RESUMEN

A feedforward-feedback aeration control strategy based on online oxygen requirements (OR) estimation is proposed for oxidation ditch (OD) processes, and it is further developed for intermittent aeration OD processes, which are the most popular type in Japan. For calculating OR, concentrations of influent biochemical oxygen demand (BOD) and total Kjeldahl nitrogen (TKN) are estimated online by the measurement of suspended solids (SS) and sometimes TKN is estimated by NH4-N. Mixed liquor suspended solids (MLSS) and temperature are used to estimate the required oxygen for endogenous respiration. A straightforward parameter named aeration coefficient, Ka, is introduced as the only parameter that can be tuned automatically by feedback control or manually by the operators. Simulation with an activated sludge model was performed in comparison to fixed-interval aeration and satisfying result of OR control strategy was obtained. The OR control strategy has been implemented at seven full-scale OD plants and improvements in nitrogen removal are obtained in all these plants. Among them, the results obtained in Yumoto wastewater treatment plant were presented, in which continuous aeration was applied previously. After implementing intermittent OR control, the total nitrogen concentration was reduced from more than 5 mg/L to under 2 mg/L, and the electricity consumption was reduced by 61.2% for aeration or 21.5% for the whole plant.

Epidermolysis bullosa acquisita (EBA) is an acquired subepidermal bullous disease characterized by IgG autoantibodies to type VII collagen, a major component of anchoring fibrils. Most patients with EBA are adult and develop autoantibodies to the noncollagenous (NC) 1 domain of type VII collagen. We describe a 4-year-old Japanese boy presenting pruritic vesicles and tense blisters over his whole body. Immunofluorescence studies revealed linear IgG/C3 deposits along the dermal-epidermal junction of the patient's skin, and circulating IgG autoantibodies mapping to the dermal side of 1 M NaCl-split skin. By immunoblotting analysis using dermal extracts as a substrate, the patient's IgG antibodies labelled a 290-kDa protein corresponding to type VII collagen. Immunoblotting studies using recombinant proteins demonstrated that the patient's circulating autoantibodies recognized not only the NC1 but also the NC2 domain of type VII procollagen. Review of the previously reported cases and the present case suggested that patients with EBA with autoantibodies to regions other than the NC1 domain are all children younger than 10 years of age with clinical features of an inflammatory phenotype.

RESUMEN

The first case of primary cutaneous squamous cell carcinoma (SCC) to cause zosteriform and epidermotropic metastasis to skin is reported. The patient is a 72-year-old Japanese woman. A cutaneous SCC appeared on the lateral side of her right knee and was removed. After dissection of the right inguinal lymph nodes, which revealed metastases, and irradiation of the right inguinal region, the patient presented with slightly pruritic and painful erythematous papules on the right hip and small brownish papules and vesicles with crusts on the anterior side of the right thigh. The eruptions were in a zosteriform distribution along the right L1 to L3 dermatomes. Histologically neoplastic squamous cell nests were observed in the epidermis, below the epidermal-dermal junction, and within lymphatic vessels in the deeper reticular dermis. We postulate that neoplastic cells with the ability to fuse with adjacent squamous epithelium may have been carried beneath the basal lamina or to the epidermis via dermal lymphatic backflow, resulting in epidermotropic metastasis.

Human T-lymphotropic virus-1 (HTLV-1) is considered to be the cause of adult T-cell leukaemia-lymphoma (ATL). Monoclonal integration of HTLV-1 proviral DNA, as is analysed by Southern blotting, has been demonstrated in ATL patients. Unusual integration patterns of HTLV-1 proviral DNA have occasionally been described, and it is suggested that the patterns have clinical implications for ATL pathophysiology. Multiple, complete and defective types of integration patterns, in that order, are apparently associated with prognoses from good to poor. We report a 73-year-old Japanese woman with lymphoma-type ATL and a bulky cutaneous tumour on the left thigh. Four bands of slightly differing intensity were seen after EcoRI digestion of skin and lymph node samples on Southern blot analysis of HTLV-1 proviral DNA. Analysis for T-cell receptor-beta gene revealed five novel bands after restriction enzyme digestion with HindIII, indicating that the patient has four separate tumour cell clones, each of which carries one copy of the provirus. She was treated with chemotherapy and radiation and remains under reasonable control despite some relapsing cutaneous nodules. The indolent course in this present case could be related to the multiple integration pattern of HTLV-1 proviral DNA detected.

Physiological reaction and oxygen intake during exercise and recovery were measured in fourteen young female Japanese during the follicular phase of their menstrual cycle at 25 degree C with 50% relative humidity and at 35 degree C with 50% relative humidity. Subjects, clad in bathing suits only, performed a bicycle ergometer exercise at a constant work load of 600 kg . m/min at a cycling rate of 50 rpm for 20 min and recovered while remaining on the bicycle ergometer for 40 min. The mean values of sweat volume and skin temperature were significantly greater at 35 degree C than at 25 degree C. It has been shown that heart rate and rectal temperature during exercise were slightly higher at 35 degree C than at 25 degree C, while those during recovery were significantly higher at 35 degree C than at 25 degree C. Oxygen intake, oxygen debt, and the fall in diastolic blood pressure after exercise were considerably greater at 35 degree C than at 25 degree C. The increase in oxygen intake in a hot environment might result from an increased metabolism due to higher body temperature and increased energy requirement for heat dissipation such as profuse sweating, higher heart rate, and increased ventilatory volume. The increase in oxygen debt in a hot environment might reflect the increased metabolism caused by higher body temperature and the increased production of lactic acid in the working muscle as a result of an insufficient blood supply to the muscle. The increases in sweat volume, oxygen intake during exercise, and oxygen debt in women in a hot environment were considerably smaller than corresponding values for men. The smaller increase in sweat volume in women in a hot environment could reflect a smaller oxygen intake and a more marked dilation of skin vessels in women than in men.
