RESUMEN

INTRODUCTION: Data on the digitization in Swiss veterinary practices and clinics were collected in a survey from June 2017 to the end of December 2017. Data of 171 practices contributed to the survey. Animal records were filed in 96.5% with a practice management program. Nine out of ten practices operate an x-ray machine whereof 70% digitally record the radiographs. While a moderate diversity of practice management systems is used, numerous different radiographic recording, archiving and viewing systems are utilized. Data exchange with other practices and owners preferably takes place via e-mail, followed by upload servers and digital data carriers. Data protection receives less attention in veterinary medicine than in comparison to human medicine. A protected data exchange platform coupled with AMICUS and ANIS is under construction via standardized DICOM (https://www.dicomstandard.org/current/) and HL7 (https://www.hl7.org/) interfaces.


INTRODUCTION: De juin 2017 à fin décembre 2017, des données relative à la digitalisation ont été collectées dans le cadre d'une enquête auprès de cabinets et cliniques vétérinaires suisses. Au total, 171 pratiques ont participé à l'enquête. 96,5% conservent les antécédents du patient avec un programme de gestion. Neuf cabinets sur dix utilisent un appareil à rayons X, dont 70% enregistrent numériquement les images. Bien que la variété des systèmes de gestion soit modérée, elle est riche en ce qui concerne l'enregistrement, l'archivage et la visualisation des images. L'échange de données avec d'autres pratiques et propriétaires se fait de préférence par courrier électronique, suivi de serveurs de téléchargement et de supports de données numériques. En comparaison avec la médecine humaine, la protection des données a beaucoup moins d'importance en médecine vétérinaire. Afin de pouvoir effectuer l'échange de données numériques de manière pratique, une plate-forme d'échange de données sécurisée couplée à AMICUS et à ANIS via DICOM normalisé (https://www.dicomstandard.org/current/) et HL7 (https://www.hl7.org/) est en construction.

The objective was to determine the agreement between scrutineers assessing canine hip dysplasia (CHD) within and between the Fédération Cynologique Internationale (FCI) and the Swiss (CH) grading schemes, and between first-opinion and second-opinion scrutineers using the CH scheme. 62 scrutineers, participating in a quality assessment programme, were asked to evaluate 12 sets of hip radiographs. All radiographs had been previously evaluated by a first-opinion and 7 by both a first-opinion and second-opinion scrutineers using the CH scheme. Inter-rater agreement (κ) and intraclass correlation (ICC) were evaluated for the Norberg angle and dysplasia grades, and ICC for the numerical CH scores. The κ and ICC were 0.290 and 0.471 for Norberg angles, 0.372 and 0.577 for FCI grades, and 0.417 and 0.567 for CH grades, respectively. The ICC for CH scores was 0.614. Total agreement for grades was found in a single case. For radiographs evaluated using both schemes, raters gave the same grade in 441 cases, a lower FCI than CH grade in 78 cases and a lower CH than FCI grade in 32 cases. Intrarater agreement between grades of the two schemes was 0.786. Agreement between the first-opinion and second-opinion scrutineers was 0.250 for Norberg angles and -0.286 for CH grades. Although agreement between the two schemes was good, agreement between scrutineers using either scheme was only fair to moderate, and no agreement was found between first-opinion and second-opinion scrutineers. Application of the FCI and CH schemes may impact the incidence of hip dysplasia in populations with both severe and moderate dysplasia, but is unlikely to impact populations with only mild dysplasia. The CH system of first-opinion and second-opinion examinations should be re-evaluated.

