RESUMEN

Background: Modern dentistry has increasingly valued conservative and biologically less invasive clinical practices, seeking to preserve the patients' tissues and natural dental elements. Most extractions in the dental clinic are preceded by periodontal disease that presents bone and gingival tissue loss, compromising the aesthetics as well as the support of dental elements. Objectives: The clinical approach in these cases often involves bone exertion followed by the successful installation of osseointegrated implants. Material and Methods: In this study, a case of extensive periodontal involvement and mobility of dental elements was carried out in a minimally invasive way, using the Er:YAG laser for periodontal decontamination and the hemolasertherapy technique to regenerate adjacent tissues, totaling nine weeks of treatment. Results: Clinical and radiographical improvement of tissue health and complete preservation of dental elements were observed. Conclusion: The patient underwent a follow-up appointment 2 years after the start of treatment when it was still possible to verify the stability of the clinical condition and the tissue gains obtained.

PURPOSE: There has been growing demand for dental bleaching worldwide, however, despite being effective, hydrogen peroxide (HP) can negatively affect the dental structure. Thus, new techniques, such as violet LED light have emerged and need to be studied. The aim of this study was to analyze and compare the effectiveness of violet LED light alone or combined with 35% HP gel. METHODS: Six different tooth bleaching techniques (n = 10) were performed in intrinsically pigmented bovine teeth: G1 - 35% HP (1x/week for 4 weeks, 45 min of gel application); G2 - 35% HP (1x/week for 4 weeks, 15 min of gel application); G3 - violet LED (1x/week for 4 weeks); G4 - violet LED (2x/week for 2 weeks); G5 - violet LED (4x/week for 1 week); G6 - Violet LED + 35% HP (hybrid technique, 1x/week for 4 weeks, 15 min of gel application). Specimens were submitted to color evaluation at predetermined times using Konica Minolta® spectrophotometer, and the surface morphology (n = 3) was qualitatively analyzed by Scanning Electron Microscope (SEM). Data of the color change test were analyzed considering a 5% level of significance. RESULTS: There was significant difference in color (p <0.05) for all groups after bleaching protocols. SEM analysis revealed that the greatest change in surface occurred in Group G1, with demineralization of the dental enamel. When considering the same time interval, there were no statistical differences for axis L*, but differences were shown for axis a* (G2, G3, G4 ≥ G1, G5 ≥ G6) and b* (G1, G2, G3, G4, G5 > G6). Regarding the comparison of ΔE00 between groups, results showed statistical difference between groups, with G1 ≥ G2, G5, G6 ≥ G3, G4. CONCLUSION: Bleaching protocols with less time (15 min) or no exposure to 35% HP (violet LED, 4x/week) could promote bleaching results as those obtained by the conventional technique using 35% HP for 45 min, with no enamel surface changes, showing to be a promising alternative to tooth bleaching.

OBJECTIVE: To evaluate the impact of desensitizing toothpastes (D), their whitening versions (DW) and whitening toothpastes (W) on tooth color change. METHODS: Tooth specimens were allocated into 11 groups (n = 10): 1. Artificial saliva (control); 2. Sensodyne Repair & Protect (SRP-D); 3. Sensodyne Repair & Protect Whitening (SRP-DW); 4. Colgate Sensitive Pro-Relief (CSPR-D); 5. Colgate Sensitive Pro-Relief Real White (CSPR-DW); 6. Colgate Total 12 (CT12); 7. Colgate Total 12 Whitening (CT12-W); 8. Sensodyne True White (ST-W); 9. Curaprox Black Is White (CB-W); 10.Oral-B 3D White Perfection (OB3D-W) and 11. Erosion-only. They were submitted to two models: A. abrasion; B. erosion/abrasion. Color change (ΔE00 ), ΔL, Δa, and Δb were determined with a spectrophotometer. Data were statistically evaluated (α = 0.05). RESULTS: For both models, ∆E00 of D did not differ significantly from DW. An overall decrease in luminosity (-∆L) and increase (+∆b) in yellowness of the tooth occurred. In abrasion, all groups reduced redness (-∆a) and only SRP-DW differed from control (p = 0.016), but for ∆E00 , ∆L and ∆b, all toothpastes did not differ from the control. In erosion/abrasion, CB-W showed significantly lower ∆E00 and ∆b than the control (∆E00 : p = 0.026; ∆b: p = 0.005) and SRP-D (∆E00 : p = 0.028; ∆b: p = 0.025). Only SRP-DW, CSPR-DW, ST-W, CB-W, and erosion-only had increased redness (+∆a), but none differed from control. CONCLUSIONS: The toothpastes did neither promote whitening nor avoid the yellowish aspect of the tooth after abrasion and erosion-abrasion, regardless of their claim. CLINICAL SIGNIFICANCE: Desensitizing and/or whitening toothpastes induced similar changes on tooth color, without promoting whitening effect or reduction in its yellowish aspect.

