OBJECTIVE: Aim: To perform a morphometric analysis of the features of vascular remodeling of the arterial bed of the testicles in post-resection portal and pulmonary hypertension. PATIENTS AND METHODS: Materials and Methods: The testes of 54 white rats were studied, which were divided into groups: 1st included 16 intact animals, 2nd - 20 rats with pulmonary hypertension, 3rd - 18 individuals with post-resection portal hypertension. Postresection pulmonary hypertension was modeled by right-sided pulmonectomy. Postresection portal hypertension was simulated by removing 58.1 % of the liver parenchyma. RESULTS: Results: The outer diameter of the small-caliber arteries of the left testicle increased by 3.4% (p<0.05) in post-resection arterial pulmonary hypertension, and by 2.9% in post-resection portal hypertension. The inner diameter of the small-caliber arteries of the left testicle decreased by 7.7% (p<0.001) in pulmonary heart disease, and by 6.5% (p<0.01) in post-resection portal hypertension. The Kernogan index decreased by 23.0% (p<0.001), the Vogenvoort index increased by 1.26 times. In case of post-resection portal hypertension, the Kernogan index decreased by 19.0% (p<0.001) and the Wogenvoort's index increased by 1.19 times. The relative volume of damaged endotheliocytes in the small-caliber arteries of the left testis increased 20.6 times (p<0.001) in pulmonary heart disease, and increased 16.3 times (p<0.001) in post-resection portal hypertension. CONCLUSION: Conclusions: Portal and pulmonary hypertension lead to pronounced remodeling of the arterial bed of the testicles, which is characterized by thickening of the arterial wall, narrowing of their lumen, significant changes in Wogenvoort and Kernogan indexes, atrophy, dystrophy, and necrobiosis of endotheliocytes.

OBJECTIVE: The aim: To conduct a morphometric analysis of the features of the remodeling of vessels of the hemomicrocirculatory channel of the testicles under the action of aluminum chloride on the body. PATIENTS AND METHODS: Materials and methods: The testicles of 52 white male Wistar rats were morphologically studied, which were divided into two groups: intact and animals injected with aluminum chloride at a dose of 100 mg/kg. RESULTS: Results: The long-term effect of aluminum chloride on the body led to pronounced changes in the quantitative morphological indicators of the vessels of the hemomicrocirculatory channel of the testes compared to the control ones. The diameter of arterioles of the left testicle decreased by 18.4%, precapillary arterioles by 19.1%, hemocapillaries by 10.1%. The diameter of the postcapillary venules of the left testicle increased by 26.3%, the venules by 26.4%, and the density of microvessels decreased by 30.0%, which indicated the deterioration of its blood supply. The degree of remodeling of microvessels in the right testis was less pronounced. The diameter of arterioles of the right testicle decreased by 17.1%, precapillary arterioles by 18.4 %, and hemocapillaris by 9.1%. The diameter of the postcapillary venules of the right testicle increased by 25.5%, the venules by 27.0%, and the density of microvessels decreased by 25.8%. CONCLUSION: Conclusions: Long-term action (during the month) of aluminum chloride on the body leads to pronounced remodeling of the vessels of the hemomicrocirculatory channel of the testicles, which is characterized by a pronounced narrowing of its supply (arterioles, precapillary arterioles), exchange (hemocapillaries) links.

Currently, there is no reliable and ideal criterion for assessing the level of tissue viability in critical limb ischemia in patients with diabetic foot syndrome, which does not allow the selection of patients for revascularization. AIM: The aim is to investigate the microcirculation of the lower extremities before and after balloon angioplasty and to develop an algorithm for its implementation depending on the characteristics of purulent-necrotic complications. MATERIALS AND METHODS: The study was performed in 67 patients with neuroischemic form of diabetic foot syndrome. 3 groups of research were created: the first group included 18 patients with dry gangrene of one toe, several toes or distal foot; in group II - 35 patients with wet gangrene with signs of purulent arthropathy of the toes, phlegmon of the foot and with chronic wounds on the feet and legs; Group 3 - 14 patients diagnosed with purulent-necrotic complications, which were observed in both the 1st and 2nd groups, where patients refused to perform balloon angioplasty. Such patients underwent conservative treatment of lower extremity ischemia. RESULTS: It was shown that all patients have low levels of TcpO2- less than 30 mm Hg. The change in the value of TcpO2 during treatment turned out to be interesting: Group I indicator before revascularization was - 15.0±1.31 mm Hg, after the restoration of blood flow for 5- 7 days - 35.53±2.92 mm Hg. after 6 months - 36.67±2.35 mm Hg; Group II before revascularization - 10.35±0.74 mm Hg, for 5-7 days - 25.06±1.13 mm Hg, after 6 months - 34.43±1.97 III group at admission to the hospital - 12.14±0.86 mm Hg, for 5-7 days - 17.14±0.9 mm Hg, after 6 months - 13.71±2.2 mm Hg. CONCLUSIONS: After revascularization, there is a reperfusion syndrome, the severity of which depends on the number of revascularized vessels of the lower extremity. Balloon angioplasty is one of the priority methods of surgical treatment of limb ischemia with lesions of the shin-foot segment.

