RESUMEN

The objective was to determine the postoperative hypersensitivity of two-step Total-etch as compared to one-step Universal adhesives followed by composite restorations on 100 patients by applying Total-etch on one tooth and Universal adhesive on another tooth. The bonds and teeth were randomly selected. Postoperative hypersensitivity was recorded by visual analog scale before, immediately after, and 24 h after the restoration using cold stimulus. The Mann-Whitney test was applied for statistical comparison of postoperative hypersensitivity between the two bonds as well as for any significant difference in genders with each bond. No significant difference was found between postoperative hypersensitivity of the two adhesives before (p-value = 0.57), immediately after (p-value = 0.604), and 24 h after (p-value = 0.728) the restoration. Males showed more hypersensitivity with Total-etch as compared to females before (p-value = 0.037), immediately after (p-value = 0.047), and 24 h after the restoration (p-value = 0.022). No significant difference was found between gender and Universal adhesive at all three stages (p-value > 0.05). The results suggest no significant difference in postoperative hypersensitivity between the two materials when good sample size and proper technique were observed along with the removal of bias like different patients having different pain perceptions and multiple operators having different operating skills. Objective: To determine the association of diet and dietary practices with dental caries among adults. Design: A case-control study. Setting: Operative Department, Rawal Institute of Health Sciences, Islamabad, Pakistan. Participants: 300 participants of both genders, aged 25-50 years. Interventions: A food frequency questionnaire and a patient proforma were used to determine the frequency and preferences of diet and dietary habits that may be associated with dental caries among adults, respectively. The diet and dietary habits of 150 adults with caries (cases) were compared with those of 150 adults without dental caries (control). An independent sample T-test was applied to determine the difference in mean age. Mann-Whitney and Chi-Square tests were applied to determine the significance of diet and dietary habits respectively. Multivariate logistic regression analysis determined the odd ratio change in significant variables. P-value ≤0.05 was considered significant. Results: Refined sugar (p-value = 0.69), fruit juices (p-value = 0.45), carbonated beverages (p-value = 0.91), duration of consumption of sugary food (p-value = 0.07), and frequency of brushing (p-value = 0.15) were not found to be significantly associated with dental caries in adults. The gender (p-value = 0.02), preferred time for eating sugary foods (p-value <0.001), smoking (p-value <0.001), and tea consumption (p-value = 0.02) were found to be significantly associated with dental caries. Conclusion: Adults who regularly consumed sugar as a snack other than regular mealtimes were more likely to be associated with dental caries. Men, smokers, and adults who frequently took shots of sugar with their tea were more likely to be associated with dental caries.

OBJECTIVE: To compare postoperative sensitivity following composite restoration placed in supra gingival class-V cavities using self etch adhesive and total etch adhesive. STUDY DESIGN: A randomized clinical trial. PLACE AND DURATION OF STUDY: Operative Dentistry Department of Armed Forces Institute of Dentistry, Rawalpindi, from July to December 2009. METHODOLOGY: A total of 70 patients having class-V supra gingival carious lesions were divided into two groups. Classes-V cavities not exceeding 3 mm were prepared. One treatment group was treated with self etch adhesive (adhe SE one Ivoclar) and the control group was treated with total-etch adhesive (Eco-Etch Ivoclar) after acid etching with 37% phosphoric acid. Light cured composite (Te-Econom Ivoclar) restoration was placed for both groups and evaluated for postoperative sensitivity immediately after restoration, after 24 hours and after one week. Data was recorded on visual analogue scale. RESULTS: Comparison of sensitivity between the two treatment groups on application cold stimulus after 24 hours of restoration showed significant difference; however, no statistically significant difference was observed at baseline, immediately after restoration and at 1 week follow-up with cold stimulus or compressed air application. CONCLUSION: Less postoperative sensitivity was observed at postoperative 24 hours assessment in restoration placed using SE adhesives compared to TE adhesives. Thus, the use of SE adhesives may be helpful in reducing postoperative sensitivity during 24 hours after restoration placement.