INTRODUCTION: Osteochondrosis (OC) is common in large-breed dogs. According to the breeding guidelines of the Swiss kennel clubs, the shoulder joints are included in the radiographic screening for joint diseases in the Greater Swiss Mountain dog (GSMD) and the Border Collie (BC) since 1993 and 2003, respectively. The aim of this study was to estimate the overall prevalence of humeral head OC in these 2 breeds in Switzerland based on the data of the Swiss National Dysplasia Committees. All radiographs were re-evaluated to assess single radiographic changes. From 1993 and 2003, accordingly, until 2013, the overall prevalence was 14% for the GSMD and 8% for the BC, respectively. Affected joints showed a focally reduced opacity or a flattened/indented contour of the caudal section of the humeral head. Articular flaps were only seen occasionally. Degenerative joint disease was significantly more common in OC affected joints (GSMD: 32%; BC: 20%) than in joints without OC. The present study is the first report on the prevalence of humeral head OC in a large cohort of GSMD and the BC over a long study period. In comparison to other breeds, the herein reported prevalences are in the mid to upper range. Results of the present study should alert veterinarians to the disease in these breeds and may serve as a starting point for further epidemiological and genetic studies.


INTRODUCTION: L'ostéochondrose (OC) est fréquente chez les chiens de grande race. Conformément aux prescriptions d'élevage des clubs cynologiques suisses, l'articulation de l'épaule est incluse dans le dépistage radiologique des affections articulaires chez le Grand bouvier suisse (GBS) et le Border Collie (BC) et ceci depuis 1993 respectivement 2003. Le but de la présente étude était d'estimer la prévalence de l'OC de la tête humérale chez ces deux races en Suisse, sur la base des données des commissions nationales suisses de dysplasie. Toutes les radiographies ont été réévaluées pour repérer des altérations radiologiques isolées. Depuis 1993 respectivement 2003 jusqu'en 2013, la prévalence était de 14% chez le GBC et de 8% chez le BC. Les articulations affectées montraient une opacité localement réduite ou un contour aplati/ dentelé de la partie caudale de la tête humérale. Des souris articulaires n'étaient constatées qu'occasionnellement. Les affections articulaires dégénératives étaient significativement plus fréquentes sur les articulations affectées d'OC que sur les autres (GBS: 32%; BC: 20%). La présente étude rapporte pour la première fois la prévalence de l'OC de la tête humérale sur une large cohorte de GBS et de BC sur une longue période. En comparaison avec d'autres races, les prévalences constatées se situent entre la moyenne et le niveau supérieur. Les résultats de cette étude doivent alerter les vétérinaires au sujet de cette affection chez ces races et peut server de pont de départ pour de futures recherches épidémiologiques et génétiques.

RESUMEN

INTRODUCTION: Diffuse idiopathic skeletal hyperostosis (DISH) is a common, non-inflammatory, systemic disease of the spine and the abaxial skeleton in humans and dogs. Spondylosis deformans (SD) must be considered as an important differential diagnosis which in humans, unlike DISH, is always accompanied by degenerative disc disease. In the veterinary literature, usually no difference is made between these diseases. The aim of the present review is to summarize essentials of DISH regarding its definition, etiology, prevalence, clinical findings and therapy in both, the human and dog. In particular, the various classification schemes and the most important differential diagnoses are discussed. Specific aspects of canine DISH are highlighted.


INTRODUCTION: L'hyperostose squelettique idiopathique diffuse (DISH) est une affection systémique non-inflammatoire de la colonne vertébrale et du squelette périphérique chez l'homme et le chien. La spondylose déformante (SD) qui, chez l'homme, contrairement à la DISH, s'accompagne d'une atteinte dégénérative des disques intervertébraux et représente donc une entité pathologique propre constitue un diagnostic différentiel important. Dans la littérature vétérinaire, par contre, on ne distingue souvent pas de façon explicite entre SD et DISH. Le but du présent travail est de donner un aperçu de la définition, de l'étiologie, de la prévalence, de la clinique et du traitement de la DISH chez l'homme et le chien. On discute en particulier les divers schémas de classification et les principaux diagnostics différentiels et on relève particulièrement les aspects spécifiques de l'affection chez le chien.

