BACKGROUND: Food allergies can substantially burden patients and families by negatively affecting finances, social relationships, and personal perceptions of health. This study was performed under the Finnish Allergy Programme aimed at reducing avoidance diets to foods in schoolchildren by 50%. The main goal of this study was to investigate how many children could be freed from diet restrictions in a Finnish school district through a diagnostic algorithm including component-resolved diagnostics and food challenge. The secondary aim was to provide a crude estimate of the burden of the elimination food diets in the region, and the savings associated with the proposed intervention. METHODS: A total of 205 children on a food avoidance diet according to the school register because of food allergy were invited into the study. One hundred and fifty-seven children were interviewed, tested for IgE to extracts and allergen components and food challenged in respective order. RESULTS: After two years, 12 children still had an avoidance diet and three of them were treated successfully with sOTI; the rest suspended their avoidance diet (n = 134) or dropped out of the study (n = 11). The cost of the elimination diets was estimated in 172 700€ per year at start and 13 200€ per year at the end of the study; total savings were 128 400€ yearly. CONCLUSIONS: The results demonstrate a 65% reduction of avoidance diets to foods in school-aged children, exceeding the 50% aim of the Finnish Allergy Programme. Therefore, it is possible to actively reduce the number of food allergy diagnoses that remain unmonitored in the society through a tailored diagnostic work-up.

RESUMEN

Asthma is an inflammatory disease of the lower airways. The typical symptoms of asthma are cough, wheezing and shortness of breath. Asthma is diagnosed based on measures of pulmonary function showing variable or reversible airways obstruction. The basic pharmacological treatment consists of alleviating the asthmatic inflammation with regular inhaled glucocorticoids and relieving sudden obstructions with as-needed inhaled beta2-agonists. The treatment is adjusted based on asthma control. If good control of asthma is not achieved with low to medium doses of inhaled glucocorticoids, additional control medication (inhaled long acting beta2-agonists, antileukotrienes, in adults also theophylline or tiotrohium) should be introduced.

RESUMEN

Immunotherapy involves the specific treatment of IgE-mediated allergic diseases, indicated for allergic rhinitis and conjunctivitis, allergic asthma, insect sting (bee and wasp) allergy, and food allergy (especially cow's milk, egg and wheat). Subcutaneous injection immunotherapy with pollens (both trees and grass), animal danders, insect venoms and house dust mite preparations for allergic rhinitis and asthma is effective for both adults and children. Sublingual immunotherapy indicated for allergic rhinitis caused by grass pollens (especially timothy), is effective and appears to be a safe route of administration. Specific oral tolerance induction is used in children over five years of age with severe food allergy.

The prevalence of hypertension is high in Finland. Only one fourth of the drug-treated hypertensive patients reach their target pressure. We evaluated a commercially available telemedicine system for impact in reaching better blood pressure control among Finnish hypertensive patients. A telemedicine system, the Doc@home, was used to assist blood pressure treatment in 19 Finnish hypertensive patients. Blood pressure control improved during the 3-month follow-up. Patient-to-Doc@home compliance was good, but study physicians found the system time consuming in the beginning. According to our results, the Doc@home telemedicine system showed a promising approach in hypertension treatment but needs some further development and trained staff to become a still more practical alternative.