This study aimed to investigate matrix metalloproteinase (MMP) activity in human dentin using in-situ and gelatin zymography, after at-home and in-office bleaching, related to their clinical exposure times. Dentin specimens (n = 5) were treated with 35% hydrogen peroxide (50 min per session/4 sessions), 10% carbamide peroxide (180 min/21 sessions), or no treatment. All were subjected to in-situ zymography. Dentin slices were, subsequently, obtained, covered with fluorescein-conjugated gelatin, and examined with confocal laser-scanning microscopy. The fluorescence intensity was quantified and statistically analyzed using one-way ANOVA and Bonferroni tests (α = 0.05). Furthermore, gelatin zymography was performed on protein extracts obtained from dentin powder (N = 8 teeth), treated with hydrogen peroxide or carbamide peroxide, with different exposure times (10/50 min for hydrogen peroxide; 252/1260 min for carbamide peroxide). The results of the in-situ zymography showed no statistical differences between the bleached specimens and the control group, with a medium level of gelatinolytic activity expressed in the dentin tubules. The results of gelatin zymography showed an increased expression of pro-MMP-9 in carbamide peroxide groups. The expression of pro-MMP-2 decreased in all the experimental groups. The bleaching treatments performed on the enamel of sound teeth do not influence dentinal enzymatic activity. However, when unprotected dentin tissue is bleached, matrix metalloproteinases are more expressed, particularly when carbamide peroxide is used, proportional to the exposure time.

PURPOSE: To evaluate push-out bond strength (PBS) and interfacial nanoleakage (NL) of adhesively luted fiber posts using different composite cements and polymerization protocols. MATERIAL AND METHODS: 100 premolars were endodontically treated and assigned to the following groups (n=10): RelyX Universal light-cure (3M Oral Care); RelyX Universal self-cure (3M Oral Care); Maxcem Elite Chroma light-cure (Kerr); Maxcem Elite Chroma self-cure (Kerr); Calibra Universal light-cure (Dentsply Sirona); Calibra Universal self-cure (Dentsply Sirona); Multilink Automix light cure (Ivoclar Vivadent); Multilink Automix self-cure (Ivoclar Vivadent); Luxacore Z Dual light-cure (DMG); Luxacore Z Dual self-cure (DMG). Half of the teeth from each group were subjected to the PBS test after 24 h (T0), while the other half was tested after 12 months (T12) of artificial saliva aging. An additional 4 teeth per group were prepared for NL expression. PBS values were analyzed using multivariate ANOVA and Tukey's post-hoc test. NL scores were analyzed using chi-squared tests (α = 0.05). RESULTS: Statistical analysis revealed that the variables "cement" and "aging" significantly influenced PBS (p < 0.05), but not "polymerization" and "root region" (p > 0.05). Significantly lower PBS values (p < 0.05) were detected for the Calibra Universal groups compared to other cements, while the RelyX Universal groups performed equally well (p > 0.05) or better than other cements (p < 0.05). At T12, PBS values increased in the majority of groups, irrespective of root region (p < 0.05). Differences in NL expression were present at T0, and in general, the aging process increased marginal infiltration. CONCLUSION: Aging and choice of composite cement influenced PBS, while root region and polymerization protocol seemed to have no influence on posts' resistance to dislodgment.

