RESUMEN

BACKGROUND: We aimed to investigate the postoperative prognosis in patients with early-stage laryngeal squamous cell carcinoma (LSCC) in association with the preoperative blood markers and clinicopathological characteristics and to develop nomograms for individual risk prediction. METHODS: The clinical data of 353 patients with confirmed early-stage LSCC between 2009 and 2018 were retrospectively retrieved from the First Affiliated Hospital with Nanjing Medical University. All patients were randomly divided into the training and testing groups in a 7:3 ratio. Univariate and multivariate analyses were performed, followed by the construction of nomograms to predict recurrence-free survival (RFS) and overall survival (OS). Finally, the nomograms were verified internally, and the predictive capability of the nomograms was evaluated and compared with that of tumour T staging. RESULTS: Univariate and multivariate analyses identified platelet counts (PLT), fibrinogen (FIB), and platelet to lymphocyte ratio (PLR) were independent factors for RFS, and FIB, systemic immune-inflammation index (SII), and haemoglobin (HGB) were independent prognostic factors for OS. The nomograms showed higher predictive C-indexes than T staging. Furthermore, decision curve analysis (DCA) revealed that the net benefit of the nomograms' calculation model was superior to that of T staging. CONCLUSIONS: We established and validated nomograms to predict postoperative 1-, 3- and 5-year RFS and OS in patients with early-stage LSCC based on significant blood markers and clinicopathological characteristics. These models might help clinicians make personalized treatment decisions.

RESUMEN

Objective@#To analysis the anxiety status of Chinese medical workers during the epidemic of COVID-19 by meta-analysis method. @*Methods@#CNKI, VIP, WanFang Data, SinoMed, PubMed, Cochrane, EMBASE, MEDLINE, Scopus, Google Scholar and other databases were searched to collect literature on the anxiety status of Chinese medical workers during the epidemic of COVID-19. The retrieval time is from the database construction to 11/03/2020. Meta-analysis was performed on the included articles by using Stata 16.0 software. @*Results@#A total of 7 articles were included, with a total sample size of 7,741 people. Meta-analysis using the random effects model showed that the anxiety score of Chinese medical during the epidemic of COVID-19 was significantly higher than that of the national norm in each study, the difference was statistically significant [SMD (95% CI)=1.145 (0.705–1.584), p<0.001]. @*Conclusion@#The anxiety level of Chinese medical workers has increased significantly during the epidemic of COVID-19.

Objective To investigate the serum expressions and clinical significance of Dickkopf-1 in patients with rheumatoid arthritis (RA). Methods The clinical data of 139 patients with RA were retrospectively analyzed, including the disease history, tender joint count (TJC), swollen joint count (SJC), platelet, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), rheumatoid factor (RF), anti-nuclear antibody (ANA), antikeratin antibody (AKA), anti-perinuclear factor (APF), anti cyclic citrullinated peptide antibody (anti-CCP), Dickkopf-1 and radiological (X-ray) staging. The disease activity scale (DAS) was evaluated, and the ESR and CRP levels were included. The relationship between Dickkopf-1 and the clinical data of RA, DAS44 score was analyzed. Results The serum level of Dickkopf-1 in patients with RA was (2.70 ± 0.46) μg/L. There was no relationship between serum Dickkopf-1 level and gender, age, course of disease, CRP, platelet, ANA, AKA, APF, RF, radiological staging in patients with RA (P>0.05). The serum Dickkopf-1 level was significantly associated with TJC, SJC, ESR, DAS44-ESR score, DAS44-CRP score and anti-CCP (r = 0.200, 0.291, 0.178, 0.222, 0.199 and 0.278, P = 0.019, 0.001, 0.037, 0.009, 0.028 and 0.012). Conclusions The serum Dickkopf-1 expression level is closely related to the occurrence and development of RA. Dickkopf-1 may contribute to diagnose the disease activity in patients with RA.

RESUMEN

Objective To compare the efficacy of vacuum sealing drainage( VSD) and the traditional treatment for breast abscess drain-age abscess.Methods A total of 59 cases with breast abscess in our hospital from February 2013 to December 2014 were randomly divided into two groups.The observation group underwent VSD and the control group treated by traditional abscess drainage of abscesses.The healing time,scarring length,the average length of hospital stay,number of dressing changes,pain degree were observed.The breast deformation and the recurrence of patients after 6 months were compared.Results The abscess healing time,the length of scar and the average days of hospi-talization were respectively (10.3 ±1.2) d,(1.5 ±0.2) cm,(12.7 ±3.5) d,the control group were respectively (16.5 ±2.6) d,(4.5 ± 0.6) cm,(26.9 ±3.3) d.The difference between two groups was statistically significant(P0.05).No breast deformation occurred in observation group,but 5 cases in the control group.The difference between two groups was statistically signifi-cant(P<0.05).Conclusion The effect of VSD for breast abscess is significant,which can accelerate the healing of the abscess and reduce the suffering of patients without affecting the appearance.

