RESUMEN

AIM: To study effect of epicardial adiposity on risk of development and severity of coronary atherosclerosis. MATERIAL AND METHODS: We registered classical metabolic risk factors (RF) and additional factors of cardiovascular risk in 138 men aged 55.47+/-9.07 years with ischemic heart disease (IHD), functional class (FC) II-III angina, and I-III degree obesity. Diagnostic coronary angiography (CAG) was performed in all patients. Thickness of epicardial fat (tEF) in millimeters was measured at transthoracic echocardiography (ECG). RESULTS: Average tEF indexes were highest (10 [8; 10] mm) in the group of patients with multiple stenoses in coronary arteries (CA). According to ROC-analysis tEF appeared to be a predictor of significant coronary atherosclerosis. Its sensitivity was 80.4%, specificity - 67.6 % (cut-off value 6 mm). Epicardial adiposity was among factors associated with presence of coronary atherosclerosis (odds ratio [OR] 4.44, 95% confidence interval [CI] 2,06 to 9.59; p<0,001) along with age (OR 6.56; 95% CI 2.59 to 16.60; p=0,001), leptin (OR 3.50; 95% CI 1.46 to 8.37; p <0,001), resistin (OR 3.13; 95% CI 1.32 to 7.42; p <0,001) and waist circumference (OR 1.65; 95% CI 0.72 to 3.80; p=0.018).

RESUMEN

The clinical signifcance of serodiagnostic assay of rheumatoid arthritis increased nowadays. This is a reason to include the citrullinated antigens' antibodies into the new criteria of diagnostics ACR/EULAR 2010. The approbation of national test system detecting the cyclic citrullinated peptide antibodies was implemented. The analysis was applied to 211 blood serum samples taken of 50 blood donors, 60 patients with rheumatoid arthritis, 66 patients with other rheumatoid diseases. In addition, 35 samples were concurrently analyzed with the comparative test system (CCP2 Euroimmun, Germany). The referential meanings of standard limits were established on the basis of results of study of samples taken from healthy blood donors. When the standard limit was less than 10 arbitrary units the sensitivity made up 75% and the specificity--87.9%. In the case of higher values of citrullinated antigens' antibodies which are more than 15 arbitrary units, the sensitivity made up 68% and the specificity--93.1%. The results of comparing with the comparative test system characterized by high convergence made up 94% (33 out of 35), but the comparative test system detected citrullinated antigens' antibodies in 2 samples. The positive qualitative results of both methods analysis of autoantibodies weakly correlated with one another (r = 0.14). The results testify that the parameters of national test system correspond to the publication data concerning the second generation methods of cyclic citrullinated peptide antibodies detection though yield to the best foreign analogues.

RESUMEN

One foreign and two Russian recombinant enzyme immunoassay test-systems were comparatively investigated under conditions of an encoded experiment. Sensitivity in the experiment with the Russian test-systems Borreliosis-ELISA-IgG and Lyme Best was 63.8 and 68.8% respectively. As for the test-system Borrelia IgG Recombinant, it was 47.5%. All the test-systems were highly specific (94.4 to 99.5%). The test-systems Lyme Best and Borrelia IgG Recombinant revealed partial cross reactions with sera from patients with systemic lupus erythematosus and leptospirosis.

RESUMEN

Three foreign and one Russian ELISA test-systems for detection of IgM to Borrelia burgdorferi sensu lato were comparatively studied with the use of the clinical material in an encoded experiment. In the foreign ELISA test-systems based on the use of native antigens of borrelia, cross reactions with sera from patients with syphilis, Epstein-Barr infection, cytomegalovirus infection and systemic lupus erythematosus were detected. The Russian recombinant ELISA test-system Borreliosis-ELISA-IgM showed high sensitivity and specificity. The simultaneous use of the test-systems for detecting IgG and IgM significantly increased the efficacy of diagnosis of early borreliosis.