This clinical study evaluated the effect of bleaching performed with violet LED light (405-410 nm), either combined with hydrogen peroxide (HP) gel, or not, on color change, dental sensitivity, participants' satisfaction and impact on their quality of life. A hundred participants were divided into one of the groups (n = 25): G1 - 35% HP (4 sessions, 1x/week); G2 - violet LED (4 sessions, 1x/week); G3 - violet LED (4 sessions, 2x/week); G4 - hybrid technique (violet LED + 35% HP; 4 sessions, 1x/week). Color evaluation was performed with colorimetric tests (objective and subjective), before, 14 days and 3 months after completion. Additionally, satisfaction with treatment, impact on quality of life (OHIP-14) and dental sensitivity were recorded. The data were submitted to statistical analysis, considering a significance level of 5%, with the exception of the data from the questionnaire on satisfaction (descriptive analysis). Two-way ANOVA and Tukey tests showed that there was no difference between color variation resulting from techniques used in G1 and G4; those used in G2 and G3 did not differ and were less effective for bleaching than those of the other groups, in both subjective and objective evaluations. Regarding tooth sensitivity, subjects in G2 and G3 experienced no sensitivity, ​​while those in G4 showed lower sensitivity values than those in G1. With respect to quality of life, only subjects in G1 and G2 showed a significant positive impact. Among the evaluated techniques, the hybrid type seemed to be a good alternative, showing effective bleaching with less tooth sensitivity.

BACKGROUND: To clinically evaluate the effect of 35% hydrogen peroxide gel renewal in association with violet LED (405-410nm) through a split-mouth randomized controlled clinical trial. METHODS: The treatment consisted in 3 bleaching sessions of 15 min each, with an interval of 7 days between them, using 35% hydrogen peroxide combined to violet LED irradiation. Selected patients had two experimental segments for the split-mouth design: No change of the bleaching gel during each session (NBGR) and 3 changes of the bleaching gel every 5 min for each session (BGR). During the 3 bleaching sessions, the selected quadrant received the same treatment. Patients had their upper canines and central incisors teeth color measured with a subjective (color scale - VITA Classical) and an objective (spectrophotometer - VITA Easyshade) method and their teeth sensitivity measured using a Visual Analog Scale (VAS) before, immediately after each bleaching session, and 14 days and 2 months (60 days) after the end of the treatment. RESULTS: The protocol adopted in the present study reached satisfactory results regarding color change. No statistical difference between groups was observed immediately after the end of the treatment and in the follow-up analysis for both subjective and objective color evaluation. No difference in tooth sensitivity between segments was observed. CONCLUSION: There is no need for bleaching gel renewal when following the clinical protocol of 3 sessions of 15 min in a bleaching protocol of 35% hydrogen peroxide combined with violet LED.

A new LED wavelength, violet LED (VL) with a wavelength between 405 - 410 nm was recently introduced to be used for in-office dental bleaching. In comparison to the blue LED system (440 to 485 nm), the shorter wavelength has more energy carried in its photons and also corresponds to the absorption peak of the stained particles, which lead to whitening utilizing a physical process. Considering the need to suggest and develop new protocols with this new technology, this article reports 2 different dental bleaching protocols developed in a split-mouth model using VL. A 25-year-old male patient was submitted to in-office dental bleaching. On the teeth from the left side, the bleaching gel (35% H2O2) was renewed 3 times (every 8 mins), and on the right side, the gel was maintained without renewal during the bleaching session. The irradiation with Violet LED Light (405 nm ± 10 nm) was performed with the following protocol: 1 min of irradiation with 30 s light off until 8 min of total time. A total of 3 cycles were performed (total time of 24 min). Two bleaching sessions were performed with an interval of 7 days between sessions. Based on the results of this split-mouth case report, there was no visible difference in the final color outcome and sensitivity between both bleaching protocols tested.

