RESUMEN

PURPOSE: Cluster analyses on inflammatory markers of chronic rhinosinusitis (CRS) in Asians from multicenter data are lacking. This multicenter study aimed to identify the endotypes of CRS in Koreans and to evaluate the relationship between the endotypes and clinical parameters. METHODS: Nasal tissues were obtained from patients with CRS and controls who underwent surgery. The endotypes of CRS were investigated by measuring interleukin (IL)-5, interferon (IFN)-γ, IL-17A, IL-22, IL-1ß, IL-6, IL-8, matrix metalloproteinase-9, eotaxin-3, eosinophil cationic protein, myeloperoxidase (MPO), human neutrophil elastase (HNE), periostin, transforming growth factor-ß1, total immunoglobulin E (IgE), and staphylococcal enterotoxin (SE)-specific IgE. We performed hierarchical cluster analysis and evaluated the phenotype, comorbidities, and Lund-Mackay computed tomography (LM CT) score in each cluster. RESULTS: Five clusters and 3 endotypes were extracted from 244 CRS patients: cluster 1 had no upregulated mediators compared to the other clusters (mild mixed inflammatory CRS); clusters 2, 3, and 4 had higher concentrations of neutrophil-associated mediators including HNE, IL-8, IL-17A, and MPO (T3 CRS); and cluster 5 had higher levels of eosinophil-associated mediators (T2 CRS). SE-specific IgE was undetectable in T3 CRS and had low detectable levels (6.2%) even in T2 CRS. The CRS with nasal polyps (CRSwNP) phenotype and LM CT scores showed no significant differences between T2 and T3 CRS, while the incidence of comorbid asthma was higher in T2 CRS than T3 CRS. In T3 clusters, higher levels of neutrophilic markers were associated with disease severity and CRSwNP phenotype. CONCLUSIONS: In Koreans, there is a distinct T3 CRS endotype showing a high proportion of CRSwNP and severe disease extent, along with T2 CRS.

RESUMEN

A new species of the genus Pseudaeginella Mayer, 1890 belonging to the family Caprellidae Leach, 1814 was collected from the South Sea in Korea. Pseudaeginellacarinaspinosasp. nov. is morphologically similar to related congeners belonging to the genera Paradeutella Mayer, 1890 and Pseudaeginella, in having dorsal projections on pereonites, triarticulate mandibular palp, small or absent molar, and uniarticulate pereopods 3 and 4. However, this new species is distinguished from its congeners by the position and size of dorsal projection. This is the first record of Pseudaeginella from the Northwest Pacific region, including Korea, and a key to species of the genus Pseudaeginella is also provided.

RESUMEN

Intermittent hypoxia (IH) has been an issue of considerable research in recent years and triggers a bewildering array of both detrimental and beneficial effects in several physiological systems. However, the mechanisms leading to the effect are not yet clear. Consequently, we investigated the effects of IH on allergen-induced allergic asthma via the mitogen-activated protein kinase (MAPK) signaling pathway. Forty BALB/c mice were dived into four groups. We evaluated the influence of IH on the cell signaling system of the airway during the allergen-induced challenge in an animal model, especially through the MAPK (mitogen-activated protein kinase) pathway. The protein concentrations of p-ERK/ERK, p-JNK/JNK, p-p38/p38, and pMEK/MEK were significantly reduced in the allergen-induced+IH group, compared to the allergen-induced group (p-value < 0.05 as considered statistically significant). The number of eosinophils, neutrophils, macrophages, and lymphocytes in the bronchoalveolar lavage fluid and Dp (Dermatophagoides pteronyssinus)-specific IgG2a and interleukins 4, 5, 13, and 17 were significantly reduced in the Dp+IH group, compared to the Dp group. These findings suggest that the MAPK pathway might be associated with the beneficial effect of IH on the attenuation of allergic response in an allergen-induced mouse model.