RESUMEN

Teeth once designated obsolete for rehabilitation and deemed for extraction should be re-evaluated for salvaging two thirds or even one-third component of the individual teeth. In our part of the continent, financial restraints can effect the orthodox procedures of root amputation if the final restoration is not provided at an earlier date. This case report enlightens on a biphasic approach combining radisection and coronal resection of a tooth with an endodontic perforation, which is a procedure not normally executed for mandibular molars but was adopted after taking the socioeconomic status of the patient into consideration.

RESUMEN

Odontogenic cutaneous sinus tract is a rare but well documented condition. It is usually misdiagnosed as a local skin lesion and maltreated by systemic antibiotics and/or surgical excision with eventual recurrence. This is because the primary etiology is incorrectly determined. We came across a 38 year old patient who presented with a cutaneous lesion on her left cheek with frequent purulent discharge and was not responding to systemic antibiotics. The case history, diagnosis and management of this condition is presented here.

OBJECTIVE: To compare the efficacy of camphorated paramonochlorophenol to a mixture of honey and mustard oil as a root canal medicament. DESIGN: An in vitro comparative study. PLACE AND DURATION OF STUDY: This study was carried out in collaboration with microbiology department of Armed Forces Institute of Pathology, Rawalpindi and was conducted from Aug-2001 to Nov-2001. MATERIALS AND METHODS: A sample of 90 infected root canal contents of decayed teeth was collected from Operative Dentistry Department of Armed Forces Institute of Dentistry, Rawalpindi. Each sample was inoculated aerobically as well as an-aerobically. Organisms were identified and isolates were preserved and refrigerated for experiments. Medicaments were procured and prepared in comparable dilutions. Minimum inhibitory concentration of both medicaments was compared by susceptibility testing against already preserved isolates. RESULTS: The tested mixture (honey and mustard oil equal v/v ) demonstrated antibacterial activity against all isolates at concentration of 12.5%(mean of effective conc.17.11%) while that of camphorated paramonochlorophenol was comparably at higher concentration 3.1%(mean of effective conc. 3.35%). However the mixture revealed better results than that of individual agents, which was 12.5% to 25% respectively. CONCLUSION: Mixture (honey and mustard oil) was effective, viable alternate endodontic medicament.

OBJECTIVE: To calculate the frequency of dens evaginatus in permanent teeth. DESIGN: A descriptive study. PLACE AND DURATION OF STUDY: The study was conducted at Armed Forces Institute of Dentistry (AFID), Rawalpindi from April 2002 to April 2003. SUBJECTS AND METHODS: A clinical and radiographic analysis of 5400 permanent anterior teeth was conducted to determine the occurrence of this developmental malformation. RESULTS: Dens evaginatus was found in about 2.4% of the permanent anterior dentition and was more frequent in the maxilla, particularly in the lateral incisor. This anomaly was less frequent in the mandible. CONCLUSION: This study shows that dens evaginatus of anterior teeth is more common in the permanent dentition particularly, the maxillary incisors. Furthermore, the frequency of dens evaginatus is high in males than females.

RESUMEN

OBJECTIVE: To determine the success rate in autotransplantation of endodontically treated third molars. DESIGN: A descriptive study. PLACE AND DURATION OF STUDY: This study was carried out at Armed Forces Institute of Dentistry (AFID), Rawalpindi (Pakistan) from January 2002 to December 2002. PATIENTS AND METHODS: A total of 50 patients meeting the inclusion criteria were selected who had their first or second molars in unrestorable condition with intact third molars. The donor teeth were extracted after the preparation of recipient site. After endodontically treated in vitro the donor teeth were carried to the recipient site and immobilized. Postoperative variables were recorded and analyzed on SPSS version 10. RESULTS: The overall success rate after six months of the transplantation of third molars was 88% with complete root formation after endodontic treatment. All the patients(12%) who had complaints were more than 35 years of age. CONCLUSION: Third molars are good substitute for the unrestorable first or second molars and would be as effective as endosseous implants. The procedure is likely to have complications in the advanced age group.
