RESUMEN

AIM: To determine clinical efficiency and tolerance of paxil (paroxetin) in the treatment of panic disorders. MATERIAL AND METHODS: An open, non-comparative 7 week study entered 60 patients (mean age 36.6 +/- 3.22 years) with panic disorders and agoraphobias with panic disorders. Paroxetin was given in a single daily dose 20-60 mg/day in the morning. The results were assessed by Hamilton and Sihan scales, scale of clinical impression, by reduction of panic attacks. RESULTS: In 72.2% of patients panic attacks ceased or occurred less frequently (by 50%). Anxiety relieved by 67 +/- 4.6%. The number of the responders was greater while the effect occurred faster in patients with panic disorders. The drug was well tolerated, side effects occurred in 43.3% of cases. They were mild or moderate. CONCLUSION: Paroxetin has a prominent anxiolytic and antiphobic action, is well tolerated and is effective in panic disorders and agoraphobia with panic disorders.

Two groups of patients (30 patients in each group) were treated by lithium carbonate and contemnol. High preventive efficiency of both drugs was found: average annual duration of manic and depressive exacerbation as well as their frequency decreased practically by equal value--40-50%. The efficiency was somewhat higher in the group of patients treated by contemnol (80%) as compared with the group on lithium carbonate (70%). Total number of side-effects and spontaneous complaints was twice as much in prophylaxis by lithium carbonate. Wide application of contemnol in psychiatric practice for prophylaxis of aggravations (fits) of affective and schizoaffective psychoses is recommended.

Comparative study of the influence of equivalent doses of lithium carbonate and contemnol was studied in patients with phasic development of endogenous psychoses. 33 patients (17 men, 16 women) of 20-58 years or age were treated. According to IDC-10 there were observed schizoaffective psychoses in 22 patients (F25), bipolar affective disorders in 5 patients (F31) and chronic mood disorders in 5 individuals (F34). 2-daily comparative pharmacokinetic profiles of lithium carbonate and contemnol were presented and on the base of standard statistic analysis were estimated: lithium presence in blood, amplitude of variations of lithium concentration, number of peak concentrations, areas under pharmacokinetic curves. The study showed that concentration of lithium in blood increased gradually and achieved the maximal level in 6-7 hours after contemnol administration and was not practically changed during following 24 hours as compared to lithium carbonate. The results of the study confirmed that contemnol occurred to be the prolong of lithium carbonate and hadn't to be used more frequently than once a day.