RESUMEN

In the present study, we evaluated the effects of different environments on the filtering rate (FR), mortality, and biodeposition (BD) of the freshwater mussel Unio douglasiae in bloom waters containing the toxic cyanobacterium Microcystis aeruginosa. The mean FR of 19 selected individuals (shell length, 5.0-9.8 cm) was 0.30 ± 0.03 L g-1 h-1 (range = 0.24-0.35 L g-1 h-1). Shell length was strongly correlated with both net and gross BD of mussels (P < 0.0001). The mean FR was higher in river water (0.405 ± 0.052 L g-1 h-1) than in lake water (0.304 ± 0.051 L g-1 h-1). In contrast, the BD of mussels was higher in RW (0.671 ± 0.609 mg g-1 h-1) than in LW (0.275 ± 0.027 mg g-1 h-1). For algal species, the FR of mussels ranged from 0.114 ± 0.024 to 0.553 ± 0.019 L g-1 h-1. The FR of U. douglasiae was higher in river water (mainly diatoms), whereas BD was higher in lake water (mainly Microcystis). U. douglasiae did not prefer toxic M. aeruginosa, which was significantly accumulated in pseudofaeces and faeces. The maximum FR of U. douglasiae in algal bloom water was recorded at a water temperature of 25°C and water depth of 50 cm. Moreover, the in situ mortality of U. douglasiae was strongly affected by water temperature and nitrogen concentration. Overall, U. douglasiae can enhance water quality in eutrophic areas by removing dominant cyanobacteria, although its removal efficiency depends on environmental parameters and site of introduction.

RESUMEN

Two dexaminid amphipod species belonging to the genus Paradexamine were collected from Korean waters. After observation and identification compared with related congeners, these two species are revealed to be new to science. In comparative identification, one of the new species, P.acuta sp. nov. is similar to P.houtete in having an acutely rounded lobe and posteroventrally pointed coxa 7. However, this new species is distinguished from P.houtete in having a larger number of medial setae on the propodus of gnathopod 1 and a rounded basis of pereopod 6. This new species is also very similar to P.marlie s.l. Hirayama from Japanese waters, and P.marlie s.l. might be re-established or synonymized with P.acuta sp. nov. in the future. The other new species, P.rotundogena sp. nov. is similar to P.tafunsaka in having a rounded eye lobe; however, it is distinguished from P.tafunsaka in having an elongate carpus on gnathopod 2 and the differently shaped basis of pereopod 7. A key to the five Korean species of Paradexamine, including the two new species, is also provided.

RESUMEN

PURPOSE: We aimed to determine appropriate antigen concentrations and the right time to evaluate intranasal changes when performing a nasal provocation test (NPT). Also, we sought to analyze the diagnostic usefulness of individual nasal symptom and peak nasal inspiratory flow (PNIF). MATERIALS AND METHODS: We divided 46 patients into allergic rhinitis (AR) group (n=19) and a non-allergic rhinitis (NAR) group (n=27). We performed intranasal challenge with 100 AU/mL of Dermatophagoides pteronyssinus (DP) and measured changes in nasal symptoms [scored using the visual analogue scale (VAS)] and PNIF%. If the patient showed significant changes, VAS and PNIF were assessed again after another 15 minutes. In patients without significant changes, we administered 1000 AU/mL and measured changes in nasal symptoms and PNIF% after 15 and 30 minutes. RESULTS: Fifteen minutes after the 100 AU/mL challenge, the AR group showed more significant VAS changes in all nasal symptoms, total nasal symptom score (TNSS), and PNIF% change than the NAR group. Among the AR group, patients who did not respond to 100 AU/mL exhibited less significant differences relative to the NAR group, even after 1000 AU/mL challenge. Receiver operating characteristic curve analysis for VAS changes 15 minutes after 100 AU/mL challenge revealed that all nasal symptoms had area under the curve (AUC) values of ≥0.84 (p<0.001). TNSS change had an AUC value of 0.929 (p<0.001), while PNIF% change had an AUC value of 0.834. CONCLUSION: We could determine the optimal concentration (100 AU/mL), timing (15 minutes after challenge), and parameters (changes in TNSS and PNIF%) when performing NPT.

RESUMEN

BACKGROUND: The efficacy of rupatadine for the treatment of AR has been confirmed in numerous clinical studies, however there are very few studies on asian patients. OBJECTIVE: To assess the safety and efficacy of rupatadine fumarate in the treatment of Korean perennial allergic rhinitis (PAR) patients. METHODS: A multicenter, double-blind, randomized, placebo-controlled, comparative study of rupatadine fumarate and bepotastine besilate was conducted. Each group was administered rupatadine, bepotastine or placebo for 4 weeks. Primary parameters for efficacy included morning and evening symptom reduction from baseline at 4 weeks. Treatment safety and tolerability were evaluated according to a self-reported incidence and type of adverse events at each follow up visit. RESULTS: Rupatadine showed a significant reduction in symptoms at morning and evening evaluations, in both 5TSS (-5.69, P < 0.0006) and 4NTSS (-4.74, P < 0.0015) compared to placebo. There was a significant reduction from baseline for 5TSS (-65.4%, P = 0.002) and 4NTSS (-63.7%, P = 0.003) with rupatadine compared with placebo. At evening evaluations, there were significant reductions of 5TSS (-63.2%, P = 0.009) and 4NTSS (-61.6%, P = 0.013) for the rupatadine group. Compared with bepotastine, rupatadine showed greater reduction in the morning symptoms at 4 weeks. When individual symptoms were assessed with 12-hour reflective mean daily symptom score, rupatadine showed better efficacy than placebo in sneezing (P = 0.016) and rhinorrhea (P = 0.097). The rate of adverse events showed no statistical significance. CONCLUSIONS: Rupatadine is a safe and effective treatment option for Korean PAR patients and possibly a better choice over bepotastine for controlling morning symptom.

