INTRODUCTION: Maintenance of dental implant with different hygiene methods or instruments may cause a surface alteration. It directly affects bacterial colonization and adhesion on titanium implant surfaces that result in peri-implant diseases. This study aimed to compare the Streptococcus sanguinis (S. sanguinis) adhesion on titanium implant abutments after instrumentation with a rubber cup with pumice and erbium, chromium-doped: yttrium, scandium, gallium, and garnet (Er, Cr: YSGG) laser using scanning electron microscope (SEM) observation and colony-forming unit (CFU) measurement. METHODS: Twenty-one MegaGen titanium implant abutments were randomly distributed into three groups. Seven abutments were respectively selected for the control/untreated (C) group, while the other two groups were treated with rubber cups with pumice (P) and Er, Cr: YSGG laser (L). All samples were cultured with S. sanguinis for bacterial colonization and adhesion. One sample for each group was selected for SEM observation, while the other samples were prepared for CFU calculation. RESULTS: For SEM results, at 2,000× magnification, machining marks were intact in the C group, roughened in the L group, and smoothened in the P group. At 5,000× and 10,000× magnifications, moderate colonies of S. sanguinis were revealed in C and L groups, while sparse bacterial colonies were detected in the P group. However, for CFU results, statistical analysis showed no significant value (p>0.05) comparing all three groups. CONCLUSION: P instrumentation revealed a lesser amount of S. sanguinis adhesion in SEM photographs, but no statistical significance of CFU results was noted for all three groups.

Oral health is crucial for young children, yet plaque and gingivitis pose significant challenges. This review evaluates oral health interventions for children under seven years to identify effective strategies. A systematic review was conducted across multiple databases using predefined criteria. Thirteen thousand five hundred records were identified, with 13 studies meeting the inclusion criteria. Various interventions were assessed, including tactile models, digital tools, fluoride varnish, and education programs. The meta-analysis showed heterogeneity in outcomes, with some interventions significantly reducing plaque and gingivitis. Tactile models and digital tools demonstrated effectiveness, particularly among children who were visually impaired and had congenital heart defects. However, fluoride varnish showed mixed results. School-based interventions and advanced toothbrushing technologies effectively reduce early childhood caries and plaque. Despite publication bias, a low risk of bias was observed across studies. The findings underscore the importance of tailored interventions and collaboration among stakeholders. Comprehensive oral health education, innovative digital tools, and multifaceted approaches are recommended to promote lifelong oral health habits. Further research is needed to standardize protocols and assess long-term effectiveness. Evidence-based interventions can significantly improve oral health outcomes for children under seven, laying the foundation for lifelong oral health.

Introduction Tooth sensitivity, or dentin hypersensitivity (DH), is characterized by sharp, sudden pain in response to stimuli such as cold, heat, sweet, or acidic foods and drinks. In Malaysia, there is limited understanding of the epidemiological aspects of tooth sensitivity, necessitating focused research. The condition results from the exposure of dentinal tubules transmitting stimuli to nerves within the pulp, with contributing factors including gingival recession, enamel erosion, and periodontal disease. This study aims to investigate the factors associated with tooth sensitivity among patients at the Hospital Universiti Sains Malaysia (USM) using advanced statistical methods. Methods This study employed a computational research design to develop an ordinal regression and bootstrap methodology using the RStudio software (Posit PBC, Boston, MA) to analyze secondary data from the Hospital Universiti Sains Malaysia. Six variables were analyzed: tooth wear severity, patient's age, gender, smoking status, alcohol status, and type of toothbrush. The study was conducted in three phases: 1) the development of an ordinal regression model, 2) the development of algorithms for ordinal regression and bootstrap method, and 3) validation using tooth sensitivity data. Results The analysis revealed that the replication with 1000 samples provided the most precise estimates with small standard errors (SE) and consistently significant effects across variables. Tooth sensitivity was influenced by age, toothpaste type, toothbrush type, and brushing frequency. Conclusion The study highlights the importance of considering multiple variables such as age, toothpaste type, toothbrush type, and brushing frequency in understanding tooth sensitivity. The combined ordinal regression and bootstrap technique significantly improved the model's accuracy, providing valuable insights for dental health professionals. These findings underscore the need for specific guidelines on oral hygiene practices to manage and reduce the risk of tooth sensitivity.

This study aims to observe the effect of electrospun scaffolds loaded with taraxasterol microspheres on the proliferation and differentiation of osteoblasts. Taraxasterol microspheres were prepared by desolventization and electrostatic adsorption technology. The structure of the microspheres was observed under transmission electron microscopy. The drug microspheres were loaded into electrospinning using electrospinning technology, followed by scanning electron microscopy and elemental analysis. Osteoblasts were cultured in vitro, and the effects of drug carriers on osteoblast proliferation and differentiation were observed through Cell Counting kit 8 (CCK-8) cell proliferation detection and alkaline phosphatase activity detection. Transmission electron microscopy showed that the prepared drug microspheres have a double-layer structure, which can effectively reduce the sudden release of drugs. The electrospinning had a porous three-dimensional structure between them, which was conducive to cell adhesion. After loading microspheres, there was a significant difference in electrospinning diameter and nodular protrusions, energy dispersive spectroscopy (EDS) elemental analysis showed that the proportion of nitrogen elements increased significantly after the addition of microspheres, a CCK-8 detection showed that drug carrier scaffolds loaded with taraxasterol had a promoting effect on osteoblast proliferation (P<0.05). Alkaline phosphatase detection showed that drug carrier scaffolds loaded with taraxasterol can promote early differentiation of osteoblasts (P>0.05). Electrospinning loaded with taraxasterol microspheres can promote osteoblast adhesion, proliferation, and differentiation.

