RESUMEN

The 2016 American Thoracic Society and Infectious Diseases Society of America (ATS/IDSA) guidelines removed the concept of healthcare-associated pneumonia (HCAP). We examined whether the 2016 ATS/IDSA guidelines are applicable in Korea.We conducted a retrospective, observational study of pneumonia patients who were hospitalized at a tertiary teaching hospital from March 2012 to February 2014. Identified pathogens that were not susceptible to ß-lactams, macrolides, and fluoroquinolones were defined as community-acquired pneumonia drug-resistant pathogens (CAP-DRPs). We analyzed the risk factors for 28-day mortality and the occurrence rate of CAP-DRPs.Of the 1046 patients, 399 were classified with HCAP and 647 with CAP. HCAP patients were older and had more comorbidities than CAP patients. Initial pneumonia severity index (PSI) was higher in patients with HCAP than with CAP. HCAP was associated with not only an increased rate of CAP-DRPs (HCAP, 19.8%; CAP, 4.0%; P < .001) but also an increased rate of inappropriate initial antibiotic therapy (IIAT) (HCAP, 16.8%; CAP, 4.6%; P < .001). HCAP was also associated with an increased 28-day mortality rate compared with CAP (HCAP, 14.5%; CAP, 6.3%; P < .001). In a multivariable analysis, PSI was an independent risk factor for 28-day mortality in HCAP patients (odds ratio 1.02, 95% confidence interval 1.01-1.04). CAP-DRPs and IIAT were not associated with mortality.Patients with HCAP revealed higher rates of CAP-DRPs, IIAT, and mortality than patients with CAP. However, CAP-DRPs and IIAT were not associated with mortality. PSI was the main predictive factor for 28-day mortality in patients with HCAP.

BACKGROUND AND OBJECTIVE: The TIOtropium Safety and Performance In Respimat (TIOSPIR) trial showed similar safety and exacerbation efficacy profiles for tiotropium Respimat and HandiHaler in patients with COPD. The TIOSPIR results for patients in Asia are presented here. METHODS: TIOSPIR evaluated once-daily tiotropium Respimat 5 and 2.5 µg with HandiHaler 18 µg in patients with COPD. Primary endpoints included time to death and time to first COPD exacerbation. Safety and exacerbation efficacy profiles were determined for the Asian region, and for Asia (all treatment arms pooled) versus the rest of the world (RoW). RESULTS: In Asia (n = 2356), time to death was similar for Respimat 5 and 2.5 µg versus HandiHaler 18 µg (hazard ratio (HR) (95% CI): 0.96 (0.67, 1.38) and 1.23 (0.87, 1.73)). Risk of COPD exacerbation was similar for Respimat 5 µg, but increased for 2.5 µg versus HandiHaler 18 µg (HR (95% CI): 0.99 (0.85, 1.15) and 1.17 (1.00, 1.35)). Time to death in Asia and RoW was similar (HR (95% CI): 1.15 (0.99, 1.35)). Time to first COPD exacerbation was longer (HR (95% CI): 0.84 (0.78, 0.89)) and exacerbation rates were lower in Asia, but severe exacerbations were more frequent than in the RoW. Risk of major adverse cardiovascular events was similar for both regions. CONCLUSION: Similar safety and exacerbation efficacy profiles were observed for tiotropium Respimat 5 µg and HandiHaler 18 µg in patients with COPD from Asia, analogous to the global analysis. Asian patients had lower risk of, and fewer exacerbations overall, but a higher proportion of severe exacerbations than in the RoW.

We have used steady-state and time-resolved neutron reflectometry to study the diffusion of fullerene derivatives into the narrow optical gap polymer poly[N-9″-hepta-decanyl-2,7-carbazole-alt-5,5-(4',7'-di-2-thienyl-2',1',3'-benzothiadiazole)] (PCDTBT) to explore the sequential processing of the donor and acceptor for the preparation of efficient organic solar cells. It was found that when [6,6]-phenyl-C61-butyric-acid-methyl-ester (60-PCBM) was deposited onto a thin film of PCDTBT from dichloromethane (DCM), a three-layer structure was formed that was stable below the glass-transition temperature of the polymer. When good solvents for the polymer were used in conjunction with DCM, both 60-PCBM and [6,6]-phenyl-C71-butyric-acid-methyl-ester (70-PCBM) were seen to form films that had a thick fullerene layer containing little polymer and a PCDTBT-rich layer near the interface with the substrate. Devices composed of films prepared by sequential deposition of the polymer and fullerene had efficiencies of up to 5.3%, with those based on 60-PCBM close to optimized bulk heterojunction (BHJ) cells processed in the conventional manner. Sequential deposition of pure components to form the active layer is attractive for large-area device fabrication, and the results demonstrate that this processing method can give efficient solar cells.

Fullerene derivatives are commonly used as electron acceptors in combination with (macro)molecular electron donors in bulk heterojunction (BHJ) organic photovoltaic (OPV) devices. Understanding the BHJ structure at different electron donor/acceptor ratios is critical to the continued improvement and development of OPVs. The high neutron scattering length densities (SLDs) of the fullerenes provide effective contrast for probing the distribution of the fullerene within the blend in a nondestructive way. However, recent neutron scattering studies on BHJ films have reported a wide range of SLDs ((3.6-4.4) × 10(-6) Å(-2)) for the fullerenes 60-PCBM and 70-PCBM, leading to differing interpretations of their distribution in thin films. In this article, we describe an approach for determining more precisely the scattering length densities of the fullerenes within a polymer matrix in order to accurately quantify their distribution within the active layers of OPV devices by neutron scattering techniques.