RESUMEN

This study aimed to investigate the impact of student e-learning on the development of clinical competencies. The study participants were 3rd year students (n = 43) at a private mid-sized medical school located in a South Korean suburb on a four-year medical program. Educational intervention was implemented to enhance student clinical performance. Students engaged in learning activities that intended to promote their self-directed learning abilities and clinical performances using e-learning resources. Intervention was conducted for the duration of six months during the 3rd year and its effectiveness was investigated by comparing student performances in OSCEs in a pre- and post- comparison format and also by comparing them with national scores. In addition, student perceptions of the impact of e-learning on their OSCE performances were assessed using a questionnaire, which included 36 items that elicited student perceptions of their experiences of e-learning and readiness for e-learning. Student OSCE scores improved significantly after educational intervention in all domains of clinical competencies assessed and for total scores (p < 0.001). Furthermore, students achieved higher OSCE scores than national average scores in the post-test, whereas they had performed lower than national average scores in the pre-test. Students showed neutral or slightly positive responses to the effectiveness of e-learning, and their perceptions of e-learning were not associated with their e-learning readiness scores. The study shows student OSCE performance improved significantly after educational intervention, which indicate the effectiveness of e-learning to support student learning of clinical performance. Despite significant improvements in student OSCE scores after e-learning, their perceptions of its effectiveness were neutral. Furthermore, student perceptions of e-learning were not associated with their readiness for it. Suggestions are made to help students use e-learning more effectively to enhance their clinical competencies.

RESUMEN

INTRODUCTION: Alzheimer's disease (AD), a heterogeneous pathological process representing the most common causes of dementia worldwide, has required early and accurate diagnostic tools. Neuropathological hallmarks of AD involve the aberrant accumulation of Amyloid beta (Aß) into Amyloid plaques and hyperphosphorylated Tau into neurofibrillary tangles, occurring long before the onset of brain dysfunction.Areas covered:Considering the significance of Aß and Tau in AD pathogenesis, these proteins have been adopted as core biomarkers of AD, and their quantification has provided precise diagnostic information to develop next-generation AD therapeutic approaches. However, conventional diagnostic methods may not suffice to achieve clinical criteria that are acceptable for proper diagnosis and treatment. The advantages of nanomaterial-based biosensors including facile miniaturization, mass fabrication, ultra-sensitivity, make them useful to be promising tools to measure Aß and Tau simultaneously for accurate validation of low-abundance yet potentially informative biomarkers of AD.. EXPERT OPINION: The study has identified the potential application of advanced biosensors as standardized clinical diagnostic tools for AD, evolving the way for new and efficient AD control with minimum economic and social burden. After clinical trial, nanobiosensors for measuring Aß and Tau simultaneously possess innovative diagnosis of AD to provide significant contributions to primary Alzheimer's care intervention.

RESUMEN

OBJECTIVES: We evaluated the usefulness of the standardized nasal provocation test (NPT) protocol recently published by the European Academy of Allergy and Clinical Immunology (EAACI) and compared the utility of several parameters for diagnosing allergic rhinitis (AR) caused by house dust mites (HDM). Subjective parameters were nasal and ocular symptoms measured using a visual analog scale (VAS), and objective parameters were peak nasal inspiratory flow (PNIF), minimal cross-sectional area (MCA), and total nasal volume (TNV). METHODS: Before and after spraying Dermatophagoides pteronyssinus (DP) allergen (1,000 AU/mL, 100 µL) into both nostrils of 13 patients with AR (AR group) and 22 patients with non-AR (NAR group), we used VAS scores to measure nasal symptoms (nasal obstruction, rhinorrhea, sneezing, and itching) and ocular symptoms and assessed PNIF, MCA, and TNV. RESULTS: The AR group had significantly worse symptoms than the NAR group 15 minutes after DP challenge (P<0.001). After 30 minutes, nasal obstruction and rhinorrhea remained worse in the AR group (P<0.001); a similar but less marked difference was seen for sneezing (P=0.012) and itching (P=0.039). Ocular symptoms, PNIF, MCA, and TNV differed between groups after both 15 and 30 minutes (P<0.05). The area under the receiver operating characteristic curve was higher for nasal obstruction (0.977), rhinorrhea (0.906), and TNV (0.979) than for sneezing (0.755), itching (0.673), and MCA (0.836). CONCLUSION: NPT performed according to the EAACI guidelines could help diagnose AR caused by HDM. TNV and VAS changes in nasal obstruction and rhinorrhea had higher diagnostic accuracy than other parameters.

