RESUMEN

Objective: In this study, we aimed to evaluate the effects of demographic data and comorbid diseases on the prognosis and treatment diagnosed with laryngeal squamous cell cancer (LSCC). Methods: Medical records of LSCC patients treated and followed up in a single referral center between 2008 and 2019 were retrospectively reviewed. In addition to the demographic data, the results of overall survival (OS), disease-specific survival (DSS), disease-free survival (DFS), locoregional-free survival (LRFS), and factors affecting recurrence were analyzed. Results: We included 573 patients with a mean age of 60.1±9.8 years. Of the 573 patients, 94.2% (540) were men, 93.7% (537) were smokers, 40.1% had at least one comorbid disease, and 69.8% (400) presented with glottic LSCC. The five-year OS, DSS, DFS, and LRFS rates for all cases were 65.7%, 79.9%, 67%, and 74.7%, respectively. In early-stage LSCC treatment, the rates of OS (p=0.008), DFS (p=0.024) and LRFS (p=0.01) were statistically significantly higher in the endolaryngeal laser surgery (ELS) group compared with the radiotherapy (RT) group. In advanced-stage LSCC treatment, total laryngectomy had statistically significantly higher five-year DFS (p=0.003) and LRFS (p=0.002) rates compared to chemoradiotherapy. Conclusion: Our study showed that ELS provided higher rates of OS, DFS, and LRFS compared to RT in the treatment of early-stage LSCC. Recurrence was significantly higher in supraglottic tumors, advanced-stage tumors, and in patients with clinical N positivity.

RESUMEN

PURPOSE: To investigate whether patients undergoing percutaneous liver mass biopsy (PLMB) can be safely discharged following a two-hour monitoring period. METHODS: A multi-center prospective analysis was conducted for 375 patients (196 males and 179 females), mean age 63 ± 12.45 years (range 37-89) who underwent PLMB between August 2023 and March 2024. Patients were monitored for 24 h, and complications were classified as minor or major. The timing of complications was categorized into three groups: within the first two hours, between the 2nd and 24th hours, and within 1 week after 24 hours. RESULTS: Minor complications occurred in 18.93% (71/375) and major complications in 2.13% (8/375). Most minor complications (80.2%, 57/71) appeared within the first two hours, 12.7% (9/71) between 2 and 24 h, and 7.1% (5/71) after 24 h. All major complications (62.5%, 5/8) except late-onset cases, occurred within the first two hours. No major complications occurred between 2 and 24 h. Late-onset major complications occurred in 37.5% (3/8) after 24 h. CONCLUSION: The two-hour monitoring period did not adversely impact patient management regarding minor complications and is safe for identifying all major complications except for late-onset ones. Extending the post-biopsy recovery period does not significantly improve patient care.

RESUMEN

BACKGROUND: In the treatment of early stage laryngeal cancers, surgery (transoral laryngeal surgery (TOLS), open partial laryngeal surgery (OPLS) and radiotherapy (RT) are used. OBJECTIVES: Compare the oncological results of patients with early stage laryngeal squamous cell carcinoma (LSCC) treated with TOLS or RT. DESIGN: Retrospective. SETTINGS: Tertiary training and research hospital. PATIENTS AND METHODS: The participants were divided into patients who underwent TOLS and RT treatment. The groups were compared with each other in terms of local recurrence, regional recurrence, distant metastasis, 3 and 5-year overall survival (OS), disease-free survival (DFS), disease-specific survival (DSS) and laryngectomy-free survival rates (LFS). MAIN OUTCOME MEASURES: The effects of TOLS and RT treatment on local control, regional control, OS, DFS, DDS and LFS in early stage laryngeal cancers. SAMPLE SIZE: 261. RESULTS: The mean follow-up time was 48 (26) months. There were 186 patients who underwent TOLS and 75 patients who underwent RT treatment. Gender, cigarette/alcohol consumption, tumor localization, anterior commissure involvement, tumor grades, recurrence rates and recurrence localizations of the groups were similar. The 5-year overall, disease specific, disease free and laryngectomy-free survival rates were 85.9%, 88%, 79.4%, 96.3% in the TOLS group and 74.3%, 76.7%, 72.3%, 85.2% in the RT group (P=.034, .065, .269, .060, respectively). CONCLUSIONS: TOLS had equal and good oncological outcomes on OS and DFS compared to RT. Anterior commissure involvement was statistically significant independent prognostic risk factor for DFS in both groups. The 5-year OS rate was greater in the TOLS groups (P=.034). LIMITATIONS: Retrospective, but to the best our knowledge, this is the first study in Turkey with a high patient volume and a long follow-up time.

