RESUMEN

A three-year-old Labrador retriever was referred for decreased appetite, a painful swelling in the region of the maxillary right fourth premolar, and a heart murmur indicative of patent ductus arteriosus (PDA) diagnosed 1.5-weeks prior to presentation. Oral examination and intraoral dental radiographs showed impaction of the maxillary right fourth premolar surrounded by reactive alveolar bone. Necrotic bone, remnants of the deciduous maxillary right fourth premolar, and the impacted permanent maxillary right fourth premolar were removed following surgical exploration of the area. The PDA was repaired without complication 2-weeks following oral surgery. The extraction site was healing appropriately, and the dog's inappetence and painful facial swelling had resolved.

RESUMEN

A technique was developed for closure of gastrotomy and enterotomy incisions using disposable skin staples. The technique was used successfully in three dogs with gastrointestinal linear foreign bodies that required a gastrotomy and one or more enterotomies. The method allows for secure closure of gastrointestinal incisions and minimizes the surgical time in patients requiring multiple gastrointestinal incisions.

RESUMEN

A two-year old neuteral/male cougar (Felis concolor) was presented because of abnormal eating habits and an irregularity of the mandibular right first molar that was noted by the caretaker. Oral examination and dental radiographs showed a lesion consistent with odontoclastic resorption of the mandibular right first molar, and a crown fracture and dilacerated root of the maxillary right first incisor. Exodontic therapy was performed on both teeth. The caretaker reported no problems associated with the patient's oral cavity 10-months following treatment.

RESUMEN

The surface roughness of tooth enamel was quantitatively evaluated following scaling with four power scalers at three different tip forces. Ceramic and tooth samples were evaluated for surface roughness using surface profilometry. Applied tip forces were produced with a horizontal balanced arm holding the scaler handpiece and load weight, and were measured using a load cell. The power scalers evaluated were the ultrasonic piezoelectric, ultrasonic magnetostrictive (ferromagnetic stack), sonic, and rotosonic. For the tooth samples, at 50 grams of force, the piezoelectric, magnetostrictive, and sonic scaler roughnesses were similar and the rotosonic scaler outcome was statistically higher. Increasing the applied force significantly increased the roughness produced by the rotosonic and piezoelectric scalers. The results of this study indicate that the type of power scaler and applied tip force used determines the post-scaling surface roughness of ceramic and tooth enamel surfaces.

Dental health was evaluated in two populations of raccoons (Procyon lotor) in western Illinois (USA); one was from a rural agricultural area with low human density and the other from a nearby state park heavily used by humans and raccoons. From 1989 through 1993, 300 raccoons were live-trapped in the agricultural area and 246 raccoons were live-trapped in the park. Oral health was assessed using gingival and calculi indices and by measuring loss of attachment and tooth wear. Raccoons from the park were significantly older and smaller, but not thinner, than raccoons from the farmed area. Gingival and periodontal indices, tooth wear, tooth loss, and caries increased significantly from juveniles to yearlings to adults, at both sites. Males had higher levels of gingivitis and loss of periodontal attachment than females, but were similar on other dental measures. There were no seasonal differences between raccoons in dental indices. Animals with high scores for one oral measure tended to have high values for all indices. Dental health was generally good for juveniles and yearlings from both sites. Among adults, periodontal indices and the prevalence of caries were significantly higher in the park, but prevalence of broken or missing teeth was similar for both populations. There was no association between body condition, and a higher dental score or more missing or broken teeth.

Partial extramural obstruction of the descending colon was diagnosed in two dogs and a cat as a complication of elective ovariohysterectomy. In each case, the obstruction was caused by fibrous tissue that encircled or crossed the descending colon, severely restricting the organ's normal mobility and luminal diameter. Clinical signs secondary to obstipation were observed in two cases, five weeks and 27 months after elective ovariohysterectomy. In one dog without clinical signs, the adhesion was an incidental finding during a laparotomy performed nine years after the ovariohysterectomy. The fibrous adhesions were removed surgically in all three cases without additional complications.

RESUMEN

A 2 year-old, spayed female Shetland sheepdog presented with a fractured maxillary right fourth premolar tooth during oral examination for a dental prophylaxis. Seven months after conventional root canal therapy was performed, the tooth was prepared for non-precious metal, full crown restoration, which was cemented in place 1 month after crown margin preparation. During re-examination for an unrelated problem 6 months later, the metal crown was in place and there was no evidence of further fracture of the tooth.

