RESUMEN

Alteration of food patterns leads to changes in nutritional status, thus contributing to the development of non-communicable diseases accounting for over a half of all causes of death of the population of our country. Poor working conditions and occupational hazards play an important role in inducing metabolic disorders and cardiovascular diseases. The objective of joint studies conducted by two Healthy Nutrition Centers located in the cities of Yekaterinburg and Moscow was to assess the diet and nutritional status of workers at two industrial enterprises of the Sverdlovsk Region. The total of 347 unrefined copper production workers (Plant 1) and 267 iron ore miners (Plant 2) were included in the study (the average age was 45.7±0.4 and 50.4±0.6 years, respectively). The study design envisaged a study of actual nutrition by a frequency method, anthropometric indices, total body composition by bio-impedancemetry, and nutritional status biomarkers using biochemical blood indices. The workers' diet was characterized by a high energy value (more than 2,500 kcal/day) with an excess of total and saturated fats (40.7-41.0 and 15.2-15.3% by the calorie content) as well as mono- and disaccharides (19.0-21.0% by the calorie content). Vitamins C and A consumption of the iron ore miners was 78% (p<0.01) and 27% (p<0.05) lower than that of the unrefined copper production workers, respectively. High (>1.0) waist to hip ratios were estimated in 28.5% of the workers. Obesity (BMI>25.0 kg/m2) was established in 36-42% of our subjects and the fat mass excess - in 81% of them. High serum concentrations of low-density lipoproteins were measured in 28 and 35% of the workers of both plants, respectively. The metabolic syndrome was identified in 27.0% of Plant 1 workers and in 44.2% of Plant 2 miners, whereas cardiovascular diseases were diagnosed in 25.9 and 56.5% of the workers, respectively. The effect of genetic factors (rs993609 polymorphism of FTO gene and Trp64Arg polymorphism in ADRB3 gene) on the development of obesity and metabolic disorders was demonstrated.

This research aims to assess catering arrangements and dietary patterns of children at preschool educational institution and at home. The article analyses catering arrange­ments for pre-school children (3-7-year-old) from Ekaterinburg, Nizhny Tagil, Kamensk-Uralsky, Vladikavkaz and Krasnoyarsk. Based on menu production records provided by preschool educational institution, and family nutrition questionnaires we have examined their compliance with the nutritional physiological standards. The summarized data on menu production records from preschool educational institutions showed whether they met food basket requirements (net weight), as well as nutritional value requirements (proteins, fats, carbohydrates, calories content). The assessment was carried out in Ekaterinburg, Nizhny Tagil and Krasnoyarsk using Public catering calculations software (version 5) and a proprietary database of childreris food formulae. Childrens food basket assess­ment in Ekaterinburg and Kamensk-Uralsky revealed underconsumptionof vegetables (17.9-38% decrease), milk (52.7-62% decrease), curd (40.2-45.3% decrease). We have also observed decreased consumption of fruits, including dried fruit, by 31.8%, as well as cereals and legumes by 43% in Kamensk-Uralsky. Macronutrient analysis has shown underconsumption of total carbohydrates (by 19%) and animal proteins. The assessment of children nutrition at home has been carried out using NUTRITEST-IP diagnostic sys­tem in Nizhny Tagil, Kamensk-Uralsky, Vladikavkaz and Krasnoyarsk. The results showed overconsumption of most foods except vegetables. Excessive intake of fats and sugars (3.6 and 3.5 fold higher, respectively) accounted for higher than normal energy consump­tion by children at home. The evaluation has also revealed a significant difference in the consumption of saturated fats, mono - and disaccharides, food fibers and vitamin C by the children from the locations included in the study. The excessive intake of simple carbohy­drates was typical for family diets and excessive fat intake was characteristic for child diet in preschool educational institution.

RESUMEN

In the Sverdlovsk region for the assessment and management of environment and health of the population over more than 10 years there is used a methodology for the assessment of the multiple environmental risk, allowing to identify priority routes of toxicants entering the body, to evaluate the possible dose-response relationships and elaborate targeted measures for their correction. In the article there is shown how are effective management decisions, elaborated on the base of the results of the assessment of multiple environmental risk to public health in cities of Pervouralsk and Revda of the Sverdlovsk region and directed to the mitigation of the exposure to chemical factors of the environment.

The authors evaluated possible increase in metabolic disorders risk in workers exposed to natural asbestos and to mixed asbestos stock dust, on the background of improper nutrition. Findings are excessive intake of food with high content of free sugar, saturated fatty acids and common salt (2.3-11.0 times) with lack of chicken meat, cereals, fish, vegetables, fruits and cultured milk foods in diet. Excessive body weight was seen in 81.0% of the workers, every third worker was obese. Chances ratio proved that the workers with excessive body weight are more probable in developing high serum levels of glucose, cholesterol, triglycerids, low density lipoproteins, higher atherogenicity index (2.4-3.1 times), and probability of arterial hypertension is higher by 78%.