RESUMEN

PURPOSE: Adolescents today live in a world surrounded by electronic devices. They did not experience life before the internet and they are therefore called 'digital natives'. As a result of increased screen exposure among adolescents, many short-term and long-term health problems may occur. No measurement tool to date includes multidimensional variables of screen exposure other than the duration of the screen exposure. The aim of this study was to develop a scale that evaluates screen exposure within different dimensions and to confirm the reliability and validity of that scale. DESIGN AND METHODS: This study was methodological. The research was completed with 347 adolescents. A personal information form and the Evaluating the Screen Exposure of Adolescents (ESEA) scale were used for data collection. IBM SPSS Statistics 24.0 and IBM AMOS 24.0 software programs were used for data analysis, and descriptive statistics were used to analyse numerical variables. Exploratory and confirmatory factor analyses were conducted to determine the factorial structure of the scale. RESULTS: The scale was finalised with a total of 23 items within five factors, which are willingness/desire for screen exposure, socialisation, family control, procrastination tendency, and effects of prolonged screen exposure. The factor loadings of the scale vary between 0.370 and 0.825. The internal consistency coefficient (Cronbach's alpha value) of the total scale was found to be 0.79. CONCLUSION: The ESEA scale, which was psychometrically evaluated in this study, assesses the screen exposure of adolescents multidimensionally and provides consistent measurements. PRACTICE IMPLICATIONS: Nurses working with adolescents, and especially pediatric nurses and public health nurses, can use this measurement tool to assess the screen exposure of adolescents in a multidimensional way. This scale supports better problem definition and effective interventions by nurses considering multidimensional screen exposure.

RESUMEN

OBJECTIVES: This study aimed to determine the impact of different types of masks on the voices of healthcare professionals who had to wear masks for an extended amount of time during the pandemic period and had a healthy voice. METHODS: Our research included 41 healthcare workers. The participants were separated into two groups: surgical (n = 21) and N95 mask users (n = 20). Healthcare workers evaluated masks before and after wearing them for at least 8 hours throughout the workday. All subjects had a videolaryngoscopic examination; the Voice Handicap Index-10 (VHI-10), GRBAS, acoustic voice analysis (F0, jitter%, shimmer%, noise/harmonic ratio, relative average perturbation [RAP]), aerodynamic measures (maximum phonation time, MPT), and blood oxygen saturation were evaluated. RESULTS: Although both groups' VHI-10 scores increased after using the mask, this rise was not statistically significant in our research. According to the GRBAS classification, voice quality deterioration was identified in 9.6% (mild-moderate) of the group using surgical masks and 15% (mild) of the group wearing N95. Only the jitter and RAP values of individuals wearing both surgical and N95 masks were determined to be statistically significant. There was no significant change in MPT following mask wear in either group. Both the surgical and N95 mask-using groups showed a substantial drop in blood oxygen saturation before and after mask usage. CONCLUSION: There was no change in voice quality between healthcare workers wearing surgical and N95 masks. It has been noticed that voice perception and quality are affected by the mask's barrier effect rather than the kind of mask.

RESUMEN

Cryogenic cooling has gathered significant attention in the manufacturing industry. There are inherent difficulties in machining materials that are difficult to machine because of high levels of hardness, abrasiveness, and heat conductivity. Increased tool wear, diminished surface finish, and reduced machining efficiency are the results of these problems, and traditional cooling solutions are insufficient to resolve them. The application of cryogenic cooling involves the use of extremely low temperatures, typically achieved by employing liquid nitrogen or other cryogenic fluids. This study reviews the current state of cryogenic cooling technology and its use in machining difficult-to-machine materials. In addition, this review encompasses a thorough examination of cryogenic cooling techniques, including their principles, mechanisms, and effects on machining performance. The recent literature was used to discuss difficult-to-machine materials and their machining properties. The role of cryogenic cooling in machining difficult materials was then discussed. Finally, the latest technologies and methods involved in cryogenic cooling condition were discussed in detail. The outcome demonstrated that the exploration of cryogenic cooling methods has gained prominence in the manufacturing industry due to their potential to address challenges associated with the machining of exotic alloys.

