RESUMEN

Insect defensins, are cationic peptides that play an important role in immunity against microbial infection. In the present study, an anionic defensin from Plutella xylostella, (designated as PxDef) was first cloned and characterized. Amino acid sequence analysis showed that the mature peptide owned characteristic six-cysteine motifs with predicted isoelectric point of 5.57, indicating an anionic defensin. Quantitative real-time polymerase chain reaction analysis showed that PxDef was significantly induced in epidermis, fat body, midgut and hemocytes after injection of heat-inactivated Bacillus thuringiensis, while such an induction was delayed by the injection of live B. thuringiensis in the 4th instar larvae of P. xylostella. Knocking down the expression of nuclear transcription factor Dorsal in P. xylostella by RNA interference significantly decreased the mRNA level of PxDef, and increased the sensitivity of P. xylostella larvae to the infection by live B. thuringiensis. The purified recombinant mature peptide (PxDef) showed higher activity against Gram-positive bacteria, with the minimum inhibition concentrations of 1.6 and 2.6 µM against B. thuringiensis and Bacillus subtilis, respectively. To our knowledge, this is the first report about an anionic PxDef, which may play an important role in the immune system of P. xylostella against B. thuringiensis.

In the present study, two experiments were designed to investigate the lethal and sublethal effects of methoxyfenozide on Spodoptera litura (F.). The first experiment was conducted to explore the fitness cost of methoxyfenozide resistance in a methoxyfenozide-resistant strain (selected with methoxyfenozide for 13 consecutive generations), leading to resistance ratio to methoxyfenozide 83.0- and 2359-fold higher when compared to the field and susceptible populations with a fitness cost of 0.17. In the second experiment, second instars of the susceptible strain were treated with sublethal doses of methoxyfenozide (LC30, LC20, and LC10) by diet incorporation for larval feeding for 3 days. It was observed that higher concentrations of methoxyfenozide significantly prolonged the larval and pupal development time of S. litura as compared to the control treatment. The number of eggs per female, egg hatching, sex ratio, and longevity of adults of methoxyfenozide-treated groups was greatly reduced as compared to untreated S. litura. Our data clearly indicated that fitness cost of methoxyfenozide and its sublethal effects on S. litura has an important impact on its population dynamics.

The intensity of genetic diversity amongst chickpea genotypes and their crosses is unknown. The current study investigated the genetic diversity of chickpea genotypes and their F1 crosses by using random amplified polymorphic DNA (RAPD) markers. We assessed the variation among six chickpea genotypes and 15 F1 crosses with the RAPD markers. The six parents and their 21 hybrids were carefully studied based on the presence or absence of bands. The level of polymorphism varied with different primers. Out 28 primers used, 21 amplified the genomic DNA in all the varieties, 15 generated polymorphic bands among all the varieties, and six produced similar banding patterns.

We examined the recollection of traumas in panic disorder patients with and without history of nocturnal panic attacks. From a sample of 154 patients seeking treatment for panic disorder, almost 85% of those with nocturnal panic reported a history of traumatic events in comparison to only 28% without nocturnal panic. Fear of loss of vigilance is considered as a potential mediator of the relationship between nocturnal panic and traumatic events.

