RESUMEN

An application of sequential automated SPE separation equipment coupled to the quadrupole-based ICPMS instrumentation with a dynamic reaction cell such as a screening test system of (90)Sr and Pu isotopes in environmental samples was developed in this work. So far, during the course of a large number of reports as to various specific radioactivities in environmental samples surveyed at radioactive contaminated area around the Fukushima Daiichi Nuclear Power Plants (FDNPP), there is a much smaller number of reports on (90)Sr and Pu isotopes than that of (134)Cs and (137)Cs since the FDNPP accident, and then it would be expected to develop the simple analysis method of these isotopes instead of radiation measurements currently in use. In particular, a screening for (90)Sr in environmental samples has been accomplished using an isotopic ratio measurement mode in comparison with the characterization on the Solid Phase Elution (SPE) separation between strontium and zirconium isotopes around the mass-90 fraction. As a result, for a trial analysis of environmental samples of a muddy snow water and a soil which were collected at Fukushima, it was found that the present developed system makes it applicable for achieving up to the specific activity levels of several hundreds Bq/kg ((90)Sr) and about 1-2Bq/kg (Pu isotopes) as the screening test system.

Phthalocyanine (Pc)-dyed fiber is reported to reduce atopic symptoms in some patients when they use underwear made of the fiber. We investigated the adsorption of allergens on Pc-fiber. Pc-fiber trapped house dust/pollen/food allergens with varied molecular weight and pI. The adsorbed allergens were released in the presence of mild detergent. Pc-fiber did not change the molecular weight or disulfide bonding of the allergens. These observations imply that Pc-fiber is applicable as an "allergen trap" for a wide variety of products.

BACKGROUND AND STUDY AIM: When a large flat colorectal tumor is excised by piecemeal endoscopic resection, it is not possible to obtain suitable specimens for histopathological examination to assess whether the resection has been complete. We prospectively analysed follow-up colonoscopy examinations of endoscopic polypectomy resection sites for residual lesions. PATIENTS AND METHODS: A total of 24 patients with large flat colorectal tumors were treated using an endoscopic submucosal saline injection technique. The resection site was prospectively examined for residual lesions using a magnifying colonoscope, at 3, 6, 12 and 24 months postoperatively. RESULTS: None of the 5 patients who underwent en bloc resection exhibited residual lesions postoperatively. Of the 19 patients who underwent piecemeal resection, one died of an asthma attack and 18 were followed up. Residual lesions were detected in four of these 18 patients (three adenomas and one cancer), which were resected endoscopically. When the tumor was resected endoscopically en bloc and was judged histologically to be completely resected, residual lesions were not detected at follow-up. However, residual lesions were detected in 22.2 % of patients 3 months after initial resection when polyps were resected piecemeal. After treatment, residual lesions were detected in two patients (11.1 %) at 24 months. CONCLUSION: After piecemeal endoscopic resection for large flat colorectal tumors, it is necessary to follow-up and remove residual lesions endoscopically every 3 months until they all are removed.

Secondary infections (SI) in skin lesions are common. In the present study 40 beta-hemolytic streptococci were isolated from 36 patients suffering from SI due to various skin diseases. Staphylococcus aureus coexisted with beta-hemolytic streptococci in 29 of these cases (81%), and beta-hemolytic streptococci were often associated with coagulase-negative staphylococci and gram-positive rods. Eighteen patients (50%) carried beta-hemolytic streptococci predominantly. In most cases of SI due to atopic dermatitis (AD), the predominant species was S. aureus, while in other skin diseases, S. aureus and beta-haemolytica streptococci were predominant in approximately 50% of the patients, except for SI due to tumors and viral diseases. The mean age of patients with SI and beta-hemolytic streptococci was 37 years and that of patients with SI and predominant S. aureus was 32 years. The lower mean age found for S. aureus was due to SI found in patients with AD. This study emphasizes the polymicrobial microbiology of SI.

