RESUMEN

A 16-year-old male presented with a 3-month history of an asymptomatic, enlarging conjunctival lesion. An excisional biopsy was performed and histologic and immunohistochemical examination showed characteristic features of a peripheral primitive neuroectodermal tumour (PNET) adjacent to a benign compound naevus. FISH analysis, demonstrating a split-signal at 22q12, confirmed the diagnosis. Staging investigations were negative confirming the primary nature of the lesion. The patient was treated with local wide re-excision and chemotherapy. He remains alive and well 29 months after initial resection.

RESUMEN

The worldwide shortage of organs and tissue for transplant has led to many ethical discussions involving restrictions concerning organ and tissue donation, including living donations and payment for donation. Efforts are being made to increase the donor pool; however, it is timely to ask whether moral decisions such as the rejection of directed donations are defensible. In this Clinical Conundrum, six specialists delve into the ethical and practical issues surrounding directed donation of human organs and tissues with particular reference to its implications in the field of ophthalmology.

RESUMEN

If development is defined as a process of enhancing human capabilities, that is, to expand choices and opportunities so that each person can lead a life of respect and value, then poverty is the deprivation of these capabilities. Nobel Laureate for Economics, Amartya Sen, states: 'as people who live minus sign in a broad sense minus sign together, we cannot escape the thought that the terrible occurrences that we see around us are quintessentially our problems'. This year's Council Lecture examines issues of individual and institutional responsibility in a developing world. Aspects of development relevant to ophthalmology are discussed and a review of Australian efforts undertaken. With a view to encouraging Fellows to take a more active role in development, it is demonstrated that there are a range of contributions that can be made. Appropriate practice models are explored and a strategy for College involvement presented.
