RESUMEN

In 2005, renal replace treatment (dialysis and transplant) was necessary for about 40,000 people, without being known the number accurate and either their basic characteristics, such as: time in treatment, modality or treatment changes. The presented data cover the 76% of the Spanish population and are the result of the cooperation among technicians of registries, nephrologists and transplant coordinations. 4,125 people started RRT in 2005, the total estimated acceptance rate for renal replacement therapy in adults in Spain was 126 pmp and regarding other European countries it locates us in an intermediate area. The incidence rate seems to keep stable in the last years although there were some differences among communities (from 104 pmp in Castile and Leon to 186 pmp in Canary Islands). Diabetes Mellitus is the most diagnosed cause of renal failure in 2005, more than 20% of patients, followed by vascular diseases. The estimated prevalence of renal replacement therapy in Spain at the end of 2005 was 903 pmp, with important variations among communities (from 806 pmp in Cantabria to 1056 pmp in Valencia Region). The 47% of prevalent RRT patients had a functioning transplant. Mortality on haemodialysis and peritoneal dialysis was 13.7% and 10.8% respectively. Mortality on transplant was 1.3%, one of the lowest values registered so far. Mortality on renal replacement therapy was around 5% among patients from 45 to 64 years, 11% between 65 and 74 years and 19% among the patients older than 75 years.

RESUMEN

Presentation of one case of vesical involvement due to lymphoma in an 82-year old female which presented as a micturition syndrome, abdominal mass and later picture of obstructive uropathy with acute renal function impairment. Diagnosis was achieved by ultrasound and abdominal CAT, cystoscopy with of vesical biopsy, transvaginal mass biopsy and immunohistochemical techniques. The difficulties of a correct differential diagnosis with other vesical neoplasias as well as the need to differentiate primary from systemic vesical lymphoma were discussed, not only because of the separate prognosis but also for the different therapeutical approaches.

RESUMEN

OBJECTIVE: An unusual case of Klinefelter's syndrome with ischemic priapism is described. METHODS: The clinical course of the present case is described and discussed. CONCLUSIONS: This unusual case cannot be completely ascribed to the hormonal changes in Klinefelter's syndrome, although hormone imbalance may have favoured the development of ischemic priapism.

RESUMEN

Presentation of one case of giant urethral lithiasis found accidentally. No associated urethral pathology was identified which could justified the bulbar urethra location or the large size. Lithiasis was removed through urethrolithotomy, closing in one operation the urethral wall.

RESUMEN

Intoxication with paraquat has been rarely described in our country. We present a case of a 66-years-old man admitted to our hospital five hours after an accidental ingestion of a sip of paraquat. Despite therapy with gastric lavage, Füller's Soil, Magnesium Sulphate, liquids and hemoperfusion, the patient developed Multiple Organic Failure and died 48 hours after his hospitalization. Plasmatic levels of the herbicide were very high. Typical clinical data, prognosis factors and therapeutical management of this intoxication are described.

RESUMEN

Fifty cases of brucellosis attending our Service of Internal Medicine during the period 1985-1990 are restrospectively studies. The number of patients in which the potential transmission mechanism and the risk factor involved are unknown is very high, being the percentage higher among women from urban areas (50%). Thus, greater attention should be paid to transmission sources considered unfrequent and to survival of the germ in several biological environments, as well as to consumption of non-pasteurized dairy products. The evolution period of the disease until hospitalization and diagnosis is large (48.6 days), especially in its oligosymptomatic forms. The diagnostic method is based mainly in clinical observations plus agglutinations, with positive hemocultive as the diagnostic method in just 6% of the cases. Treatment with streptomycin and tetracycline is still highly effective.