RESUMEN

PURPOSE: To determine the preferences of Ear, Nose, and Throat (ENT) and Head-Neck Surgery (HNS) specialists and residents undergoing training for online or face-to-face training and the factors affecting the preference. METHODS: An 16-item questionnaire was created using Google Forms and was administered to residents undergoing training in ENT departments and to specialists working in the same field. The questionnaires were distributed online and requested to be completed between August and October 2020. The study participants were analyzed in two groups according to the preference for online training or face-to-face training. RESULTS: Evaluation was made of a total of 173 participants, as 68 (39.3%) in online training, and 105 (60.7%) in face-to-face training. Online training comprised 47 (69.1%) females and 21 (30.9%) males with a mean age of 38.9 ± 8years and face-to-face training comprised 68 (64.8%) females and 37 (35.2%) males with a mean age of 37.9 ± 7.5years (gender: P = 0.55, age: P = 0.10). Of the total sample, 39 (22.5%) subjects were single and 134 (77.5%) were married. Face-to-face training was preferred by 61.2% of the married respondents and by 59% of those who were single. While 43.4% stated that visuals and documents were similar in both forms of training, those who stated that documentation was better in face-to-face training constituted 90.2% of the group that preferred face-to-face training (P = 0.0001). Of the total participants, 65.4% stated that concentration was easier in face-to-face training (P = 0.0001). When the groups were compared according to areas of interest, more of those involved in rhinology and head-neck surgery were in face-to-face training, and those with an interest in otology were seen to be in online training (P = 0.002). A wish to continue online training after the pandemic was expressed by 80.9% of the whole sample, and 68.6% wished to continue with face-to-face training (P = 0.0001). Hybrid meetings were determined to have been selected by 86.1% (P = 0.0001). CONCLUSION: Online web seminars have an important role as a teaching and learning tool. There is a need for further research to evaluate how these clinically focused seminars can be presented at high quality and how they can provide benefit in training.

RESUMEN

Although chip morphology changes according to the machining method and related cutting parameters, chip formation affects the quality of the machined surface. In this context, it is very important to understand the relationship between chip morphology and surface quality, especially in materials that are difficult to machine. In the presented study, the changes in chip morphology, surface morphology, and surface quality criteria (Ra and Rz) that occurred during the milling of precipitation-hardened steel in different cutting environments were analyzed. Milling experiments were carried out in dry, MQL (minimum quantity lubrication), nano-MQL (graphene), nano-MQL (hBN), Cryo, and Cryo-MQL environments using TiAlN-coated inserts and three different cutting speeds and feed rates. While the highest values in terms of Ra and Rz were measured in dry machining, the minimum values were obtained in a nano-MQL (hBN) cutting environment. Due to the lubrication and low friction provided by the MQL cutting environment, chips were formed in thinner segmented forms. This formation reduced the chip curve radius and thus provided a more stable surface morphology. On the other hand, Cryo-ambient gas could not effectively leak into the cutting zone due to the intermittent cutting process, but it increased the brittleness of the chips with the cooling effect and provided a similar surface morphology. The values of minimum Ra and Rz were obtained as 0.304 mm and 1.825 mm, respectively, at a 60 m/min cutting speed and 0.04 mm/rev feed. Consequently, the use of nano-MQL cutting medium is seriously recommended in terms of surface quality in milling operations of difficult-to-machine materials.

