BACKGROUND: The objectives of the researchers are as follows: First, to investigate whether intraoperative or postoperative administration of Intravenous (IV) iron supplements in patients undergoing primary total knee arthroplasty (TKA) can contribute to the hemoglobin recovery during the postoperative period (between 4 and 8 weeks after surgery). Second, to examine whether the administration of IV iron supplements during or immediately after TKA in patients undergoing primary TKA can reduce the need for allogenic blood transfusion during hospitalization. METHODS: Articles published between January 1, 1990, and June 30, 2023 were searched in PubMed, Cochrane, and Embase. The population, intervention, comparison, and outcome of this study are as follows; Population: Patients undergoing primary total knee arthroplasty; Intervention: Administration of IV iron supplements during or immediately after surgery; Comparison: Non-administration of IV iron supplements; Outcome: Degree of hemoglobin recovery (between 4 and 8 weeks after surgery) and the need for blood transfusion during hospitalization. RESULTS: There was a statistically significant difference in the amount of change in hemoglobin between iron supplementation group and non-iron supplementation group. The effect size were -0.44 (95% confidence interval: -0.69 to -0.19, P value < .001) in all patients. This means that the amount of change in hemoglobin were significantly reduced in the iron supplementation group than in the non-iron supplementation group. There was a statistically significant difference for post-operative transfusion rate between 2 groups. The effect size were 0.28 (95% confidence interval: 0.10-0.81, P value = .02) in all patients. This means that the post-operative transfusion rate was significantly less in the iron supplementation group than in the non-iron supplementation group. CONCLUSION: The administration of IV iron supplements during or after TKA surgery increases hemoglobin recovery between 4 and 8 weeks after surgery and reduces the need for allogeneic blood transfusion during hospitalization.

We retrospectively investigated the natural course of hemoglobin levels after allogenic blood transfusion in total knee arthroplasty. All patients were treated according to the same clinical pathway, and blood tests were performed on the same day. All blood tests were done on pre-op, immediate post-op, midnight of op day, 1st, 2nd, 3rd, 5th, 7th, and 11th day after surgery. Of the total 593 cases, a total of 197 cases (33.2%) were performed within 3 days of surgery. Hemoglobin level was significantly lowest on the 3rd day after surgery and tended to increase afterwards in the non-transfusion group. In the case of blood transfusion on the day of surgery, the hemoglobin level showed an increase on the next day and then showed a minimum value on the fifth day of surgery and then increased. The same pattern was identified when blood transfusion was done on the 1st and 2nd day of surgery. However, when blood transfusion was done on the 3rd day, the hemoglobin level showed a steady increase afterwards. The hemoglobin level of total knee arthroplasty patients with no blood transfusion was the lowest on the 3rd day after surgery and increased afterwards. If blood transfusion was done within 2 days after surgery, the hemoglobin level was the lowest on the 5th day after surgery and increased afterwards. If blood transfusion was done on the 3rd day after surgery, the hemoglobin level increased afterwards.

BACKGROUND: We aimed to find out whether the combined treatment of acupuncture and oral medication is more effective than sole oral medication in reducing pain and improving knee function at the end of treatment and after short-term period (4-6 weeks after treatment). Second, if it is effective, we investigated whether the effect surpasses the minimal clinically important difference. METHODS: Articles published between January 1, 1992, and August 31, 2022, were searched in PubMed, Cochrane, and Embase. The PICO (population, intervention, comparison, and outcome) of this study are as follows: Population: knee osteoarthritis patients; Intervention: acupuncture (non-sham acupuncture) + oral medication (analgesic or non-steroidal anti-inflammatory drugs); Comparison: oral medication (analgesic or non-steroidal anti-inflammatory drugs); Outcome: visual analog scale (VAS) or Western Ontario and McMaster University (WOMAC) osteoarthritis index. RESULTS: The combined treatment of oral medication and adjuvant acupuncture showed statistically significant improvement in VAS and WOMAC scores at the end of acupuncture treatment and short-term follow-up time (between 4 and 6 weeks after acupuncture). In addition, the degree of improvement of VAS and WOMAC index showed effects beyond minimal clinically important differences compared to pretreatment at both the end of acupuncture treatment and the short-term follow-up of acupuncture treatment. CONCLUSION: The existing evidence suggests that adjuvant acupuncture may play a role in the treatment of knee osteoarthritis. However, physicians should be aware of adverse effects such as hematoma in adjuvant acupuncture treatment.

RATIONALE: Galeazzi fractures are uncommon, and are less common in children than in adults. Galeazzi-equivalent fractures, a variant of the classic Galeazzi fractures, mostly occur in children or adolescents. Galeazzi equivalent fractures may commonly accompany distal radial fractures or injury of the epiphyseal plate of the distal ulna. PATIENT CONCERNS: A 13-year-old man visited our emergency department after stumbling over a rock. Manual reduction and splinting was already done in a nearby medical center. Plain radiographs revealed fractures in the distal radius and fracture of the ulnar epiphyseal plate. DIAGNOSIS: Plain radiographs showed Galeazzi-equivalent fracture. The result of primary closed reduction was not enough and there was still displacement of fracture. INTERVENTIONS: Anatomical reduction of distal radius was fixed with a plate and screws, and K-wires were inserted percutaneously for reduction and fixation of ulnar fracture. OUTCOMES: Complete bone union was achieved and normal range of motion is shown 2 years postoperatively. The patient is able to perform daily activities and sport activities without any signs of ulnar growth arrest. LESSONS: Open reduction is required in patients with malalignment, failure to reduce the DRUJ or maintain its reduction, or older ages which are hard to expect sufficient bone remodeling.

