Objetivo Describir, mediante pruebas funcionales y biomecánicas, cómo la intervención personalizada de fisioterapia acuática puede contribuir a mejorar la marcha y funcionalidad en un paciente con accidente cerebrovascular crónico. Descripción del caso Mujer de 63 años que experimentó un accidente cerebrovascular en 2006. Presentaba hemiplejía del miembro superior izquierdo y hemiparesia del miembro inferior izquierdo, que limitaban su marcha y funcionalidad. Se evaluaron la fuerza muscular isométrica, la percepción del esfuerzo para actividades de la vida diaria, la marcha mediante pruebas funcionales y el centro de presiones, utilizando una plataforma de fuerzas. Intervención Dos sesiones de terapia acuática a la semana, durante 8 semanas, basadas en la terapia acuática específica Halliwick ®, el método de los anillos de Bad Ragaz ® y en entrenamiento de marcha con peso y dobles tareas. Resultados La percepción del esfuerzo al caminar disminuyó 30mm, la prueba Timed Up and Go disminuyó 5,88 segundos, la fuerza muscular aumentó más de 4kg en todos los músculos, excepto flexores de cadera y extensores de rodilla, y el desplazamiento del centro de presiones disminuyó en todos los planos y tareas evaluadas. Conclusión Este estudio respalda la idea de que la terapia acuática puede contribuir a mejorar aspectos biomecánicos que ayudan a explicar las mejoras clínicas en pacientes con accidente cerebrovascular (AU)


Objective To describe, through functional and biomechanical tests, how a personalized aquatic physiotherapy intervention can contribute to improving gait and functionality in a patient with chronic stroke. Description of the case Woman with 63-year-old who suffered a stroke in 2006. She presents hemiplegia of the left upper limb and hemiparesis of the left lower limb, which limit her gait and functionality. Isometric muscle strength, perception of effort for activities of daily living, gait through functional tests, and center of pressure displacement were evaluated using a force platform. Intervention Two aquatic therapy sessions per week, for 8 weeks, based on Specific Aquatic Therapy-Halliwick ®, Bad Ragaz ® Ring Method and dual task weight gait training. Results Perception of effort when walking decreased 30mm, Timed-Up-and-Go decreased by 5.88seconds, muscle strength increased more than 4kg in all muscles, except hip flexors and knee extensors, and center of pressure displacement decreased in all the plans and tasks evaluated. Conclusion This study supports the idea that aquatic therapy can contribute to improving biomechanical aspects that can have an impact on clinical improvements in stroke patients (AU)

RESUMEN

Spinal automatisms and reflexes, peripheral neurogenic and myogenic reactions are common in patients with irreversible brain death. They are therefore compatible and are even understood by experienced investigators as confirmation of irreversible brain death. This article provides an overview of the phenomenology of irreversible brain death and discusses it from a neuropathological perspective. Furthermore, irreversible brain death is described in order to distinguish it from pathological movements and motor reactions in comatose patients or patients with disturbed consciousness due to severe brain disorders.

A significant change in the fourth update of the German guidelines on determining brain death is that it includes an explicit profile of requirements on physicians involved in ILBF diagnosis. These requisite qualification criteria have also been formulated due to the fact that, in many hospitals, ILBF diagnosis is only rarely carried out and, as a result, uncertainty frequently arises. Typical difficulties emerge at all stages of ILBF diagnosis, and numerous relevant pitfalls arise that need to be taken into consideration and which might also be relevant in the selection of the method(s) to detect irreversibility. The approaches presented here are suited to achieving a valid result in the evaluation of equivocal ILBF.

RESUMEN

A 58-year-old woman developed rapidly progressive neurological symptoms and finally loss of vigilance 5 weeks following primarily successful lung transplantation. A posterior reversible encephalopathy syndrome (PRES) under treatment with tacrolimus as well as hyperammonemia due to sepsis with Ureaplasma urealyticum could be identified as the causes. Infections with Ureaplasma, bacteria which produce ammonia as a product of metabolism, are increasingly being identified in immunocompromised people by specific PCR (polymerase chain reaction) procedures and should routinely be taken into consideration as the cause of unspecific neurological symptoms.