The identification of factors involved in herpes virus latency and reactivation is critical to a better understanding of the mechanisms essential to viral neuroinvasiveness and neurovirulence. Recurrent episodes of ocular herpes infections cause irreversible corneal scarring and are the primary cause of loss of vision due to an infectious agent in industrialized countries. In this study, we examined the ability of nicotine, a compound known to be involved in stress-associated immunomodulation and recognized as one of the most frequently used addictive agents, to induce ocular shedding in rabbits latently infected with herpes simplex virus type 1 (HSV-1) strain McKrae. New Zealand white rabbits latently infected with HSV-1 at 3-4 weeks post-inoculation were randomly divided into two groups. The corneas of all rabbits were free of lesions as verified by slit lamp biomicroscopy. One group received nicotine by transdermal patch (21 mg/day) for 20 days and the other group served as the control. Reactivation data were obtained by detection of virus in tear film collected by ocular swabbing performed concurrently with the administration of nicotine. Compilation of data from three separate experiments demonstrated that 16.5% (258/1560) of the swabs taken from rabbits treated with nicotine were positive for virus, compared with 8.3% (53/639) of swabs taken from controls. Rabbits receiving nicotine exhibited a significantly (P < 0.0001) higher rate of ocular shedding than controls. The concentration of nicotine in the serum was determined at various times (0-24 hrs) after new patch replacement. Peak (average) serum level of nicotine was obtained 8 hours after patch replacement and exhibited a broad range of values (0.233 microg/mL-6.21 microg/mL). These results suggest that an initial systemic exposure to nicotine significantly increases HSV-1 reactivation. Further studies are needed to reveal any effects of nicotine dependency and nicotine withdrawal on herpesvirus reactivation.

Anaerobes isolated from skin specimens from 1999 to 2001 were examined. The most common type was Peptostreptococcus spp., especially P. magnus and P. assaccharolyticus and Bacteroides fragilis. Dominance was seen for P. magnus, P. acnes and P. prevotii. Peptostreptococcus spp. and P. acnes showed high susceptibility to four antimicrobial agents. Prevotella spp. and B. fragilis showed low or no susceptibility to ampicillin, while B. fragilis showed low susceptibility to ceftizoxime. Evaluation of anaerobes is important for the balance of skin flora as well as for the choice of antimicrobial agents, when the anaerobes are pathogenic.

We examined Propionibacterium acnes lipase in skin diseases and Unsei-in. Butyric acid production in axillary seborrheic dermatitis (ASD) was higher than in other dermatitis, and that in acne vulgaris (AV) was significantly higher than in controls. P. acnes lipase is the pathogenic factor in AV and fatty acids produced by lipase might be the pathogenic factor in ASD. Unsei-in suppressed P. acnes lipase probably because some ingredients have antimicrobial and anti-inflammatory activities.

Enterococcus faecalis was isolated at a frequency of 70% and was chiefly isolated from secondary infections due to ulcer/decubitus. Seven sole/predominant E. faecalis were isolated. Penicillins were more effective against E. faecalis and the sensitivities of E. faecalis to antimicrobials were higher than those of E. faecium. Some nonpredominant E. faecalis strains were sensitive to erythromycin probably due to less resistant mechanisms. The characterization of Enterococcus spp. is especially important when choosing appropriate antimicrobials for therapy.

BACKGROUND AND STUDY AIMS: Ulcerative colitis is a chronic inflammatory bowel disease with repeated remission and relapse, although the occurrence of relapse is difficult to predict. We performed a prospective study to determine whether there is a relationship in ulcerative colitis between the inflammatory changes identified by endoscopic ultrasonography (EUS) and relapse. PATIENTS AND METHODS: Participants were 23 ulcerative colitis patients who had not suffered a relapse for 1 month, with a Seo activity index less-than-or-equal 150 and Baron grade 1 at colonoscopy. The thickness of the first to the third layer of the rectal wall in these patients was measured by EUS at the start of the study. They were subsequently followed up for 1 year to record any relapses, defined as having a Seo activity index > 150 and Baron grade greater-than-or-equal 2 at colonoscopy. RESULTS: Relapse occurred in eight patients. The mean activity index of the relapsed patients was 187.3 (95 % confidence interval (CI), 166.4 - 208.2) at the end of the study. The thickness of the first to the third layer of the rectal wall, as evaluated by EUS at the beginning of the study, was significantly larger in the relapse group (mean 2.73 mm, 95 % CI 2.13 - 3.33 mm) than in the non-relapse group (1.79 mm; 1.56 - 1.99 mm; P = 0.0001). CONCLUSIONS: Catheter probe-assisted endoluminal ultrasonography may predict the occurrence of relapse of ulcerative colitis.