RESUMEN

Objective To explore the effect of a multimodal electroencephalogram ( EEG) data visualiza-tion system on the motor imagery ability of stroke survivors. Methods Twenty stroke patients were randomly di-vided into an experimental group and a control group, each of 10. Both groups were provided with brain-computer interface-based motor imagery ( MI) training. At the same time, the experimental group was monitored and guided using an online, multimodal EEG data visualization system developed in our department. The classification accuracy ( CA) and event-related desynchronization ( ERD) of the 2 groups′ motor imagery were compared before and after the treatment. Results Before the treatment, no significant differences in the average CA of MI were found be-tween the experiment group (50.92±2.08) and the control group (49.35±4.20)(P>0.05). After the treatment, however, the experimental group′s average CA had increased to (64.52±5.27), significantly higher than that of the control group (51.18±5.02). When the stroke patients imaged affected upper extremity movements, obvious ERD was observed in the α frequency around the bilateral central motor regions of both groups, especially in the experi-mental group, but without significant differences between the two groups. However, no significant changes were found in the ERD of theβwaves of the two groups( P>0.05) . Conclusion The proposed online multimodal elec-troencephalogram data visualization system can help stroke patients imagine movements actively. It is worth sprea-ding in clinical practice.

OBJECTIVE@#To evaluate the diagnostic value of liver imaging reporting and classification criteria of data system (LI-RADS) MRI on primary hepatic carcinoma (HCC).
@*METHODS@#The MRI images of 100 patients (72 patients with HCC, 4 with regenerative nodules, 13 with hemangioma, 4 with liver cyst, 3 with Liver abscess, 2 with inflammatory pseudotumor, 1 with hepatic arteriovenous fistula, 1 with Limitations of steatosis) with liver disease were collected in Xiangya Hospital from January, 2014 to January, 2015. All patients were at the risk of HCC. The images were reviewed according to the LI-RADS categorization by American College of Radiology (ACR) under a condition of blindness to the pathological data. The sensitivity, specificity and the area under the ROC curve in the diagnosis of HCC with the LI-RADS categorization were calculated by three associate professors from the Department of Radiology, Xiangya Hospital.
@*RESULTS@#The area under the ROC curve was 0.925 in the 100 patients with HCC. Of the 100 pathologically confirmed patients, 20 cases were LR1 and LR2. However, the post-operation pathological examination showed benign tumor (negative predictive value was 100%). There were 25 HCC in 31 cases of LR3, 27 HCC in 29 cases of LR4 (positive predictive value was 93.1%). All of 20 cases of LR5 were HCC (positive predictive value was 100%). If LR1 and LR2 were considered negative, and LR3, 4, 5 were positive, then the sensitivity, the specificity and the area under the ROC curve was 100%, 71.4% and 0.925, respectively. If LR1, 2 and 3 were considered negative and LR4 and LR5 were positive, the sensitivity, the specificity and the area under the ROC curve was 100%, 90.9% and 0.974, respectively.
@*CONCLUSION@#The MRI LI-RADS categorization is highly sensitive and specific in the diagnosis of HCC, which can improve the diagnostic accuracy.

