RESUMEN
THE AIM OF THE STUDY: To develop and implement a comprehensive algorithm for the rehabilitation of patients after partial resection of the mandible using a titanium «growing¼ endoprosthesis. MATERIAL AND METHODS: The study included 16 patients aged 2 to 7 years, with benign (6 cases) and malignant (10 cases) tumors of the mandible. The patients were divided into 2 groups depending on the time of fixation of the endoprosthesis. Group 1 included patients with simultaneous installation of a prosthesis (7 people). Group 2 included patients with delayed installation of an endoprosthesis (9 people). For the reconstruction of the mandible, «growing¼ titanium endoprostheses made of Ti6Al4V alloy of various designs were used. Removable orthodontic devices of mechanical and functional type of action, standard elastic mouthguards were used in the process of dental treatment. RESULTS: A comprehensive algorithm has been developed for the rehabilitation of children after partial resection of the mandible, depending on the time of fixation of the prosthesis and the volume of surgical intervention. CONCLUSION: The developed algorithm of complex rehabilitation using a «growing¼ endoprosthesis and dental support at the pre and postoperative stages allows to reduce the volume of secondary deformation of facial structures and dentition.

Comparison of methods comprehension was done with intraoral roentgenography, orthopantomography (OPG), cone-beam computer tomography (CBCT) on the devices with optical amplifier of roentgenologic image (OARI) and sensor resolution (SR) 2 pair of lines per mm and without OARI with SR 2.5 pair of lines per mm. There were used extracted earlier teeth with adequate root canal obturation. According to the results of the study the image received with the help of CBCT with high SR let have the biggest information volume about filling quality of root canal and root canal structure peculiarities that testified to pro of the method over CBCT with OARI. The most widespread in the practice methods of intraoral roentgenography and orthopantomography not in all cases reflected reliably the root canal obturation degree.

In 6 patients with sarcomas of the bones of the crus at different periods following radiotherapy the authors have revealed pathologic reconstruction of the structure of the tibia. A roentgenologic picture of the changes in the course of the dynamic observation of the patients and their treatment is presented. It has been demonstrated that the conservative treatment effective.

The results of examination of 12 patients with vertebral radiation injuries were analyzed. The authors considered clinical and x-ray signs of radiation osteoporosis (5), radiation osteonecrosis (7 patients), and complications in the form of pathological fractures of vertebral bodies. Vertebral radiation injuries were most frequently detected 6-12 yrs. after the discontinuation of radiotherapy.

The data on 53 cases of bone injuries caused by radiation treatment for cancer of the uterus were analysed. The clinico-roentgenological signs of osteoradionecrosis (33), pathological fracture and dislocation (13), osteomyelitis (5) and radiation-induced sarcoma of bone (2) were considered. Lesions were found to occur most frequently in the upper flaring portion of the ilium and bones of the hip joints. Regular X-ray examinations of the pelvic bones are necessary for early detection of radiation injuries as well as conservative treatment to prevent such grave complications as pathologic fracture and osteomyelitis.

Secondary tumors developed in the zone of exposure fields in 6 patients treated with radiation for different malignancies. Tumors appeared, on the average, 12 years after radiotherapy.

The authors have summed up their experience gained in the diagnosis and treatment of radiation injuries of the head and maxillofacial region developing after radiotherapy of malignant tumors (105 patients) and non-tumorous diseases (47 patients).