The results of treatment of patients with diabetic foot syndrome (DFS) remain unsatisfactory, 34% of patients have another wound defect in the foot within 1 year, and the recurrence rate within 5 years is 70%. AIM: The aim of the study was to determine the indicators of physical capabilities and self-care of patients after organ-sparing surgery due to DFS. MATERIALS AND METHODS: 6-12 months after discharge from the surgical hospital, all patients were sent questionnaires of the functional scale of the lower extremities. The questionnaire consists of 20 questions related to physical difficulties associated with the function of the lower extremities. The answers were evaluated on a scale: inability to perform actions - 0 points, in the absence of difficulties, the score was 4 points. RESULTS: 87.4% of responses to 176 letters were received, of which 84.6% contained answers to questionnaires, and 2.8% reported relatives' deaths. Despite self-monitoring of glycemia, 7 out of 10 patients did not reach its target level. When assessing physical activity in everyday life associated with the condition of the lower extremities after surgery, the indicator in the study group was 32.17 ± 17.88 points, and in the control group - 34.71 ± 16.13 points. The lowest mean score, 20.45 ± 11.53 was found in the group of patients after "high" amputations. The estimated score of self-care within the apartment was below 12 in 3 (27.2%) patients from the group after "high" amputations and was 13.27 ± 4.53, and in patients after operations within the foot - 15.71 ± 5 , 51 points. With limited physical activity, patients adjust satisfactorily to simple home activity within the home. CONCLUSIONS: Indicators of physical capabilities and self-care of patients after organ-sparing operations for DFS do not differ significantly from those in patients without this syndrome. The ability to perform physical activities within the home is influenced by the duration of diabetes, the presence of circulatory failure, decreased vision, as well as overweight patients.

As is known, phytochemical substances can play an important role in maintaining homeostasis and the prevention of chronic diseases, including diseases of the mouth. The most popular are polyphenols, which are very common in fruits, vegetables, beverages, cereals. There are data on their polyvalent pharmacological activity: antioxidant, antiinflammatory, detoxifying properties, antihypoxic, immunocorrective action, the ability to improve the endothelial function, etc. Quercetin (3,3', 4', 5,7-pentahydroxyflavone) is the main flavonoid in our daily diet. Of particular interest in the therapeutic activity of quercetin is its use in dental practice, in particular in the treatment of one of the leading problems - periodontal disease. Moreover, in modern pharmacotherapy, the strategic goals define the use of combination drugs, which is explained by the limited effectiveness of classical (traditional) monotherapy. Despite the good characteristics of quercetin, like many flavonoids, it is characterized by reduced bioavailability due to limited absorption. Therefore, increasing the bioavailability of quercetin is one of the most important goals of modern research. A modern pharmacy has proposed a number of dosage forms that improve bioavailability, including dental films or chips, which are intended for topical use and belong to the transdermal therapeutic systems and are applied by application to the mucous membrane of the mouth and periodontium. Based on the literature, we can predict that quercetin preparations water-soluble form and liposomal form, will be effective in dental practice, in particular in the treatment of periodontitis, affecting the main pathogenetic links. It has been proven that quercetin has an organ protective effect, increases the effectiveness of basic therapy, promotes a positive clinical effect, improves the course of the disease regardless of the severity, reduces the risk of complications. Moreover, quercetin has no toxicity and cumulative effect stimulates the activity of the body's enzymatic systems.

Obesity is one of the most serious and common risk factors for cardiovascular disease that increases the risk of type 2 diabetes. AIM: The aim of the study was to analyze the features of disorders of diastolic function of the left ventricle and the oxygenation of arterial and venous blood in patients with diabetic foot syndrome and metabolic syndrome with diastolic dysfunction of the left ventricle in its complex treatment with the using a course of hyperbaric oxygen therapy. MATERIALS AND METHODS: An examination of 32 patients with diabetic foot syndrome, neuro-ischemic form (I stage - 8 patients, II stage - 24 patients) and metabolic syndrome, to determine an assessment was made of diastolic function of the left ventricle, determination of saturation of arterial and venous blood, oxygen content in arterial and venous blood, arterio-venous difference in oxygen. The control group consisted of 20 healthy individuals. These indicators were determined before starting the course of hyperbaric oxygen therapy and after completing the full course used 100 % oxygen, with the following parameters: working pressure of 1.7-1.8 at, with periods of compression and decompression of 10 minutes, isopression - 40-60 minutes. The total number of sessions was 8-10. RESULTS: The ratio peak rates of early- and late diastolic filling of the patients in the study group was significantly lower by 24.48 % (p <0.05) compared to the control group. Iso-volumetric relaxation and slowing of early diastolic filling also increased: by 21.02 % and by 7.26 % (p <0.05), respectively. Saturation of arterial blood after hyperbaric oxygenation sessions was 4.1% higher than the initial data. Saturation of venous blood before and after oxygen therapy was 52.64 ± 0.08 % and 58.25 ± 0.02 %, respectively. Oxygen content in arterial and venous blood before the start of the course was 121.57 ± 0.85 ml· L-1 and 127.20 ± 3.5 ml·L-1, respectively, and after treatment respectively 135.61 ± 0.06 ml·L-1 and 82.13 ± 0.01 ml·L-1. In addition, the volume of oxygen consumed by body tissues increased from 46.85 ± 0.84 ml·L-1 to 57.83 ± 4.3 ml·L-1. CONCLUSIONS: The use of the course of the hyperbaric oxygen therapy contributed to the reduction of signs of diastolic heart failure and increased oxygenation rates of arterial and venous blood, the alignment of oxygen metabolism, contributed to the elimination of both systemic oxygen deficiency and local tissue hypoxia, due to which a complex therapeutic effect revealed.