The purpose of this study was to determine the effects of a one-minute chlorhexidine gluconate skin preparation protocol prior to cephalic vein catheterization. Twenty-three healthy beagle dogs had one leg aseptically prepared and the opposite leg served as a control. Twenty-six- and 77-hour time groups were studied. Chlorhexidine-treated legs had significantly lower cutaneous bacterial counts than the control legs prior to catheter insertion and prior to catheter withdrawal for both time groups. Control legs developed significantly more dermatitis than the treated legs after 77 h. A one-minute preparation with 4% chlorhexidine gluconate was an effective method for sustained reduction of cutaneous bacterial counts at peripheral intravenous catheter insertion points in dogs. Increased cutaneous bacterial counts were associated with significantly more microscopic dermatitis in untreated legs after 77 h of catheterization.

A 20-month-old, intact male shih tzu was evaluated for chronic upper airway disease. Endoscopic examination established a diagnosis of choanal atresia, a developmental anomaly of the posterior nasal cavity. Although surgical intervention provided temporary relief, stenosis of the nasopharynx by obstructive scar tissue was confirmed within four weeks. A permanent tracheostomy provided long-term relief of the respiratory difficulty. This is the first report of choanal atresia or nasopharyngeal stenosis in a dog.

Maxillofacial surgery in dogs includes the management of jaw fractures various palatal surgical procedures, the management of oral tumors, and the surgical management of miscellaneous maxillofacial problems. Miscellaneous maxillofacial problems that may require surgical intervention include dentigenous cysts, bone sequestra or osteomyelitis, and an inability to open or close the month. Careful perioperative planning of maxillofacial surgical procedures will help ensure a successful outcome.

Ophthalmic manifestations of dental disease may occur in dogs and cats because of the proximity between posterior maxillary teeth and the orbit. Ophthalmic disorders may be diagnosed promptly, but the initiating dental disease may be overlooked. Inappropriate diagnosis and treatment of dental disease may result in loss of teeth, irreversible vision-threatening ophthalmic disease, or, ultimately, loss of the globe. When ophthalmic examination results are suggestive of a primary dental disorder, thorough examination of the maxillary teeth is imperative to diagnose appropriately the underlying dental disease. Therapy should include treatment of the dental disease and secondary ophthalmic disorders.

Seven different suture materials were implanted into the oral tissues of 12 cats. The sutures and surrounding tissues were retrieved en bloc from each of two cats on days one, three, seven, 14, 21, and 28 postimplantation. Tissue reaction and suture duration were evaluated by gross visual observation and histological examination. Chromic gut disappeared between days three and seven; polyglactin 910 disappeared between days 14 and 21; and polyglycolic acid disappeared as early as seven to 14 days. Polydioxanone still was intact at day 28 and is recommended as an absorbable material for procedures in which longer healing time is anticipated. All the nonabsorbable suture materials (i.e., polypropylene, stainless steel, and nylon) were intact at day 28 postimplantation. Visual inspection showed polypropylene to have the least tissue reaction.

Microscopic evaluation of the margins of excised cutaneous neoplasms is of paramount importance for determining that neoplastic tissue does not extend to the excision borders. Dyes or pigments that indelibly mark the tissue should be more reliable than sectioning techniques or suture markers for identifying the surgical margins before and after tissue processing. We evaluated 5 pigments to select a material that could be rapidly applied by surgeons, and readily identified on histologic section by the pathologists. Twenty normal canine skin specimens were assigned to each of 5 groups. Each group was treated with artists' pigments in acetone, India ink in acetone, alcian blue, typists' correction fluid, or a commercially available marking kit. Ten specimens within each group were marked before formalin fixation, and 10 were marked after fixation. Application properties, fixation and processing properties, and microscopic characteristics were evaluated for each material. Application properties were acceptable for all marking materials on unfixed specimens, and for alcian blue, India ink in acetone, and correction fluid on fixed specimens. Fixation and processing properties were acceptable for all materials except correction fluid. All marking materials survived fixation and processing, and colors were readily visualized under the microscope. Microscopic characteristics were acceptable for alcian blue, India ink in acetone, and the commercial kit. Overall, alcian blue was the best marking material, with India ink in acetone and the commercial kit also acceptable. Correction fluid and artist' pigments in acetone were not acceptable because pigment fragmentation and incomplete tissue coverage hindered microscopic evaluation of resection margins.

A force transducer was developed to measure bite force in dogs. A total of 101 readings was obtained from 22 pet dogs ranging in size from 7 to 55 kg. Bite forces ranged from 13 to 1394 Newtons with a mean for all dogs of 256 Newtons and a median of 163 Newtons. Most measurements fell within the low end of the range, with 55% of the biting episodes less than 200 Newtons and 77% less than 400 Newtons.

