RESUMEN

Ureteral injuries due to gunshots are tend to be misdiagnosed because of concomitant vascular and intraabdominal organ wounds. Our case is a 23-year-old man who was admitted to the hospital with multiple abdominal gunshot wounds. Laboratory findings showed worsening anemia, and the computed tomography (CT) scan showed multiple lead bullets inside the abdomen and retroperitoneum. Patient was then taken to the operation room for laparatomy. There were many intestinal injuries and also a stable retroperitoneal hematoma. There was no ureteral injury mentioned in the operation note or the initial CT report. Two weeks after this operation, the patient developed significant urine leakage from the abdominal wounds. Ureteral J stent placement was our first choice of treatment. This was complicated with a late ureteral obstruction. The final treatment was an ureteroureterostomy. Diagnosing ureteral injuries due to traumatic causes can be hard in most of the cases. Physicians should consider early evaluation of the patient for ureteral leakage if there is suspicion about ureteral injury.

Increases in platelet count and platelet indices such as mean platelet volume (MPV), platelet distribution width (RDW) and plateletcrit (PCT) have been reported in a wide range of pathological settings. The aetiology of varicocele, a vascular disease, is poorly defined. In this study, we aimed to examine the relationship between varicocele, platelet count and a series of platelet indices. A total of 69 patients with varicocele and 56 patients without varicocele were enrolled in the study. Patient sperm parameters, platelet, MPV, PDW and PCT values were analysed. There were semen abnormalities in 37 (53.6%) patients in the varicocele group and 19 (33.9%) patients in nonvaricocele group (P < 0.05). There was no significant difference in platelet count or platelet indices (MPV, PDW and PCT) between the patients with and without varicocele (P < 0.05). There was no statistically significant difference in platelet count or MPV, PDW and PCT between patients with varicocele and the control subjects.

OBJECTIVE: To evaluate the effect of the Valsalva maneuver (VM) during micturition on intraocular pressure (IOP) in patients with benign prostatic hyperplasia (BPH). PATIENTS AND METHODS: IOP were measured before and during micturition in uroflowmetry testing by Tonopen-Avia (Reichert Inc., NY, USA) in 35 eyes of 35 patients with BPH. International Prostate Symptoms Score (IPSS) questionnaire was used for quantification of BPH symptoms. The subjects were divided into two groups according to IPSS questionnaire: patients with low IPSS scores and patients with high IPSS scores. RESULTS: The mean IOP before and during micturition in patients with BPH were 15.26 ± 4.39 mmHg and 16.23 ± 4.43 mmHg, respectively (p = 0.047). When patients with BPH were assigned to two distinct groups according to the IPSS scores, IOP did not differ in patients with low IPSS scores (p = 0.590), whereas mean IOP was significantly higher in patients with high IPSS scores (p = 0.007). CONCLUSIONS: The Valsalva maneuver, performed during micturition, can lead to significant IOP increase in patients with BPH, particularly in advanced disease.
