RESUMEN

OBJETIVO: La información es una de las necesidades más importantes de los familiares de pacientes críticos. Los profesionales sanitarios identifican también el gran valor de la comunicación con los familiares para reducir su ansiedad y su estrés. A pesar de ello, los familiares pueden sentirse indebidamente informados, lo cual causa un sufrimiento añadido. El objetivo de este estudio es tratar de comprender las perspectivas de los familiares y los profesionales sanitarios sobre la información que se aporta en las unidades de cuidados intensivos (UCI). MÉTODO: Se realizó una revisión narrativa utilizando las bases de datos MEDLINE, CINAHL, PsycINFO y Cochrane Library para identificar los estudios publicados en inglés y en español de 2002 a 2018. RESULTADOS: Se incluyeron 46 estudios, identificándose cinco categorías: 1)«Necesidad de saber»; 2)«Satisfacción del familiar con la información recibida»; 3)«Impacto de la información en las experiencias de los familiares en la UCI»; 4)«Percepciones de las enfermeras y médicos sobre la transmisión de información», y 5)«Proceso de información en la UCI». CONCLUSIONES: La aportación de información honesta y veraz a los familiares de los pacientes críticos es esencial para reducir su ansiedad e incrementar su control, aunque parece que esto no siempre es adecuadamente realizado por el personal. La implicación interdisciplinaria en la aportación de información puede ser beneficiosa para los familiares y los profesionales de las UCI. Esta revisión aporta una nueva comprensión acerca del proceso de información a los familiares de los pacientes de la UCI y puede utilizarse para mejorar la calidad y la humanización de los cuidados en dichas unidades

Interdisciplinary involvement in information giving may be beneficial for both families and ICU professionals. This review brings new understanding about the process of information to families of ICU patients and it can be used to improve the quality and humanization of care in the ICUs

RESUMEN

OBJETIVO: Conocer el significado que atribuyen pacientes adultos y sus familias a la ausencia familiar durante la atención de urgencia. MÉTODO: Estudio cualitativo siguiendo la propuesta de la teoría fundamentada con recogida de datos en 2 unidades públicas de urgencias hospitalarias, localizadas en el sur de Brasil. Se llevaron a cabo 15 entrevistas con pacientes y 15 entrevistas con familiares durante los meses de octubre de 2016 a febrero de 2017. Los datos fueron analizados siguiendo el método comparativo contante. RESULTADOS: Los pacientes y familiares vivencian la ausencia familiar en la atención de urgencia como un proceso de sufrimiento causado por la separación del binomio familia-paciente; por la falta de entendimiento acerca de los motivos que justifican la exclusión familiar, y por enfrentarse a la situación con resignación. CONCLUSIÓN: La urgencia per se conlleva un sufrimiento en el paciente y sus familiares; este sufrimiento se intensifica cuando la familia es apartada y no puede acompañar al paciente durante la atención de urgencia. En vista de estos resultados, es necesario desarrollar estrategias y políticas sanitarias que contribuyan a la atención integral de pacientes y familias en unidades de urgencias hospitalarias

AIM: The study explored the views and attitudes of nurses and physicians on family presence during resuscitation in emergency departments in Brazil. BACKGROUND: International emergency associations endorse family presence during resuscitation; however, the extent to which it is practiced remains unclear, particularly in the Brazilian context. Research of emergency staff views and attitudes towards this practice is desirable so that actions can be identified to support families at the bedside. METHODS: A qualitative research was conducted. Thirty-two health professionals (11 physicians and 21 nurses) working in two emergency departments in southern Brazil were purposefully recruited in January 2015. In-depth interviews were conducted, and data were analysed using content analysis. FINDINGS: Nurses and physicians found family presence during resuscitation controversial and the general attitude towards this practice was negative. They reported that 'changes are needed' to adapt hospital infrastructures for family presence, and to train staff to respond to the information and emotional needs of families. DISCUSSION: Translating a family nursing framework into clinical practice involves the need for reassessing educational and management policies in clinical contexts. CONCLUSION: This research brings new understanding about the attitudes of some Brazilian nurses and physicians on the implementation of family presence during resuscitation and identifies the need to develop policies and strategies to improve family presence in emergency departments. IMPLICATIONS FOR NURSING AND HEALTH POLICY: Hospital-based policies are required to improve family-centred care in emergency departments while providing a criterion of legality and safety to professionals to invite families to be present during invasive procedures. Also, family-focused education in health science degrees, continuing education and in the community is required.

The process of nursing home placement can be a stressful event for both the dependent elderly person and the family. During admission, especially the first few months, the family may suffer feelings of loss, sadness, failure and guilt. In this context, support and guidance from health professionals are essential to address the needs of residents and families through effective interventions. The aim of this systematic review is to identify the most effective interventions to help families during the process of institutionalization of a relative in a nursing home. Two types of family interventions were identified: those that focused on family-staff relationship and those that focused on family support groups, the latter being the most effective. On the other hand, most interventions have an individualistic approach, focusing on the primary caregiver. Finally, there is a shortage of quality studies that present the results of family interventions in the geriatric field and also a lack of such studies in the Spanish context.

La institucionalización en un centro geriátrico supone en muchas ocasiones un acontecimiento estresante, tanto para la persona mayor dependiente como para su familia. Durante el ingreso, especialmente en los primeros meses, la familia puede tener sentimientos de pérdida, tristeza, fracaso o culpabilidad. En este contexto, resulta fundamental el apoyo y orientación de los profesionales del centro, a través de intervenciones efectivas que aborden las necesidades tanto del residente como de su familia. El objetivo de esta revisión sistemática es identificar cuáles son las intervenciones familiares más efectivas para ayudar a las familias durante el proceso de institucionalización en un centro gerontológico. Se han identificado dos tipos de intervenciones: aquellas que se centran en la relación familia-personal sanitario y, las que se enfocan a grupos de apoyo a familiares, siendo estas últimas las más efectivas. Por otro lado, la mayoría de intervenciones tienen un enfoque individual, centrándose en el cuidador principal, sin abordar la unidad familiar. Por último, hay una escasez de artículos de calidad que presenten resultados sobre intervenciones familiares en el ámbito geriátrico, identificándose una ausencia de estos estudios en el contexto español (AU)