OBJECTIVE: The present study investigated the ability of a chlorhexidine (CHX)-containing primer (0.2% aqueous solution) to inhibit dentinal enzymes, preserve the hybrid layer (HL) and remain within the HL, after 10 years of aging in artificial saliva at 37°C. METHODS: Non-carious extracted molars were assigned to two groups, cut into slabs exposing middle/deep dentin, etched and bonded with Adper Scotchbond 1XT (SB1XT) with or without 0.2% CHX aqueous solution pretreatment. Composite build-ups were made, and the specimens were cut in 1-mm thick bonded sticks. In situ zymography was performed on freshly prepared specimens (T0) and specimens aged for 10 years (T10-yr) at 37°C in artificial saliva, to investigate endogenous gelatinolytic activity within the HL. At T10-yr, specimens were also decalcified and embedded in epoxy resin for TEM analysis. Micro-Raman spectroscopy was performed at T0 and T10-yr to evaluate the chemical profiles in intertubular dentin and the HL. RESULTS: In situ zymography showed less pronounced enzymatic activity in the CHX-pretreated group (p<0.05) regardless of aging, maintaining a similar level of fluorescence at T0 and T10-yr (p>0.05). TEM results showed that 98% of the HL had been degraded in the control group, while 95% of the HL was intact in the experimental group. Moreover, all the Raman spectra peaks assigned to CHX could be identified only in the CHX-pretreated group (T0 and T10-yr). SIGNIFICANCE: In vitro, CHX remains in the HL after 10 years with its inhibitory effect preserved. This may be the underlying factor for HL preservation after this long aging period.

Objective: To clinically evaluate the effect of the violet light-emitted diode (LED) light (405-410 nm) associated to in-office dental bleaching with 35% carbamide peroxide, on tooth color change and sensitivity. Participant's satisfaction after treatment was also evaluated. Materials and methods: A total of 50 participants were selected and randomized into a treatment group (n = 25): G1-two bleaching sessions of 30 min each, with 7-day interval (D0 and D7); G2-two bleaching sessions (same protocol as G1) associated with violet LED light, 30 min each session. The color evaluation was performed before and after 7 days of the second session (D0 and D14); the medium third of buccal surface of canine and central incisor teeth were evaluated under VITA Shade Guide and spectrophotometry (subjective and objective). Tooth sensitivity was evaluated immediately after the treatment and after 48 h of each bleaching session (D0, D2, D7, and D9), and a self-perception questionnaire was applied on D14. Results: Data were subjected to nonparametric Mann-Whitney test and both canine and central incisor teeth presented color change. Group G2 showed significantly more change than G1. Fisher's exact test showed no statistically significant dental sensitivity differences between groups. Both groups reported a high level of satisfaction but chi-square test showed statistically higher satisfied patients with dental bleaching in G2 than G1. Conclusions: The violet LED light (405-410 nm) could improve dental bleaching effectiveness, without sensitivity increase.

In-office dental bleaching allows the dentist to have greater control of the procedure and prevents patients from ingesting chemicals. To obtain optimum results, in-office bleaching usually requires a longer period of application as well as changes of the bleaching agent applied to the tooth surfaces at each appointment. The objective of this case report was to assess, by means of a split-mouth design in a single patient, the final tooth color and tooth sensitivity resulting from 2 different bleaching protocols: 1 application of 35% hydrogen peroxide for 45 minutes and 3 applications of 35% hydrogen peroxide for 15 minutes each. Neither the patient nor 5 individuals who were blinded to the techniques noted a difference in the final esthetic results of the 2 protocols immediately after the procedure. In addition, the patient reported that no tooth sensitivity was associated with either protocol. The results of dental bleaching on both sides were maintained after 15 days. The results shown in the present case report suggest that there may be no need to renew the gel during in-office dental bleaching.