Alveolar echinococcosis (AE), a parasitic disease primarily of the liver caused by the larval stage of Echinococcus multilocularis, is highly endemic in Switzerland. In contrast to well-established management protocols in people, little is known with regard to optimal treatment strategies in dogs. The objective of this study was to describe the clinical signs and diagnostic procedures in dogs with AE and to evaluate outcome following medical treatment alone or surgery and medical treatment. Of 23 putative AE cases between 2004 and 2014, 20 were classified as confirmed (n=18) or probable (n=2) AE, based on abdominal ultrasound, serology, cytology, histology and/or PCR. Most dogs presented with abdominal distension in an advanced stage of disease. Dogs receiving specific treatment (radical or debulking surgery together with medical treatment, or medical treatment alone) survived longer than dogs left untreated, but no difference was found between treatment types. Survival at one year was associated with absence of free abdominal fluid, absence of abdominal distension and treatment of any type. However, dogs treated with debulking surgery all faced relapse. Findings of this study suggest that in AE-affected dogs for which a therapeutic approach is regarded appropriate by owners and veterinarians, radical surgical resection and medical treatment or, if total resection is not possible, medical treatment alone should be considered. However, studies on larger numbers of dogs are necessary before definitive treatment recommendations can be made.

A two-year-old female Lucerne Hound was presented with a one-week history of signs of progressive neck pain, inappetence, apathy, and an elevated rectal temperature. Findings of magnetic resonance imaging (MRI) were consistent with a foreign body abscess in the epidural space at the level of the first and second cervical vertebrae. A left-sided dorso-lateral atlantoaxial approach was performed, revealing an epidural abscess containing a grass awn. The clinical signs resolved within three days of surgery and the dog made a full recovery. This case report shows that grass awns can migrate to the atlantoaxial region in dogs and MRI findings lead to a suspicion of caudo-cranial migration within the spinal canal.

A 2-years old male Labrador retriever dog was presented with intermittent therapy-resistant diarrhoea, accompanied by vomiting, inappetence, apathy, and mild fever. The blood analysis showed an anaemia, neutrophilia, eosinophilia, and increased liver enzymes. Abdominal palpation was slightly painful. X-rays and echography revealed a severely enlarged liver with multiple cavernous structures. Histopathologic examination of liver biopsies showed a severe chronic granulomatous hepatitis with numerous parasitic cysts. Morphology of the cysts was compatible with the metacestode stage of Echinococcus multilocularis. The dog was only 2-years old at the time of diagnosis. Although alveolar hydatid disease of the liver is rare in dogs, it should be envisaged as a possible differential diagnosis in cases of space-occupying processes in the liver, even in young animals, as the incubation period of this disease in the dog can be considerably shorter than in humans.

RESUMEN

A heifer with developmental skeletal disorder was presented to the ruminant clinic of the University of Berne. Abnormal long bone growth and a wobbling gait were the main clinical signs. All long bones were examined radiologically, several parameters of body size were measured and the results were compared to the measurements of a healthy control animal. Haematology and blood chemistry were normal. Based on the poor prognosis the animal was slaughtered. The final diagnosis of hyena disease was based on the characteristic growth disturbances in the caudal parts of the body, giving the animal a hyena-like appearance. For the first time a case of hyena disease is reported in Switzerland.

RESUMEN

The therapeutic possibilities in veterinary cardiology have developed rapidly in the past few years. Whereas until recently cardiac intervention in dogs could only be performed by thoracotomy, new minimally invasive techniques are adopted. Procedures like balloondilatation of pulmonic stenosis, coil embolisation of patent ductus arteriosus, pacemaker implantation in symptomatic bradyarrhyhtmia, and palliative balloon pericardiotomy are becoming more and more established. These alternative interventional methods are attractive, because no postsurgical pain and no complications potentially associated with thoracotomy ensue. The knowledge of such new treatment modalities and particularly the indications for an intervention are prerequisites to apply them optimally and broadly.

A focal uterine adenomyosis is described in two bitches. In both cases, the uterus showed knobbly enlargements of 4 to 8 cm in diameter, which resulted in distinct clinical symptoms. Other pathological changes of the uterus were not present. One bitch was presented because of a history of vaginal discharge of several months' duration. Radiographs, as well as ultrasonography, revealed a soft tissue lesion at the cervix. The other bitch showed a marked reduction in its general condition and a sudden onset of a tense abdomen. Radiologically, a lesion of soft tissue opacity was observed in the mid-abdomen and was seen to originate from the left uterine horn during exploratory laparotomy. A torsion of the lesion was present, which explained the clinical signs in this second case.

