Hematochezia due to recurrent bleeding from rectal Dieulafoy can be challenging for endotherapy. We report the first case of successful endoscopic ultrasound-guided coil embolization of a feeding arteriole to the peripheral Dieulafoy vessels in the region of a colorectal anastomosis. The patient presented with recurrent lower gastrointestinal hemorrhage despite multiple previous conventional endoscopic treatment modalities.

We report the first case of lumen apposing metal stent (LAMS) use in malignant stricture of the duodenal apex to treat gastric outlet obstruction (GOO) and to facilitate endoscopic retrograde management of synchronous malignant biliary obstruction due to pancreatic head adenocarcinoma. This technique may be an effective alternative and a simpler approach than current methodology to this endoscopically challenging condition.

We submit the first case report of a successful EUS-guided transmural LAMS placement to drain the small bowel directly into the transverse colon in a high surgical risk patient with obstruction due to severe anastomotic stricture. We describe our technique for endoscopic colo-enterostomy and are hopeful that this will become a viable option to manage non-operable high-grade distal small bowel obstruction.
