RESUMEN
Primary pulmonary hypertension is a rare disorder of unknown etiology with a poor prognosis. There is no cure, and drug therapy is effective in only a few patients. Calcium-channel antagonists and anticoagulants are the mainstay of therapy. Prostacyclin therapy leads to significant clinical improvement but its use is restricted due to high cost and complex drug delivery systems. Sildenafil is a selective vasodilator and has been shown to be effective in decreasing pulmonary vascular resistance in animal models of pulmonary hypertension. We report the use of sildenafil in two patients of primary pulmonary hypertension who were refractory to conventional drug therapy.

A case of XXY Klinefelter's syndrome associated with bilateral cryptochidism, -ultiple capillary hemangiomas and vesical telangiectasia is reported. Testicular biopsy revealed Leydig cell hyperplasia with hyalinization of the seminiferous tubular membrane. The patient presented with recurrent hematuria. Excretory urography, urethroscopy and renal angiography were normal. Cystoscopy revealed multiple telangiectasia, which was responsible for the hematuria. To our knowledge this is the first report of multiple capillary hemangiomas and vesical telangiectasia associated with calssical Klinefelter's syndrome.

Hypocalcaemia may manifest with tetany, convulsions and even status eqilepticus. Recognition of underlying hypocalcaemia in convulsions is mandatory because the fits may not be adequately controlled by anti-convulsant drugs which may also aggravate hypocalcaemia. Vitamin D, by relieving hypocalcaemia, reduces the frequency of convulsions and may even eliminate them.

In three cases of dextrocardia with situs inversus affecting a father and his two sons, the father had married his first cousin, the daughter of his father's sister, so that the two sons were the products of a consanguineous marriage. The general and familial incidence of the condition and the genetic influences responsible for it are discussed.