Patients with traumatic brain injury (TBI) frequently exhibit varied forms of visual system dysfunction including: binocular, oculomotor, accommodative, refractive error shift, visual field loss, and visual perceptual deficits. A 5-year collaborative study between optometry and ophthalmology was initiated to follow documented mild TBI patients utilizing diagnostic methods to assess the quantity and quality of visual system deficits and recovery. A group of patients with mild TBI receiving optometric rehabilitation were compared with a group of age-matched, gender-matched, and headsize-matched TBI patients not receiving such treatment. Eighteen patients diagnosed with mild TBI underwent a treatment regimen of optometric rehabilitation (group I); 32 patients diagnosed with mild TBI did not receive optometric rehabilitation (group II). Pattern visually evoked cortical potential (VECP) testing and electroretinography (ERG) evaluation were utilized initially, repeated 6-12 months later and then 12-18 months after baseline. All TBI patients' VECP and ERG results were compared to age-matched, headsize-matched controls. Once the ERG had been used to exclude retinal involvement, identification of visual pathway dysfunction was possible with the VECP. Full-field ERG results in all groups were not remarkable and not sensitive for patients with mild TBI. Initial testing results revealed that 72% of those TBI patients in group I demonstrated VECP waveform abnormalities and 81% of those patients in group II showed waveform dysfunction. In the testing performed 12-18 months later, 38% of group I TBI patients, after receiving a treatment regimen of optometric rehabilitation, showed VECP waveform abnormalities; 78% of group II TBI patients demonstrated waveform abnormalities. VECP evaluation in patients with mild TBI can provide a useful and reliable tool for objective assessment of visual system deficit and recovery. Significant differences in visual system recovery were shown when comparing group I and group II.

Expectations about arousal were examined in relation to nocturnal panic (NP). Eighteen panic disorder patients suffering from NP attacks and 18 control individuals were assigned randomly to conditions in which they were informed that audio feedback signals reflected heightened arousal that was either (a) expected and harmless or (b) unexpected. Participants relaxed and slept for 45 to 60 min, followed by presentation of periodic audio feedback signals. Physiological recording was continuous while subjective measures were collected at completion of the signals phase. NP patients in the unexpected--no reassurance condition were significantly more anxious and symptomatic than their counterparts in the expected reassurance condition, whereas control individuals did not differ across the 2 conditions. Physiological and behavioral data were less consistent than subjective measures. The results are interpreted as supportive of a cognitive-behavioral model of NP.

BACKGROUND: Patients suffering from a traumatic brain injury (TBI) show many types of visual dysfunctions, including binocular, oculomotor, accommodative and visual field loss. This study evaluates the visual status of patients suffering a mild TBI. METHODS: The visual findings of a control group of 16 non-TBI patients are compared to the visual findings of 16 patients with documented mild TBI. Functions compared include symptomology, refractive status, stereopsis, ocular alignment by cover test, relative accommodation, pursuits, near point of convergence break and recovery, phorias at distance and near and vergence ranges at distance and near. RESULTS: Significant differences were found between the two groups in the areas of refractive status, near point of convergence break/recovery, near cover test, stereo acuity and base-in break at distance. There were also significant differences in symptomology, pursuit function, base-in recovery at distance, base-in break/recovery at near and base-out recovery at distance. CONCLUSIONS: In-depth visual evaluation of mild TBI patients is critical for documentation purposes as well as for possible treatment regimens.

We performed a prospective study to evaluate fine-needle aspiration (FNA) cytology as a screening tool of carcinoma of the prostate in 159 men with normal digital rectal examinations and acid phosphatase prior to open prostatectomy for voiding symptoms. The incidence of carcinoma of the prostate was 5.6%. 4 patients had A1 lesions and 5 had A2 lesions. Only one A2 lesion was malignant cytologically. The sensitivity was 56%, specificity 69%, positive predictive value 24% and negative predictive value 90%. Sufficient cytologic specimens were provided in 66% of cases. While FNA is at least equal to core biopsy as a diagnostic modality of palpable prostatic abnormalities, it does not prove to be an adequate screening modality for occult carcinoma of the prostate in the prostatectomy candidate.

Thirty-five patients being treated for one of three anxiety disorders were asked to rate the likelihood of a negative outcome prior to entering a feared situation, while in the situation and after leaving the situation. Ratings made in anticipation were consistently higher than ratings made either in the situation or after leaving it. In contrast, normal Ss, about to undertake tourist rides perceived as being mildly dangerous, rated the likelihood of negative outcome as highest when actually in the situation.

