RESUMEN

AIM: To identify and determine the mechanisms of an association between arterial hypertension (AH) and proatherogenic systemic metabolic disturbances in hypertensive disease (HD), as well as the possibility of eliminating these disturbances by antihypertensivetherapy. SUBJECTS AND METHODS: Fifty-four patients with HD and 64 persons randomly selected from an unorganized urban population were examined. Systolic and diastolic blood pressure (BP), insulin sensitivity, the most important metabolic parameters of lipids, lipoproteins, and glucose, the degree of systemic inflammation and oxidative stress, and the rate of lipoprotein proatherogenic and immunogenic modification were determined. RESULTS: All the patients with HD were found to have a regular concurrence of AH with systemic inflammation and activated free-radical reactions, metabolic abnormalities, such as atherogenic dyslipidemia, insulin resistance, atherogenic and immunogenic modified lipoproteins. Antihypertensive treatment failed to eliminate metabolic disturbances even when BP control was fully restored. CONCLUSION: AH and systemic metabolic disturbances in HD have a common pathogenetic basis and the treatment of hypertensive patients should provide the normalization of not only BP, but also inflammatory and oxidative status and systemic metabolism.

RESUMEN

Luminescent and scintillation properties of polystyrene-based plastic scintillators with ß-diketone Eu complexes are investigated. A scintillator with dibenzoylmethane Eu complex containing two phenyl groups demonstrates the maximum scintillating efficiency. It is shown that plastic scintillators efficiency is dramatically decreased if ß-diketone derivatives contain no phenyl groups as substituents. This fact can be explained by exciplex mechanism of energy transfer from a matrix to Eu complex.

Physiological stresses (heat, hemodynamics, genetic mutations, oxidative injury and myocardial ischemia) produce pathological states in which protein damage and misfolded protein structures are a common denominator. The specialized proteins family of antistress proteins - molecular chaperons (HSPs) - are responsible for correct protein folding, dissociating protein aggregates and transport of newly synthesized polypeptides to the target organelles for final packaging, degradation or repair. They are inducible at different cell processes such as cell division, apoptosis, signal transduction, cell differentiation and hormonal stimulation. HSPs are involved in numerous diseases including cardiovascular pathologies, revealing changes of expression and cell localization. We studied the possible changes in expression level of abundant mitochondrial chaperon Hsp60 and main human cytochrome P450 monooxygenase (2E1 isoform) at dilated cardiomyopathy (DCM) progression at the end stage of heart failure using Western blot analysis. The ischemic and normal humans' hearts were studied as control samples. We observed the decrease of Hsp60 level in cytoplasmic fraction of DCM- and ischemia-affected hearts' left ventricular and significant increase of Hsp60 in mitochondrial fractions of all hearts investigated. At the same time we detected the increase of P450 2E1 expression level in ischemic and dilated hearts' cytoplasmic fractions in comparison with normal myocardium and no detectable changes in microsomal fractions of hearts investigated which could be linked with increased level of oxidative injury for DCM heart muscle. In addition, all the changes described are accompanied by significant decrease of ATPase activity of myosin purified from DCM-affected heart in comparison with normal and ischemic myocardia as well. The data obtained allow us to propose a working hypothesis of functional link between antistress (HSPs) and antioxidative (cytochromes) systems at DCM progression.

AIM: To try polyenzyme drug vobenzim in chronic glomerulonephritis (CGN). MATERIALS AND METHODS: 174 CGN patients were randomized into 2 groups. The study group of 39 patients received conventional pathogenetic treatment plus vobenzim (initial dose 18-24, maintenance 12-15 dragees a day). Control group of 135 patients were given only pathogenetic therapy. 67 patients appeared ineligible. RESULTS: 3-6 months after the treatment the response was seen in 61.5% patients of the study and 17.8% patients of the control group. Side effects of vobenzim in CGN treatment were absent. CONCLUSION: Enzyme therapy with vobenzim in CGN is safe and effective.

RESUMEN

Correcting action of vitamin E and it's short chain derivative on the activity of some mitochondria electron transport chain enzymes were investigated on models of acute and chronic toxic hepatitis. Inhibition of NADH- and succinate-cytochrome c oxidoreductase complexes activity was established in short term action of xenobiotics. Treatment of rats with CCl4 during 60 days lowered activity of NADH-cytochrome c oxidoreductase complex and significantly increased activity of succinate-cytochrome c oxidoreductase complex and succinate dehydrogenase. Obviously, as a result of long term influence of hepatotoxic agents switching over in rat mitochondria electron transport from NAD-dependent way of substrate oxidation to succinate-dependent way took place. This event could be a part of the body adaptation mechanisms. Vitamin E and its short chain analogue corrected activities of investigated enzymes of mitochondria liver in the animals with acute and chronic hepatitis.

