RESUMEN

PURPOSE: Using a pediatric head phantom constructed in our department, we examined a method to reduce exposure by using organ-effective modulation (OEM; Toshiba Medical Systems Corporation, Tochigi) to tilt the gantry during pediatric head computed tomography (CT) scanning. METHOD: The radiation reduction and CT image standard deviation (SD) were measured at gantry angles at which the orbit was slightly irradiated, partially irradiated, and completely irradiated. The OEM incident surface dose reduction rate was measured using an automatic exposure control (AEC) phantom with a diameter of 6-18 cm. RESULTS: The lens surface dose reduction rate using OEM was 21.2%. When the gantry was tilted and the orbit was completely out of the scanning range, the rate of reduction was 47.8%. OEM incident surface dose reduction rates were 27.4% for a phantom diameter of 18 cm, 22.0% for that of 16 cm, 17.8% for that of 14 cm, 17.2% for that of 12 cm, 8.4% for that of 10 cm, and 0% for that of 8 cm and 6 cm. OEM effectiveness decreased with decreasing phantom diameter. The use of OEM increased the rate of change of SD by 1.25´ when the gantry inclination was 0°, 1.27´ when the gantry inclination was 10°, and 1.27´ when the gantry inclination was 20°in the 12 o'clock position. CONCLUSION: The degree of reduction in exposure dose to the lens in pediatric head CT imaging was 47.8% by completely removing the lens from the irradiation range using gantry tilt and 21.2% by using OEM. The effect of OEM changed in proportion to tube current. The exposure reduction effect of the OEM decreases with decreasing head size, indicating its reduced effectiveness in head CT scans of smaller infants.

RESUMEN

Pharmacokinetics of SN-38 in patients with end-stage kidney disease (ESKD) is partially varied because of fluctuations in transporters expression and/or function by high protein bound-uremic toxins concentration. The fluctuations may induce variations in anticancer drugs sensitivity to cancer cells. We aimed to clarify the variations in sensitivity of SN-38 to cancer patients with ESKD and investigate this mechanism, by human colon cancer cells exposed to uremic serum residue. LS180 cells were exposed to normal or uremic serum residue (LS/NSR or LS/USR cells) for a month. IC50 values of SN-38 in LS/NSR or LS/USR cells were calculated from viability of each cells treated SN-38. mRNA expression and intracellular SN-38 accumulation was evaluated by RT-PCR and HPLC-fluorescence methods, respectively. The IC50 value in LS/USR cells was higher than that in LS/NSR cells. Organic anion transporter polypeptide (OATP) 2B1 mRNA expression was lower in LS/USR cells than in LS/NSR cells, and SN-38 accumulation in LS/USR cells was lower than that in LS/NSR cells. Only co-treatment baicalin, which is OATP2B1 inhibitor, almost negated the difference in SN-38 accumulation between LS/NSR and LS/USR. Anticancer effects of substrates of OATP2B1, such as SN-38, were reduced in ESKD patients at the same plasma substrate concentration.

PURPOSE: Organ-effective modulation (OEM) is a mechanism to reduce radiation dose to selected organs on computed tomography (CT). The purpose of this study was to measure radiation dose to the breast in Asian patients undergoing chest CT and to clarify the degree of exposure reduction. METHOD: We randomly selected 60 female patients undergoing non-contrast chest CT after breast cancer surgery. To measure radiation dose, an optically stimulated luminescence dosimeter had been attached directly to the gown over the nonoperated breast in 30 patients. Radiologists evaluated the image quality with and without OEM. In order to clarify the characteristics of OEM, the effects of angle and object size were measured using a phantom and an ionization chamber dosimeter. RESULTS: The OEM group received 9.1 ±â€¯1.9 mGy and the non-OEM group received 10.7 ±â€¯2.4 mGy. OEM reduced the exposure by 12.2% (P <  0.01). OEM caused no reduction in diagnostic quality. In the phantom study, the results of the angle effect were 3.2%, 11.2%, 28.7%, 31.3, 25.9%, 14.9% and 6.0% dose reductions at -90, -60, -30, 0, 30, 60 and 90°, respectively. The effect of the subject thickness was 3.7%, 17.5%, 30.2%, 31.7%, and 34.1% at 16, 20, 24, 28 and 32 cm diameters, respectively. CONCLUSIONS: OEM is a useful mechanism for reducing radiation exposure to the breast without affecting diagnostic imaging quality. The reduction rate correlated negatively with body habitus.

BACKGROUND: The estimation of emphysematous changes is very sensitive to computed tomography (CT) threshold level. In clinical practice, the predetermined threshold is usually set at -950 Hounsfield units (HU) for the detection of low attenuation volume (LAV). However, threshold levels that are tightly connected to pulmonary function abnormalities have not been determined. PURPOSE: To determine the threshold level for calculating an LAV that closely reflects airflow limitation in patients with chronic obstructive pulmonary disease (COPD). MATERIAL AND METHODS: Seventy-six consecutive non-COPD smokers and COPD patients underwent paired inspiratory and expiratory multidetector CT (MDCT). LAV% was segmented every 10 HU between -1000 and -750 HU to examine the correlation between LAV% and indexes of obstructive impairment. RESULTS: LAV% gradually increased as the threshold level increased on both inspiratory and expiratory images. LAV% on inspiratory images was higher than that on expiratory images at all threshold levels between -1000 and -750 HU. The threshold level that correlated with obstructive impairment differed between the two images: -930 HU on inspiratory and -870 or -880 HU on expiratory images. CONCLUSION: LAV% dramatically changed according to the threshold level on both inspiratory and expiratory images, indicating that LAV% is dependent on the attenuation threshold level in patients with COPD. The threshold linking LAV% to airflow limitation was higher on expiratory than on inspiratory images.

OBJECTIVE: The purpose of the present study was to evaluate the usefulness of coronal reformatted images obtained from 64-slice multi-detector computed tomography to assess the ablative margin (AM) in hepatocellular carcinoma (HCC) treated with radio frequency ablation (RFA). METHODS: Ninety-five HCC nodules were analyzed in 66 HCC patients treated with RFA. Two radiologists and one hepatologist independently reviewed axial CT images with or without coronal reformatted images in HCC treated with RFA. Nodules were determined as AM-sufficient (≥5 mm) or AM-insufficient (<5 mm). The level of interobserver agreement was measured using the weighted kappa test. The sensitivity, specificity, and positive and negative predictive values (NPVs) of an insufficient AM (<5 mm) to predict local recurrence were evaluated. RESULTS: The numbers of AM-sufficient nodules judged by readers 1-3 based on axial images and both axial and coronal images were 56, 49, and 58, and 47, 33, and 48, respectively. Excellent agreement and good to excellent agreement were obtained among the three readers on axial image readings and both axial and coronal image readings, respectively. The mean sensitivity, specificity, and positive and NPVs of an insufficient AM on axial images and both axial and coronal images to predict local recurrence were 64%, 60%, 17%, and 93%, and 95%, 50%, 20%, and 97%, respectively. CONCLUSIONS: Coronal reformatted CT images should be utilized to evaluate the AM in HCC treated with RFA in order to decrease the risk of local recurrence following treatment.