This evidence- and consensus-based practical guideline for the diagnosis and treatment of Guillain-Barré Syndrome (GBS) in childhood and adolescence has been developed by a group of delegates from relevant specialist societies and organisations; it is the result of an initiative by the German-Speaking Society of Neuropediatrics (GNP), and is supported by the Association of Scientific Medical Societies (AWMF, Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften). A systematic analysis of the literature revealed that only a few adequately-controlled studies exist for this particular age group, while none carries a low risk of bias. For this reason, the diagnostic and therapeutic recommendations largely rely on findings in adult patients with GBS, for which there are a higher number of suitable studies available. Consensus was established using a written, multi-step Delphi process. A high level of consensus could be reached for the crucial steps in diagnosis and treatment. We recommend basing the diagnostic approach on the clinical criteria of GBS and deriving support from CSF and electrophysiological findings. Repetition of invasive procedures that yield ambiguous results is only recommended if the diagnosis cannot be ascertained from the other criteria. For severe or persistently-progressive GBS treatment with intravenous immunoglobulin (IVIG) is recommended, whereas in cases of IVIG intolerance or inefficacy we recommended treatment with plasmapheresis. Corticosteroids are ineffective for GBS but can be considered when acute onset chronic inflammatory demyelinating polyneuropathy (A-CIDP) is suspected due to a prolonged disease course. PURPOSE: Guillain-Barré Syndrome (GBS) is a life-threatening disease due to respiratory muscle involvement. This study aimed at objectively assessing the course of respiratory muscle function in GBS subjects within the first week of admission to an intensive care unit. METHODS: Medical Research Council Sum Score (MRC-SS), vigorimetry, spirometry, and respiratory muscle function tests (inspiratory/expiratory muscle strength: PImax/PEmax, sniff nasal pressure: SnPna) were assessed twice daily. GBS Disability Score (GBS-DS) was assessed once daily. On days one (d1) and seven (d7), blood gases and twitch mouth pressure during magnetic phrenic nerve stimulation (Pmo,tw) were additionally evaluated. RESULTS: Nine subjects were included. MRC-SS, vigorimetry, PImax, and SnPna increased between d1 and d7. GBS-DS, spirometry and Pmo,tw remained unaltered. Only SnPna correlated closely with the MRC-SS on both d1 (r = 0.77, p = 0.02) and d7 (r = 0.74, p = 0.02). CONCLUSION: SnPna was the only parameter that correlated with MRC-SS, while the current gold standard of spirometry measurement did not.

Introducción: Un síntoma principal de la enfermedad de Parkinson es la alta incidencia en caídas y deterioro en el equilibrio, tanto estático como dinámico. El objetivo de este estudio es determinar el efecto de un programa de entrenamiento de Ai Chi acuático sobre la prevención de caídas en pacientes con Parkinson, mejorando su autonomía funcional y su percepción del dolor físico. Método: Quince pacientes diagnosticados de Parkinson (Hoehn y Yahr, rango: 1-3) participaron en un programa de intervención de Ai Chi acuático de 10 semanas de duración, con sesiones 2 veces por semana. Se emplearon las escalas EVA de dolor, Tinetti de equilibrio y marcha y el test Get up and Go. Resultados: Los resultados se obtuvieron aplicando la prueba de Friedman para 3 muestras relacionadas, en los pacientes antes de la terapia, después de la terapia y al mes de la terapia. Los datos obtenidos mostraron una mejoría significativa (p < 0,001) al comparar los resultados mostrados en las 3 mediciones, donde los valores de percepción de dolor, equilibrio y funcionalidad en marcha han disminuido significativamente, incluso al mes de finalizar las mismas los pacientes seguían mostrando mejoría y mantenimiento en sus resultados. Conclusión: Un programa de Ai Chi en el agua parece ser un tratamiento factible para los pacientes diagnosticados con EP en un grado leve y moderado en el tratamiento del dolor, equilibrio y capacidad funcional

The data obtained showed a significant improvement (p < .001) in scores for pain perception, balance, and gait function after the treatment programme. Furthermore, patients continued to show significant improvements and the benefits remained at the one-month follow-up visit. Conclusion: Ai Chi is a promising and feasible aquatic treatment for improving pain perception, balance, and functional capacity in patients diagnosed with mild or moderate Parkinson's disease

BACKGROUND: Treatment options for rheumatoid arthritis (RA) include pharmacological interventions, physical therapy treatments and balneotherapy. AIM: To evaluate the benefits and harms of balneotherapy in patients with RA. DESIGN: A systematic review. POPULATION: Studies were eligible if they were randomised controlled trials consisting of participants with definitive or classical RA. METHODS: We searched various databases up to December 2014. Balneotherapy had to be the intervention under study, and had to be compared with another intervention or with no intervention. We considered pain, improvement, disability, tender joints, swollen joints and adverse events among the main outcome measures. We excluded studies when only laboratory variables were reported as outcome measures. Two review authors independently selected trials, performed data extraction and assessed risk of bias. RESULTS: This review includes nine studies involving 579 participants. Most studies showed an unclear risk of bias in most domains. We found no statistically significant differences on pain or improvement between mudpacks versus placebo (1 study; N.=45; hand RA; very low level of evidence). As for the effectiveness of additional radon in carbon dioxide baths, we found no statistically significant differences between groups for all outcomes at three-month follow-up (2 studies; N.=194; low to moderate level of evidence). We noted some benefit of additional radon at six months in pain (moderate level of evidence). One study (N.=148) compared balneotherapy (seated immersion) versus hydrotherapy (exercises in water), land exercises or relaxation therapy. We found no statistically significant differences in pain or in physical disability (very low level of evidence) between groups. We found no statistically significant differences in pain intensity at eight weeks, but some benefit of mineral baths in overall improvement at eight weeks compared to Cyclosporin A (1 study; N.=57; low level of evidence). CONCLUSION: Overall evidence is insufficient to show that balneotherapy is more effective than no treatment; that one type of bath is more effective than another or that one type of bath is more effective than exercise or relaxation therapy. CLINICAL REHABILITATION IMPACT: We were not able to assess any clinical relevant impact of balneotherapy over placebo, no treatment or other treatments.