RESUMEN

PURPOSE: It was aimed to analyze the relationship of the respiratory functions, respiratory muscle strength, magnitude of the curvature, angle of trunk rotation (ATR) and brace-wearing duration in patients with adolescent idiopathic scoliosis (AIS). METHODS: Sixty patients with AIS (43 females), with maximum Cobb angles between 20° and 66° were included in the study. ATR values were measured with a scoliometer. Respiratory function parameters (forced vital capacity [FVC], forced expiratory volume in one second [FEV1], FEV1/FVC ratio, peak expiratory flow [PEF]) and respiratory muscle strengths (maximum inspiratory pressure [MIP] and maximum expiratory pressure [MEP]) were measured by combined spirometry. RESULTS: There was a negative significant correlation between maximum Cobb angle and ATR with respiratory function parameters except for PEF and FEV1/FVC (r = -0.258-0.441; p = <0.001-0.047). There was a moderate negative correlation between ATR with MIP (cmH2O) (r = -0.377; p = 0.003) and MEP (cmH2O) (r = -0.362; p = 0.005). On the other hand, no correlation was found between brace-wearing duration with respiratory functions, maximum Cobb angle and brace-wearing duration with respiratory muscle results. CONCLUSION: The results of this study showed that, respiratory functions were normal or mildly affected, and respiratory muscle strengths were weak in AIS. Increased Cobb angle and ATR negatively affected respiratory function; increased ATR was associated with decreased inspiratory and expiratory muscle strength. It is extremely important to carefully evaluate the respiratory system and to know the variables that affect respiratory functions and respiratory muscle strength in achieving optimum recovery in the holistic treatment of individuals with scoliosis.

RESUMEN

BACKGROUND AND OBJECTIVES: Idiopathic sudden sensorineural hearing loss (ISSHL) is a rapid loss of hearing, exceeding 30 dB in at least 3 consecutive frequencies within 3 days, without any identifiable cause despite thorough investigations. Currently, the etiology and pathogenesis of ISSHL have not been fully elucidated. This study aimed to assess the size of the cochlear nerve in patients with ISSHL and explore its relationship with pretreatment audiograms and treatment response. Subjects and. METHODS: A total of 125 patients (59 [47.2%] women; mean age 47.7±13.8 years [minimum-maximum: 21-76]) and 60 healthy participants (27 [45%] women; mean age 45.7±16.8 years [minimum-maximum: 20-76]) as a control group were included in this study. The size of the cochlear nerve was assessed on the affected side, compared to the control group, as well as on the unaffected side. Pretreatment and posttreatment audiological values were also analyzed. RESULTS: The cross-sectional area (CSA), vertical diameter (VD), and horizontal diameter (HD) of the CN were found to be smaller on the affected side of ISSHL patients compared to the control group (p<0.01; p=0.04; p=0.02, respectively). In the study group (affected side of ISSHL patients), there were no significant differences in VD, HD, and CSA values between pretreatment audiogram types (p=0.23; p=0.53; p=0.39, respectively), and initial hearing levels (p=0.16; p=0.22; p=0.23, respectively). Furthermore, there were no significant differences in VD, HD, and CSA values between the recovery groups according to Furuhashi criteria (p=0.18; p=0.37; p=0.27, respectively). CONCLUSIONS: The size of the CN may be a risk factor for ISSHL, but it does not affect the type of audiogram curves and was not prognostic in terms of treatment response.

RESUMEN

Introduction: It is important to predict success before the treatment of urolithiasis. We aimed to predict the success of shock wave lithotripsy (SWL) by comparing twinkling artifact (TA) revealed through colour Doppler ultrasonography (CDUS) with stone density in non-contrast computed tomography (NCCT). Material and methods: Eighty patients who underwent SWL between January 2021 and January 2022 were included in the study. Patients with stones of 5-20 mm in the renal pelvis and proximal ureter at NCCT were included. Patients' demographics, Hounsfield units (HU) in NCCT, and TA grades in CDUS were recorded. The stone-free rate after SWL, additional treatments, overall success rates, and the association between TA and success rates were evaluated. Results: The mean age was 47.41 ±15.08 years. The mean BMI was 24.49 ±3.67 kg/m2. Twenty-three (28.8%) patients were TA grade 0, 33 (41.2%) patients were grade 1, and 24 (30%) were grade 2. The mean HU of TA grades 0, 1, and 2 of stones were 628 ±107, 864 ±123, and 1166 ±292, respectively. The HU increased along with the increase in the TA grade of the stone (p <0.01). The mean number of SWL sessions was 2.26 ±0.75 in patients with TA grade 0, and 2.92 ±0.40 in patients with TA grade 2. The mean number of SWL sessions increased along with the increase in TA grade (p <0.01). The stone-free rate decreased as the TA grade increased. Stone diameter and TA were the only predictors of SWL success. Conclusions: We think that TA may be useful in predicting SWL success.