This analysis of the relationship of infant and childhood illness and death to ghost beliefs is based on holistic fieldwork in the late 1950s and the late 1970s in Shanti Nagar (a pseudonym), a village in North India. Illness and the supernatural world are linked by the concepts of ghosts and Fever, the latter an index of ghost illness, deriving from a supernatural being. The links between ghosts, Fever, and ghost illness involve basic Hindu beliefs, tales from Hindu and Sanskritic texts, ancient curing practices, stress, and local and family histories. A limited number of cases from the many in Ghosts: Life and Death in North India (R. Freed and S. Freed 1991) are here presented to illustrate particular points and general characteristics of ghost illness, including ghost possession, when found in children. The village health culture includes curing practices from the Atharva-Veda (the most ancient Sanskritic literature), Ayurvedic Medicine, Unani Prophetic Medicine, and Western Biomedicine.

Laterality of prostatic lymphatic drainage was successfully assessed in nine patients with prostatic carcinoma after direct unilateral transrectal injections of 99Tcm-antimony sulphide colloid by Franzen needle. Pelvic lymphatic drainage was equally divided between ipsilateral, contralateral and bilateral patterns. The frequent observation of radiocolloid migration to contralateral lymph node groups in patients with prostate carcinoma, including those with primary disease localized clinically to a single lobe of the prostate, indicates the potential for contralateral pelvic lymph node metastases in this population. This finding may be important in determining the appropriate surgical staging and treatment of patients with prostate carcinoma. The clinical applicability of lymphoscintigraphy in the evaluation of metastatic spread and in lymph node biopsy planning requires further study.

The substantial number of ghost possessions that came to our attention during fieldwork in a North Indian village in 1977-78 led to a thorough study of ghost beliefs as part of a holistic study of village life. Ghosts are not phantoms floating on the periphery of village life, the concern only of children and the credulous. Rather, the study shows that ghosts are linked with basic Hindu beliefs, village lore, ancient curing practices and theories, the diagnosis of illness and treatment of disease, individual stress and anxiety, and family, lineage, and village histories. Ghost possession, a subsidiary and dramatic form of ghost illness, is behavior in which the ghost speaks from its victim who undergoes a range of alternate states. Unpredictable events and heightened personal stress generally precede episodes of ghost possession. Cases of ghost illness and ghost possession include children and adults of both sexes and a range of ages. Our data contradict the village stereotype that only women suffer from ghost possession. Villagers have recourse to both traditional remedies and Western biomedicine to treat ghost illness.

Comparative DNA ploidy measurements were carried out by flow cytometry and by image analysis on cells in 71 bladder washing specimens from 50 patients with past histories of bladder tumors. Among the specimens classified as diploid or questionable by flow cytometry, 14 showed the presence of aneuploid DNA values documented by image analysis. In 18 of the 50 patients, recurrent tumors were observed during a relatively brief period of follow-up. In 15 of them the DNA pattern was aneuploid and in three it was questionable. In nine of the 15 patients, both methods of DNA analysis disclosed aneuploidy, but in six patients aneuploidy was detected by image analysis only. A combination of DNA aneuploidy, whether observed by flow cytometry, image analysis, or both, and of positive or suspicious urine cytology is highly predictive of recurrence of high grade bladder tumors. Image analysis of DNA content in bladder washings adds information of clinical value above and beyond that obtained by flow cytometry.

Detailed functional isomorphism had been observed (Freed, 1984) between induced (conditioned) immunogenicity and classical conditioned defensive reflexes, possibly as evolutionary adaptation against danger at micro and macro levels respectively. Similarly, functional isomorphism is postulated between conditioned specific tolerogenicity of the immunotolerance system and behavioral reflexes. Isomorphism requires that sensory signals elaborated with intrinsic (unconditioned) behavioral tolerogens as carriers do not subsequently combine classically with unconditioned aversive stimuli and evoke conditioned defensive responses. Unconditioned behavioral tolerogenic carriers were identified with behavioral (physiological) activities of Oriental meditation. Confirmation of conditioned behavioral tolerogenicity appeared in the unresponsiveness of Yogi mediators to sensory stimuli as reflected in unchanged alpha rhythms of their encephalograms. Conditioned behavioral specific unresponsiveness maintains the "quiet" of meditation and mediates the experience of Zen mediators, namely, sharpened, clearer perceptions and unresponsiveness to aversive components of current conditioned signals ordinarily reactivating residues of affect. Conditioned behavioral specific unresponsiveness has survival value.