Pseudomonas aeruginosa is a major nosocomial pathogen that causes severe disease including sepsis. Carbapenem-resistant P. aeruginosa is recognised by the World Health Organisation as a priority 1 pathogen, with urgent need for new therapeutics. As such, there is renewed interest in using bacteriophages as a therapeutic. However, the dynamics of treating pan-resistant P. aeruginosa with phage in vivo are poorly understood. Using a pan-resistant P. aeruginosa in vivo infection model, phage therapy displays strong therapeutic potential, clearing infection from the blood, kidneys, and spleen. Remaining bacteria in the lungs and liver displays phage resistance due to limiting phage adsorption. Yet, resistance to phage results in re-sensitisation to a wide range of antibiotics. In this work, we use phage steering in vivo, pre-exposing a pan resistant P. aeruginosa infection with a phage cocktail to re-sensitise bacteria to antibiotics, clearing the infection from all organs.

Primarily sourced from Asteraceae family herbs such as the Dandelion, Taraxasterol is a pentacyclic triterpenoid lauded for its extensive biological functionalities. Its therapeutic potency is demonstrated in various disease models, encompassing enteritis, arthritis, acute hepatic injury, and pneumonia. Scientific literature underscores its anti-inflammatory, antioxidant, and antineoplastic attributes. The primary aim of this study is to thoroughly explore the diseasemodulating mechanisms and effects of taraxasterol. We endeavor to provide an exhaustive review of the experimental subjects, intervention components, distinct action modalities, contributing factors, and protein pathway expressions associated with taraxasterol, systematically represented via diagrams and tables. Such a schematic representation encourages a continued academic dialogue concerning taraxasterol's pharmacological characteristics. This review is envisioned as a practical guide for the selection of experimental subjects and methodologies in prospective research. It is intended to further illuminate taraxasterol's pharmacodynamics, thereby offering theoretical and empirical justification for its clinical application.

Peri-implant diseases can still develop despite oral hygiene practices being maintained. Consequently, regular debridement must be carried out to ensure the implant is sustained. This review evaluated bacterial colonization on implants following the use of different hygiene instruments. A literature search was conducted in PubMed, ScienceDirect, and Scopus databases for articles published from 2012 to 2022. A total of 19 full-text papers were selected. The number of bacteria colonized was most commonly evaluated with a scanning electron microscope (SEM) or by colony-forming unit (CFU) counts, crystal violet assays, plaque index, probing depth, bleeding on probing, turbidity test, and live-dead assays. Rubber cup polishing with an abrasive paste showed a significantly greater reduction in biofilm formation compared with air abrasion with glycine powder, while the air abrasion treatment was found to be more efficient than piezoelectric, carbon, and stainless steel scalers. Surface treatment with Er, Cr: YSGG laser, and Er: YAG laser resulted in statistically significant superior dental biofilm removal compared with titanium curettes and photodynamic therapy. Air abrasion, plastic curette, titanium curette, and ultrasonic scaler showed no significant differences in bacterial colonization, but air abrasion and plastic curette were safer for zirconia implant decontamination. Furthermore, the titanium brush showed better results in decontaminating the implant surface than the Er: YAG laser. Although no single instrument or method could be considered as offering a gold standard in treating peri-implant diseases, the use of air abrasion with glycine powder, laser therapies, rubber cup polishing with an abrasive paste, and a titanium brush had high levels of cleaning efficacy and acceptance by patients.

Peri-implant disease is usually caused by the accumulation of dental biofilm around the implant, and this biofilm can irradiate the gingiva tissue, which leads to inflammation and, more severely, to a deterioration of the bone structure. There is a concern regarding the removal of biofilm from the implant surface by using different hygiene instruments. Some hygiene instruments may have some effect on the dental implant surface, resulting in roughening or damage to the implant surfaces. This study reviewed the effects of titanium implant surfaces on different hygiene instruments. A literature search was conducted from PubMed, ScienceDirect, and Scopus databases for articles published from 1992 to 2021. A total of 19 full-text papers with keywords of interest that met all the eligibility criteria were selected. Surface roughness was evaluated with a scanning electron microscope and also using a profilometer, laser scanning, scanning probe, and atomic force microscopes. A metal curette produced a roughened surface on the titanium implant, but a plastic curette did not alter the surface. Instrumentation with rubber cups left the surface unchanged and appeared to smoothen the surface, whereas the air-powder abrasive instrumentation altered the surface with the presence of micro pits and pores. A conventional metal ultrasonic scaler showed significant surface topographical changes and scratches on both titanium surfaces, as a diode laser, light-emitting diode (LED), and laser treatment did not show any alteration on the rough and smooth titanium surfaces. Thus, a non-metallic instrument such as a plastic curette, rubber cups, and novel technology including diode laser, LED, and laser treatment is appropriate and can be used for debridement on smooth and machined titanium implant surfaces as well as sandblasted and acid-etched (SLA), titanium plasma-sprayed (TPS), and resorbable blasted media (RBM) surfaces. The use of metallic instruments should be avoided, and it is not recommended.