RESUMEN

OBJECTIVES: This study aims to analyse the way immigrants and their personality traits get affected by traumatic events in the post-migration process. METHODS: This descriptive study was conducted through Google Forms with the involvement of 2,509 immigrants. A descriptive questionnaire, the "IFOMA Post-Migration Post-Traumatic Effect Scale" and the "GADOT Personality Types Determination Scale" were used to collect the research data. Independent samples t-test, one-way ANOVA, and related sample Friedman's two-way analysis tests were used in data analysis. RESULTS: Immigrants were exposed to significant effects in all sub-dimensions of the Post-Migration Post-Traumatic Effect Scale. Experiencing post-traumatic stress was found to be significantly related to the research parameters, which, respectively, are gender, age, marital status, educational background, legal status, years of living in the current country, employment status, ethnicity, Turkish language proficiency, and post-migration psychological problems (p < 0.05); 42.8% of the immigrants had the Type 9 personality, and all personality types were affected by the Psychological Affection, Physical Affection, Anxiety, and Social Adaptation sub-dimensions, respectively (p < 0.05). CONCLUSION: By analysing the impact of migration-induced trauma on immigrants within their society, it can be recommended to implement initiatives specific to immigrants' personality traits and to carry out protective/preventive projects that will minimize immigrants' exposure to trauma and encourage their participation in social adaptation processes.

RESUMEN

OBJECTIVE: Spinal fusion, which is widely used in the surgical treatment of adolescent idiopathic scoliosis (AIS), limits the movements of the vertebral column. In this study, it was aimed to investigate the relationship between spinal mobility in the postoperative period with functional balance, quality of life, and perception of appearance in individuals with AIS. METHODS: Thirty patients with AIS who underwent posterior spinal fusion surgery 1 to 3 years ago were included in the study. A universal goniometer was used to measure a spinal range of motion (ROM), functional reach test for functional balance, Scoliosis Research Society-30 (SRS-30) for quality of life, and Spinal Appearance Questionnaire (SAQ) for perception of appearance. RESULTS: Surgical fusion length was significantly correlated with movements in the sagittal and transverse planes (r = - 0.383-[- 0.608]; p = < 0.001-0.037). Except for spinal left rotation, there was a significant positive correlation between functional balance level and ROM results (r = 0.374-0.523; p = 0.003-0.42). The SRS-30 total score correlated significantly with all other ROM measures except for rotations (r = 0.434-0.574; p = 0.00-0.016). SAQ total data correlated significantly with all ROM measurements (r = - 0.553-[- 0.395]; p = 0.002-0.031). CONCLUSIONS: In AIS, limitation of movement in the coronal, sagittal, and transverse planes is observed after fusion surgery. Spinal ROM is affected by the level of fusion performed at surgery. Postoperative spinal limitation of motion adversely affected functional balance, quality of life, and perception of appearance. We believe that comprehensive rehabilitation approaches that improve postoperative range of motion and increase functionality are important for optimal postoperative recovery.