Functional isomorphism holds between four essential properties of acupuncture and of meditation, namely, 1) alpha rhythm prominent in electro-encephalograms (EEG); 2) deep general relaxation; 3) high degree of unresponsiveness to ordinarily painful stimuli; 4) participation of virtually the entire body. It is postulated, subject to experimental test, that a "stillness" prevails during acupuncture similar to the quiet of meditation. The quiet of meditation and by postulate, the "stillness" of acupuncture, provide high degree of unresponsiveness to aversive components of conditioned stimuli which had habitually reactivated affective trauma. This marked unresponsiveness accounts for the "stillness" and its EEG alpha rhythm. With its low noise level the "stillness" also provides the cerebral cortex better resolved, more intense signals relative to background and more comprehensive, clear information. The cortex can then call upon newly mobilizable, more precise regulation for removing imbalances throughout the body. The same factors apply to the therapy by acupuncture of phantom limb pain syndrome if the pain impulses from the limb, while attached, is the unconditioned stimulus of a reflex in which impulses from inner organs function as conditioned stimuli. Successful therapy of the syndrome using laser-stimulated acupuncture points is discussed accordingly. Emphasized is the desirability to maximize the "stillness," possibly by monitoring the course of therapy by displayed EEG.

Changes taking place in both Qi Gong Masters and their patients during Qi Gong treatment were evaluated using the Bi-Digital O-Ring Test. During the Qi Gong state, on the Qi-Gong Master's body, as well as the body of the patient being treated, acupuncture points CV5 (Shi Men) and CV6 (Qi Hai)-- located below the umbilicus-- show changes from +4 in the pre-Qi Gong state to between -3 and -4 during the Qi Gong state. Before and after the Qi Gong, there is a normal +4 response to the Bi-Digital O-Ring Test at these acupuncture points. Similar changes were also observed on acupuncture points CV17 (Shan Zhong), CV 22 (Tian Tu), Yin Tang (at an area just between the eyebrows: the pituitary gland representation area, colloquially known as the "third eye") and GV20(Bai Hui), the entire pericardium meridian & triple burner meridian, their acupuncture points, the adrenal glands, testes, ovaries and perineum, as well as along the entire spinal vertebrae, particularly on and above the 12th thoracic vertebra, medulla oblongata, pons, and the intestinal representation areas of the brain located just above and behind the upper ear. Using these findings as criteria for evaluating the effectiveness of reaching the Qi Gong state, we were able to reproduce during the experimental trials similar changes in ourselves and the patient being treated with therapeutic effects comparable to those of the Qi Gong Master. Beneficial effects of external Qi Gong treatment given by a Qi Gong practitioner 1 to 3 times for 10-20 seconds each (although most Qi Gong masters take 3-20 minutes per treatment) often resulted in improvement of circulation and lowering of high blood pressure, as well as relaxation of spastic muscles, relief of pain, and enhanced general well-being, all of which resemble acupuncture effects. In order to reproduce the same procedure with others, we selected 4 children ranging between 8 and 11 years of age who had no knowledge of Qi Gong or Oriental medicine. One of these four children, the 8 year old, was able to consistently reach the same Qi Gong state after less than a half day and another child, 11, after less than 2 days. Within a week, the other two were sometimes able to reproduce the Qi Gong state but not always. Using the Qi Gong state thus obtained, it was found that this type of Qi Gong energy is directed to specific directions from the hand and can even penetrate wooden or metal doors.(ABSTRACT TRUNCATED AT 400 WORDS)