A dolicocephalic canine skull was used to develop a teaching and study model for surgical endodontic access sites of the maxillary and mandibular canine teeth, the maxillary fourth premolar and the mandibular first molar teeth. Coronal endodontic access sites were created, and endodontic files were inserted through the access preparation into the root canal system. The teeth were radiographed to determine the ideal surgical access points. Barium markers were placed over these points and each tooth was radiographed again to confirm the appropriate location of the marker. Surgical accesses to the apices were created by drilling through the barium markers and underlying bone. Following localization of the apices, apicoectomies were performed, exposing the apical portion of the pulp canal. The maxillary and mandibular canine teeth, the maxillary fourth premolar tooth, and the mandibular first molar tooth and surrounding bone were harvested from a similar dolicocephalic dog following euthanasia. The specimens were embedded in methylmethacrylate, sectioned, and mounted on slides to reveal the structures located around the surgical access sites.

Twenty-one mandibular fractures in 11 cats and 6 dogs were repaired during a 20-month period. A new technique using dental composite was used to stabilize the mandible. The canine teeth were pumiced, acid etched, and aligned with dental composite, leaving the mouth opened approximately 1 cm. Six weeks after surgery the composite was removed so that radiographs of the mandible could be made with the animal under general anesthesia. In 1 dog in which the fracture had not healed, the composite was replaced. The composite broke before 6 weeks in 8 animals; 2 required replacement of the dental composite. There were no other complications. The median time for fracture healing was 6 weeks. All fractures healed with anatomic dental occlusion. The advantages of this technique are that no further damage is caused to the teeth or to the blood supply of the bone, the occlusion is anatomic, dermatitis (which is seen with tape muzzles) is not a complication, and the technique is easy to use in brachycephalic breeds, cats, and animals with poor bone quality. The limiting factor of this technique is that it does require four salvageable canine teeth. This repair technique is still a viable option even if one or more of the canine teeth are fractured. Dental composite stabilization is fast, easy, inexpensive, and, in our series, it was 100% effective for the repair of mandibular fractures.

A dolichocephalic canine skull was utilized to develop a teaching and study model for coronal access cavity preparation of the dentition. The teeth from the right side of the skull were extracted. Each tooth was radiographed in two planes to localize the "ideal" endodontic access point. Barium markers were placed over these points and each tooth was radiographed again to confirm the appropriate location of the marker. Access was created by drilling through the barium marker. Then, K-files were inserted through the access preparation into the root canal system. The teeth were radiographed with the K-files in place to verify straight pathways to the apices. Straight line access to the apex was obtained in all teeth. Creation of this model provides an opportunity for the veterinary dental student to develop and refine skills used in performing endodontic therapy. The model can be utilized by the veterinary dental practitioner in clinical practice.

Whenever an animal is examined because of chronic rhinitis, the dentition should be carefully examined to rule out the possibility of a primary dental problem. Oral examination under heavy sedation or general anesthesia is necessary to diagnose many of these cases. A periodontal probe is used in detecting deep periodontal pockets that extend into the nasal cavity or maxillary sinus. A dental explorer is used to reveal pulpal exposure secondary to dental fractures or rapid dental wear or attrition. Careful oral examination is used to localize any oronasal or oroantral fistula from a previously lost tooth. Dental radiographs help to reveal the extent of alveolar bone pathology. An assessment of all these findings will result in an accurate diagnosis of the primary cause of the chronic rhinitis, which will in turn facilitate appropriate treatment.

Partial mandibulectomy was performed for the treatment of benign or malignant oral tumors in 142 dogs. Forty-two dogs with a benign tumor (ameloblastoma) had a 22.5 month (range, 6 to 74 months) median disease-free interval, with a 97% 1-year survival rate; there was local recurrence in one dog. Twenty-four dogs with squamous cell carcinoma had a disease-free interval of 26 months (range, 6 to 84 months), with a 91% 1-year survival rate; recurrence and metastasis developed in two dogs and metastatic disease in one dog. Based on survival curves, 37 dogs with a melanoma had a median survival time of 9.9 months (range, 1 to 36 months), with a 21% 1-year survival rate; 20 dogs died or were euthanatized for recurrent or metastatic disease. Twenty dogs with osteosarcoma had a median survival time of 13.6 months (range, 3 to 28 months), with a 35% 1-year survival rate; nine dogs died or were euthanatized for recurrent or metastatic disease. Nineteen dogs with fibrosarcoma had median survival time of 10.6 months (range, 3 to 32 months), with a 50% 1-year survival rate; 12 dogs died or were euthanatized for recurrent or metastatic disease. Results of this and previous studies demonstrated that partial mandibulectomy was effective in prolonging survival and decreasing recurrence for squamous cell carcinoma and ameloblastoma. Progressive disease and corresponding low survival times were common in dogs with melanoma, osteosarcoma, and fibrosarcoma. There were no differences in survival times or the progression of disease among five partial hemimandibulectomy procedures. The high rates of recurrence and metastasis in dogs with these tumors suggest a need for evaluation of ancillary chemotherapy and local radiation therapy to decrease the prevalence of progressive disease.
