OBJECTIVE: To assess factors associated with emotional changes and Hyperactivity/Inattention (HI) motivated by COVID-19 quarantine in adolescents with immunocompromising diseases. METHODS: A cross-sectional study included 343 adolescents with immunocompromising diseases and 108 healthy adolescents. Online questionnaires were answered including socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and validated surveys: Strengths and Difficulties Questionnaire (SDQ), Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0). RESULTS: The frequencies of abnormal emotional SDQ scores from adolescents with chronic diseases were similar to those of healthy subjects (110/343 [32%] vs. 38/108 [35%], p = 0.548), as well as abnormal hyperactivity/inattention SDQ scores (79/343 [23%] vs. 29/108 [27%], p = 0.417). Logistic regression analysis of independent variables associated with abnormal emotional scores from adolescents with chronic diseases showed: female sex (Odds Ratio [OR = 3.76]; 95% Confidence Interval (95% CI) 2.00‒7.05; p < 0.001), poor sleep quality (OR = 2.05; 95% CI 1.08‒3.88; p = 0.028) and intrafamilial violence during pandemic (OR = 2.17; 95% CI 1.12‒4.19; p = 0.021) as independently associated with abnormal emotional scores, whereas total PedsQL score was inversely associated with abnormal emotional scores (OR = 0.95; 95% CI 0.93‒0.96; p < 0.0001). Logistic regression analysis associated with abnormal HI scores from patients evidenced that total PedsQL score (OR = 0.97; 95% CI 0.95‒0.99; p = 0.010], changes in medical appointments during the pandemic (OR = 0.39; 95% CI 0.19-0.79; p = 0.021), and reliable COVID-19 information (OR = 0.35; 95% CI 0.16‒0.77; p = 0.026) remained inversely associated with abnormal HI scores. CONCLUSION: The present study showed emotional and HI disturbances in adolescents with chronic immunosuppressive diseases during the COVID-19 pandemic. It reinforces the need to promptly implement a longitudinal program to protect the mental health of adolescents with and without chronic illnesses during future pandemics.

Conclusion: The present study showed emotional and HI disturbances in adolescents with chronic immunosuppressive diseases during the COVID-19 pandemic. It reinforces the need to promptly implement a longitudinal program to protect the mental health of adolescents with and without chronic illnesses during future pandemics.

Introduction: Bioimpedance spectroscopy (BIS) devices utilize biophysical modeling to generate body composition data. The addition of body mass index (BMI) to modified Xitron-Hanai-based mixture equations improved BIS estimates of intracellular water (ICW), particularly at the extremes of BMI. A 3-compartment model for distinguishing excess fluid (ExF) from normally hydrated lean (NHLT) and adipose tissue may further improve BIS estimates. Objective:We aimed to validate a BIS approach based on the Chamney model for determining fat mass (FM) in healthy individuals (NHANES) and for measuring FM changes in individuals undergoing massive weight loss. Methods: Using adult NHANES 1999-2004 (2821 female, 3063 male) and longitudinal pre-topost-RYGB (15F) data, we compared dual-energy-X-ray absorptiometry (DXA) and BIS for FM. We applied BIS adiposity-corrected values to Chamney equations for normally hydrated lean and adipose tissue (NHLT, NHAT) and FM. Method agreement was evaluated by correlations, paired t-tests, root mean square error (RMSE), BlandAltman (B-A) analysis, and concordance correlation coefficients (CCC). Results: Method agreement between BIS and DXAFM was good in healthy adults (r=0.96, CCC=0.93, p<.0001), and pre-to-post-RYGB (r=0.93-0.98, CCC=0.81-0.86, p<.001). Although cross-sectional FM measures differed, FM change measures post-RYGB did not (35.6±8.9 vs. 35.2±9.2 kg, BIS vs. DXA) and agreed well (r=0.84, p<.0001). The 15 subjects with follow-up measurements at 1 year lost 11.5±9.8 kg FFM by DXA, but only 1.3±2.5 kg of NHLT by BIS, suggesting that the FFM loss may have been mostly adipose tissue water. Conclusions: Incorporation of the Chamney model into BIS algorithms is a major conceptual advancement for assessing and monitoring body composition. Its ability to differentiate ICW and extracellular water (ECW) in NHLT and NHAT, as well as excess ECW is promising, and would facilitate lean tissue monitoring in obesity and acute/chronic disease.(AU)