RESUMEN

Seijyo-bofu-to, Jumi-haidoku-to and Toki-shakuyaku-san effectively suppressed acne rashes as well as incidental symptoms. The synergistic activities of the ingredients in the Kampo formulations might produce these effects. In contrast, distinct suppression of incidental symptoms was not found with antimicrobials. The cause of adverse effects in antimicrobials has not yet been clarified and different degrees of suppression of incidental symptoms among the Kampo formulations exist.

An evaluation was carried out of the types of Streptococcus spp. (excluding Streptococcus pyogenes) isolated from infectious skin diseases, and their susceptibilities to eight antimicrobial drugs of the Streptococcus spp. isolated. The types of diseases were also evaluated. A total of 29 Streptococcus spp. were identified and Streptococcus agalactiae was the most common. Streptococcus spp. were found to be susceptible to the eight antimicrobials used except gentamicin; susceptibility to gentamicin varied according to species. Susceptibility to beta-lactams was 100% and this drug should be considered the antimicrobial of choice in Streptococcus spp. treatment.

Keratocytes express MHC class I molecules constitutively, and keratocytes stimulated with IFN-gamma express MHC class II molecules. Unstimulated keratocytes constitutively express B7-1 and ICAM-1, as well as low levels of CD40 and 4-1BBL. These findings indicate that keratocytes may deliver both antigen-specific and costimulatory signals to CD4(+) and CD8(+) T cells. To demonstrate that keratocytes expressing B7-1 provide a costimulatory signal to T cells, CD4(+) or CD8(+) mouse T cells were incubated with anti-CD3 mAb and irradiated keratocytes. Enhanced proliferation of both CD4(+) and CD8(+) T cells occurred, and could be inhibited by anti-B7-1 mAb, indicating T cell costimulatory activity by B7-1 on the keratocytes. To demonstrate that keratocytes can deliver an antigen-specific signal, CD4(+) and CD8(+) T cells from herpes-infected mice were incubated with HSV-1-infected, irradiated keratocytes. The resulting T cell proliferation and production of Th1 cytokines (IL-2, IFN-gamma) indicated T cell activation by antigens presented by the infected keratocytes. These results show that keratocytes in the corneal stroma of the mouse can function as antigen-presenting cells and, thus, may play a role in immune-mediated stromal inflammation such as herpetic stromal keratitis.

Two trichilemmal cysts (TC) on the scalps of two Japanese women were examined at the electron microscopic level. The ultrastructural characteristics of the TC included an abrupt transition from nucleate to anucleate cells, a few oval, small, keratohyaline granules mixed with increasing numbers of tonofibrils from the peripheral layer towards the center, spherical particles with lipid droplets, desmosomal structures in keratinized cells, and the interdigitation of keratin in the uppermost keratinizing cells. The keratinization pattern of the TC was unique in that typical soft or hard keratinization processes did not occur. Trichilemmal keratinization is observed in the trichilemmal sacs that surround the lower ends of catagen or telogen hairs, as well as in the outer root sheaths in the follicular isthmus of anagen hairs. Comparison of the ultrastructure of these two known types of trichilemmal keratinization with the keratinization pattern of the TC in our patients indicates that TC may differentiate into, or originate from, the proliferation of the outer root sheath in the follicular isthmus of anagen hairs.

Famciclovir (FCV) is efficacious in the treatment of acute herpes zoster and recurrent genital infections but has not been used to treat ocular herpes simplex virus (HSV) infections. We evaluated the efficacy of orally administered FCV in treating HSV-1 epithelial keratitis and determined its effects on the establishment of latency and subsequent reactivation. Rabbits were inoculated with HSV-1 strain 17 syn+ and treated twice daily with increasing concentrations of FCV (60 to 500 mg/kg of body weight). This resulted in a significant, dose-dependent improvement in keratitis scores, as well as prolonged survival. Regardless of the dose of drug used, all groups exhibited the high rates of spontaneous and induced reactivation characteristic of 17syn+. The efficacy of 250 mg of FCV per kg was also compared to topical treatment with 1% trifluorothymidine (TFT). Although TFT treatment was more effective at reducing eye disease, FCV-treated rabbits had a better survival rate. Real-time quantitative PCR analysis of rabbit trigeminal ganglia (TG) demonstrated that FCV significantly reduced the HSV-1 copy number compared to that after treatment with TFT or the placebo but not in a dose-dependent manner. In summary, oral FCV treatment significantly reduces the severity of corneal lesions, reduces the number of HSV-1 genomes in the TG, improves survival, and therefore may be beneficial in reducing the morbidity of HSV keratitis in the clinic.

