RESUMEN

THE AIM OF THE STUDY: Was to create a finite element design to evaluate the masticatory stress distribution on the full fixed prosthesis with three supporting implants and the mandibular bone in the conditions most closely resembling the real ones. MATERIALS AND METHODS: The geometric design was created with the results of a CBCT of a real patient. The transfer of the DICOM into STL format was carried out in the Mimics Medical 21 program, in which the author's splinting device for the implant was designed. The actual thicknesses of the patient's compact and cancellous bone layers were measured with Romexis ver. 4.5.1. Scanning of the artificial dentition was performed with the iTero Element 2 Version 1.7 which created an object in STL format. Design was performed in the package for finite element analysis Ansys 19.3. Tetrahedral elements of the second order were used for calculation. The structural elements were connected in the model by a rigid «Bonded¼ contact. RESULTS: A finite element model has been created for the experimental load design on the lower jaw for immediate implant restoration with few supports in combination with a device for stabilizing the implant in unfavorable clinical conditions. It is shown that the designed finite element model takes into account the biomechanics of the prosthesis with few supports. When calculating, the entire model was fixed to the mandibular process at its base, and was loaded with a force of 500 N, distributed over the teeth. The values of the maximum stresses obtained in the elements were 51.8 MPa for the compact layer and 245 MPa for titanium elements. CONCLUSION: Thus, we created personalized geometric and finite element models for calculating the distribution of functional load on a long full fixed implant-supported prosthesis with few artificial supports, which makes the experimental results more accurate and correct.

OBJECTIVE: To study the effect of the occlusal surface of a natural tooth, cement-retained and screw-retained fixed partial dentures supported by dental implants and the coefficient of friction on the stress distribution in the peri-implant bone at maximum and minimum principal stresses. MATERIAL AND METHODS: Study of maximum and minimum principal stresses in models with natural teeth and artificial crowns supported by dental implants, taking into account the coefficient of friction using the finite element analysis. RESULTS: In models represented by fixed partial dentures supported by dental implants and a natural tooth, the maximum tensile stresses arise in the cortical bone in the cervical region of the artificial crown, and the maximum compressive stresses occur both in the cortical layer in the cervical region of the artificial crown and in the cervical region of the tooth. In models with two fixed partial dentures supported by dental implants or two natural teeth, the stress distributions in the cortical layers in the upper and lower jaw are almost identical. CONCLUSION: Modeling the antagonist and adding to the FEA model is important in order to determine the precise and realistic direction of the resulting force vector. Amplification of the number of contact areas should be considered when modeling the occlusal surface of artificial crowns supported by dental implants.

OBJECTIVE: To study the effect of morphological parameters of natural teeth and fixed partial dentures supported by dental implants on the time of occlusion. MATERIAL AND METHODS: The study of the morphological parameters of the first right upper and lower molars of natural teeth and fixed partial dentures supported by dental implants in the positions of the first molars of the upper and lower jaws, followed by determination of the relationship between the obtained data and the occlusion time using mathematical and statistical analysis. RESULTS: The study of the morphology of the occlusal surface of natural teeth and fixed partial dentures supported by dental implants did not reveal significant differences in the results of measuring the distances between the cusps of the upper and lower first molars, both natural teeth and fixed partial dentures supported by dental implants. However, the angles of the slopes of the cusps and the angles between cusps in natural teeth are sharper than those of artificial crowns. CONCLUSION: According to the correlation analysis in both groups, the relationship between the occlusion time and the morphological parameters of the lower molars was determined; the relationship between the morphological parameters of the upper molars and the occlusion time was not revealed.

RESUMEN

The article proposes a methodology for creating an assessment of the quality of dental care based on the use of the method of analysis of hierarchies and the theory of fuzzy logic. Approbation of the methodology was carried out on the assessment of the quality of dental services for the manufacture of metal-ceramic crowns. When conducting a new assessment, you can achieve a reduction in the time of its conduct by 6 times, increasing the number of evaluation criteria by 2 times and get results with a confidence factor (p=0.95). Evaluation allows you to combine existing descriptive, quantitative, radiological, functional assessments into a system and rank their significance for the final result. This allows assessing the quality of treatment more objectively.

The article the literature review on dental materials for the manufacture of gum formers used in dental implantology. Adhesion of microorganisms to titanium, PMMA and PEEK resins is discussed. According to published studies PEEK polymer is characterized by a similar bacterial contamination compared to titanium but shows lower degree of contamination when compared to acrylic resins in equal conditions.

RESUMEN

The paper presents statistical data on implant placement procedures in Central Research Institute of Dentistry and Maxillofacial Surgery (Moscow, Russia) in 2010-2015. In 64% of cases inadequate bone volume was attributed to alveolar bone atrophy. Bone deficiency was equally often in upper and lower jaws (in 49.3 and 50.7%, correspondently) but varied in forms with complex configurations to be more specific for maxilla. The study also includes a series of clinical cases illustrating implant placement procedures in anatomically unfavorable settings.

RESUMEN

Jaw fragment with complete absence of 3 teeth in the chewing area was simulated with masticatory loads corresponding to different types of antagonists: full dentures, partial dentures, intact teeth, implant-refained restavations. Simulated consoled elements of different lengths and sizes of dental implants. The analysis of the stress-deformation state of the teeth construction in the ANSYS program. As a result, it was found that with increasing length of the cantilever element 2 times - stress in the bone grows almost linearly. By reducing the diameter of the implant - is quadratic.

The comparative analysis of efficiency of various types of diagnostic templates for planning orthopedic treatment of 50 patients with partial or complete absence of teeth was performed. The patients were divided in 3 groups according to template manufacturing technique. All diagnostic templates made in accordance with clinical conditions allowed transfering digital data of the scheduled prosthetic construction for virtual implantation planning. Advantages and pitfalls of each template type are discussed.

RESUMEN

The aim of the study was to evaluate the marginal fit changes in implant supported prosthetic bars by electroerosive processing. The experimental model included 9 bars supported by 2, 4 and 6 implant analogues (3 bars for each number of supports). Marginal gap size was assessed by digital microscopy before and after electroerosive processing conducted in 3 stages (10, 8 and 5 minutes long). Electroerosive processing proved to be useful tool for marginal fit correction.

RESUMEN

Microhemodynamics by gingival mucosa contour formation in implant placement sites was studied by laser doppler flowmetry. When installing healing abutment customized by Estelite Flow Quick noted reduction of tissue blood flow was revealed indicating the development of ischemia in the gingival mucosa. Conventional blood flow pattern restored in 7 days.

RESUMEN

Materials for custom-made impression trays used for impression by implant fixed restorations were compared in the study. The analysis included such values as flexural strength and elasticity modulus, impression material adhesion strength with the use of adhesive and without it. Light-cured plastic Elite LC Tray had the best rates of bending strength and elasticity modulus and the Protakril M had the highest adhesion strength both with and without adhesive.

RESUMEN

Microcirculation dynamics in marginal gingiva by its contouring after second stage of dental implantation was examined by means of laser Doppler flowmetry. After installation of the 5.5 mm flared healing abutments there was a significant reduction in tissue blood flow indicating severe ischemia in marginal gingiva. Efficient prosthodontic treatment caused beneficial impact on microcirculation.
