RESUMEN

Erythrocytic morphological changes were studied in 48 patients with coronary heart disease (CHD) at the stages of coronary artery bypass grafting, in whom intraoperative continuous apparatus (CATS, Fresenius, Germany) reinfusion of lost autoblood was used. The patients with CHD were found to have a significant erythrocytic morphological transformation with the oxygen status compensated by the preserved nonpathological forms of erythrocytes. Intraoperatively stable tissue oxygen delivery and rapid blood loss compensation during coronary artery bypass grafting could be provided by continuous reinfusion of lost autoblood that contained valuable erythrocytes after apparatus elimination of disrupted and degenerative forms.

RESUMEN

The article presents an analysis of results of different operations disuniting the gastroesophageal collector in patients with portal hypertension. Unsatisfactory long-term results of the Pazior operation widely used in CIS are known although this operation meets the requirements of emergency surgery aimed at saving the patient's life. The authors have developed an original method of disuniting the gastroesophageal collector. The method was tested in planned and emergency operations. An analysis of the preliminary results shows good perspectives of this direction.

RESUMEN

The priority of the surgical correction of portal hypertension in patients with liver cirrhosis belongs to proximal spleno-renal anastomosis. The investigations performed have proved that preliminary embolization of the splenic artery with a metallic spiral should be carried out before placing this anastomosis. It has positive effects on the course of the nearest postoperative period. In addition, the preliminary embolization of the splenic artery may be used as a test of endurability of great cavitary operation on the vessels.

RESUMEN

The paper presents the results of repeated investigations in the tick relapsing fever nidus (TRF) conducted after a ten-year interval in Gava settlement situated in the foothill area of the Namangan Province of Uzbekistan. A considerable deterioration in the habitat conditions of the vector, Ornithodoros papillipes Bir., due to the intensified anthropogenic effect, and decrease in its number have been noted. The distribution range of the tick (percent of infested farmsteads) in this case did not reduce. The infection rate of the vector's population with the tick relapsing fever agent decreased that points indirectly to a higher mortality of infected ticks under unfavourable conditions. Wavy course of morbidity with TRF during the period between investigations shows that changes that took place in the nidus were not uniform in their character and may have resulted to some extent from natural cycles. A similar character of anthropogenic effect on the populations of ticks in the settlement natural nidi suggests that the above changes are common to the nidi of this infection.

RESUMEN

The analysis of postoperative complications, which developed in 33 patients after peritoneoatrial shunting (PAS), was carried out. The effectiveness of PAS is determined by proper choice of the indications, adequate performance of surgical intervention. The use of intraoperative teleroentgenoscopy for control of the venous catheter situation is recommended. Reoperations for postoperative complications are to be performed in presence of a pressure gradient in the valvular system and in patients with shunt infection after elimination of the inflammatory process.

RESUMEN

Analysis of the abundance of the tick O. papillipes in the Namangan and Andizhan districts was conducted on vast material. According to the tick abundance (from very high to very low) settlements were arranged into five groups. Most common are settlements with a high and average tick abundance. In most cases the agent of TRF was isolated from the ticks. Natural and anthropogenic factors determining the abundance rate of ticks in some settlements and possibilities of their practical use for the quick annihilation of TRF nidi are discussed.

RESUMEN

The contemporary state of settlement nidi of tick-borne relapsing fever is discussed on the material collected in the Namanganskaya and Andizhanskaya regions of Uzbek SSR. Despite the control measures and rise of the sanitary culture of population the conditions are still preserved for the maintenance of nidi. As in ancient estates, classical nidi of tick-borne relapsing fever, so in modern ones there are preserved conditions for existence of flourishing populations of vectors, i.e. favourable habitats and the availability of hosts. The analysis of factors is given affecting the contemporary level of sick-rate with tick-borne relapsing fever. Within the limits of the distribution area of the vector a higher sick-rate should be expected than it is registered.
