RESUMEN

Pseudophakic cystoid macular edema (CMO) known to appear in 0.1-2.35% of patients after phacoemulsification is one of the common causes of vision decrease during the postoperative period. The authors analyze a case of CMO development in a pseudophakic patient diagnosed 7 months after uncomplicated phacoemulsification. Optical coherence tomography (OCT) was suggested for patients with low vision and a sensation of shroud before the eye for early diagnosis of pathological process. Intravitreal injection of the 0.7 mg dexamethasone micro-implant performed soon after establishing diagnosis was proved to be viable for postoperative CMO arrest in patients without previous treatment. One month after intravitreal implant injection the visual acuity in the patient's eye improved from 0.4 to 0.7, and retinal thickness in the macular area decreased from 705 to 255 µm. The result remained stable for 10 months. Intravitreal micro-implant appeared to have higher efficiency in contrast with traditional treatment methods (according to literature data), which was attributed to direct drug delivery to the area of pathological process. The use of 0.7 mg dexamethasone implant did not increase the intraocular pressure indicating its potential viability for patients with compensated glaucoma.

RESUMEN

Based on literature and own experience recommendations for intracapsular ring use in cataract surgery in patients with different types of suspensory ligament deficiency were given. Operation technique and prevention of complications during ring implantation with or without retractors in different etiology of subluxation and individual degree of suspensory ligament deficiency were described for every principal step of phacoemulsification.

RESUMEN

The study deals with the development of an effective, atraumatic and easy-to-use treatment for patients with progressive myopia, by using a discoid xenograft. The treatment involves implantation of a discoid xenograft to the episclera of the posterior pole of the eye. The xenograft is a white disk prepared from the treated bovine pericardium and it has 2 mm in thickness and a diameter of 13 mm. Clinical studies were performed in 89 patients (161 eyes) operated on by the proposed procedure, by evaluating the stabilized progression of myopia 4 years later. In the late period of a follow-up, the stabilization effect was 88.3 to 92.6% in relation to the degree of myopia. There was evidence for increases in the thickness of the sclera (and its acoustic density) on the posterior pole of the eye, for improved regional hemodynamics and better electrophysiological functions of the retina in the postoperative period. Xenoscleroplasty of the posterior pole of the eye with a bovine pericardial xenograft is an easy-to-use, pathogenetically oriented, highly effective, and safe treatment in patients with progressive myopia.

Eighty-four eye were examined in 67 patients after phacoemulsification with intracapsular IOL implantation. The patients were shared, with respect to IOL rigidity, between 2 groups: group 1--rigid PMMA lenses and group 2--soft hydrogel and acryl lenses. The inclination angle was evaluated for different lens types by using an elaborated mathematical formula. The lenses had capsular fixation in all cases. The deviation angle ranged from 0.58 to 1.22 degrees. The difference between the inclination angle and ocular visual axis for rigid and soft lenses was statistically reliable (p < 0.05). The inclination angle of the optic axis in case of soft lenses versus rigid lenses was to a greater extent pronounced after 2 months of observation. However, the conclusion is not final due to continuous fibrosis of the capsular sac, which affects dynamically the IOL positioning. Therefore, similar studies will be made after 12-14 months to enable final conclusions.

RESUMEN

The authors demonstrate a high informative value of preoperative lacrimal fluid examinations to predict postoperative uveitis in patients with complicated cataracts. Detection of antituberculin antibodies in titers 1:128 or higher is a factor of risk of development of a postoperative inflammatory reaction. The risk of postoperative uveitis is increased to 100% if antibodies to tuberculin, tissue-specific retinal S antigen, and lenticular proteins are simultaneously detected in the lacrimal fluid.

RESUMEN

Physical and chemical parameters of immune complexes were studied in cataract patients before and after surgery with laser nephelometry. The authors suggest a possible uveitogenous role of the circulating immune complexes of medium size (from 7 to 21 IgG) and high concentration (over 1.2 mg/ml) in the development of postoperative complications of an inflammatory nature.

RESUMEN

Relative informative value of various functional methods of investigation was studied in 87 patients with tapetoretinal abiotrophy and complicated cataracts in order to find adequate methods for the assessment of the retinal status before surgery and to define the criteria of visual function prediction. The efficacy of the diagnosis was verified after cataract extraction in these patients. The methods of investigation employed were instrumental electroretinogram (ERG) recording, Hanz-Feld stimulated microERG, high-frequency (40 Hz) rhythmic ERG, electrocoagulograms, determination of electric sensitivity thresholds for the retina and of the optic nerve axial bundle lability (ESL), perimetry, and vision acuity measurements. Rhythmic ERG and ESL were found the most informative; rhythmic ERG amplitude proved to be the most sensitive to macular area involvement, this prompting employment of this method for the prediction of visual functions. Rhythmic ERG helped single out three groups of patients with relatively favorable, doubtful, and unfavorable outcomes.

The authors sum up the results of comprehensive examination of all the principal structures of the eyeball in patients with tapetoretinal abiotrophy. Present-day methods of investigation were employed, that helped reveal a generalized pattern of the pathologic process in this condition. The detected shifts should be borne in mind when choosing a method for cataract extraction in such patients.

Experimental trials of a new intraocular irrigation solution, one of the enriched balanced salines, were carried out. A comprehensive morphologic study has revealed the protector characteristics of lactosol plus (lactosol + taurin) used in the management of irrigation injury to the posterior epithelium of the cornea in the course of experimental intrachamber perfusion. The protective properties of lactosol + taurin, a new enriched balanced saline, are explained by the similarity of its chemical composition to that of the intraocular fluid and by the presence of a stable polyfunctional bioprotector, taurin amino acid.

NADH fluorescence may serve a convenient test indicating the tissue redox status. Rabbit cornea has been incubated at ambient temperature in media of different compositions with 5.10(-5) M NADH at pH 7.0-7.3. Oxidation of NADH in the humor-substituting medium for corneal tissue incubation has been monitored by fluorescent analysis (stimulation maximum at a wavelength of 360 nm, emission maximum 460 nm). Characteristic kinetics of changes in the pyridine nucleotide fluorescence consists in its maximum intensity occurring in 10 min of the tissue incubation, followed by the fluorescence intensity reduction by the 20-30th min of the corneal tissue incubation. These changes are explained by a release of substrates, characterized by NADH oxidizing activity, from the corneal tissue, this resulting from injury to the corneal cells with humor-substituting solutions. Carnosine (10 mM) and taurine (0.05% solutions) have been found to protect the corneal tissue from injuries, in contrast to common salines.

It has been shown that intraocular lenses (IOL) prepared from polymethylmethacrylate and silicon let pass the light of short-wave visible and long-wave spectrum regions. Unlike natural lens, IOL have no yellowish coloration, which results in the appearance of chromatic aberrations and brightness of IOL during implantation. Mechanisms of damaging effects of visible light on the retina were studied. It has been shown that a week after the rat eyes were illuminated the waves "a" and "b" ERG were inhibited. At the ultrastructural level damage of the cells of the retinal pigment epithelium and of the retinal layers formed by photoreceptor cells took place. Dynamics of the damage of tissue structures of the retina by visible light was investigated. It is suggested that application of colored lenses and prophylaxis by the inhibitors of free radical oxidation will protect the retina from photochemical damage.