As part of an ongoing study of objective parameters of prognostic value in prostatic carcinoma, a routine procedure was developed to aspirate all prostates prior to surgery. These targets were different from those of other workers in the field of prostatic fine needle aspiration (FNA), who generally advocate that FNA be confined to suspicious nodules. The aspirations were performed by a large group of practicing urologists who had no special training in prostatic FNA except for guidelines provided by their peers and information available in the literature. This approach permitted an assessment of the performance of FNA as a screening test rather than as a diagnostic procedure. During the period from January 1983 to February 1987, 1,683 patients had prostatic FNAs performed (plus subsequent histologic study). The following diagnoses were rendered: "inadequate/scanty specimen" in 625 cases (37%), "negative/atypical" in 844 cases (50%) and "suspicious/positive" in 214 cases (13%). Histologic examination showed stage A1 prostatic adenocarcinoma in 18 patients. The cytologic diagnoses on these 18 patients were inadequate/scanty in 3 (17%), negative/atypical in 13 (72%) and suspicious/positive in 2 (11%). Of the 214 patients with a positive/suspicious diagnosis by FNA, the diagnosis of prostatic carcinoma was confirmed by tissue evidence in 200; the other 14 patients had either no evidence of prostatic carcinoma on surgical biopsy (needle biopsy/transurethral resection/suprapubic prostatectomy) or had no surgical biopsy. Eight of the 14 patients developed clinical evidence of carcinoma, 1 died of urinary bladder carcinoma and 1 was lost to follow-up. In the remaining four patients, there is still no evidence of prostatic carcinoma after about one-and-one-half years of follow-up. These results indicate that (1) specialized training is required in order to obtain adequate smears by prostatic FNA; (2) prostatic FNA is not a good screening technique for detecting stage A1 prostatic carcinoma; and (3) a positive diagnosis by prostatic FNA, even when not confirmed by tissue biopsy, is still an indication of disease.

We studied 14 postpubertal patients at an average of 33 months after treatment for testicular torsion. Of these patients 11 had been treated by detorsion and 3 by orchiectomy. Five normal male volunteers of the approximate age of the study group served as controls. The patients treated by detorsion were subdivided into 3 groups based on the degree of atrophy of the detorsed testicle: group 1--no testicular atrophy (5), group 2--25 per cent testicular atrophy (2) and group 3--greater than 90 per cent testicular atrophy (4). Mean duration of torsion was greatest in the orchiectomy group (161 hours) compared to 6, 16 and 29 hours for groups 1, 2 and 3, respectively. The serum luteinizing hormone and follicle-stimulating hormone response to an intravenous bolus of 100 mcg. synthetic gonadotropin releasing hormone was measured in all patients. All groups had a greater mean follicle-stimulating hormone response to gonadotropin releasing hormone stimulation than controls (p less than 0.05). Patients who underwent orchiectomy had the greatest follicle-stimulating hormone response to gonadotropin releasing hormone stimulation. Mean luteinizing hormone response to gonadotropin releasing hormone stimulation was normal in patients without atrophy (group 1) but it was greater than controls in patients who had atrophy (groups 2 and 3) or who underwent orchiectomy (p less than 0.05). Several conclusions could be made from our study. All patient groups treated for torsion had evidence of testicular dysfunction. Patients who underwent orchiectomy displayed more testicular dysfunction than patients who had atrophy after detorsion. Testicular dysfunction after torsion is more likely to involve spermatogenic before Leydig cell function.

RESUMEN

Urachal disorders are uncommon and present with varied appearances. Three cases of urachal disease, one congenital and two acquired, are reported. Each case is representative of the symptoms and findings of its respective category. A review of the literature is presented. A basic understanding of urachal development is necessary to suspect a diagnosis of urachal disease.
