RESUMEN

Comparative analysis of the treatment results in 120 patients, suffering destructive form of biliary calculous disease (DFBCD) was performed. Depending on operative access and postoperative conservative therapy applied the patients were divided into two groups. In the main group in 58 patients, suffering DFBCD, a minilaparotomy access was applied, using "mini-assistant" apparatus named after M. I. Prudkov and postoperatively--low-intensive laser irradiation and regional lymphotropic therapy. In a comparison group in 62 patients various laparotomic accesses were used, and a standard postoperative therapy was performed. Normalization of clinical, laboratory and immune indices in the main group were observed in twice earlier than in a control one, and economical together with moral-psychological effect was measured in reduction of the medicines quantity used as well as the patients stationary stay.

BACKGROUND: The arrival of 610,000 new immigrants to Israel from the former Soviet republics accounted for 58% of the population growth in the early 1990's. OBJECTIVE: To compare the coronary angiographic findings and risk factors between the new immigrants and local Jewish and Arab patients in this era of cost containment. METHODS AND RESULTS: A total of 550 consecutive patients--314 Jews, 95 new immigrants and 141 Arabs--were catheterized and analyzed during a 5 month period in 1995. Of this group 403 were males (73%). The mean age was 63.6 +/- 10.2 years among new immigrants, 62.4 +/- 9.4 among Jews, and 55.1 +/- 10.9 among Arabs (P < 0.05). Immigrants, including those under age 60, had the highest prevalence of multivessel disease (88.7%). Arabs had a high prevalence of single vessel disease (34.6%) and a low prevalence of multivessel (65.4%) and left main coronary disease (5.6%). Age, gender, risk factors and ethnic origin in descending order were determinants of the extent of coronary angiographic disease as revealed by multiple regression analysis. CONCLUSION: New immigrants had the most extensive angiographic coronary involvement, while Arab patients were younger and had less severe coronary artery disease. More intensive risk factor modification may have a major impact on disease progression particularly in the new immigrant subgroup.

PIP: Turkmenistan has a population of 4,361,300. Its composition by nationality is 72% Turmen; 9.5% Russian; 9% Uzbek; 2.5% Kazakh; and 7% other. The majority of the population is Muslim. The annual rate of increase in population is 3%. The birth rate is 33.1 per 1000, and the natural growth rate is 25.2 per 1000. The maternal mortality rate is 105.3 per 100,000 live births. The main cause of maternal mortality is obstetrical hemorrhage, which accounts for one-third of deaths. The perinatal mortality rate is 23.8 per 1000 live births (stillbirths, 14.5; neonates, 9.3). Preterm infants account for 42.2% of perinatal mortality. A family planning service has existed in the country since 1989. The IUD is the most popular contraceptive method; 41,666 coils were inserted in 1993. Hormonal contraception was received by 0.2% (1610 women). In the past five years, the rate of abortion per 1000 women of childbearing age has declined by 27.2%. The widespread revival of the traditional practice of exclusive breast feeding has aided family planning; a level of up to 98% natural contraceptive protection can be achieved as a result of lactation amenorrhea until the infant is six months old, and up to 90-92% until the infant is 12 months old.^ieng

RESUMEN

Selective proximal vagotomy (SPV) was performed in 435 patients whose age ranged from 16 to 80. The isolated variant of SPV was applied in 130 and SPV with a stomach draining operation was conducted in 305 patients; 320 patients were examined after the operation in follow-up periods of 7 years and more. Postoperative lethality was 0.6%. A recurrent ulcer was revealed in 4.3% of patients. The causes of the recurrence are indicated. The results were found to be excellent and good in 76%, satisfactory in 16.9%, and poor in 7.1% of the examined patients.