A adição do índice de massa corporal (IMC) às equações de mistura modificadas com Xitron-Hanai modificadas melhorou as estimativas de DEB de água intracelular (AI), particularmente nos casos extremos do IMC. Um modelo de 3 compartimentos para distinguir o excesso de fluido (ExF) de magro normalmente hidratado (NHLT) e tecido adiposo pode ainda melhorar as estimativas do DEB. Objetivo: Pretendemos validar uma abordagem do DEB com base no modelo de Chamney para determinar a massa de gordura (MG) em indivíduos saudáveis (NHANES) e para medir mudanças de MG em indivíduos submetidos à perda de peso maciça. Método: Usando o NHANES adulto 1999-2004 (2821 mulheres, 3063 homens) e dados longitudinais pré-pós-RYGB (15 F), comparamos a absorção de raios-X de dupla energia (DXA) e DEB para MG. Resultados: O acordo de método entre DEB e DXA MG foi bom em adultos saudáveis (r=0,96, CCC=0,93, p<.0001) e pré-pós-RYGB (r=0,93-0,98, CCC=0,81-0,86, p<0,001). Embora as medidas de MG transversais diferissem, as medidas de mudança de MG pós-RYGB não (35,6±8,9 vs. 35,2±9,2 kg, DEBvs. DXA) e concordaram bem (r=0,84, p<.0001). Os 15 sujeitos com medidas de seguimento ao 1 ano perderam 11,5±9,8 kg FFM por DXA, mas apenas 1,3±2,5 kg de NHLT pelo DEB, sugerindo que a perda de FFM pode ter sido principalmente água do tecido adiposo. Conclusões: A incorporação do modelo de Chamney em algoritmos DEB é um grande avanço conceitual para avaliar e monitorar a composição corporal. A sua capacidade de diferenciar AI e água extracelular (AE) no NHLT e NHAT, bem como o excesso de AE é promissor e facilitará a monitorização do tecido magro na obesidade e doença aguda/crônica.(AU)

RESUMEN

In February 2013, the Organ Procurement and Transplantation Network mandated that transplant centers perform screening of living kidney donors prior to transplantation for Strongyloides, Trypanosoma cruzi and West Nile virus (WNV) infection if the donor is from an endemic area. However, specific guidelines for screening were not provided, such as the optimal testing modalities, timing of screening prior to donation and the appropriate selection of donors. In this regard, the American Society of Transplantation Infectious Diseases Community of Practice, together with disease-specific experts, has developed this viewpoint document to provide guidance for the testing of live donors for Strongyloides, T. cruzi and WNV infection, specifically identifying at-risk populations and testing algorithms, including advantages, limitations and interpretation of results.

Rotavirus nonstructural protein NSP4 has recently been suggested to function as a viral enterotoxin and play a role in the pathophysiological mechanism whereby rotaviruses induce diarrhea. The ability of rotavirus NSP4 to stimulate a humoral immune response was examined in naturally infected children and adults, rotavirus vaccinated children, as well as a cellular immune response in adults. In this study, 10 of 10 naturally infected children and 9 of 10 rotavirus-vaccinated children showed a weak humoral IgG immune response to recombinant NSP4 (rNSP4) and/or a synthetic peptide corresponding to residues 114-134 of NSP4. Modest serum IgG antibody responses were detected in 20 of 20 adults. A cellular immune response to rNSP4 and/or NSP4(114-134) were detected in 8 of 10 adults measured either as a T-cell proliferative response (7 of 10), an increased production of IL-2 (6 of 10), or increased production of interferon-gamma (8 of 10). These results indicate that NSP4 induces a humoral immune response in humans and show for the first time that NSP4 stimulates a cellular immune response, possibly including cytolytic T-cells.

