RESUMEN

Rates of psychological trauma are increasing in the community. Psychological trauma symptoms may prevent patients from being able to access or tolerate dental treatment. The importance of dental teams providing equitable person-centred care by adopting a trauma-informed approach is discussed. One way to support dental services to be trauma-informed is through training staff in psychological trauma. An example from a Scottish Dental Hospital is provided. Directions for future research are discussed and the next steps in the journey to develop trauma-informed dental services are outlined.

OBJECTIVE: Psychological flexibility and fear of cancer recurrence are important variables that influence psychosocial outcomes in individuals diagnosed with a range of different types of cancer. Their role and how they impact on psychological distress and quality of life in men with prostate cancer specifically have not been established. METHODS: A cross-sectional sample of 144 men with prostate cancer was recruited. Multiple regression and conditional process analysis were used to assess whether psychological flexibility moderates the relationship between fear of recurrence and distress and quality of life. RESULTS: Psychological flexibility significantly predicted psychological distress (ß = -0.56, p < 0.0001) and quality of life (ß = 0.21, p < 0.0001), appearing a stronger predictor of psychological distress than fear of recurrence (ß = 0.25, p < 0.0001). Fear of recurrence was a stronger predictor of quality of life (ß = -0.41, p < 0.0001) than psychological flexibility. Psychological flexibility moderated the relationship between fear of recurrence and psychological distress (ß = -0.01, p < 0.001). At low and average levels of psychological flexibility, psychological distress mediated the relationship between fear of recurrence and quality of life (ß = -0.33 to -0.16, p < 0.05). At high levels of psychological flexibility, distress no longer mediated this relationship (ß = 0.01, ns), supporting the role of psychological flexibility as a moderator. CONCLUSIONS: These findings suggest that psychological flexibility might be a useful treatment target, through interventions such as Acceptance and Commitment Therapy, to buffer the effects of fear of recurrence and distress and improve psychosocial outcomes in this population.

The relationship between working hours and sustainability has attracted research attention since at least the early 2000s, yet the role of care giving in this context is not well understood. Focusing on Australians between 40 and 60 years who have reduced their working hours and income, we explore the relationship between working hours, care giving and consumption. Data from the national census (Australian Bureau of Statistics, 2006, Australian Bureau of Statistics, 2011, Australian Bureau of Statistics, 2016c) were analysed to contextualise patterns in paid working hours, income and carer roles for men and women aged between 40 and 60 years. Findings from a national survey on informal carers (ABS, 2016a) were also consulted. Taken together, the two sources of national data showed that two thirds of all informal carers are women, that the likelihood of assuming informal carer roles increases with age, and that men and women in carer roles work fewer paid hours per week and have a lower weekly income than non-carers of the same age. To gain qualitative insights into these patterns in Australian national data, and the likely implications of carer roles for household consumption, semi-structured interviews were conducted with ten households who subsequently recorded details of their consumption-related expenses over a seven-day period. The interview data showed the strong connection between carer roles, reduced income and paid working hours and its strongly gendered dimension. We argue that women primarily 'downshift' to undertake care rather than for sustainability motivations and that there is consequently a need to connect scholarship on gender and care with that on downshifting. The link between reducing paid working hours, care-giving and household consumption appeared to be less straight forward and varied between households. Our findings suggest that a complex relationship exists between environmental and social welfare concerns that has policy implications and warrants further exploration.

BACKGROUND: Alcohol related violence is a troubling backdrop to the social lives and relationships of many young people in post-industrial societies. The development of the night-time economy where young people are encouraged to drink heavily in entertainment precincts has increased the risk of violence. METHODS: This paper reports on 60 individual structured in-depth interviews about the drinking biographies of young people (aged 20-24) living in Victoria, Australia. Twenty-six males and 34 females participated in the research. The participants discussed their experiences with alcohol over their life course to date. The material on alcohol related violence is analysed in this paper. RESULTS: Just over half of the participants (33/60) recounted negative experiences with alcohol related violence. The findings demonstrate the continuing gendered nature of experiences of perpetration and victimization. Participants reported that aggression and violence perpetrated by some men was fuelled by alcohol consumption and required ongoing management. Experiences of violence were also spatialized. Men were more likely to report managing and avoiding violence in particular public settings whilst more women than men discussed managing violence in domestic settings. CONCLUSION: The central argument of this paper is that incidents of alcohol related violence and reactions to it are specific gender performances that occur in specific socio-cultural contexts. In contrast to research which has found some young people enjoy the adventure and excitement of alcohol related violence the mainstream participants in this study saw violence as a negative force to be managed and preferably avoided. Understanding violence as a dynamic gender performance complicates the development of policy measures designed to minimize harm but also offers a more holistic approach to developing effective policy in this domain. There is a need for greater acknowledgement that alcohol related violence in public venues and in families is primarily about particular performances of masculinity and this is where policy should be targeted in addition to venue based interventions.

In this paper we document the experience of participating in novel randomised controlled trials for panic disorder - where face-to-face and Internet delivery of cognitive behavioural therapy are compared. Our analysis is based on 18 months of observation and in-depth interviews with 10 trial participants and 8 trialists in Victoria, Australia. We argue that the participants are positioned as active health consumers and approach the trial as they would other self-help practices. High levels of individual responsibility are assumed of participants in these trials, which they accept by approaching the trials reflexively and searching for information and strategies they can employ while building their health literacy on panic disorder. Although the researchers set the parameters of the treatment and interaction, increasingly the participants choose the extent to which they will comply with their defined role. For the participants the trial is one of the 'pick and mix' options of available treatment and we suggest it is a compelling example of contemporary health consumption.

RESUMEN

Twenty Australian lesbian-parented families were interviewed in multigenerational family groups about the interface between their public and private worlds. Experiences of the health care bureaucracy were difficult, whereas many participants found individual providers to be approachable and caring. Three strategies were used for disclosure of their sexual orientation to health care providers: private, proud, and passive. Influences on the strategy used included family formation, role of the non-birth parent, geographic location, and expected continuity of care. Parents displayed a high degree of thoughtful planning in utilizing their preferred disclosure strategy in order to optimize safety, particularly for their children.