Objective To invetigate clinical characteristics,pathogenesis and its risk factors of neuropsychi-atric lupus erythematosus(NPLE).Methods 160 cases of SLE patients,including 30 cases of diagnosed NPLE,were analyzed retrospectively.The relationship between the appearance of NPLE and common autoantibodies,other organ involvement and SLE disease activity score(SLEDAI)were analyzed.Results Such antibody positive rates of anti nuclear antibody(ANA),anti Sm antibody,anti -RNP antibody,anti SSA antibody,anti ds -DNA antibody had no significant difference between NPLE and non NPLE,the value of χ2 was 0.947,0.013,1.194,0.023,0.745 respectively,the value of P was 0.194,0.910,0.274,0.879,0.388 respectively.Renal involvement,raynaud phenomenon,interstitial pneumonia,pulmonary thromboembolism,oral ulcer,arthritis,facial erythema,pleuritis,pericarditis,fever,pulmonary hypertension,photo -allergy and alopecia in the two groups had no difference,the value of χ2 was respectively 0.419,1.383,0.721,0.201,1.368,1.194,0.055,0.946,0.262,2.503,0.628,2.898 and 0.075,the value of P was 0.517,0.324,0.396,1.000,0.242,0.274,0.815,0.331,0.609,0.114,0.428,0.089 and 0.785 respectively.But the occurrence of hand and foot vasculitis in NPLE was significantly higher(χ2 =3.996,P =0.046).SLEDAI of NPLE was higher than non NPLE(t =8.446,P =0.000).Conclusion There was no correlation with the occurrence of NPLE and common autoantibodies,other organ involvements.Hand and foot vasculitis and higher SLEDAI(more than 15 points)were the risk factors of NPLE.Encephalopathy may be the initial manifestation to some of SLE cases. Early diagnosis and methylprednisolone pulse treatment combined with immunosuppressive therapy can effectively improve the remission of the disease,reduce mortality and improve prognosis.

DraIII is a type IIP restriction endonucleases (REases) that recognizes and creates a double strand break within the gapped palindromic sequence CAC↑NNN↓GTG of double-stranded DNA (↑ indicates nicking on the bottom strand; ↓ indicates nicking on the top strand). However, wild type DraIII shows significant star activity. In this study, it was found that the prominent star site is CAT↑GTT↓GTG, consisting of a star 5' half (CAT) and a canonical 3' half (GTG). DraIII nicks the 3' canonical half site at a faster rate than the 5' star half site, in contrast to the similar rate with the canonical full site. The crystal structure of the DraIII protein was solved. It indicated, as supported by mutagenesis, that DraIII possesses a ββα-metal HNH active site. The structure revealed extensive intra-molecular interactions between the N-terminal domain and the C-terminal domain containing the HNH active site. Disruptions of these interactions through site-directed mutagenesis drastically increased cleavage fidelity. The understanding of fidelity mechanisms will enable generation of high fidelity REases.

RESUMEN

[Objective] To explore the clinical effect of Buyang Huangwu Decoction and Guipi Decoction together on BTS. [Method] Select 48 cases of BTS in accordance of diagnosis standard, divide them into treatment group 48 cases(take revised Buyang Huangwu Decoction and Guipi Decoction ) and control one 20(routine western medicine), judge their cure effect after 2~4 courses. [Result] The cure rate and total effective rate were respectively 60.42%and 87.50% for treament group, for control one, they were respectively 30.00% and 65.00%; with χ2 test, their differences were respectively of marked statistical meaning and statistical meaning. [Conclusion] Buyang Huangwu Decoction and Guipi Decoction together had definite cure effect on BTS.

Gene knockout by ZFNs (zinc-finger nucleases) is efficient and specific, and successfully applied in more than 10 organisms. Currently, it is unclear whether this technology can be used for knocking-out enhanced green fluorescent protein (EGFP) gene in transgenic goats. Here we constructed and used ZFN-coding plasmids to produce genetic knockouts in the cells of cloned fetus produced from donor cells by microinjection of EGFP gene. Following introduced plasmids into caprine primary cultured fetus fibroblasts by electroporation, targeting of a transgene resulted in sequence mutation. Using the flow cytometric analysis, we confirmed the disappearance of EGFP expression in treated cells. Sequence from PCR products corresponding to targeted site showed that insertion of a G into the exon of EGFP resulted in frame shift mutation. These results suggest that ZFN-mediated gene targeting can apply to caprine fetus fibroblasts, which may open a unique avenue toward the creation of gene knockout goats combining with somatic cell nuclear transfer.

Objective To evaluate the morbidity trend of prostate cancer since the clinical usage of PSA was introduced in Beijing Hospital.Methods Retrospectively we analyzed prostate cancer cases diagnosed in Beijing Hospital from 1995 to 2008.The incidence, age, PSA and clinical stage at diagnosis were taken into account.Results Four hundred and thirty-two cases were enrolled into the study.Who were aged 40 - 90 years old, mean age 72.0 ± 7.8 years.The most frequent age at diagnosis was 70 to 79 years.The incidence increased annually with the most significant increase taking place in 2007 and 2008.Compared with the period 1995 to 1999, the localized prostate cancer rate between 2004 and 2008 increased from 23.9% to 36.3%; the metastatic prostate cancer rate decreased from 49.3% to 32.1%; the rate of patients with PSA 4 - 10 ng,/ml increased from 12.7% to 29.2%; the rate of PSA > 100 ng/ml decreased from 22.5% to 13.2%.Conclusions The incidence and early detection rate of prostate cancer in Beijing Hospital increased from 1995 to 2008.The age at diagnosis had not significantly changed.However, the early detection rate should be improved.