Serologic assays could be useful for determining circulating subtypes in different geographic regions. A total of 175 serum samples from the same number of Argentinian HIV-infected patients from Buenos Aires and Rosario were tested against a panel of peptides representing V3 consensus subtypes A through H. A V3 peptide enzyme immunoassay was used for screening the sera. Most sera were reactive with peptides representing subtypes B (58.28%), F (13.14%), and A (8.57%). Cross-reactivity between the remainder of the peptides was observed. Genotypes of eight patients from Rosario were determined and compared with serotyping. Results showed that seven of eight genotyped patients reacted with their respective consensus B peptide and one reacted with consensus B and F. V3 peptide serology proved to be useful for determining HIV-1 clades circulating in Argentina.


PIP: 175 serum samples were collected from 175 HIV-infected Argentineans in Buenos Aires and Rosario during 1987-95, for testing against a panel of peptides representing V3 consensus HIV-1 subtypes A through H. A V3 peptide enzyme immunoassay was used to screen the sera. 58.28% of the sera were infected with HIV-1 subtype B, 13.14% with subtype F, 8.57% with subtype A, 4% with subtype H, 2.85% with subtype D, 2.28% with subtype G, and 1.71% with subtype C. Some cross-reactivity between peptides was observed. Peripheral blood mononuclear cells (PBMCs) were obtained from 8 HIV-infected subjects from Rosario for use in determining genotypes. 7 of the 8 genotyped patients reacted with their respective consensus B peptide and 1 reacted with consensus B and F. V3 peptide serology proved useful in determining which HIV-1 clades are circulating in Argentina.

RESUMEN

The authors present a case of tuberculosis of the breast as the AIDS--defining illness in a male adult patient, with CD4 counts still moderately low (475 cells/mm3) and without evidence of tuberculosis in any other organ. Literature review emphasizes the rarity of the isolated infection of the breast, especially in males, with no increase in the number of cases of this infection site even in patients with AIDS.

A highly conserved gp41 epitope (amino acid sequence ELDKWA) has been described to elicit antibodies neutralizing a broad variety of HIV-1 strains. We analyzed the antibody reactivity of HIV-1-infected individuals from Argentina and Sweden to overlapping synthetic peptides covering aa647-684 of HIV-1 MN gp41. An immunodominant antigenic determinant was discovered in the C-terminal region adjacent to the ELDKWA sequence. Most of the sera from both Argentina and Sweden reacted only with peptides representing this region. Two other patterns of reactivity were also observed: some sera reacted only with peptides spanning the central region of gp41, including the ELDKWA sequence; other sera reacted with both the central and C-terminal regions. Differences in reactivity were noted between Argentinian and Swedish sera in terms of peptides covering the central region. In addition, to determine the amino acids essential for antibody binding, seroreactivity against a set of substitution peptides covering the immunodominant epitope was studied. Results obtained indicated that carboxyl amino acids WNWFDI close to the ELDKWA sequence were the most important for antibody binding. Ability of sera, tested for peptide reactivity, to neutralize HIV-1 was also analyzed. High antibody reactivity to the central region was frequently found in sera with high neutralizing titers. This observation was not seen when seroreactivity to peptides spanning the C-terminal region was analyzed. These results suggest that the immunodominant epitope C terminal to the ELDKWA sequence is not involved in inducing neutralizing antibodies.

RESUMEN

Scores on the Buss-Durkee Hostility Inventory (BDHI) were analyzed in relation to violent versus nonviolent criminality and ethnic group membership within a sample of adult, male offenders. Although the white, Mexican-American, and black subjects differed on both psychometric and criminological variables, there was no statistically significant association between these predictors and criteria for either the total sample or the individual ethnic groups. The results were seen to contradict recent claims regarding the validity of the BDHI as a discriminator of violent behavior.