Despite nearly a century of research, the treatment of cervical dentinal hypersensitivity (DH) remains challenging. This case report discusses the indications for different approaches to the treatment of DH in a single patient; the chosen alternatives took into account the different degrees of tooth wear and levels of pain at different sites. A 31-year-old woman reported DH in the maxillary right canine and first premolar and the maxillary left lateral incisor and canine in response to thermal, tactile, and osmotic stimuli. Clinical examination revealed that the teeth on the right side presented noncarious cervical lesions deeper than 1 mm, while the teeth on the left side presented only minimal wear. Therefore, the right canine and premolar were restored with composite resin to create a mechanical barrier against stimuli and reestablish form, function, and esthetics. Prior to restoration, the teeth on the right side were irradiated with a low-power laser (808 nm, 100 mW, 1.1 J/point, 10 seconds), which was applied in a single session at 2 locations on each tooth. In contrast, the left lateral incisor and canine were irradiated in 3 sessions with the low-power laser, which reduces pain levels and depolarizes nerve fibers by means of cell biomodulation, and received no restorations. A visual analog scale (0.0-10.0) was used to record the patient's pain, and it was found that pain levels for the restored teeth decreased from 9.4 initially to 0.0 immediately after restoration, and pain levels for the irradiated teeth decreased from 5.4 initially to 2.0 after 3 sessions. After 6 months of clinical evaluation, both sets of teeth showed scores of 0.0 (no pain). Based on the results presented, it can be concluded that both treatments provided satisfactory outcomes when applied for the appropriate indication.

In Endodontics, photosensitizers' such as methylene blue and toluidine blue have been used in Photodynamic Therapy due to their positive results. However, they can stain the dentin from the root canal after Photodynamic Therapy (PDT). The present in vitro study aimed to evaluate different stain removal protocols from root canal after PDT using methylene blue (MB) dye. After mechanical preparation of the root canal of 40 uni-radicular human teeth, PDT was performed using 0,01% MB with parameters of 3min of pre-irradiation and a diode laser irradiation emitting at 660nm, 40mW, 4min, 9.6J. After PDT, different protocols of MB removal were performed: Group 1 - control (0.9% saline solution); Group 2 - sodium hypochlorite (2.5% NaOCl); Group 3-17% ethylenediamine tetraacetic acid (EDTA); Group 4 - passive ultrasonic irrigation (PUI); The color of the dentin of the root canal was measured, before, immediately after the PDT and immediately after the cleaning using a spectrophotometer. The ΔE values found were statistically compared using the ANOVA and Tukey's tests (α=0.05). All the treatments lead to some cleaning of root canal after PDT, however, none of the treatments tested completely removed all staining caused by MB photosensitizer of the root canal. Among the treatments tested, PUI and Hypochlorite 2.5% promoted greater cleaning, with no statistically significant difference between them. In conclusion, within the protocols tested in the present study, no treatments were able to completely remove MB staining of the root canal after PDT.

Postoperative sensitivity is one of the most common side effects of in-office bleaching with hydrogen peroxide. Laser phototherapy (LPT) has been suggested as an adjunctive treatment to prevent or minimize tooth sensitivity. This case report aimed to verify the efficacy of LPT in the reduction of sensitivity after in-office bleaching. Tooth bleaching was performed with 35% hydrogen peroxide activated with a hybrid LED-laser device. Immediately after the bleaching treatment, the patient reported dental sensitivity, as measured with a visual analog scale (VAS). To reduce sensitivity, LPT was applied with a 780-nm laser using the following parameters: 70 mW, exposure time of 10 seconds per point of irradiation (middle region of the buccal surfaces of each compromised tooth) in contact mode, energy of 1 J per point. Immediately after LPT, the patient reported a substantially lower level of pain on the VAS. Twenty-four hours after bleaching, the score on the VAS indicated that sensitivity levels had rebounded, and the patient received additional LPT. After 48 hours, the patient reported no dental sensitivity. The results in this patient indicated that irradiation with an infrared low-power laser substantially reduced dental pain generated by bleaching, suggesting that LPT should be considered as an auxiliary method to reduce postbleaching tooth sensitivity.