Objective To study the clinical significance of Raynaud phenomenon (RP) in systemic lupus erythematosus(SLE).Methods One hundred and twelve cases with SLE were divided into two groups according to the condition of RP and were analyzed retrospectively.The similarities and differences were investigated in system injury and laboratory examination in two groups.Results The occurrence rates of pulmonary arterial hypertension (PAH),interstitial lung disease (ILD),pericarditis,fatigue,pulmonary embolism (PE) and deep venous thrombosis (DVT) in patients with RP were higher than those in patients without RP [89.3%(25/28) vs.2.4%(2/84),57.1%(16/28) vs.16.7%(14/84),75.0%(21/28) vs.39.3% (33/84),67.9%(19/28) vs.33.3%(28/84),21.4%(6/28) vs.4.8%(4/84),14.3%(14/28) vs.1.2%(1/84) ](P < 0.05 ).The positive rates of anti-cardiolipin (ACL) antibody,anti-RNP antibody,anti-Sm antibody and LA in patients with RP were higher than those in patients without RP [46.4%(13/28) vs.9.5%(8/84),75.0%(21/28) vs.33.3%(28/84),57.1%(16/28) vs.23.8%(20/84),17.9%(5/28) vs.2.4%(2/84)] (P <0.05),while leucopenia in patients with RP was lower than that in patients without RP [7.1%(2/28) vs.45.2% (38/84)] (P =0.001).Conclusion Heart,lung and vascular lesions such as PAH,ILD,pericarditis,PE and so on are easier to occur in SLE with RP,furthermore the positive rates of ACL,anti-RNP antibody,anti-Sm antibody,and LA are higher than those without RP.

Objective To use enzyme-linked immunosorbent assay (ELISA) for measuring thrombospondin-1 (TSP-1),and to analyze its diagnostic value for prostatic carcinoma.Methods The possible difficulties and the way to solve the difficulties with ELISA spot were explored first.Three agents which could segregate idio-antigen and one technique which could depurate proteinum were designed and developed.The non- idio- proteinum cross reaction problems were solved and the routine method to measure TSP-1 with ELISA was set up successfully.The serum TSP-1 was measured in 14 patients with benign prostatic hyperplasia (BPH) and 18 patients with prostatic carcinoma.Results The TSP-1 values were (73.77±12.72)% and (121.86±-19.47)% in prostatic carcinoma group and benign prostatic hyperplasia group,respectively (t= 8.44,P<0.01).The diagnostic sensitivity and specificity of TSP-1 and prostate specific antigen (PSA) for prostatic cancer were 92.7%,88.9% and 85.7%,66.7%,respectively (P<0.01).The area under the receiver operating characteristic curve (ROC) of TSP-1 and PSA were 0.9663 and 0.7421 (P<0.05).Conclusions The determination of TSP-1 with ELISA is feasible.TSP-1 is an ideal diagnostic parameter for prostatic carcinoma and it may distinguish BPH from malignant prostatic disease more exactly than PSA.

BACKGROUND: A series of basic researches have confirmed that,the olfactory ensheathing cell transplantation can promote spinal cord regeneration and recover some neurological functions of spinal cord in animal models of spinal cord injury.Some clinical trials also prove that transplantation of olfactory ensheathing cells can indeed improve neurological function in patients with spinal cord injury,and then improve their quality of life.OBJECTIVE: To verify the effectiveness and safety of olfactory ensheathing cell transplantation in repair of neurological function of spinal cord injury patients.METHODS: The aborted embryonic olfactory bulb was collected and digested into single olfactory ensheathing cells.After they were cultured and purified 2 weeks,olfactory ensheathing cell suspension was prepared.A total of 213 cases of spinal cord injury were selected.Under general anesthesia,the prepared olfactory ensheathing cell suspension was injected through several target sites surrounding the injured spinal cord.ASIA scale was used to assay the patients before transplantation,3 weeks to 2 months after transplantation,so as to evaluate spinal cord recovery.RESULTS AND CONCLUSION: The spinal cord nerve function in all patients altered to different degrees at 3 weeks postoperation.Spinal cord function score,the sensory and motor functions were significantly increased compared with preoperation(P < 0.001),and showed a trend of continuous improvement with time; the patients were visited as follow-up for no more than 5 years,and no impairment of the restored nervous function or transplant adverse reactions were observed.It is confirmed that olfactory ensheathing cell transplantation can promote the recovery of nerve function in patients with spinal cord injury,it can restore and improve some spinal cord functions,and the treatment is safe.

