RESUMEN

Results of surgical treatment of cavernous liner hemangiomas in 47 patients have been analysed. In 7 cases the original method of surgery in unresectable liver hemangiomas was used. The method was designed by the authors of the article. The proper diagnosis of liver hemangiomas is possible in the specialized divisions only. Ultrasound examination, CT and laparoscopy play the major role in diagnosis. Anatomical liver resection is the major surgical method. In cases of unresectable hemangiomas the method of omento-hepatopexy with subsequent X-ray therapy is indicated.

RESUMEN

Under analysis is an experience with examining and treatment of 167 patients with focal lesions of the liver using computed tomography. Percutaneous transhepatic puncture and draining the hepatic abscesses and cysts under control of computed tomography is an independent method of treatment used in 53 patients. The technique of performing the puncture and drainage is described. Specific features of surgical treatment of focal lesions of the liver are described. Decompression of bile ducts in the postoperative period is shown to be necessary. A conclusion is made about high efficiency of computed tomography in diagnosis and treatment of focal lesions of the liver.

RESUMEN

From 1977 till 1993 in the clinic 108 patients were treated for the injury of extrahepatic biliary ducts occurred during cholecystectomy or gastric resection, performed mainly in the other medical departments. Injury of the ducts was diagnosed while operation performance in 68 patients. In 58 (54,6%) observations the reason of injury was the lack of surgeon's qualification and in 22 (20,3%)--nonadequate conditions of operation performance.

RESUMEN

The automatized value of treatment results and postoperative period course was done according to special programs in 218 patients with choledocholithiasis using the united computer system. Integral risk factors and four regressional patterns of postoperative period course were obtained using the factor analysis. Prognosis was quite right in 87,5-97,5% of patients.

RESUMEN

In the clinic, the special appliances and apparatus for collection of the bile, its filtration, sterilization and introduction into the stomach of a patient were developed and used in patients with external biliary fistula. Since 1987, the original method for treatment of the alcoholic disease has been introduced. It was used in 40 patients.

RESUMEN

Of 7000 patients operated on biliary ducts in 622 the recovering and reconstructive interventions were conducted. Late follow-up result was studied in 313 (51%) patients. Good result was noted in 179 (57.7%) patients, fair--in 99 (32.2%), poor--in 35 (10.1%) in terms from 3 months till 17 years. In patients operated on for the "forgotten" biliary ducts concrements, frequently combined with stenosis of common biliary duct terminal portion, most frequently good and fair result was noted. Late follow-up result of reconstructive operations using external transhepatic carcass drainage was significantly better than after application of other methods of correction for high stricture and injury of biliary ducts.

RESUMEN

In treatment of 86 patients with a iatrogenic injury, or cicatricial stricture of the extrahepatic bile ducts, the Prader-Smith, Saypole-Kurian transhepatic drainage of hepatico-digestive anastomosis and that with the use of the method suggested by the authors have been used. The technique for performance of the operations is described, the special instruments are offered. After the operation, only one female patient has developed a subphrenic abscess.

RESUMEN

The authors have an experience with operations on 78 patients with scarry strictures and iatrogenic injuries of the hepatocholedochus who had transhepatic drainage of the hepaticodigestive anastomosis after Pradery--Smith and Seipol--Kurianu. The authors have developed a special method of operations. For each step there are special instruments facilitating performance of operations, reducing traumatism and minimizing the amount of postoperative complications.

RESUMEN

The authors studied the results of sonography of the breast and zones of regional lymph drainage in 15,973 patients. In 154 of them puncture aspiration biopsy was conducted under ultrasonic control. Breast carcinoma was found in 23 patients, in 2 of them the tumor was not palpable. The examination was performed on the Aloka-SSD-630 apparatus. The sonographic picture was that of a structure with diminished echogenic properties, irregular notched contours, and non-uniform echo signals from the internal structures. Echography with puncture biopsy made it also possible to reveal the presence of nonpalpable axillary metastatic lymph nodes and to establish the precise differential diagnosis of benign and malignant intraductal tumors. The method of puncture aspiration biopsy under control of ultrasonic examination is highly effective in the differential diagnosis of breast carcinoma.

RESUMEN

Drainage of the thoracic lymph duct and lymphosorption were used in 63 patients with obstructive jaundice of different etiology and cholestatic hepatitis. Lymph was returned into the umbilical vein with transfusion of proteins, glucose, steroid hormones and other preparations.

RESUMEN

In the clinic, 56 patients with an external pancreatic fistula was treated. After conservative treatment, a fistula closed in 43 patients, and was functioning in 13. Fistuloenterostomy was performed in 6 patients, resection of the left lobe of the pancreas--in 7. There were no complications and lethal outcomes. The method for return of the pancreatic juice into the duodenum has been suggested.

The article analyses experience in diagnosing the pathological condition and treating 125 patients with acute injury to the diaphragm (61 patients) and chronic traumatic diaphragmatic hernias (64 patients). The authors indicate the therapeutic and diagnostic means in this pathological emergency due to the acute trauma of the chest and abdomen or incarceration of the abdomen. The diagnostic errors and their consequences are analysed. Operation was conducted on 104 patients. In ruptures of the diaphragm in the zone of the esophageal hiatus the cardia was drawn downwards and fastened under the diaphragm; in cases of fractures of the bony framework of the chest with the formation of fragments, panel fixation of the costal fragments and the sternum was carried out by the authors' methods. The results of the surgical interventions are shown (18 patients--17.4% died) with emphasis on the character of the disorders and the lapse of time from the development of the emergency to the application of therapeutic aid.
