RESUMEN
Treatment results of 1219 patients of elderly and senile age, hospitalized in surgical departments of Turkestan during 2001-2008 were analyzed. Demographic characteristics, spectrum of profile pathology as well as concurrent diseases were given. Operative activity and average hospital stay, hospital lethality and complication rates were defined.

Hemaggregatology was formed as a new trend in blood science in the second part of the 20th century. Hemaggregatology underwent several stages in its development. This gave rise to a theory of the systematic regulation of blood aggregation and a new trend in surgery, such as blood gravitation surgery. Subsequently all instrumental corrective techniques for blood aggregation were integrated into a special part surgery--blood surgery.

More than 400 gravitation plasmapheresis operations conducted in patients with coronary heart disease (CHD) have been analyzed. Plasmapheresis was combined with thrombocytapheresis, erythrocyte oxygenation, plasma sorption and plasma filtration. Removed plasma was substituted for rheologically active solutions and albumin. Gravitation plasmapheresis was used in critical cases when alternative therapeutic methods had failed to provide a favourable outcome. Inclusion of gravitation plasmapheresis into the combined treatment of CHD patients was conductive to decreasing lethality in complicated forms of acute myocardial infarction attended by circulatory insufficiency, stage III-IV, from 82 to 33%. Hypoxia and acytosis are eliminated as a result of normalization of blood circulation and the system of blood aggregation regulation, the functions of the lungs and other internal organs were improved, that was manifested in the clinical course of CHD: pain symptoms disappear, stress tolerance increases, sensitivity to drug therapy rises, lethality decreases.

Plasmapheresis was used to correct blood lipid composition in patients with angina at rest and angina of effort (functional classes 3-4). In addition to a drop in total cholesterol and triglycerides, cholesterol in low and very-low-density lipoproteins fell abruptly, while high-density-lipoprotein cholesterol, on the contrary, increased considerably. Clinical improvement of the patients was also recorded.

Gravitation plasmapheresis (PP) on PF-0.5 fractionator was used in 32 patients with acute myocardial infarction to control blood rheologic disorders. Central hemodynamics and myocardial metabolism were monitored by means of right heart probing and echocardiography. A control group of 7 patients were subjected to "mock PP" with a rheopolyglucin infusion. PP limited manifestations of heart failure, normalized the levels of compounds with high molecular weights which conditioned plasma viscosity, reduced significantly total peripheral and total pulmonary resistance, increased the cardiac index, stroke volume and ejection fraction, and improved regional left-ventricular contractility. Myocardial metabolic studies demonstrated a decline in left-ventricular myocardial ischemia, as evidenced by the arteriovenous difference in lactate levels. It is therefore suggested that PP can be effectively used as part of combined treatment for complicated acute myocardial infarction.

A study of 32 patients with complicated acute myocardial infarction, treated with gravitation plasmapheresis during the acute phase of the disease, demonstrated a significant drop in blood concentrations of compounds with high molecular weights (fibrinogen, cholesterol, triglycerides, total protein) as well as a significant activation of the heart's pumping function, while total peripheral resistance declined, as did the degree of myocardial ischemia. This is interpreted as an evidence of positive effect of plasmapheresis on central hemodynamics, myocardial metabolism and clinical course of myocardial infarction in such patients.

The article gives a generalized experience of using the home equipment for gravitational plasma- and cytapheresis for correcting the disturbances of the aggregate state of the blood in acute myocardial infarction, angina pectoris and rheumatic fever. The possibility of a wide-scale use of these devices in the intensive therapy and resuscitation departments has been shown. The basic tenets of the extracorporeal theory of regulating the aggregate state of blood have been formulated. A possibility of the combined use of blood fractionaters with other equipment for changing biochemical, biophysical and cytologic properties of blood has been shown.

Marked microcirculatory changes, hemorheologic and immunologic disorders were found in anginal patients. Plasmapheresis with repeated thrombocytopheresis sessions as part of combined treatment improves these parameters, the improvement correlating with a stable clinical remission.
