RESUMEN

The authors analyzed an experience of surgical treatment in patients with aortic valve failure and ischemic heart disease. All patients have been operated in clinic of hospital surgery No 2 of State Pavlov Medical University. The work presented basic approaches to the management and strategy choice of the treatment of these patients. On the basis of given research, the authors suggested to perform a reconstruction of aortic valve by using original method in combination with coronary bypass surgery, which allowed avoiding a prosthetic-patient inadequacy. At the same time, it could improve an intracardiac haemodynamics and postoperative course.

RESUMEN

A comparative examination of the anatomy of human and porcine aortic valves was carried out. The similarity and differences are shown in the anatomical structure and geometrical relationships of certain structures of the allogenic and xenogenic aortic valves which should be taken into account when performing reconstructive-plastic operations on the aortic valve and in implantation of stentless bioprostheses, and when developing their new models.

RESUMEN

A comparative investigation of biomechanical and hydrodynamic characteristics of stentless and stented bioprostheses was carried out. It was demonstrated that stentless bioprostheses are functionally superior to stented ones. The differences between biomechanics and hydrodynamics of the examined stentless bioprostheses "BioLab", "AB-Mono-Kemerovo" and DD2 and stented bioprostheses "Wessex", "KemCor" and "Bionox-2" were shown.

RESUMEN

A comparative analysis of experiments with biomechanics and hydrodynamic stentless xenografts implanted into the subcoronary position with completely (n = 7) and partially (n = 7) scalloped sinuses was made. The implantation was performed into the human aortic root complexes in vitro. Factors of the influence on biomechanical and hydrodynamic performances of xenografts before and after implantation have been established. Subcoronary implantation of stentless bioprostheses with the completely scalloped sinuses proved to result in better flow and biomechanical characteristics than the implantation technique with the saved noncoronary sinus.

RESUMEN

Mycotoc aneurysms (MA)--a rare and extremely dangerous lesion of the aorta. CT, MRA and aortography were used to diagnose MA of the thoracoabdominal areas in 2 men of 64 and 67 years of age. The clinical picture included: fever with shivering with the temperature 38-39 degrees C and increasing pains in the inferiothoracic and lumbar areas of the spine. Hemoculture was positive in 1 patient only. The patients were operated on. Shunting of the aorta from the inferiothoracic to infrarenal segments was performed in one patient with the exclusion of the involved portion and resection of MA. Metalloosteosynthesis of the spine due to destruction of LI-LII and compression of the spine were performed in the second patient followed by analogous shunting of the aorta and visceral arteries with the resection of the aneurysm. To prevent infection of the prostheses the shunts were wrapped up with a piece of the greater omentum. One patient died two months later from recurrent sepsis. The other one is alive, within 28 months the shunts are patent, there are no signs of a reinfection.

RESUMEN

A comparative analysis of geometrical, biomechanical and hydrodynamic characteristics of the valve-aortic complexes taken from humans of different age has shown a determining role of distensibility of the aortic root in the functioning of valve cusps. An alteration of geometrical relationships of elements of the aortic root, its less distensibility, worse biomechanical and hydrodynamic characteristics of the aortic valve in humans older than 50-60 years of age is established.

RESUMEN

Results of examinations and treatment of 49 patients with carcinoma of the operated stomach were analyzed. The operations had been previously performed for carcinoma, polyposis and ulcer disease. Results of the surgical treatment of such patients were shown to depend on terms of detection of the tumor growth. So, the patients who had had surgical interventions of the stomach should be systematically observed at dispensaries.

RESUMEN

A comparative assessment of energy losses and functional shifts in the system of circulation and respiration in various types of the professional activity in 22 patients 14,7 +/- 1,38 years after successful radical operations for the open arterial duct, defects of interatrial and interventricular septa, 64 healthy people and 12 nonoperated patients with similar congenital heart diseases was made. The patients subjected to timely radical operations were shown to possess working capacity and ability. They can fulfill a professional work with light and average strain under systematic control.

RESUMEN

The correlation of most important parameters of the central hemodynamics with the intensity of anaerobic metabolism was studied in 65 patients operated on having congenital heart diseases and in 37 healthy patients under increased physical exercise. A mild decrease of the functional reserve of the cardiovascular system (by 7-15%) was revealed in the patients as compared with the control group.

The hemodynamic indices under conditions of growing physical exercise were studied in 15 healthy women and 17 patients operated for the congenital heart disease. The operated women were found to have a restricted ability to increase the stroke volume. The individual choice of the level of everyday domestic and professional activity should be made with special reference to individual hemodynamic reactions upon the physical exercise.

The physical working capacity was investigated in 25 men operated on for congenital heart disease and in 12 healthy men not accustomed to heavy physical load. It was shown that the main factor responsible for lower physical efficiency of the operated patients was their less pronounced ability to increase the minute circulation volume under conditions of the maximum physical load.

RESUMEN

The functional cardiac reserve was studied in 26 non-operated and 42 operated patients with congenital heart diseases. The ratio of the maximum utilization of oxygen (VO2 max) to the heart volume (HV) was shown to be an objective quantitative index of the functional cardiac reserve. The authors believe that the value of the functional cardiac reserve may be used for the determination of risk as well as for the assessment of the rehabilitation of the rehabilitation significance of operations in patients with congenital heart diseases.
