RESUMEN

A 6-month-old female Labrador Retriever mix rescued by a local shelter developed respiratory distress and later became tetraplegic. After transferring to a specialty centre, diagnostic imaging (CT and MRI) revealed spina bifida at C3 and dermoid sinuses at the level of C3 and T1. Surgery was performed to remove the dermoid sinuses. The dog was placed on broad-spectrum antibiotics and a tapering anti-inflammatory dose of prednisone, postoperatively. Independent ambulation was regained within 14 days with no recurrence of neurologic clinical signs.

RESUMEN

OBJECTIVE: To determine the percentage of cats with a phenobarbital (PB) concentration between 15 and 45 µg/mL that had a ≥ 50% reduction in the number of seizures and to investigate applicability of the 2011 International League Against Epilepsy (ILAE) classification system in cats. DESIGN: Retrospective case series. ANIMALS: 30 cats with suspected or confirmed epilepsy. PROCEDURES: Medical records for 2004 to 2013 at 3 veterinary hospitals were searched. Information collected included signalment, duration of observation before treatment, frequency of seizures before PB administration, seizure phenotype, dose of PB, serum PB concentration, number of seizures after PB administration, duration of follow-up monitoring, and survival time. A modified 2011 ILAE classification system was applied to all cats. RESULTS: Seizure control was achieved in 28 of 30 (93%) cats with a serum PB concentration of 15 to 45 µg/mL. This comprised 10 of 11 cats with structural epilepsy, 14 of 15 cats with unknown epilepsy, and 4 of 4 cats with presumptive unknown epilepsy. Thirteen cats had no additional seizures after initiation of PB treatment. CONCLUSIONS AND CLINICAL RELEVANCE: Seizure control was achieved in most cats with a serum PB concentration between 15 and 45 µg/mL, regardless of the cause of the seizures. A modified 2011 ILAE classification was applied to cats with seizures and enabled classification of cats without specific genetic testing and without identified structural or inflammatory disease. This classification system should be incorporated into veterinary neurology nomenclature to standardize communication between veterinarians and improve comparisons among species.

OBJECTIVE: To determine whether changes in the cutaneous trunci muscle (CTM) reflex are an early predictor of outcome in dogs with severe acute thoracolumbar intervertebral disc extrusion (IVDE). STUDY DESIGN: Multicenter prospective cohort study. ANIMALS: Dogs (n = 36) with acute IVDE causing paraplegia, loss of nociception in pelvic limbs and tail, and an abnormal CTM reflex postoperatively. METHODS: The caudal border of the CTM reflex was established 24 hours after surgery and at discharge, and was reported as moving cranially, caudally, or staying static. Dogs were re-evaluated at 12-20 weeks and at 7-36 months postoperatively. Outcome was classified as improved or unimproved, successful or unsuccessful, and ascending myelomalacia or not, and compared with early movement of the CTM reflex by construction of contingency tables and performing a Fisher's exact test. RESULTS: By discharge (mean, 4.7 days; SD = 2.10), CTM reflex progression was caudal in 19 dogs, static in 11, and cranial in 6. Five of 6 dogs with cranial movement developed ascending myelomalacia (P < .0001). Seventeen of 19 dogs with caudal movement showed an improvement by 12-20 weeks (P = .0046) and none developed ascending myelomalacia (P = .0013). CONCLUSIONS: Postoperative changes of the caudal border of the CTM reflex are an early indicator of outcome in dogs with severe acute IVDE. Cranial movement of the CTM reflex is significantly associated with the development of ascending myelomalacia. Caudal movement is significantly associated with improvement, but not associated with a long-term successful outcome.