OBJECTIVES: Cerebral autoregulation is particularly challenged in acute ischemic stroke. We investigated (1) clinical and radiological factors related to dynamic cerebral autoregulation (DCA) in acute stroke and (2) the relationship between DCA and clinical outcome of stroke. METHODS: A total of 45 patients with middle cerebral artery (MCA) stroke were analyzed pooling two previous studies. DCA was measured by transcranial Doppler in both MCAs early (within 48 h from onset) and late (day 5-7) using low-frequency phase and correlation analysis (index Mx). Outcome was assessed by modified Rankin scale after a mean period of 4 months. RESULTS: Mx increased (i.e. autoregulation worsened) between the early and late measurement, more so on affected (P = 0.005) than on unaffected sides (P = 0.014). Poorer autoregulation as indicated by lower ipsilateral phase (early and late) and higher Mx (late measurement) were significantly related to larger infarction. More severe stroke was significantly related to poorer ipsilateral Mx and phase. Ipsilateral phase in the early (P = 0.019) and Mx in the late measurement (P =0..016) were related to poor clinical outcome according to univariate analysis. CONCLUSIONS: Impairment of DCA ipsilateral to acute ischemic stroke is associated with larger infarction. Dysautoregulation tends to worsen and spread to the contralateral side over the first days post-stroke and is associated with poor clinical outcome.

BACKGROUND: Balneotherapy (or spa therapy, mineral baths) for patients with arthritis is one of the oldest forms of therapy. One of the aims of balneotherapy is to soothe the pain and as a consequence to relieve patients' suffering and make them feel well. In this update we included one extra study. OBJECTIVES: To assess the effectiveness of balneotherapy for patients with osteoarthritis (OA). SEARCH STRATEGY: We searched the following databases up to October 2006: EMBASE, PubMed, the Cochrane 'Rehabilitation and Related Therapies' Field database, PEDro, CENTRAL (Issue 3, 2006) and performed reference checking and communicated with authors to retrieve eligible studies. SELECTION CRITERIA: Randomised controlled trials (RCT) comparing balneotherapy with any intervention or no intervention. At least 90% of the patient population had to be diagnosed with OA. DATA COLLECTION AND ANALYSIS: Two authors independently assessed quality and extracted data. Disagreements were solved by consensus. In the event of clinical heterogeneity or lack of data we refrained from statistical pooling. MAIN RESULTS: Seven trials (498 patients) were included in this review. Two studies compared spa-treatment with no treatment. One study evaluated baths as an add-on treatment to home exercises and another compared thermal water from Cserkeszölö with tap water (placebo). Three studies evaluated sulphur or Dead Sea baths with no treatment or mineral baths with tap water baths or no treatment. Only one of the trials performed an intention-to-treat analysis and two studies provided data to perform an intention-to-treat analysis ourselves. A 'quality of life' outcome was reported by one trial. We found: silver level evidence concerning the beneficial effects on pain, quality of life and analgesic intake of mineral baths compared to no treatment (SMD between 1.82 and 0.34). a statistically significant difference in pain and function of Dead Sea + sulphur versus no treatment, only at end of treatment (WMD 5.7, 95%CI 3.3 to 8.1), but not at 3 month follow-up (WMD 2.6, 95%CI -1.1 to 6.3). no statistically significant differences in pain or function at one or three months of Dead Sea baths versus no treatment (WMD 0.5, 95%CI -0.6 to 1.6) or at one or three months of sulphur baths versus no treatment (WMD 0.4, 95%CI -0.9 to 1.7). Adverse events were not measured in the included trials. AUTHORS' CONCLUSIONS: We found silver level evidence (www.cochranemsk.org) concerning the beneficial effects of mineral baths compared to no treatment. Of all other balneological treatments no clear effects were found. However, the scientific evidence is weak because of the poor methodological quality and the absence of an adequate statistical analysis and data presentation. Therefore, the noted "positive findings" should be viewed with caution.
