RESUMEN

BACKGROUND: Surgical wound infection is the most frequent type of care health associated infection. Lack of knowledge about the prevention of surgical wound infection in patients undergoing left ventricular assist device implantation could significantly undermine the potential benefits of surgical intervention. OBJECTIVES: This study aimed to map the recommendations for adult patients undergoing left ventricular assist device implantation. DESIGN: This is a scoping review, being registered in the Open Science Framework under DOI https://doi.org/10.17605/OSF.IO/Q76B3 (https://osf.io/q76b3/). METHOD: Left ventricular assist device coordinators and nurse specialists in dermatology and stomatherapy conducted a scoping review in Scopus, The Cochrane Database of Systematic Reviews, Cumulative Index to Nursing and Allied Health Literature (CINAHL), limited to the period between 2015 and 2022. The results of this scoping review will be discussed and presented in separate articles. This paper will synthesize research evidence on the perioperative topic. RESULTS: The initial searches resulted in 771 studies. Sixty nine met the eligibility criteria and were included in the scoping review. Eight articles addressing the perioperative topic that answered the question of this article were included. CONCLUSION: Although this scoping review included heterogeneous, and scarce studies with left ventricular assist device patients. As such, there are many promising future research directions for this topic. IMPLICATIONS FOR CLINICAL PRACTICE: Infection surveillance should be an integral part of left ventricular assist device implantation programs in health care institutions. Velvet completely buried in subcutaneous tissues reduces transmission system infection. Triple tunnel method reduces transmission system infection risk.

OBJECTIVE: to evaluate the effect of implementation of hospital discharge planning based on the taxonomies of NANDA-International, nursing interventions classification (NIC) and nursing outcomes classification (NOC) for patients with heart failure (HF) or diabetes mellitus (DM). METHODS: quasi-experimental quantitative study conducted in a public university hospital located in the state of Rio Grande do Sul, Brazil. Convenience sampling included 28 adult patients hospitalized for HF or DM with the nursing diagnosis Ineffective Health Management (00078), who received the following nursing interventions: Teaching: Disease Process, Teaching: Prescribed Medication and Teaching: Prescribed Diet. Before and after the intervention, the following nursing outcomes were evaluated : Knowledge: Diabetes Management and Knowledge: Heart Failure Management. RESULTS: the score of the nursing outcome Knowledge: Heart Failure Management went from 2.05±0.28 to 2.54±0.30 (P=0.002), and of the nursing outcome Knowledge: Diabetes Management went from 2.61±0.55 to 3.21±0.57 (P=0.000). CONCLUSION: discharge planning based on the NIC improves the NOC score and may interfere in the health outcomes.

RESUMEN Objetivo: evaluar el efecto de la implementación de un plan de alta hospitalaria basado en la taxonomía de NANDA-Internacional, clasificación de intervenciones de enfermería (NIC) y clasificación de resultados de enfermería (NOC), para pacientes con insuficiencia cardíaca (IC) o diabetes mellitus (DM). Métodos: estudio cuantitativo cuasi experimental realizado en un hospital universitario público ubicado en el estado de Rio Grande do Sul, Brasil. El muestreo de conveniencia incluyó a 28 pacientes adultos hospitalizados por IC o DM con el diagnóstico de enfermería Gestión de Salud Inefectiva (00078), que recibieron las siguientes intervenciones de enfermería: Enseñanza: Proceso de la enfermedad, Enseñanza: Medicación prescrita y Enseñanza: Dieta prescrita. Antes y después de la intervención, se evaluaron los siguientes resultados de enfermería: Conocimiento: Control de la diabetes y Conocimiento: Control de la insuficiencia cardíaca congestiva. Resultados: la puntuación del resultado de enfermería Conocimiento: Control de la insuficiencia cardíaca congestiva pasó de 2.05±0.28 a 2.54±0.30 (P=0.002), y del resultado de enfermería Conocimiento: Control de la diabetes pasó de 2.61±0.55 a 3.21±0.57 (P=0,000). Conclusión: la planificación de alta basada en la NIC mejora la puntuación de NOC y puede interferir en los resultados de salud.