RESUMEN

Three new species of Cumacea belonging to the genus Bodotria were collected from the Yellow Sea in Korea. Among them, Bodotria (Bodotria) hwanghaensis sp. nov. and Bodotria (Bodotria) pseudomaculosa sp. nov. share the dorso-lateral carina on the carapace and uniarticulated uropod endopod with other Korean Bodotria species. In contrast, the last one, Bodotria (Atlantobodotria) incarinata sp. nov. lacks the dorso-lateral carina on the carapace and has a biarticulated uropod endopod. The new species B. (A.) incarinata sp. nov. can be distinguished from all other Bodotria species by the combination of carapace without dorso-lateral carina or ridge, dorso-median carina marked over whole length of carapace, antero-lateral corner of carapace rounded, antennal notch shallow, maxilliped 3 merus and carpus dilated, pereopod 1 carpus not dilated, and pereopod 2 basis with plumose seta. The new species B. (B.) hwanghaensis sp. nov. can be distinguished from all other Bodotria species by the combination of carapace almost ovoid in dorsal view, carapace lower edge of mid-lateral depression forming a faint ridge, pereopod 1 carpus not dilated, and pereopod 2 basis without plumose seta. The new species B. (B.) pseudomaculosa sp. nov. can be distinguished from all other Bodotria species by the combination of carapace surface covered with coarse squamosa-reticulate patterning formed by large, shallow pits, carapace lower edge of mid-lateral depression forming an angular ridge, pereopod 1 carpus little dilated, and pereopod 2 basis with plumose seta. We described with full illustrations, including mouthparts for the three new species. Also, a key to the Korean Bodotria species is provided.

RESUMEN

Emerging nanomaterials providing benefits in sensitivity, specificity and cost-effectiveness are being widely investigated for biosensors in the application of Alzheimer's disease (AD) diagnosis. Core biomarkers amyloid-beta (Aß) and Tau have been considered as key neuropathological hallmarks of AD. However, they did not sufficiently reflect clinical severity and therapeutic response, proving the difficulty of the Aß- and Tau-targeting therapies in clinical trials. In recent years, there has still been a shortage of sensors for non-Aß-Tau pathophysiological biomarkers that serve as advanced reporters for the early diagnosis of AD, predict AD progression, and monitor the treatment response. Nanomaterial-based sensors measuring multiple non-Aß-Tau biomarkers could improve the capacity of AD progression characterization and supervised treatment, facilitating the comprehensive management of AD. This is the first review to principally represent current nanobiosensors for non-Aß-Tau biomarker and that strategically deliberates future perspectives on the merit of non-Aß-Tau biomarkers, in combination with Aß and Tau, for the accurate diagnosis and prognosis of AD.

RESUMEN

Two new species of the genus Microlysias Stebbing, 1918 belonging to the family Tryphosidae Lowry Stoddard, 1997 were collected from shallow Korean waters. The two new species are described as Microlysias rectangulatus and M. triangulus. The new species M. rectangulatus sp. nov. is similar to M. xenokeras, M. soela, and Orchomenella japonica. However, the new species can be distinguished from these species by having a subrectangular peduncular article 5 of antenna 2 and an elongated article 1 of the mandibular palp, nearly equal to palp article 3. M. triangulus sp. nov. is similar to M. xenokeras and M. soela. However, this species is characterized by the triangular shaped peduncular article 5 of antenna 2 and the slender propodus of gnathopod 1. These species are illustrated and compared to related species.

RESUMEN

A new species of Cumacea belonging to the genus Iphinoe Bate was collected from the Yellow Sea in Korea. The new speices Iphinoe indenticulata sp. nov. resembles I. sagamiensis Gamô, 1962 in the shape of carapace and the type of setae on the uropod peduncle. However, the new species is distinguished from congeners by having only setules on dorso-median carina of carapace. Also, the number of setae of uropodal peduncle and endopod is different from the I. sagamiensis. The new species is described with full illustration including mouthparts. A key to the Korean Iphinoe species is also provided.

