positive end-expiratory pressure valve to increasing lung volumes and the use of chest compressions to optimize expiratory flow bias, the negative influence of operator experience on the increase in peak inspiratory flow, the performance of different manual resuscitators when used with the technique and the safety of application in terms of maintaining hemodynamic stability and increasing peripheral oxygen saturation. The available studies point to a positive effect of the manual hyperinflation maneuver in children who are admitted to intensive care units. Registration PROSPERO: CRD42018108056.

RESUMO O objetivo do estudo é descrever o perfil de três grupos de mulheres: aparentemente saudáveis (não doentes-ND); com doenças cardiovasculares (DC); e com doenças respiratórias (DR) crônicas. A pesquisa comparou esses grupos conforme variáveis sociodemográficas (faixa etária, escolaridade, etnia e renda familiar) e dados sobre qualidade de vida (QV), segundo domínios: físico, psicológico, relações sociais e meio ambiente. Este estudo é caracterizado como quantitativo, analítico, observacional e transversal de base populacional, com informações extraídas do Inquérito de Saúde da Mulher, realizado na cidade de Uberaba-MG. Foram entrevistadas 1.387 mulheres, com o objetivo de coletar informações sobre DC, DR, idade, escolaridade, etnia e renda familiar. A QV foi estudada por meio do WHOQOL - Bref, segundo os quatro domínios. Foi realizado levantamento estatístico, descritivo e inferencial. Identificou-se que o grupo com DC é formado por mulheres com idade a partir de 50 anos e baixa escolaridade, em oposição ao grupo de mulheres com DR, que são mais jovens e possuem escolaridade superior. Quanto à QV, as mulheres com DC (31,65%) apresentaram níveis inferiores em relação às mulheres com DR (19,10%), nos domínios: físico (50,6;54,0;<0,0001), psicológico (55,1;58,7;<0,0001), relações sociais (75,8;77,2;0,0055) e meio ambiente (41,7;43,0;0,0173), valores em média. Portanto, o domínio de QV que obteve menor pontuação para todos os grupos foi o meio ambiente e as mulheres com DC apresentaram valores mais baixos, estatisticamente significativos, para todas as categorias.


RESUMEN El objetivo del estudio es describir el perfil de tres grupos de mujeres: aparentemente sanas (no enfermas-NE); con enfermedades cardiovasculares (EC); y con enfermedades respiratorias crónicas (ER). La investigación comparó estos grupos según variables sociodemográficas (grupo de edad, escolaridad, etnia e ingresos familiares) y datos sobre calidad de vida (CV), según dominios: físico, psicológico, relaciones sociales y medio ambiente. Este estudio se caracteriza por ser cuantitativo, analítico, observacional y transversal de base poblacional, con información extraída de la Encuesta de Salud de la Mujer, realizada en la ciudad de Uberaba-MG. Se entrevistó a un total de 1.387 mujeres, con el objetivo de recopilar información sobre EC, ER, edad, escolaridad, etnia e ingresos familiares. La CV se estudió utilizando el WHOQOL - Bref, según los cuatro dominios. Se realizó encuesta estadística, descriptiva e inferencial. Se identificó que el grupo con EC está formado por mujeres con edad de 50 años o más y baja escolaridad, en contraposición al grupo de mujeres con EC, que son más jóvenes y tienen estudios superiores. En cuanto a la CV, las mujeres con EC (31,65%) presentaron niveles más bajos en comparación con las mujeres con ER (19,10%), en los siguientes dominios: físico (50,6; 54,0;<0,001), psicológico (55,1; 58,7;<0,0001), relaciones sociales (75,8;77,2;0,0055) y medio ambiente (41,7;43,0; 0,0173), valores medios. Por lo tanto, el el dominio de CV que obtuvo la puntuación más baja para todos los grupos fue el medio ambiente y las mujeres con EC presentaron valores más bajos, estadísticamente significativos, para todas las categorías.


ABSTRACT This study aims to describe the profile of three groups of women; apparently healthy (non-sick - NS); with cardiovascular diseases (CD); and with chronic respiratory diseases (RD). These groups were compared according to sociodemographic variables (age group, education, ethnicity, and family income) and quality of life (QoL) data, composing the domains: physical, psychological, social relations, and environment. This study is characterized as quantitative, analytical, observational, and cross-sectional population-based, with information extracted from the Women's Health Survey, conducted in the city of Uberaba-MG. A total of 1,387 women were interviewed, and information on CD, RD, age, education, ethnicity and family income were collected. The QoL was studied using WHOQOL - BREF, according to its four domains. Descriptive and inferential statistics were performed. The CD group is composed of women aged 50 years and older and low education level, in contrast to the women of the RD group , who are younger and have higher education level. As for QoL, women with CD (31.65%) had lower levels than women with RD (19.10%), in the domains: physical (50.6;54.0;<0.0001), psychological (55.1;58.7;<0.0001), social relations (75.8;77.2;0.0055), and environment (41.7;43.0;0.0173), values on average and p, respectively. Therefore, the QoL domain that obtained the lowest score for all groups was the environment and women with CD presented lower values for all categories, which were statistically significant.

BACKGROUND: There are few reports in the literature supporting the understanding of the physiological mechanisms of intolerance in patients with COPD to perform unsupported upper limb activities. The aims of this study were to quantify the electrical activity and oxygenation of inspiratory and upper limb muscles, and to investigate whether electromyographic manifestations of muscle fatigue are related to upper limb function as assessed by the 6-min pegboard and ring test (6PBRT) in subjects with COPD and in healthy subjects. METHODS: Thirty subjects with COPD (FEV1 42.1 ± 16.4% predicted; 68.0 ± 7.6 y old) comprised the COPD group, and 34 healthy subjects (66.8 ± 8.0 y old) comprised the control group. Both groups were assessed for body composition with dual-energy radiograph absorptiometry and spirometry. The 6PBRT was performed with simultaneous assessment of electromyography, near-infrared spectroscopy, and gas analyses (expiratory minute volume). RESULTS: Differences were observed between groups for performance (number of rings) in the 6PBRT, with the COPD group achieving lower values than the control group (P < .001). The ventilatory demand (expiratory minute volume/maximum voluntary ventilation) and root mean square amplitude of the sternocleidomastoid muscle were higher in the COPD group than in the control group (P < .04). Lower values for oxyhemoglobin and total hemoglobin were found in intercostal muscles of the COPD group compared to the control group. The root mean square amplitude of the intercostal muscles was lower in the COPD group, while it was similar between groups for anterior deltoid and trapezius muscles. Median frequency of anterior deltoid muscles presented a decreased in both groups. CONCLUSIONS: Our results indicate that the 6PBRT was performed at a higher electrical activity in the accessory inspiratory muscles, such as the sternocleidomastoid muscle, and a lower oxygenation profile in the intercostal muscles in subjects with COPD compared with healthy controls, but without muscle fatigue signs. These findings suggest that the higher ventilatory demand presented in subjects with COPD could have contributed to the worse performance in this group without signals of peripheral muscle limitation.
