RESUMEN

OBJECTIVE: To assess the efficacy and safety of our 4-week caregiver-mediated exercise (CME) in improving trunk control capacity, gait, and balance and in decreasing concerns about post-stroke falls when there is an increase in its efficacy. METHODS: Acute or subacute stroke survivors were assigned to either the trial group (n=35) or the control group (n=37). Changes in Modified Barthel Index (MBI), Functional Ambulation Categories (FAC), Berg Balance Scale (BBS), and Trunk Impairment Scale (TIS) scores at 4 weeks from baseline served as primary outcome measures. Correlations of primary outcome measures with changes in Fall Efficacy Scale-International (FES-I) scores at 4 weeks from baseline in the trial group served as secondary outcome measures. Treatment-emergent adverse events (TEAEs) served as safety outcome measures. RESULTS: There were significant differences in changes in MBI, FAC, BBS, TIS-T, TIS-D, TIS-C, and FES-I scores at 4 weeks from baseline between the two groups (all p<0.0001). There were no significant (p=0.0755) differences in changes in TIS-S scores at 4 weeks from baseline between the two groups. MBI, FAC, BBS, and TIS scores showed significantly inverse correlations with FES-I scores in patients receiving CME. There were no TEAEs in our series. CONCLUSION: CME was effective and safe in improving the degree of independence, ambulation status, dynamic and static balance, trunk function, and concerns about post-stroke falls in stroke survivors.

PURPOSE: The present study investigated the prevalence and risk factors of adhesive capsulitis of the shoulder in breast cancer patients between 13 and 18 months after surgery. METHODS: This study included 271 women who underwent surgery for breast cancer with a postoperative period of 13-18 months. Current adhesive capsulitis was defined as restriction of external rotation and one or more additional directional restrictions with history of shoulder pain. Cumulative adhesive capsulitis was defined as current adhesive capsulitis or a previous history of adhesive capsulitis after breast cancer surgery. Multivariate logistic regression analysis was performed to examine associations between current or cumulative adhesive capsulitis and potential risk factors. RESULTS: Among the 271 study patients, 28 (10.3%) and 21 (7.7%) had cumulative or current adhesive capsulitis, respectively. The incidences of cumulative and current adhesive capsulitis were higher in those aged 50-59 years (odds ratio [OR], 9.912; 95% confidence interval [CI], 1.790-54.880; and OR, 12.395; 95% CI, 1.187-129.444, respectively) and those who underwent mastectomy (OR, 6.805; 95% CI, 1.800-25.733; and OR, 9.645; 95% CI, 2.075-44.829, respectively) or mastectomy with reconstruction (OR, 13.122; 95% CI, 2.488-69.218; and OR, 20.075; 95% CI, 2.873-140.261, respectively). CONCLUSIONS: Adhesive capsulitis of the shoulder is a common problem after breast cancer treatment. An age of 50-59 years and mastectomy are major risk factors for adhesive capsulitis, and breast reconstruction additionally increases the risk. Patients with these risk factors require greater attention for early diagnosis and proper treatment.

RESUMEN

Tuning the bandgap in ferroelectric complex oxides is a possible route for improving the photovoltaic activity of materials. Here, we report the realization of this effect in epitaxial thin films of the ferroelectric complex oxide Bi3.25La0.75Ti3O12 (BLT) suitably doped by Fe and Co. Our study shows that Co (BLCT) doping and combined Fe, Co (BLFCT) doping lead to a reduction of the bandgap by more than 1 eV compared to undoped BLT, accompanied by a surprisingly more efficient visible light absorption. Both BLCT and BLFCT films can absorb visible light with a wavelength of up to 500 nm while still exhibiting ferroelectricity, whereas undoped BLT only absorbs UV light with a wavelength of less than 350 nm. Correlated with its bandgap reduction, the BLFCT film shows a photocurrent density enhanced by 25 times compared to that of BLT films. Density functional theory calculations indicate that the bandgap contraction is caused by the formation of new energy states below the conduction bands due to intermixed transition metal dopants (Fe, Co) in BLT. This mechanism of tuning the bandgap by simple doping can be applied to other wide-bandgap complex oxides, thereby enabling their use in solar energy conversion or optoelectronic applications.

The wide band gap of complex oxides is one of the major obstacles limiting their use in photovoltaic cells. To identify an effective route for tailoring the band gap of complex oxides, this study examined the effects of cobalt and iron doping on lanthanum-modified Bi4Ti3O2-based oxides synthesized using a solid reaction. The structural and optical properties were analyzed by X-ray diffraction and ultraviolet-visible absorption spectroscopy. As a result, the optimal iron to cobalt doping ratio in bismuth titanate powder resulted in an ~1.8 eV decrease in the optical band gap. This new route to reduce the optical bandgap can be adapted to the synthesis of other complex oxides.