Hirsutism was the most often observed symptom in horses with a pituitary gland tumor and was present in all 13 examined horses. Other symptoms were atrophy of muscles (n = 10), hyperhidrosis (n = 8), polyuria/polydipsia (n = 5), bulging or supraorbital fat (n = 3), polyphagia (n = 2), apathy (n = 2) and seizures (n = 2). Laminitis was the most frequently observed concurrent disease (n = 8). Hyperglycaemia (mean, 9.9 +/- 3.71 mmol/l) in 13 horses and glucosuria (median, 55 [range, 2-55] mmol/l) in 7 horses were the most important laboratory results. The dexamethasone suppression test was positive in all tested horses (n = 9) 20 h after administration of dexamethasone. The pituitary gland tumor was visible in every case underwent computed tomography (n = 7). From these results it can be concluded that a pituitary gland tumor can be suspected based on typical clinical signs. Hyperglycaemia and glucosuria support the preliminary diagnosis and a positive dexamethasone suppression test allows a final diagnosis.

RESUMEN

For the past 2 years computed tomography is used at the Veterinary School, University of Zürich. This new imaging modality enables the detection of abnormalities occurring in small and large animals which were previously not visible with imaging techniques. Subjects of this study were a foal and a small pony, both with suspected head trauma. Routine radiography could not explain any of the neurological deficiencies. In the first case a basilar skull fracture along with a focal brain hemorrhage was detected, in the second case multiple basilar skull fractures were seen. The computed tomographic examination after intravenous injection of an iodine-containing contrast medium also revealed epidural hemorrhage. These two cases demonstrate the diagnostic use of computed tomography as an extremely valuable asset when diagnosing head trauma in horses.

Obtaining high-quality radiographs of the proximal aspect of equine limbs is difficult because of the large muscles in these regions. The use of scintigraphy may provide further information. Abnormal bone scan findings of the ischial tuberosity or the third trochanter were found in 29 adult horses with obscure hind limb lameness between 1986 and 1996 at the Large Animal Clinic of the University of Bern. Each had abnormal radiopharmaceutical uptake but not all had radiographic changes. Radiopharmaceutical uptake ratios between the ischial tuberosity and the greater trochanter were calculated. The uptake ratio in a control group of 11 clinically sound horses was lower than in 11 lame horses with subjectively enhanced radiopharmaceutical uptake.

This report describes the case history of a 1.5-year-old ram admitted to the Clinic for Food Animals and Horses, University of Berne, Switzerland, because of an acute severe lameness of the left rear limb. Clinical and radiographic examination revealed disruption of the stifle joint with rupture of both cruciate ligaments, the medial collateral ligament, and disruption of both menisci from their attachments. The ruptured ligaments were replaced by synthetic ligament prostheses. Long-term examination at 12 months after implantation revealed slight instability of the stifle joint, caused by degeneration of the medial meniscus, and signs of arthrosis deformans. The new formed periarticular connective tissue contributed to the stability of the stifle joint, so that a good functional result was achieved. Necropsy findings at 14 months after implantation confirmed that the synthetic ligament prostheses were intact and the medial meniscus was detached from its insertion and incorporated into the joint capsule. At microscopic examination of the synthetic band prosthesis, ingrowth of collagen and fibrous tissue was evident.

RESUMEN

This case report describes history, clinical, laboratory, radiographic, and endoscopic findings, therapy, and outcome of a 5-year-old cow, suffering from retropharyngeal abscessation. Clinical findings at admission included elevated body temperature, head and neck held in extended position, excessive salivation, swelling of the retropharyngeal area, and slight inspiratory noise. Radiographic and endoscopic examination confirmed the tentative diagnosis of retropharyngeal abscessation. Treatment consisted of parenteral application of procaine-penicillin for 10 days and local application of linseed rubefacients for 2 weeks. At that time, abscessation had developed. The abscess was subsequently surgically, approached by a modified Whitehouse approach. During this procedure, the abscess drained into the oropharynx, allowing interruption of surgery. Five months later, clinical, radiographic, and endoscopic follow-up examination revealed complete healing of the retropharyngeal abscess.