Objective To explore the prospects for applying a brain-computer interface system in the reha-bilitation of dyskinesia by testing the brain-electricity signals of spinal cord injury patients and stroke patients during motor imagery. Methods The brain-electricity signals of 5 healthy people, 4 spinal cord injury patients and 2 stroke patients were tested during motor imagery, and the results were analyzed with a computer. Results All of the 5 healthy people showed event-related synchronization and desynchronization (ERS/ERD) in the α and β bands in the motor related cortex. Three of the 4 spinal cord injury patents showed similar characteristics, and 1 showed ob-vious and stable characteristics, but the 2 stroke patients did not show meaningful characteristics. Conclusions Spinal cord injury patients have brain-electrlcity characteristics similar to those of healthy people when they are expe-riencing motor imagery, whilst in the affected hemisphere of stroke patients the brain-electricity characteristics are not similar to those of healthy people.

Objective To explore the classification and risk factors of nocturia in patients with benign prostatic hyperplasia (BPH). Methods The 120 males aged 60 years old and over were enrolled who were first diagnosed as BPH or had ceased the BPH medication for at least 3 months.The patients were divided into six groups according to the nocturnal voiding episodes estimated according to the international prostate symptom score (IPSS). All 120 males were inquired about general information and medical histories. The time and volume of intake and voided urine were recorded for 72 hours. The indexes of prostate and bladder function in all cases were also examined and recorded. Results The incidence of nocturia in our study was 79.2%, the incidence of nocturnal polyuria and overactive bladder were 3.3% and 10.8%, respectively. The quality of life was affected by the duration between the time of patients' going to bed and the first time getting up at night. After excluding the cases of nocturnal polyuria and overactive bladder, statistical analysis indicated that the age, and residual urine volume of the bladder were positively associated with nocturia frequencies (NF), the nocturnal voiding volume negatively with NF, while the prostate volume and peak flow rate were not associated with NF. Drinking volume in 24-hours affected the nocturia (P<0. 01). Conclusions Nocturia is a common complaint in elderly males with BPH and significantly affects the quality of life. The age and the storing function of bladder are more important factors as compared with prostate hyperplasia itself.

RESUMEN

Taking into account the near infrared spectra(NIR) on numerous predictor variables with serious collinearity and having nonlinear quantitative relationship with the chemical compositions, a novel nonlinear partial least squares(PLS) approach, termed as Spline-PPLS, was constructed by combining the penalized partial least squares(PPLS) regression with B-splines transformation. Firstly, the observed spectral predictors were considered as discrete observations of curves of the wavelength and were nonlinearly transformed using B-spline basis functions. The choice of the degree of the polynomial pieces and of the number of knots was performed using the cross-validation strategy. Then, the PPLS algorithm was performed on the high dimensional transformed data matrix to build the calibration model by imposing a penalty term to the optimization criterion of PLS. The roughness penalty term indeed controlled the curvature of the functions and its smoothing parameter could also be obtained by the cross-validation. Finally, the proposed Spline-PPLS approach was applied to the wheat NIR diffuse reflectance spectra reconstruction and quantitative analysis. The result indicates that the Spline-PPLS approach not only can yield high accuracy reconstructing spectrum, but also improves the model prediction accuracy in the case of nonlinear relationships.

RESUMEN

Objective To study the clinic manifest-rations, mechanism, diagnosis and treatment of pulmonary arterial hypertention (PAH) in systemic lupus erythamatosus (SLE). Methods The clinic symptoms, laboratory tests. Doppler tests of patients who were diagnosed with PAH and SLE during last five years were analyze retrospectively. Results Anti-RNP antibody's prevalence rate was high. The presence of pulmonary arterial perssure (PAP) was not related to SLE disease active index (SLEDAI). Those who had PAP higher than 45 mm Hg were prone to have Raynaud's phenomenon PAH had no association to auto-antibodies and other systemic lesions. There were no differences in clinic features, auto-antibodies and other examinations between those with kidney injure and without kidney injure. Conclusion PAH may develop insidiously in SLE, so early diagnosis and prompt treatment of PAH can improve the prognosis.