To investigate the presence and/or agenesis of third molar (M3) tooth germs in orthodontics patients in Malaysian Malay and Chinese population and evaluate the relationship between presence and/or agenesis of M3 with different skeletal malocclusion patterns and sagittal maxillomandibular jaw dimensions. Pretreatment records of 300 orthodontic patients (140 males and 160 females, 219 Malaysian Malay and 81 Chinese, average age was 16.27±4.59) were used. Third-molar agenesis was calculated with respect to race, genders, number of missing teeth, jaws, skeletal malocclusion patterns and sagittal maxillomandibular jaw dimensions. The Pearson chi-square test and ANOVA was performed to determine potential differences. Associations between various factors and M3 presence/agenesis groups were assessed using logistic regression analysis. The percentages of subjects with 1 or more M3 agenesis were 30%, 33% and 31% in the Malaysian Malay, Chinese and total population, respectively. Overall prevalence of M3 agenesis in male and female was equal (P>0.05). The frequency of the agenesis of M3s is greater in maxilla as well in the right side (P>0.05). The prevalence of M3 agenesis in those with a Class III and Class II malocclusion was relatively higher in Malaysian Malay and Malaysian Chinese population respectively. Using stepwise regression analyses, significant associations were found between Mx (P<0.05) and ANB (P<0.05) and M3 agenesis. This multivariate analysis suggested that Mx and ANB were significantly correlated with the M3 presence/agenesis.

PURPOSE: The level of the implant above the marginal bone and flap design have an effect on the bone resorption during the healing period. The aim of this study is to detect the relationship between the level of the implant at the implant placement and the bone level at the healing period in the mesial and distal side of implants placed with flapless (FL) and full-thickness flap (FT) methods. METHODS: Twenty-two nonsubmerged implants were placed with the FL and FT technique. Periapical radiographs were taken of the patient at implant placement, and at 6 and 12 weeks. By using computer software, bone level measurements were taken from the shoulder of the healing cap to the first bone implant contact in the mesial and distal side of the implant surface. RESULTS: At 6 weeks, the correlation between the crestal bone level at the implant placement and crestal bone level of the FT mesially was significant (Pearson correlation coefficient=0.675, P<0.023). At 12 weeks, in the FT mesially, the correlation was nonsignificant (Spearman correlation coefficient=0.297, P<0.346). At 6 weeks in the FT distally, the correlation was nonsignificant (Pearson correlation coefficient=0.512, P<0.107). At 12 weeks in the FT distally, the correlation was significant (Spearman correlation coefficient=0.730, P<0.011). At 6 weeks in the FL mesially, the correlation was nonsignificant (Spearman correlation coefficient=0.083, P<0.809). At 12 weeks in the FL mesially, the correlation was nonsignificant (Spearman correlation coefficient=0.062, P<0.856). At 6 weeks in the FL distally, the correlation was nonsignificant (Spearman correlation coefficient=0.197, P<0.562). At 12 weeks in the FL distally, the correlation was significant (Pearson correlation coefficient=0.692, P<0.018). CONCLUSIONS: A larger sample size is recommended to verify the conclusions in this preliminary study. The bone level during the healing period in the FT was more positively correlated with the implant level at implant placement than in the FL.

PURPOSE: To detect the correlation between crestal bone resorption and implant stability during healing period using resonance frequency analysis (RFA). MATERIALS AND METHODS: Twenty-two International Team for Implantology Straumann implants were placed in the posterior maxilla or mandible in 9 patients. RFA reading was taken immediately after implant placement. Periapical radiographs were taken, and the distance from the shoulder of the healing cap to the first bone-implant contact was measured, and the average mesial and distal distances were taken. Patients were followed up at 6 and 12 weeks for data collection. RESULTS: At 6 weeks, the correlation between crestal bone resorption and implant stability was significant (Spearman correlation test, P < 0.05) and negative correlation coefficient (r) was -0.522. At 12 weeks, the correlation between crestal bone resorption and implant stability was not significant (Spearman correlation test, P > 0.05) and negative correlation coefficient (r) was -0.119. CONCLUSIONS: There was a significant negative correlation between the crestal bone resorption and implant stability at 6 weeks, whereas the negative correlations between the crestal bone resorption and implant stability at 12 weeks were nonsignificant.