RESUMO Objetivo: avaliar o efeito de um planejamento de alta baseado nas taxonomias NANDA-I, Classificação das intervenções de enfermagem (NIC) e Classificação dos resultados de enfermagem (NOC), implementado para pacientes com insuficiência cardíaca (IC) ou diabetes mellitus (DM). Método: Estudo quantitativo quase-experimental realizado em hospital público e universitário do Sul do Brasil. Amostragem por conveniência com 28 pacientes internados por IC ou DM com Diagnóstico de enfermagem Controle ineficaz da saúde, que receberam as intervenções Ensino: processo de doença, Ensino: medicação prescrita e Ensino: dieta prescrita. Antes e após, foram avaliados os Resultados Conhecimento: controle do diabetes e Conhecimento: controle da insuficiência cardíaca congestiva. Resultados: O NOC Conhecimento: controle da insuficiência cardíaca congestiva passou de 2,05±0,28 a 2,54±0,30 (P=0,002) e o Resultado Conhecimento: controle do diabetes passou de 2,61±0,55 a 3,21±0,57 (P=0,000). Conclusão: O planejamento de alta utilizando as intervenções NIC melhora a pontuação dos resultados NOC e pode interferir nos desfechos de saúde.

RESUMEN

OBJECTIVE: To report the role of multiprofessional teams in the effective communication and safe care of patients with left ventricular assist device. METHODS: This is an experience report about the use of effective communication for patients with ventricular assist device in a university hospital in southern Brazil. Care based on individualized action and centered on the patients started in 2017. RESULTS: At the institution, the multiprofessional teams attended training sessions and care protocols were established. The patients and caregivers attended systematic educational sessions and home visits were made to arrange patient discharge and urgency plans. Also, the hospital teams located near the patients' homes received basic training. CONCLUSION: Effective communication between multiprofessional teams, patients, and their families when planning care plays a pivotal role in the early identification of possible complications and their prevention, resulting in a greater survival rate and a better quality of life.

RESUMEN

Resumo OBJETIVO Relatar a atuação da equipe multidisciplinar na comunicação efetiva e no cuidado seguro aos pacientes com dispositivo de assistência ventricular. MÉTODO Relato de experiência sobre o uso da comunicação efetiva no cuidado a pacientes com dispositivo de assistência ventricular em hospital universitário do sul do Brasil. A referida assistência iniciou em 2017, pautada na individualização das ações e na perspectiva do cuidado centrado no paciente. RESULTADOS Na instituição foram capacitadas equipes multiprofissionais, estruturados protocolos assistenciais, realizadas sessões educativas sistemáticas para pacientes e cuidadores. Foram executadas visitas domiciliares para planejamento de alta hospitalar e plano para situações de urgência, além de capacitações básicas para as equipes dos hospitais próximos à residência dos pacientes. CONCLUSÕES A comunicação efetiva entre equipe multiprofissional, pacientes e cuidadores no planejamento do cuidado em saúde tem papel fundamental para identificar precocemente possíveis complicações, preveni-las e possibilitar maior sobrevida e qualidade de vida a esses pacientes.


Resumen OBJETIVO Informar la actuación del equipo multidisciplinario en la comunicación efectiva y cuidado seguro a los pacientes con dispositivo de asistencia ventricular. MÉTODO Se trata de un relato de experiencia sobre el uso de la comunicación efectiva en la asistencia a pacientes con dispositivo de asistencia ventricular em un hospital universitario del sur de Brasil. Esse cuidado, inició en 2017, pautado em la individualización de las acciones y em la perspectiva del cuidado centrado em el paciente. RESULTADOS En la institución se capacitaron equipos multiprofissionales, estructurados protocolos asistenciales, realizadas sesiones educativas sistemáticas para pacientes y cuidadores, visita domiciliares para la planificación de alta hospitalaria y plan para situaciones de urgência, además de capaccitaciones de urgência, además de capacitaciones básicas para los equipos de los hospitales cercanos a la residencia de los pacientes. CONCLUSIONES La comunicación efectiva entre el equipo multiprofesional, pacientes y familiares en la planificación del cuidado en salud tiene un papel fundamental para identificar precozmente posibles complicaciones, prevenirlas y posibilitar mayor sobrevida y calidad de vida.