AIMS: Because collagen is fundamental to wound healing and skin formation, collagen-containing dressing materials might be beneficial in treating diabetic foot ulcers (DFU), but supporting evidence is needed. Here, we examined the effectiveness and safety of collagen dressing material in DFU treatment. METHODS: This prospective, randomized, placebo-controlled, single-center study included patients with type 1 or 2 diabetes and palpable foot pulse who had Wagner grade 1 or 2 ulcers ≥1.0 cm2 with no signs of healing for ≥6 weeks. Patients were treated with foam dressing alone (control group) or with a porcine type I collagen dressing material (collagen group). Complete ulcer healing rate was the primary endpoint, and healing velocity and time to 50% size reduction were secondary endpoints. RESULTS: Thirty patients were included (collagen group: 17, control group: 13). There were no significant differences in demographic factors or baseline DFU characteristics. Compared to the control group, the collagen group presented a higher rate of complete healing [82.4% vs. 38.5%, P = .022], faster healing velocity (P < .05), and shorter median time to 50% size reduction (21 versus 42 days; hazard ratio = 1.94, P < .05). CONCLUSIONS: Wound management using collagen materials in DFUs showed faster and complete healing rate.

RATIONALE: Cases of pseudotumor complicated after THA with metal-on-metal articulation as a bearing surface have been reported. Most of the pseudotumors occurred near or inside the hip joint, and usually have similar features with the infectious condition. We herein report a case of pseudotumor with huge size in the pelvic cavity mimicking features of iliacus muscle abscess. PATIENTS CONCERNS: A 70-year-old female was referred to our emergency department due to huge mass in her right pelvic area on abdominal CT. She complained of mild febrile sensation, generalized weakness, and pain in her right leg for several months. The patient underwent metal-on-metal bearing THA 11 years ago for treatment of right-sided hip pain due to avascular necrosis. DIAGNOSIS: Percutaneous catheter drainage was performed for both therapeutic and diagnostic reasons. No microbes were cultured from the drained fluid, and signs of chronic inflammation were shown on pathology slide samples. The patient showed positive allergic reactions to cobalt and captan on allergic patch test, and serum cobalt levels was increased as 46.78 µg/L (normal value, 0.11-0.64 µg/L). Biopsy samples of the cyst were consistent with the histological findings of pseudotumor reported by Willert et al. INTERVENTION:: For the treatment of infection, primarily, the patient was surgically treated by irrigation, debridement, and bone bead insertion. Then, after 4 weeks of antibiotic treatment, revision surgery changing the metal-on-metal articulation into poly-on-metal articulation was performed. OUTCOMES: CT scan at 1 year after surgery showed no findings of cyst inside the pelvic cavity. Although the patient complained of right sided hip pain (VAS score 2), she was able to perform activities of daily living. LESSONS: In patients who underwent metal-on-metal THA, pseudotumor mimicked the feature of periprosthetic infection may occur not only inside the hip joint but also in the pelvic cavity.

PURPOSE: Aim is to investigate the changes of mediolateral soft tissue gaps in total knee arthroplasty (TKA) after suturing medial extensor. METHODS AND MATERIALS: We compared the differences of medial and lateral gap values that were shown by the computer navigation at 0°, 45°, 90°, and 120° knee flexion during patella in situ and during patella repaired by a towel clip on two constant sites. Fifty consecutive knees (43 patients) scheduled for TKA due to varus knee osteoarthritis, from February 2017 to May 2017, were enrolled in this prospective study. RESULTS: The medial gaps with patella repaired were significantly lower ( p < 0.05) than the medial gaps with patella in situ at 45°, 90°, and 120° knee flexion. Differences in the medial gap were largest at 90, with the difference of 0.87 mm. Twenty-four of 50 cases (48%) showed medial gap differences of 1 mm or over, and 13 of 50 cases (26%) showed medial gap differences of 2 mm or over. The variation in the medial gap at 90° following patellar repair showed significant association (correlation coefficient = 0.78, p = 0.001) with the difference between medial and lateral gaps (medial gap - lateral gap) at 90° of patella in situ. At 90° knee flexion, when the medial and lateral gap difference in patella in situ was 1 mm or less, 73.5% (25/34) of the cases showed variation in the medial gap of less than 1 mm after patellar repair. CONCLUSION: During TKA, while measuring the medial gap with patella in situ, overestimation might occur, especially in the position of knee flexion. Thus, reevaluation using towel clips should be considered when the medial and lateral gap difference is 1 mm or larger when patella in situ during evaluation of the medial and lateral gaps at 90° knee flexion.
