INTRODUCTION: this paper aims to evaluate the main direct and indirect costs of the first laparotomies and laparoscopies in bariatric surgeries with a clinical-economical retrospective and cross-sectional analysis from 2017 to 2020 at a hospital with specialties besides the basic ones in southern Brazil. METHODS: the study sample included 26 participants. The first 13 laparotomies, and the first 13 laparoscopies performed at the bariatric surgery service of the institution were evaluated. The values evaluated in such comparison analyzed the costs of operation and hospitalization. It is important to highlight that, in addition to the cost benefit, other costs take significance in the health area, such as: cost-utility, cost-effectiveness and cost-minimization, in addition to the cost-opportunity that is reassessed in the observation of the broad context associating all the values raised here. The software used for data analysis was Excel version® 365. The economic analysis was performed evidencing the profile of the patients and the direct and indirect costs involved in each segmentation. RESULTS: the direct and indirect costs of videolaparoscopy amounted to BRL 10,108.10 and laparoscopy to the amount of BRL 12,568.14. CONCLUSION: it was concluded that laparoscopy presents more savings in the aspects of all health valuations to the detriment of laparotomy. It was concluded that the videolaparoscopy presents more savings in the aspects of all health valuations than the laparotomy.

RESUMO Introdução: o presente estudo tem como objetivo avaliar os principais custos diretos e indiretos das primeiras laparotomias e videolaparoscopias em cirurgias bariátricas em uma análise clínica-econômica, retrospectiva e transversal de 2017 a 2020 em um hospital terciário do sul do Brasil. Métodos: a amostra do estudo incluiu 26 participantes. Foram avaliadas as primeiras 13 laparotomias e as primeiras 13 videolaparoscopias realizadas no serviço de cirurgia bariátrica da instituição. Os valores avaliados em tal comparação analisaram os custos da operação e da internação. É importante ressaltar que além do custo-benefício, outros custos tomam significância na área da saúde. São eles: o custo-utilidade, o custo-efetividade e o custo-minimização, além do custo-oportunidade que é reavaliado na observação do contexto amplo associando todas as valorações aqui levantadas. O software utilizado para a análise dos dados foi o Excel® versão 365. A análise econômica foi realizada evidenciando o perfil dos pacientes e os custos direto e indireto envolvidos em cada segmentação. Resultados: os custos diretos e indiretos da videolaparoscopia somaram o montante de R$ 10.108.10 e da laparoscopia o montante de R$ 12.568,14. Conclusão: concluiu-se que a videolaparoscopia apresenta mais economia nas vertentes de todas as valorizações em saúde em detrimento da laparotomia.

Objective: The present study's purpose is to evaluate the economic context in which the Brazilian public health system, the only universal public health system with more than 200 million users, stands out. This evaluation will be made through the lens of the execution of gestational health care services in a city of approximately 500 thousand inhabitants in southern Brazil. The care costs of patients with gestational diabetes mellitus (GDM) will be compared to those of patients without GDM, analyzing the different economic valuation methods. And lastly, there was an intent to explore the generated costs in the context of economic valuation applied to health to comprehend better the complexity of the union of the financial and health areas to optimize the services offered. Methods: For the economic context in health, an analysis of health investments was performed through the Transparency Portal. The costs involved in preventing GDM were raised by the Sistema Único de Saúde (SUS) table of procedures performed ordinarily in low-risk pregnancies. The expenses involved in DMG patients were increased at the High-Risk Pregnancy and Fetal Medicine Clinic of DMG patients. Results: Preventing GDM is more cost-effective, cost-minimizing, and cost-useful than treating patients diagnosed with GDM. Conclusion: The result is an extremely interesting costopportunity, given the economic context in which it is presented
Objetivo: O presente estudo tem como objetivo avaliar o contexto econômico em que se encontra o sistema público de saúde brasileiro, único sistema público universal de saúde com mais de 200 milhões de usuários. Essa avaliação será feita sob a ótica da execução de serviços de saúde gestacional em um município de aproximadamente 500 mil habitantes no Sul do Brasil. Os custos assistenciais de pacientes com diabetes mellitus gestacional (DMG) serão comparados aos de pacientes sem DMG, analisando os diferentes métodos de valoração econômica. Também serão analisados os custos gerados no contexto da valoração econômica aplicada à saúde para uma melhor com preensão da complexidade da união das áreas econômica e da saúde com o objetivo de otimizar os serviços oferecidos. Métodos: Para a contextualização econômica em saúde, foi feita a análise dos investimentos em saúde pelo Portal da Transparência. Os custos envolvidos na prevenção da DMG foram levantados pela tabela de procedimentos realizados ordinariamente em gestações de baixo risco do Sistema Único de Saúde (SUS). Os custos envolvidos em pacientes com DMG foram levantados no Ambulatório de Gestação de Alto Risco e Medicina Fetal de pacientes com DMG. Resultados: Prevenir o DMG apresenta maiores custo-benefício, custo-efetividade, custo-minimização e custo-utilidade em comparação com o tratamento das pacientes com o diagnóstico de DMG. Conclusão: O resultado é um custo-oportunidade extremamente interessante, dado o contexto econômico em que se apresenta

In this study, microparticles of bionanomaterials were obtained by polyvinylpyrrolidone, montmoril-lonite, and zinc oxide bionanosystems produced through solution intercalation technique combined with a spray-drying process, aiming for possible application as drug delivery systems. The final microparticles obtained were evaluated in terms of their production yield, which varies between 39.2% and 56.9%. Thermal analysis showed no major changes in Tg of the nanocomposites, compared to the pure PVP polymer. Scanning electron microscopy analysis revealed a pseudo-spherical shape and confirmed the micrometric size of the microparticles. Transmission electron microscopy analysis corroborated the embedding of montmorillonite and ZnO within the polymer phase. Nuclear magnetic resonance and X-rays diffraction were used to study the nanoparticles dispersion, indicating a predominant intercalated morphology. This study suggests that the applied methodology is suitable for the high yields synthesis of nanocomposites PVP based microparticles with uniform size and shape, which can be promising for the production of a new drug delivery system.

