RESUMEN

AIM: The objective of the present study was to identify the peculiar features and advantages of different methods for the mechanical impact on the thoracic tissues of the patients presenting with chronic obstructive pulmonary disease (COPD) and to develop specific indications for their clinical applications. MATERIAL AND METHODS: This randomized prospective comparative study included 137 patients with COPD. In accordance with the currently accepted classification (GOLD, 2013), all the patients had COPD of medium severity. The smoldering inflammatory process was diagnosed in 75 (54.7%) patients, grade I and II respiratory insufficiency in 80 (58.4%) and 57 (41.6%) patients, respectively. The external respiration function was evaluated by means of pneumotachometry techniques during the forced expiratory maneuver and by spirometry. The pulmonary hemodynamics and myocardial contractility of the right ventricle were studied with the use of rheopulmonography and central hemodynamics by tetrapolar thoracic rheography. The routine inflammatory and immune tests were employed. RESULTS: Investigations of the systemic circulation have demonstrated the prevalence of its hyperkinetic type (54,0%) over the hypokinetic and eukinetic ones (23,3% and 22,7% respectively). All the patients were divided into three group identical in terms of clinical and functional characteristics. The patients comprising group 1 (n=46) were prescribed the rehabilitative treatment in the form of classical chest massage, those of group 2 (n=47) were treated by means of intense massage of asymmetric chest zones, and the patients included in group 3 (n=44) underwent manual therapy. It was shown that intense massage produced the most pronounced beneficial effect. Classical massage also resulted in the reduction of the inflammatory manifestations but its effectiveness was significantly lower than that of the intense treatment (р<0,05-0,02). Manual therapy failed to cause any appreciable changes in the character and severity of the inflammatory process (р>0,5). CONCLUSION: The intense massage of asymmetric chest zones proved to provide the most efficient tool for the anti-inflammatory treatment of the patients presenting with chronic obstructive pulmonary disease in comparison with other known methods for the mechanical impact on the thoracic tissues. However, the application of this technique is limited in the patients with the hypokinetic type of systemic circulation and pulmonary hypertension. The most important advantage of chest massage by the conventional method is the possibility of its application for the treatment of the patients suffering from severe forms of COPD associated with pulmonary and systemic cardiohemodynamic disturbances with alveolar hypoxia. Manual therapy can be recommended in the first place to the patients with COPD in remission and the accompanying functional blockade of the vertebral segments.

RESUMEN

The proposed technique of cryomassage in outpatient setting promotes a complete healing of duodenal ulcer (DU) in patients with DU exacerbation. Healing of the ulcer is accompanied by improvement of blood flow in the zones of segmental innervation of the gastroduodenal system and by recovery of impaired cellular and humoral immunity in most of the patients.

RESUMEN

97 females at reproductive age with chronic nonspecific salpingo-oophoritis (CNSO) were examined and treated. The results of the treatment (vaginal and external impact) demonstrate positive effects of various cryotherapeutic techniques on CNSO clinical course, on hormonal and immune unbalance, functional activity of the uterine tubes, regional hemodynamics, psychoemotional status. Thus, cryotherapy is an effective adjuvant in combined therapy of CNSO.

RESUMEN

Massotherapy in different regimens has been used in 30 patients in remission of chronic salpingo-oophoritis (CSO). The massage produced positive changes in blood coagulation, immune status, regional hemodynamics of the small pelvis, bioelectric activity of the muscles of the anterior abdominal wall and lumbosacral region. A strong anesthetic and antiinflammatory effect of intensive massage in 78%, recovered reproductive function in 33% of the patients allow to recommend intensive massage as possible monotherapy of patients in remission of CSO.

RESUMEN

74 patients with chronic salpingo-oophoritis (CS) in remission were assessed as to effects of classic massage of the uterine and epoöphoronic reflexogenic zones on clinical pattern of the disease, bioelectric activity of the abdominal muscles, spine and pelvis, blood coagulation. The combined massage produced a more potent effect on electric activity of the above muscles and blood coagulation than massage on a single area. It is better to start massage from the abdomen and continue in the lumbosacral area.

RESUMEN

Clinical and instrumental studies have revealed differences in effects of nerve ending and classic massage on hemodynamics in 41 patients early after transitory ischemic attacks in the vertebrobasilar area. Point massage produced more potent vasotropic effect, contraindications to it are minimal. It can be considered as a pathogenetic therapy aimed at correction of cerebral circulation in patients with vertebrobasilar area applicable early after acute cerebrovascular episodes.

RESUMEN

The comprehensive studies involving rheoencephalography and tetrapolar thoracic rheography made in 120 patients with dyscirculatory encephalopathy (DE) indicated that pointed massage caused a noticeable rearrangement of hemodynamic processes, which depended on the clinical cause of the disease and the baseline status of the circulatory system. This makes it possible to consider pointed massage as a therapeutical factor aimed at correcting cerebral circulation, which is indicated particularly in DE patients having a history of transitory ischemic attacks in the vertebrobasilar bed.

RESUMEN

Therapeutic response to treatment (vaginal and external cryotherapy, drugs) in 97 patients with chronic salpingo-oophoritis at reproductive age provides the conclusion on effectiveness of various cryotherapeutic regimens in this disease. Cryotherapy improves regional blood circulation, restores contractility of the uterine tubes.

RESUMEN

The authors have developed a technique of abdominal cryomassage and tried it in patients with coronary heart disease (CHD). The massage improved the disease course, diastolic function, being most effective in hypertrophic left ventricular myocardial affection; produced lipotropic action important for early secondary prevention. Thus, abdominal cryomassage as a nonpharmaceutical method can serve an efficient adjuvant modality in combined treatment and rehabilitation of CHD patients.
