RESUMEN

Developing synthetic materials, with enzyme-like molecular recognition capabilities, as functional receptors in electronic or electrochemical devices for the timely diagnosis of major diseases is a great challenge. Herein, we present the development of Fe/MIPpy nanozymes, characterized as enzyme-like artificial receptors, for the precise and non-invasive monitoring of cancer biomarkers in aqueous solutions and human saliva. Through the integration of PVA-stabilized FeFe2O4 nanocrystals in a molecularly imprinted conducting polypyrrole matrix, the Fe/MIPpy nanozymes demonstrate 424 nA cm-2 nM-1 sensitivity and 220 pM detection limit. Charge-transfer mechanisms, Fe/MIPpy-spermine interactions, and the principle of spermine recognition are investigated by electrochemical impedance spectroscopy (EIS) and cyclic voltammetry (CV). The disposable Fe/MIPpy sensor operates wirelessly and offers rapid and remote quantification of spermine, making it a promising material for the development of cost-effective tools for non-invasive cancer diagnosis and prognosis.

INTRODUCTION: Effective critical care requires the placement of a central venous catheter (CVC), which is frequently indicated for volume resuscitation, hemodynamic assessment, and the administration of vasopressors, blood products, and parenteral nourishment. However, central venous catheterization is not without its complications. The majority of these problems are avoidable and treatable with proper patient selection, cautious insertion technique, and attention after catheter insertion. CASE: We present a case of a 34-year-old male patient who developed unilateral hydrothorax as a result of a central venous catheter malposition in the lung parenchyma. The condition was resolved since the complication was identified and treated quickly. CONCLUSION: Previous studies have revealed a few unusual incidences of catheter misplacement. This case report can be serviceable to the medical community as they should be aware of this unique presentation, it's management and outcome.

BACKGROUND: Increased exposure of humans to toxic metals and high-fat diet (HFD) consumption severely damages brain health. Natural plant extracts have shown huge potential to treat multiple human diseases. OBJECTIVE: The present study was designed to evaluate the protective effects of Shogaol (an active component of ginger) in neuroinflammation and behavioral paradigms in mice treated with metals and HFD. METHODS: 8-11 weeks old male mice model was developed by giving a combination of metals, i.e., Arsenic (As), Lead (Pb) and Aluminum (Al), 25mg/kg each mixed in drinking water with laboratory prepared HFD (40% fat) for a total duration of 72 days. Shogaol treated groups received two doses (2mg/kg & 12mg/kg) of Shogaol along with metals and HFD. The biochemical parameters, including body weights, blood glucose, and kidney and liver functions, were assessed along with the integrity of the blood-brain barrier (BBB). The expression analysis of neuroinflammatory genes (TNF-α, IL-1ß & GFAP) was performed using q-PCR in the hippocampus and cortex. The exploratory and anxiety-like behavior was assessed using an open field test, and depressive behavior was assessed through the forced swim test, while learning and memory were assessed using the Morris water maze test and y-maze test. RESULTS: Shogaol (2mg/kg & 12mg/kg) treatment improved metabolic profile and reduced expression of neuroinflammatory genes in the cortex and the hippocampus. Shogaol treatment improved BBB integrity. Results of the behavioral analysis showed that Shogaol treatment (2mg/kg & 12mg/kg) rescued behavioral impairment and improved anxiety and depression. CONCLUSION: Shogaol treatment showed strong therapeutic potential in metals & HFD induced neuroinflammation and improved cognitive functions; thus, can be considered a potential drug candidate in the future.

Diclofenac sodium is an anti-inflammatory drug commonly used to cure pain in various treatments. The remarkable potential of this pain-killer leads to its excessive use and, therefore, a persistent water contaminant. Its presence in aqueous bodies is hazardous for both humans and the environment because it causes the growth of harmful drug-resistant bacteria in water. Herein, we present a comparative study of the ZnO and ZnFe2O4 as photocatalysts for the degradation of diclofenac sodium, along with their structural and morphological studies. A simple co-precipitation method was used for the synthesis of ZnO and ZnFe2O4 and characterized by various analytical techniques. For instance, the UV-Vis study revealed the absorption maxima of ZnO at 320 nm, which was shifted to a longer wavelength region at 365 nm for zinc ferrite. The optical band gaps obtained from the Tauc plot indicated that the incorporation of iron has led to a decreased band gap of zinc ferrite (2.89 eV) than pure ZnO (3.14 eV). The metal-oxygen linkages shown by FTIR indicated the formation of desired ZnO and ZnFe2O4, which was further confirmed by XRD. It elucidated the typical hexagonal structure for ZnO and spinel cubic structure for ZnFe2O4 with an average crystallite of 31 nm and 44 nm for ZnO and ZnFe2O4, respectively. The micrographs obtained by SEM showed rough spherical particles of ZnO, whereas for ZnFe2O4 flower-like clustered particles were observed. The photocatalytic investigation against diclofenac sodium revealed the higher degradation efficiency of ZnFe2O4 (61.4%) in only 120 min, whereas ZnO degraded only 48.9% of the drug. Moreover, zinc ferrite has shown good recyclability and was stable up to five runs of photodegradation with a small loss (3.9%) of photocatalytic activity. The comparison of two catalysts has suggested the promising role of zinc ferrite in wastewater remediation to eliminate hazardous pharmaceuticals.

Recent facts and figures published in various studies in the US show that approximately 27,510 new cases of gastric infections are diagnosed. Furthermore, it has also been reported that the mortality rate is quite high in diagnosed cases. The early detection of these infections can save precious human lives. As the manual process of these infections is time-consuming and expensive, therefore automated Computer-Aided Diagnosis (CAD) systems are required which helps the endoscopy specialists in their clinics. Generally, an automated method of gastric infection detections using Wireless Capsule Endoscopy (WCE) is comprised of the following steps such as contrast preprocessing, feature extraction, segmentation of infected regions, and classification into their relevant categories. These steps consist of various challenges that reduce the detection and recognition accuracy as well as increase the computation time. In this review, authors have focused on the importance of WCE in medical imaging, the role of endoscopy for bleeding-related infections, and the scope of endoscopy. Further, the general steps and highlighting the importance of each step have been presented. A detailed discussion and future directions have been provided at the end.

INTRODUCTION: Safety of extended lymphadenectomy (EL) ± vascular resection in patients undergoing pancreaticoduodenectomy (PD) in resource limited settings is not well established. The objective of this study was to report outcomes of PD resection with EL ± vascular resection from Pakistan and review national literature. METHODS: Data of patients who underwent PD between 2011 and 2014 was reviewed. A total of 66 patients were included in the study. Primary outcome was 90 day morbidity and mortality. Secondary objective was 2 year overall survival. RESULTS: Median age was 57 (32-82) years. Majority patients had locally advanced (pT3/T4) tumors and nodal involvement i.e. 44 (71%) and 42 (67.8%). Mean number of excised lymph nodes and positive lymph nodes was 28.2 ± 12.8 (range 3-62) and 4.8 ± 6.9 (range 0-27) respectively. There were 13 vascular resections including 3 arterial resections. Overall 90 day morbidity was 31.8% and mortality was 3%. No difference in complication rate was observed in patients who did and did not undergo vascular resection i.e. 18% versus 34.5% (P = 0.1). Similarly, median survival was 11 (3-24) months and 11 (1-36) months and not significantly different (P = 0.5). CONCLUSION: In developing countries, extended lymphadenectomy with vascular resection can be safely performed with pancreaticoduodenectomy in specialized hepatobiliary units.