In recent years, there has been increasing interest in the bandgap engineering of ferroelectric oxides to improve absorbance of the solar spectrum, which is governed by their band gap. To enhance the photovoltaic efficiency by tuning the optical bandgap of complex oxides, an attempt was recently made to reduce the optical band gap of iron doping of lanthanum-modified Bi4TiO12-based oxides (Fe-BLT) using oxygen vacancy doping. To study the tunability of the optical band gap from the generation of oxygen vacancies, the thermal treatment time and temperature were controlled during heat treatment under a vacuum environment. The structural, optical properties of the synthesized podwers were examined by X-ray diffraction, scanning electron microscopy, and ultraviolet-visible spectroscopy. Typically, an oxygen vacancy in a complex oxide can alter their structure very easily. On the other hand, the ultraviolet-visible absorption spectra of iron-doped bismuth titanate ceramics under optimal conditions (12 h, 800 °C) showed a decrease in optical bandgap. from 2.02 eV to 1.8 eV without a corresponding change in their crystallographic structure. This study suggests that optimal control of the thermal treatment time and temperature critically effects the optical band gap of complex oxides.

Achieving wide band gap tunability in ferroelectric perovskite oxides is desirable for the development of photovoltaic device applications and solar cells. To tune the wide band gap of perovskite oxides, ferroelectric iron doped BLT thin films (BLFT) on SrTiO3 substrates were fabricated by RF sputtering with simple control of the oxygen content. The structural and optical properties were analyzed by X-ray diffraction, scanning electron microscopy and ultraviolet-visible absorption spectroscopy. As the oxygen content in the mixed Ar+O2 atmosphere was increased from 0% to 50%, the optical band gap of the thin films were decreased from 2.8 eV to 2.64 eV. The BLFT film deposited at an Ar/O2 ratio of 1/1 exhibited a significantly lower optical band gap than the other samples. This simple sputtering approach to controlling the band gap with a simple method can provide a new candidate tool for manipulating optoelectronics devices.

OBJECTIVE: To analyze publication rate, time to publication and the characteristics of the abstracts presented at the annual Korean Academy of Rehabilitation Medicine (KARM) meetings. METHODS: A total of 1,027 abstracts presented at the 2008 and 2009 annual KARM meetings were enrolled in the database and searched for their subsequent citation in PubMed, KoreaMed, and Google Scholar. RESULTS: The data analysis revealed that 317 (30.87%) abstracts, were published as full-length journal articles and publication rates by subject were not significantly different. The mean time to publication was 17.17±10.48 months, and the journals written in English (20.39±10.20) required a longer duration than those written in Korean (11.94±8.44) with statistical significance (p<0.001). There was no statistical difference (p=0.284) in the duration between domestic (17.61±10.37) and foreign (16.48±10.51) of the 220 domestic journal articles, 190 (86.76%) were published in the Annals of Rehabilitation Medicine and the 97 articles published in foreign journals were scattered over 60 different journals. CONCLUSION: The publication rate of abstracts presented at the 2008 and 2009 annual KARM meetings was 30.87% and the mean time to publication was 17.17±10.48 months.

OBJECTIVE: To investigate the therapeutic effects of mechanical horseback riding for gait and balance parameters in post-stroke patients. METHOD: This study was a non randomized prospective positive-controlled trial over a 12 week period. From May 2011 to October 2011, 37 stroke patients were recruited from our outpatient clinic and divided into two groups. The control group received the conventional physiotherapy while the intervention group received the conventional physiotherapy along with mechanical horseback riding therapy for 12 weeks. Outcome measurements of gait included the Functional Ambulation Category (FAC) and gait part of the Performance Oriented Mobility Assessment (G-POMA) while those of balance included the Berg Balance Scale (BBS) and the balance part of the Performance Oriented Mobility Assessment (B-POMA). These measurements were taken before and after treatment. RESULTS: There were no significant differences in the baseline characteristics and initial values between the two groups. When comparing baseline and 12 weeks post treatment in each group, the intervention group showed significant improvement on BBS (39.9±5.7 → 45.7±4.8, p=0.001) and B-POMA (10.4±2.6 → 12.6±2.1, p=0.001), but significant improvement on gait parameters. When comparing the groups, the dynamic balance category of BBS in post treatment showed significant difference (p=0.02). CONCLUSION: This study suggests that mechanical horseback riding therapy may be an effective treatment tool for enhancing balance in adults with stroke.