OBJECTIVE: Pneumococcal pneumonia is a significant cause of morbidity and mortality among adults. The study's main aim was to evaluate the in-hospital mortality and related costs of community-acquired pneumococcal pneumonia in adults. METHODS: This cross-sectional study used medical records of adult patients with pneumococcal pneumonia hospitalized in a university hospital in Brazil from October 2009 to April 2017. All patients aged ≥ 18 years diagnosed with pneumococcal pneumonia were included. Risk factors, intensive care unit admission, length of hospital stay, in-hospital mortality, and direct and indirect costs were analyzed. RESULTS: In total, 186 patients were selected. The mean in-hospital mortality rate was 18% for adults aged < 65 years and 23% for the elderly (≥ 65 years). Bacteremic pneumococcal pneumonia affected 20% of patients in both groups, mainly through chronic respiratory disease (adjusted OR: 3.07, 95% CI: 1.23-7.65, p < 0.01). Over 7 years, annual total direct and indirect costs were USD 28,188 for adults < 65 years (USD 1,746 per capita) and USD 16,350 for the elderly (USD 2,119 per capita). CONCLUSION: Pneumococcal pneumonia remains an important cause of morbidity and mortality among adults, significantly affecting direct and indirect costs. These results suggest the need for prevention strategies for all adults, especially for patients with chronic respiratory diseases.

OBJECT: Lumbar discectomy is one of the most common surgical spine procedures. In order to understand the value of this surgical care, it is important to understand the costs to the health care system and patient for good results. The objective of this study was to evaluate for the first time the cost-effectiveness of spine surgery in Latin America for lumbar discectomy in terms of cost per quality-adjusted life year (QALY) gained for patients in Brazil. METHODS: The authors performed a prospective cohort study involving 143 consecutive patients who underwent open discectomy for lumbar disc herniation (LDH). Patient-reported outcomes were assessed utilizing the SF-6D, which is derived from a 12-month variation of the SF-36. Direct medical costs included medical reimbursement, costs of hospital care, and overall resource consumption. Disability losses were considered indirect costs. A 4-year horizon with 3% discounting was applied to health-utilities estimates. Sensitivity analysis was performed by varying utility gain by 20%. The costs were expressed in Reais (R$) and US dollars ($), applying an exchange rate of 2.4:1 (the rate at the time of manuscript preparation). RESULTS: The direct and indirect costs of open lumbar discectomy were estimated at an average of R$3426.72 ($1427.80) and R$2027.67 ($844.86), respectively. The mean total cost of treatment was estimated at R$5454.40 ($2272.66) (SD R$2709.17 [$1128.82]). The SF-6D utility gain was 0.044 (95% CI 0.03197-0.05923, p = 0.017) at 12 months. The 4-year discounted QALY gain was 0.176928. The estimated cost-utility ratio was R$30,828.35 ($12,845.14) per QALY gained. The sensitivity analysis showed a range of R$25,690.29 ($10,714.28) to R$38,535.44 ($16,056.43) per QALY gained. CONCLUSIONS: The use of open lumbar discectomy to treat LDH is associated with a significant improvement in patient outcomes as measured by the SF-6D. Open lumbar discectomy performed in the Brazilian supplementary health care system provides a cost-utility ratio of R$30,828.35 ($12,845.14) per QALY. The value of acceptable cost-effectiveness will vary by country and region.

INTRODUCTION: The state of value-based management of spinal disorders and ongoing Brazilian strategies toward its implementation are highlighted in this article. METHODS: The health care system, economic impact of spine surgery, use of patient-reported outcomes, ongoing studies about health economics, and current strategies toward implementation of quality assessment of spine care in Brazil are reviewed. RESULTS: During the past 20 years, there has been an increase of 226% in the number and 540% in the total cost of spine surgeries in the public health system. Examples of economic regulatory mechanisms involve the process of health technology assessment and the auditing processes imposed by health insurance companies. Some barriers to implementing clinical registries were identified from a large Latin American survey. Strategies based on education and technical support have been conducted to improve the quality of comparative-effectiveness research in spine care. Only 1 cost-utility study on spine care has been published until now. CONCLUSIONS: The paradigm of value-based management of spinal disorders is still incipient in Brazil. Some issues from our analysis must be emphasized: (1) Brazil presents many regional disparities and scarce resources for health care; it is crucial for the health system to allocate resources based on the value of interventions; (2) because of the high economic and social burden of developing new technologies for diagnosis and treatment, research in health economics of spine care in Brazil should be prioritized; (3) these efforts would help to provide a more accessible and effective health system for patients with spinal problems.
