[Central and renal hemodynamics in hypertension with high, normal and low renin activity in the blood plasma]. / Tsentral'naia i pochechnaia gemodinamika u bol'nykh gipertonicheskoi bolezn'iu s vysokoi, normal'noi i nizkoi aktivnost'iu renina v plazme krovi.

Simultaneous study of renin activity in plasma of peripheral blood and values of central and renal hemodynamics in 180 patients with hypertensive disease showed no significant differences in the hemodynamic values in patients with stage I of the disease which differed in the activity of blood plasma renin. No dependence of the volume of circulating blood on the type of reninemia was revealed in stage II, but a significant decrease in the coefficient of circulation effectiveness and in cardiac output and an increase in peripheral vascular resistance were disclosed in patients with low renin activity in blood plasma. Diminished effective renal blood flow and increased renal vascular resistance and filtration fraction were the most marked findings in patients with high renin activity. The changes in renal hemodynamics may be associated with the effect of intrarenal angiotensin II.

A double-sensor gamma-topograph model GSG-2 has been constructed to realize colour shade lining and black-and-white photographic registration of images reflecting spatial distribution of radioactive isotopes introduced to patients for diagnostic purposes. On the ground of trial tests the valuation of functional and clinical qualities of the device is given.

Glomerular filtration was examined by way of a radioisotope technique using Yb169-labelled ethylenediaminetetracetic acid. The method permits to conduct the examinations without radio-isotope equipment at the bedside of myocardial infarction patients, which is of great practical convenience. The study was conducted in 65 myocardial infarction patients and in 14 control individuals. Glomerular filtration was studied dynamically in different age-groups, in patients with acute and recurrent myocardial infarction with due regard of concomitant hypertension and complications. A correlation of the disorders in glomerular filtration and the age of the patients was revealed. The lowest figures of glomerular filtration were received in patients with myocardial infarction complicated by circulatory insufficiency and, especially, cardiogenic shock. The indices of glomerular filtration correlated with the circulating blood volume and venous pressure not in all patients.