Lipodystrophia centrifugalis abdominalis infantilis (LCAI) in a 2.5-year-old Japanese girl is reported. Clinically she had a curved cutaneous depression with a slightly elevated erythematous border on the left abdomen beyond the left groin. The regional lymph node was palpable. Histologic examination showed a decrease of the fat, an inflammatory infiltrate mainly composed of lymphocytes, and septal fibrosis in the subcutaneous tissue. Immunohistochemical analysis found that infiltrating lymphocytes were mainly positive against CD4. Ultrastructurally nuclei in the adipocytes were crescent shaped and located toward the periphery of the cell. Some banded structures with a periodicity of 150 nm, and with intraperiodic bands, referred to as fibrous long-spacing collagen (FLSC), were observed in the septal area of the fatty tissue. These results indicated that FLSC was correlated with the breakdown of fibrillar collagens in LCAI.

We studied the lipase activities of Propionibacterium granulosum, P. acnes and the suppression of these activities by Jumi-haidoku-to (JHT). Lipase activity of P. acnes biotype III (BIII) was strongest, while that of P. granulosum was faintly expressed. Compared with the control medium, the production of propionic and butyric acids was suppressed by all the tested mediums combined with JHT. The decrease in these acids produced by JHT was significantly higher in P. granulosum than in P. acnes. Although P. acnes BIII may produce a strong effect on acne, the presence of P. granulosum should not be ignored. Further research is required on the correlation between P. acnes and P. granulosum.

Bleeding Meckel's diverticulum tends to be a complication of ectopic gastric mucosa. It has been known that the gastric acid and digestive fluid produced from the ectopic gastric mucosa cause peptic ulcer, which leads to bleeding. When the ectopic gastric mucosa is entirely formed by the pyloric gland, however, ulcer formation may result from factors other than gastric acid and digestive fluid. This is a report of an adult case of bleeding Meckel's diverticulum with ectopic gastric mucosa in whom mechanical stimulation was suggested as a cause of the erosion and ulceration, resulting in bleeding.

An understanding of the cellular genes whose expression is altered during HSV reactivation will enable us to better understand host responses and biochemical pathways involved in the process. Furthermore, this knowledge could allow us to develop gene-targeted inhibitors to prevent viral reactivation. Mice latent with HSV-1 strain McKrae and uninfected control mice were subjected to hyperthermic stress (43 degrees C for 10 min) and their trigeminal ganglia (TG) collected 1 h later. Two additional groups included HSV-1 latently infected and uninfected mice not subjected to hyperthermic stress. Poly A+ mRNA was enriched from total mouse TG RNA and reverse transcribed using MMLV RT. Radioactively labeled cDNAs were analyzed by microarray analysis. A stress/toxicology array of 149 mouse genes on a nylon membrane was used. The labeled cDNAs prepared from latently infected, stressed mice demonstrated 3-fold or greater increases in certain mRNA-early response genes (ERGs) compared to cDNAs from uninfected, stressed control mice. The ERG mRNAs that showed increases included two heat shock proteins (HSP60 and HSP40), a basic transcription factor (BTF T62), a DNA repair enzyme, two kinases [MAP kinase and a stress-induced protein kinase (SADK)], an oxidative stress-induced protein, a manganese superoxide dismutase precursor-2 (SOD-2), and cyclooxygenase 2 (COX-2). The gene expression in unstressed, infected TGs was similar to the gene expression in unstressed, uninfected controls. These results suggest that there is a significant difference in the ERG expression profile in latently infected TGs undergoing stress-induced reactivation compared to uninfected TGs.