RESUMEN

As indicated by EEG and EMG data, adjuvant microwave resonance therapy included in the complex of routine aftertreatment of 24 children with spastic diplegia improved therapeutic efficacy and promoted positive trends in the patient's condition of 63% of those treated. The changes in ENMG parameters induced by microwave resonance therapy reflect shifts in the function of peripheral neuromotor system via optimisation of suprasegmental effects.

RESUMEN

A study of 62 workers with arterial hypertension of the ore mining industry revealed significant and various changes of the thrombocytic and coagulation hemostasis in patients. Hyperfibrinemia, increase of adhesive-aggregation properties of cellular elements, depression of fibrinolysis creates a thrombogenic situation. For choice of treatment tactics in clinical conditions it is recommended to evaluate the thrombocytic and procoagulant links of hemostasis. Disorders in the system of primary hemostasis (increase of adhesive-aggregational properties of cellular elements) are an indication to the use of antiaggregants (tyclid, tyclopidin). In case of fibrinolysis depression, anticoagulants should be used in the therapy complex.

RESUMEN

Lincomycin was found to inhibit tick-borne encephalitis (TBE) virus. To test antiviral potential of this drug, a clinical trial was initiated entering TBE patients from known focuses of the disease (Novosibirsk, Kemerovo, Perm and Irkutsk Provinces). The drug was given to 23 patients with meningeal and meningoencephalitic TBE. A control group of 22 matched subjects received specific immunoglobulin. Resultant efficacy of lincomycin appeared not inferior to that of anti-TBE immunoglobulin. Lincomycin can be successfully introduced in the treatment of meningeal and meningoencephalitic TBE.

It is shown that a day after introduction of adrenaline which evokes experimental focal myocarditis the level of ubiquinone and vitamin E content in the myocardial mitochondria increases by 56.8 and 122%, respectively. Succinate-ubiquinone-reductase activity in mitochondria remains practically unchanged, while NADH-ubiquinone-reductase activity considerably falls. 5 days after the focal myocarditis reproduction the content of ubiquinone and NADH-ubiquinone-reductase activity of mitochondria return to the norm, while the vitamin E amount remains higher than in intact animals. 24h after adrenaline introduction the level of succinate-ubiquinone-reductase activity of blood leucocytes considerably grows. It is not normalized even on the 5th day after adrenaline administration. It is supposed that the level of this activity may be an index showing development of the focal myocarditis.

A study is presented of 38 patients with severe iron-deficiency anemia and 70 patients with non-severe iron-deficiency anemia. All patients showed hypotension with a mean arterial pressure from 70.0 to 76.6 mm Hg. With progression of the severity of iron- deficiency anemia the patients exhibited a reduction of the contractile function of the left ventricular myocardium accompanied by a simultaneous dilatation of its cavity. The hyperkinetic type of hemodynamics is considered as a compensatory reaction of the circulation in response to hypoxia while the state of peripheral hemodynamics evidences venous dilatation furthering high minute blood volume utilization of oxygen at the level of hemotissue barrier is disturbed. Correction of the cardiohemodynamic disorders in combination with ferrotherapy improved the contractile function of the myocardium.

An in-depth study of the prevalence of hypertonic disease among 3075 underground miners of Krivoi Rog iron-ore basin was carried out. The incidence of the disease was 11.3 +/- 0.6 percent. A correlative dependence between incidence of HD, age and length of work underground was found; there were also found differences in the incidence of HD among different occupational groups and occupations. A single-factor disperse analysis indicated that the proportion of seven identified risk factors for HD accounted for 22.0 percent. Under favourable and unfavourable combination of factors the risk of HD, as demonstrated by estimates made with the help of Bayes method, varied from 6.5 to 95.1 percent. Based on the calculated standard intensive indicators the groups of prognosis for HD were determined. Curative and health-promoting activities were conducted in sanatoria facilities among groups of special attention and unfavourable prognosis. The experience was summarized in methodical recommendations.

RESUMEN

Results are reported of a clinico-epidemiological study of 9000 workers with the purpose of evaluation of the incidence of arterial hypertension (AII) and ischemic heart disease (IHD). The incidence of AH was 29% and IHD was 6.5%. This high incidence of these conditions was low according to findings of dispensarization examination of professional collectives, namely, 1-1.5% for AH and 0.8-1% for IHD.