RESUMEN

In this article, we present a case of diffuse follicular variant papillary thyroid carcinoma with pituitary metastasis, which is a rare cause of pituitary metastasis. The follicular variant of papillary thyroid carcinoma is an uncommon variant of papillary carcinoma. A 74-year-old male was presented with weakness, fatigue, and a decreased appetite. The patient was diagnosed with secondary adrenal and thyroid insufficiencies. Imaging revealed a pituitary mass with suprasellar extension, right cavernous sinus invasion, and optic chiasm compression. Thyroid ultrasonography revealed a nodule with a maximum size of 7.2cm in the right lobe. Cytological examination via fine-needle aspiration suggested papillary thyroid cancer. Total thyroidectomy with central and right lateral neck dissection confirmed the diagnosis of diffuse follicular variant of papillary thyroid carcinoma. Owing to visual field defects, the patient underwent transsphenoidal surgery. Histological and immunohistochemical evaluations confirmed pituitary metastasis from the papillary thyroid cancer. Radioactive iodine treatment and gamma knife radiotherapy of the pituitary gland were performed. The initiation of sorafenib treatment was deemed appropriate during the follow-up. A significant decrease in the thyroglobulin levels was observed after sorafenib treatment. Pituitary metastasis should be considered in patients diagnosed with hypopituitarism and pituitary lesions at initial evaluation. The presence of visual field defects may be an indication for neurosurgical intervention and guide both diagnosis and treatment. The management of papillary thyroid cancer and the role of treatment modalities in prognosis depend on the biological behavior of the tumor. Early diagnosis and multidisciplinary management are crucial for the treatment of these patients.

RESUMEN

OBJECTIVE: To examine the treatment outcomes of the EXTREME regimen as the first-line therapy for recurrent/metastatic squamous cell carcinoma of the head and neck. STUDY DESIGN: Observational study. Place and Duration of the Study: Department of Medical Oncology, Ankara Diskapi Yildirim Beyazit Training and Research Hospital, Turkiye, between January 2014 and December 2021. METHODOLOGY: A total of 60 patients with recurrent/metastatic head and neck cancers were treated with EXTREME regimen. Survival curves were estimated using the Kaplan-Meier method. Cox proportional hazards regression models were used to determine the factors associated with progression-free survival (PFS) and overall survival (OS). RESULTS: Patients receiving cetuximab in combination with chemotherapy for metastatic or unresectable disease were obtained from patient files and electronic medical records. Majority of patients were male (78.3%). The median PFS of patients was 7 months. The median survival of the patients was 9.06 months. During follow-up, 55 patients (91.7%) relapsed, and 51 patients (85%) died. The median survival of patients with eastern cooperative oncology group (ECOG) performance status 0, 1, and 2 was 16, 9, and 4 months, respectively. And the survival of patients was found significantly lower with lower ECOG performance scores. CONCLUSION: OS was determined as 9.06 months. ECOG performance score had an important effect on the survival. KEY WORDS: Cetuximab, Head and neck cancer, Chemotherapy.

RESUMEN

BACKGROUND: Gastroesophageal reflux disease is a common condition worldwide. There is no curative treatment for gastroesophageal reflux disease. Endoplasmic reticulum stress leads to the activation of the unfolded protein response and has an important role in inflammation. The aim is to determine the role of endoplasmic reticulum stress in the follow-up of individuals with gastroesophageal reflux disease and the temporal changes of endoplasmic reticulum stress markers with treatment. METHODS: Twenty-four subjects in total were recruited prospectively, of whom 15 had nonerosive reflux disease. Two biopsies from 2 cm above the esophagogastric junction, 2 biopsies from gastric antrum mucosa, and 2 biopsies from gastric corpus mucosa were taken. Simultaneously, 2 tubes of venous blood samples were drawn from each individual (1 tube for studying the genetic markers and 1 tube for analyzing the CYP2C19 polymorphism). RESULTS: The mean age was 42.3 ± 17.6 for women and 34.66 ± 11.2 for men. Pantoprazole, esomeprazole, rabeprazole, and lansoprazole preparations were used for treatment. There was no significant difference between tissue and blood samples for panel genes ATF-6, XBP-1, DDIT-3, DNAJC-10, and EIF-2-AK before treatment. There was a significant decrease in the level of ATF-6, XBP-1, DNAJC-9, EIF2-AK, and NF-2L-2 genes in blood after treatment. In the comparison of proton pump inhibitors, significant decreases in the expression of the ATF-6, XBP-1, and DNAJC-9 mRNAs were detected in blood from individuals after treatment. RESUMEN