RESUMEN

Hybrid concepts are often used to improve existing methods in many fields. We developed a hybrid optical system that consists of multiple color cameras and one depth camera to make up the concavity problem of the visual hull construction. The heterogeneous data from the color cameras and the depth camera is fused in an effective way. The experimental results show that the proposed hybrid system can reconstruct concave objects successfully by combining the visual hull and the depth data.

RESUMEN

Cavity optoelectromechanical regenerative amplification is demonstrated. An optical cavity enhances mechanical transduction, allowing sensitive measurement even for heavy oscillators. A 27.3 MHz mechanical mode of a microtoroid was linewidth narrowed to 6.6 ± 1.4 mHz, 30 times smaller than previously achieved with radiation pressure driving in such a system. These results may have applications in areas such as ultrasensitive optomechanical mass spectroscopy.

RESUMEN

In this paper we present a technique to obtain a diffusion profile of layered turbid media in real time by using the quasi fast Hankel transform (QFHT) and the latest graphics processing unit technique. We apply the QFHT to convolve the diffusion profiles of each layer so as to dramatically reduce the time for the convolution step while maintaining the accuracy. In addition, we also introduce an accelerated technique to generate individual discrete diffusion profiles for each layer through parallel processing. The proposed method is 2 orders of magnitude faster than the existing method, and we validate its efficiency by comparing it with Monte Carlo simulation and another relevant methods.

RESUMEN

We implement a cavity optoelectromechanical system integrating electrical actuation capabilities of nanoelectromechanical devices with ultrasensitive mechanical transduction achieved via intracavity optomechanical coupling. Electrical gradient forces as large as 0.40 microN are realized, with simultaneous mechanical transduction sensitivity of 1.5x10{-18} m Hz{-1/2} representing a 3 orders of magnitude improvement over any nanoelectromechanical system to date. Optoelectromechanical feedback cooling is demonstrated, exhibiting strong squashing of the in-loop transduction signal. Out-of-loop transduction provides accurate temperature calibration even in the critical paradigm where measurement backaction induces optomechanical correlations.

RESUMEN

We experimentally demonstrate thermo-optic locking of a semiconductor laser to an integrated toroidal optical microcavity. The lock is maintained for time periods exceeding twelve hours, without requiring any electronic control systems. Fast control is achieved by optical feedback induced by scattering centers within the microcavity, with thermal locking due to optical heating maintaining constructive interference between the cavity and the laser. Furthermore, the optical feedback acts to narrow the laser linewidth, with ultra high quality microtoroid resonances offering the potential for ultralow linewidth on-chip lasers.

This study describes the methodology for analyzing skin movement with respect to the predefined flexional motion of the hand. The 3D in vivo geometric data were acquired through magnetic resonance (MR) scanning of multiple hands in various postures. Custom software was developed to segment MR images and to generate polygonal isosurfaces. A new bone coordinate system (BCS) was defined to describe the skin movement measured with the skin-surface markers. The BCS was defined by the surface registration technique, which could minimize the distance among the 3D polygonal surfaces of different postures. MR images of six subjects in the 3 postures were captured with skin-surface markers for the experiment. Skin movement on the second metacarpal bone, which was fixed during the flexional motion, ranged from 1.169 to 10.885 mm. While the distance of the skin movement varied from one subject to another, a common skin movement direction was observed among the six subjects. For the repeatability evaluation of the proposed method, the MR images of the same subject were independently processed 10 times by a trained operator. The standard deviation of the skin movement on the second metacarpal of the index finger with respect to the predefined flexional motion ranged from 0.45 to 0.74 mm.

BACKGROUND: An os acromiale results from the failure of fusion of the acromial secondary centers of ossification. It is most easily seen radiographically on an axillary lateral view. The purpose of the present study was to describe two simple radiographic findings, the double-density sign on a standard anteroposterior view of the shoulder and a cortical irregularity found on a supraspinatus outlet view, that are highly suggestive of an os acromiale. METHODS: Anteroposterior, axillary lateral, and supraspinatus outlet radiographs of thirty-four shoulders in thirty adult patients with an os acromiale were reviewed by two independent observers and were compared with those of a control group of thirty-one shoulders in twenty-nine patients without an os acromiale. Statistical analysis was performed with use of a generalized logistic regression model to determine if an os acromiale could be detected on all three radiographic views. A kappa analysis was performed to determine interobserver reliability. RESULTS: In the group with an os acromiale, twenty-eight shoulders had a meso-acromion and six had a pre-acromion. A double-density sign was noted on the anteroposterior radiograph of 82.4% of the shoulders, an os acromiale was noted on the axillary lateral radiograph of 95.6% of the shoulders, and a cortical irregularity was noted on the supraspinatus outlet radiograph of 75.8% of the shoulders. In the control group, a double-density sign was noted on the anteroposterior radiograph of 4.8% of the shoulders, no os acromiale was seen on the axillary lateral radiograph of any of the shoulders, and a cortical irregularity was noted on the supraspinatus outlet radiograph of one shoulder. These differences between the os acromiale and control groups were significant (p < 0.0001). The overall sensitivities of the anteroposterior, axillary, and supraspinatus outlet views for the detection of an os acromiale were 82.4%, 94.1%, and 73.5%, respectively. The overall specificities of the three views were 95.2%, 100%, and 98.4%, respectively. The interobserver reliabilities of the three views were 0.66, 0.88, and 0.7, respectively (p < 0.0001). CONCLUSIONS: The double-density sign on a standard anteroposterior radiograph of the shoulder and a cortical irregularity on the supraspinatus outlet view are highly suggestive of an os acromiale. An os acromiale should be suspected in a patient with these radiographic findings. LEVEL OF EVIDENCE: Diagnostic study, Level IV-1 (case-control study).