RESUMEN

This study on Korean cumaceans is based on the specimens collected from Geomun Island in Korea. The one species of the Genus Bodotria (Bodotriidae) is identified as new species, Bodotria paraspinifera sp. nov., which is similar to B. spinifera Gamô, 1986 in having the spinulose on body, dorso-lateral carina of carapace well-developed, carapace almost rectangular in dorsal view, carpus of pereopod 1 dilated, pereopod 2 without ischium, and uropod endopod uniarticulate. However, shape of carapace anterior portion, spinulose distribution pattern, and setae patterns and segment ratios of appendages are characteristics which serve to distinguish the new species from B. spinifera. Herein we described with full illustration containing information of the mouthparts for the new species. Also, a key to the Korean Bodotria species is provided.

RESUMEN

OBJECTIVE: Nucleophosmin (NPM1) has been suggested to be involved in the pathophysiologic mechanism of inflammatory disorders. We measured the expression level of NPM1 in nasal polyp (NP) tissues of patients with chronic rhinosinusitis with nasal polyposis (CRSwNP). We also assessed the correlation between NPM1 expression and other parameters such as eosinophilic infiltration, inflammatory cytokines, and clinical indicators such as Lund-Mackay computed tomography (CT) score. METHODS: Thirty patients with CRSwNP were included. We performed pre-operative CT scan to determine Lund-Mackay CT scores. During endoscopic sinus surgery, we harvested NP tissues from patients with CRSwNP. We performed Sirius red staining to evaluate eosinophilia and conducted immunohistochemical staining for NPM1 and real-time PCR for cytokines including interleukin (IL)-5, IL-17A, and IL-32. RESULTS: The mRNA expression of NPM1 was significantly up-regulated in eosinophilic NP tissues (RQ 0.58 ± 0.06), compared to non-eosinophilic NP tissues (RQ 0.38 ± 0.08, p < 0.05). In the epithelium of NP tissue, a significant positive correlation was observed between eosinophilic infiltration and NPM1 expression. The expression of NPM1 was significantly correlated with that of IL-5 (r = 0.6229, p = 0.0004), IL-17A (r = 0.5971, p = 0.001), and IL-32 (r = -0.5985, p = 0.0068). There was no significant correlation between the mRNA expression of NPM1 and the Lund-Mackay CT score (Spearman r = -0.2563, p = 0.1879). CONCLUSION: Expression of NPM1 was significantly increased in eosinophilic NP tissues from patients with CRSwNP. We observed an association between NPM1 expression and various pro-inflammatory cytokines such as IL-5, IL-17, and IL-32 and eosinophilic infiltration, which is thought to contribute to the pathophysiology of NP.

RESUMEN

A new species, Gammarus baengnyeongensis sp. nov., belonging to the family Gammaridae Leach, 1814 was collected from the Baengnyeongdo and Daecheongdo islands in South Korea. This new species is characterized by the flagellum of antenna 2 with calceoli, and the length ratio of outer ramus to inner ramus of uropod 3. Descriptions of diagnostic characteristics of the species are provided in the text. We performed statistical analysis to confirm the significance of morphological differences of interspecies. Additionally, we compare the new species to related species by the partial sequence of the mitochondrial cytochrome c oxidase subunit 1 (CO1) gene. Genetic distances between the new species and closely related species rated from 29.1-40.8%, which highly supports Gammarus baengnyeongensis sp. nov. as a valid species. In this study, we add one new Gammarus species in Korean waters.

RESUMEN

Mast cells express high-affinity IgE receptor (FcεRI) on their surface, cross-linking of which leads to the immediate release of proinflammatory mediators such as histamine but also late-phase cytokine secretion, which are central to the pathogenesis of allergic diseases. Despite the growing evidences that mammalian target of rapamycin (mTOR) plays important roles in the immune system, it is still unclear how mTOR signaling regulates mast cell function. In this study, we investigated the effects of 3-benzyl-5-((2-nitrophenoxy) methyl)-dihydrofuran-2(3H)-one (3BDO) as an mTOR agonist on FcεRI-mediated allergic responses of mast cells. Our data showed that administration of 3BDO decreased ß-hexosaminidase, interleukin 6 (IL-6), and tumor necrosis factor-α (TNF-α) release in murine bone marrow-derived mast cells (BMMCs) after FcεRI cross-linking, which was associated with an increase in mTOR complex 1 (mTORC1) signaling but a decrease in activation of Erk1/2, Jnk, and mTORC2-Akt. In addition, we found that a specific Akt agonist, SC79, is able to fully restore the decrease of ß-hexosaminidase release in 3BDO-treated BMMCs but has no effect on IL-6 release in these cells, suggesting that 3BDO negatively regulates FcεRI-mediated degranulation and cytokine release through differential mechanisms in mast cells.