BACKGROUND: Health promotion interventions for immigrant health are generally directed toward women and youth. There is no program specific to migrant male in the global and national literature to protect, improve and promote their health. The aim of this study was to examine the effect of the Increasing Health Awareness of Immigrant Men (IHAPIM) program on health perceptions, health responsibilities, perceived stress level, attitudes toward utilizing health care services and the types of coping strategies of immigrant male. METHODS: Researchers applied the short-term IHAPIM program experimental group for 5 weeks. This study was held in the two districts where predominantly immigrants live. The immigrant male's health perceptions, health responsibilities, perceived stress level, attitudes toward utilizing health care services and the types of coping strategies measured before and after 3 months of the IHAPIM program was performed. RESULTS: The study results showed that the levels of health perceptions, health responsibilities and the types of coping strategies of immigrant male were significantly different between the two study groups. CONCLUSIONS: At the end of the study, the experimental group showed improvement in measures of the health perceptions, health responsibilities, attitudes toward utilizing health care services, the types of coping strategies and perceived stress level of male decreased. Nursing interventions specific to immigrant male and sensitive to their language have led to positive changes in the health variables of immigrant male.

RESUMEN

BACKGROUND AND STUDY AIMS: Intestinal metaplasia (IM), and Helicobacter pylori (HP) infection can be shown as risk factors in the development of gastric cancer (GC). WNT signaling pathway plays a critical role in carcinogenesis. However, the literature studies are limited on the significance of this pathway for the transition from IM to GC. PATIENTS AND METHODS: We aimed to investigate the importance of the genes of WNT signaling pathways diagnostic and prognostic markers in the presence and absence of HP in conversion from IM to GC. 104 patients, (GC group n = 35, IM group n = 45, control group n = 25) were included in this case-control study. Expression of genes in WNT signalling were searched in study groups with qRT-PCR array and qRT-PCR method. Data were analysed using PCR array data analysis software. RESULTS: Statistically significant overexpression of RHOA, CSNK1A1, DVL2, FZD8 and LRP5 genes was detected in the GC and IM groups compared to the control group (p < 0.05). Statistically significant overexpression of RHOA, CSNK1A1, DVL2, FZD8 and LRP5 genes was observed in patients with metastatic GC compared to patients with GC without metastasis (p < 0.05). It was found that the RHOA, CSNK1A1, DVL2, FZD8 and LRP5 genes were statistically significantly over-expressed in diffuse GC patients compared to non-diffuse GC patients (p < 0.05). Statistically significant overexpression of RHOA, CSNK1A1, DVL2, FZD8 and LRP5 genes was detected in HP positive IM patients compared to HP negative IM patients (p < 0.05). CONCLUSION: Overexpression of RHOA, CSNK1A1, DVL2, FZD8 and LRP5 genes in IM may suggest that these genes are important markers in the development of IM and inflammation with HP. In addition, these genes are linked to tumor burden in the GC group. Consequently, we can conclude that these genes are poor prognosis biomarkers for GC and have the potential to be used as markers for future treatment monitoring.

RESUMEN

BACKGROUND: To compare the effect of five days of intensive postoperative rehabilitation and early mobilization following scoliosis surgery. METHODS: Forty adolescent patients who had undergone scoliosis surgery were randomly allocated into a rehabilitation group (RG, n = 20) and a mobilization group (MG, n = 20). The RG received five days of intensive exercise program and early ambulation, the MG received five days of a standard gait training and early ambulation. The patients were evaluated for severity of pain using a visual analog scale, thorax mobility with the thoracic mobility index, balance with the functional reach test, walking distance with the 2-min walk test, and quality of life with the Scoliosis Research Society-22 questionnaire. The length of hospital stay was recorded. RESULTS: The RG was favored over the MG for improvements from 0 to 1 week for pain. Improvements from 0 to 1 week were significantly better in the RG than the MG group for thorax mobility, balance, and walking distance. Quality of life scores improvements from 0 to 1 week were significantly better in the RG group than in the MG group. The length of hospital stay was significantly shorter in the RG group. CONCLUSION: Five days of intensive postoperative rehabilitation were superior to early mobilization in reducing the length of hospital stay, and in improving physical and functional outcomes following scoliosis surgery.
